The allorecognition of H-2Kb-specific CD4-CD8- T cell hybridomas is influenced by the substitution at residue 256 of MHC class I molecules.

Our previous studies demonstrated that allorecognition of HTB176.10 and HTB177.2, H-2Kb-reactive CD4-CD8- T cell hybridomas is markedly influenced by the exchange of the alpha 3 domain between H-2Kb and H-2Dp. The recombinant genes of the exon 4 between H-2Kb and H-2Dp were constructed to determine the residues of the alpha 3 domain that influence the allorecognition of these T cell hybridomas. Seven recombinant genes of the exon 4 were generated by in vivo recombination in Escherichia coli. Chimeric genes containing these recombinants were transfected into L cells and the transfectants expressing equivalent amounts of chimeric molecules were selected by flow cytometry. Studies on responses of these T cell hybridomas to the chimeric molecules confirmed our previous observation that the primary structure of the alpha 3 domain influences the allorecognition by the hybridomas. Moreover, it was indicated that residue 256 on the alpha 3 domain markedly affects the allorecognition by the T cell hybridomas, although substitutions at residues 184, 193, 195, 197, 262, and 264 exerted some effects on the T cell recognition. Further studies with the use of a single amino acid mutant of H-2Kb at residue 256 confirmed the effect of substitution at residue 256 on allorecognition of the T cell hybridomas. Taken together, results of this study demonstrated that polymorphism of the alpha 3 domain is indeed involved in the formation of allodeterminants recognized by TCR.

Is picolinic acid a glycine agonist at strychnine-sensitive receptors?

By means of unit recording and electrophoretic application, the effect of picolinic acid on feline spinal interneurons in situ was studied in comparison with glycine. Picolinic acid inhibited neuronal firing in 60% of neurons and in some cases the inhibitory actions were antagonized by strychnine. Inhibition of firing by glycine, which was also strychnine-sensitive, was reduced in case of concomitant administration of picolinic acid. These results suggest that picolinic acid might act as a glycine agonist at strychnine-sensitive receptors.

[Targeting anticancer chemotherapy dispersed in lipid contrast medium in hepatoblastoma in childhood].

Fourteen cases of malignant liver tumor in childhood were experienced in our department during past 14 years. Since 1984 we have performed preoperative targeting anticancer chemotherapy using oily anticancer agents such as THP-adriamycin 30 mg/m2. These oil emulsion was making with 20 mg amounts of THP-adriamycin dissolved in 5 ml urographin and 15 ml volume of lipiodol. These mixture were administered by catheterizing the hepatic artery under X-ray monitoring in 6 cases with hepatoblastoma. Remarkable anticancer effects of this targeting chemotherapy were achieved, the serum AFP level and tumor size both showing a decrease in all cases, and the resectability of tumor showing a increase in 5 among 6 cases in comparison with 50% resectability before 1983.

Synthesis and antiinflammatory activity of novel 12 beta-substituted analogues of betamethasone.

A series of 9 alpha-halo-12 beta-hydroxy and 12 beta-acyloxy analogues of betamethasone 17,21-dipropionate were synthesized and tested for topical antiinflammatory potency in the croton oil ear assay. The compounds were assayed for systemic absorption in the contralateral ear assay, in which it was found that 12 beta-hydroxy analogues 9, 13, and 15 were all absorbed but the corresponding 12 beta-esters 11a-e, 14, and 16 were not. On repeated high-dose applications to the mouse ear, there was no evidence of systemic absorption of any 12 beta-propionate ester as gauged by thymus weights (thymic involution) and plasma cortisol levels (adrenal suppression). Results of limited SAR studies showed that topical antiinflammatory activity and systemic absorption were not greatly influenced by the 9 alpha-halogen but were largely dependent on the polarity and size of the 12 substituent. While the optimal compounds 14 and 16 were less topically active than the controls beta- and beclomethasone dipropionate, unlike the controls, they displayed no systemic effects, even after repeated high-dose applications. Surprisingly, propionate 14 was devoid of atrophogenic activity.

Spinal alpha 1- and alpha 2-adrenoceptors mediate facilitation and inhibition of spinal motor transmission, respectively.

The role of descending noradrenergic fibers in the spinal motor systems was investigated using spinal reflexes in acutely spinalized rats. In rats pretreated with the MAO inhibitor clorgyline-HCl (1 mg/kg, i.v.), L-3,4-dihydroxyphenylalanine (L-dopa) (5 mg/kg, i.v.), a precursor of dopamine and noradrenaline, markedly potentiated the mono- (MSR) and polysynaptic reflexes (PSR). Selective blockade of alpha 1-adrenoceptors by pretreatment with prazosin-HCl abolished these facilitatory effects on the MSR and the PSR and revealed the inhibitory effect of L-dopa on the PSR. The depression of PSR was antagonized by the alpha 2-antagonist piperoxan. Clonidine-HCl (0.05 mg/kg, i.v.), a so-called alpha 2-agonist, and tizanidine-HCl (0.1 mg/kg, i.v.) decreased the MSR and the PSR in rats pretreated with prazosin. These inhibitions were antagonized by piperoxan. These results suggest that alpha 1- and alpha 2-adrenoceptors mediate facilitation and attenuation of motor transmission in the rat spinal cord, respectively.

[Perfusion study of the pulmonary hilar region by SPECT--normal cases].

Alveoli in the hilar region comprise the peripheral area containing daughter branches from subsegmental or one more divisional peripheral bronchi. Pulmonary perfusion in hilar region was examined by SPECT (single photon emission CT) in ten normal volunteers. ROI (region of interest) in the axial images were set in the hilar region, the upper, middle and lower lung fields with 10.8 mm thickness. Counts/one pixel (C/P) were calculated in these ROI. There was a tendency of C/P increase from upper to lower lung field. And there were no significant differences in C/P increase between hilar region and other axial fields. In the chronic obstructive pulmonary diseases, however, ventilation studies in the previous reports using Xe dynamic CT or PET (positron emission tomography) showed differences between outer region and hilar region. This method will be expected to evaluate the pulmonary perfusion not only in the whole lung but in different lung areas, including the hilar region in the chronic obstructive pulmonary diseases.

[An endoscopy-guided plan of radiotherapy for detecting esophageal cancer using a lugol staining].

The Lugol staining method has lately come to play an important role in the detection of superficial esophageal cancers which are extremely difficult to detect by using only a barium double-contrasted esophagiogram and conventional esophagioscopy. Thus we have tested an endoscopy-guided plan of radiotherapy for detecting an esophageal cancer using Lugol stain. This method was found useful in detecting intraepithelial small lesions, such as intramural metastasis or multiple origin cancers. Additionally, it has been found useful in determining the definite shape of superficial malignant lesions by evaluating biopsy specimens taken from the border of the non-stained regions.

[A case of alveolar cell carcinoma with multiple cavities, with special reference to the radiological pathological correlations].

Presented is the case of alveolar cell carcinoma in a 66-year-old woman that manifested diffuse multiple cystic cavity-forming lesions with infiltrations into the lungs. An autopsy revealed a bronchiolo-alveolar cell adenocarcinoma with mediastinal metastatic lymph nodes. Most of the cavities, whose walls were microscopically lined by tall columnar tumor cells in the pattern typical of an alveolar cell carcinoma, were round-shaped and had thin walls that showed no necrosis and were filled with mucus that contained tumor cells. The majority of these cavities were characteristic tension cavities or cysts resulting from an increased in the intraalveolar pressure caused by the mucus produced by the tumor cells in the alveolar space.

[A case of pseudoinfarct of the liver (Zahn's infarct)].

We reported a case of pseudoinfarct (Zahn's infarct) of the liver caused by the inflammatory pseudotumor. A dense segmental staining in the area of Zahn's infarct, which was thought to be characteristic finding, was found on the hepatic angiography. This finding was supposed to be consistent with sinusoidal hyperemia according to the pathological finding.

[A case of the Stewart-Treves syndrome (post-mastectomy angiosarcoma) with special reference to a radiological study].

Reported is the case of a 54-year-old female manifesting the Stewart-Treves syndrome (a postmastectomy angiosarcoma.) The patient had developed an angiosarcoma in her lymphoedematous right shoulder and lower neck develop ten years after a radical mastectomy for a papillo-tubular adenocarcinoma of the right mammary gland. Radiologically, a well-enhanced tumor was revealed by computed tomography, which angiographically had more stains and tumor vessels than usual for a mammary carcinoma and less than usually seen in a cavernous hemangioma. Histologically, this tumor showed less of a luminal differentiation than a conventional angiosarcoma and resembled a fibrosarcoma. Computed tomography and angiography were found useful in achieving a diagnosis and the subsequent therapy for this Stewart-Treves syndrome.

[Pulmonary lesion of paraquat poisoning on high resolution CT].

It is well known that paraquat causes severe organ-toxicity and pulmonary damage. We observed the progress of a patient who survived paraquat poisoning, and we recorded the changes in the lung by high resolution CT. The patient was a 35-year-old man who attempted suicide by paraquat (Guramoxone 100 ml) ingestion. At the time of hospitalization, there was no respiratory involvement. Five days after ingestion, an X-ray examination showed only indistinct vascularity of both lung fields, but high resolution CT showed increased density in the central part of both lung fields. According to the clinical progress after ingestion, mediastinal and subcutaneous emphysema were noted by chest X-ray examination. On the other hand, severe interstitial pneumonia progression of severe lung fibrosis with a decrease in lung volume and interstitial pulmonary emphysema in addition to mediastinal, subcutaneous emphysema were seen by high resolution CT. High resolution CT is useful for detecting morphologic change and diagnosing clinical stages. Observing the course of changes by high resolution CT is useful for deciding the course of clinical therapy, and we have no hesitation in affirming that it should be used in such cases.

[A case of major pulmonary laceration caused by blunt open chest trauma successfully treated by operation].

A 42-year-old man had his right arm pulled into a belt conveyer and his right upper extremity was amputated only partially leaving the scapula and the clavicle. A penetrating wound was present at the third intercostal space and the rupture of the upper lobe of the lung was diagnosed through this wound. After tracheal intubation, operation was performed under general anesthesia. The penetrating wound at the third intercostal space was widened and the thoracic cavity was examined. The rupture existed between S1 and S2 and it extended to subsegmental bronchus. The ruptured upper lobe was resected. The defect in the chest wall was reconstructed with mobilized anterior serratus muscle and major pectoral muscle after stabilizing the fractured ribs by inserting Kirschner wire into the marrow of the fractured ribs. Postoperative course was uneventful and the wound healed primarily without infection. The patient was sent to a rehabilitation facility 52 days after injury.

Hyperhistaminemia in patients with histidinemia due to increased decarboxylation of histidine.

Histidinemia results from a defect in the major catabolic pathway of histidine. Among the various alternative metabolic pathways, transamination, methylation and acetylation of histidine have been found to be increased, but increased decarboxylation to histamine has not been reported in histidinemia. In an attempt to confirm the possibility of increased decarboxylation of histidine, plasma histamine levels were determined by reversed-phase high speed liquid chromatography in histidinemic, atopic, and control groups. In the histidinemic group, plasma histamine levels were higher than those in the other two groups, and correlated positively with plasma histidine level. Serum IgE levels, the numbers of eosinophils and basophils, and the histaminopexic power in the histidinemic group were within the normal range. These results suggest that hyperhistaminemia is not due to allergic reactions but results from increased decarboxylation of histidine.
