Activation and processing of non-anchored yapsin 1 (Yap3p).

A C-terminally truncated form of yapsin 1 (yeast aspartic protease 3), the first member of the novel sub-class of aspartic proteases with specificity for basic residues (designated the Yapsins), was overexpressed and purified to apparent homogeneity, yielding approximately 1 microg of yapsin 1/g of wet yeast. N-terminal amino acid analysis of the purified protein confirmed that the propeptide was absent and that the mature enzyme began at Ala68. The mature enzyme was shown to be composed of approximately equimolar amounts of two subunits, designated alpha and beta, that were associated to each other by a disulfide bond. C-terminally truncated proyapsin 1 was also expressed in the baculovirus/Sf9 insect cell expression system and secreted as a zymogen that could be activated upon incubation at an acidic pH with an optimum at approximately 4.0. When expressed without its pro-region, it was localized intracellularly and lacked activity, indicating that the pro-region was required for the correct folding of the enzyme. The activation of proyapsin 1 in vitro exhibited linear kinetics and generated an intermediate form of yapsin 1 or pseudo-yapsin 1.

Transanal approach to rectocele repair may compromise anal sphincter pressures.

PURPOSE: This study prospectively assessed the functional results, particularly anal sphincter impairment, following transanal repair of rectocele for chronic intractable constipation. METHOD: Twenty-one consecutive women (mean age, 47.7 (standard error of the mean, 2.7) years) had the diagnosis of rectocele obstructing defecation made on synchronized anal manometry, electromyography, and cinedefecography. All underwent a standardized transanal repair with controlled anal stretching (maximum of 4 cm) from self-retaining anal retractors. The clinical function and anorectal manometry were assessed before surgery and were repeated six months later. RESULTS: All 21 patients were subjectively satisfied with the relief from constipation after surgery. There were significant improvements in the straining at defecation (before, n = 19; after, n = 3; P = 0.001), need to digitate per vagina (before, n = 16; after, n = 0; P = 0.001), stool frequency (before, 3.8 (0.7) times weekly; after, 8.6 (1.2); P = 0.004), and laxative requirements (before, n = 7; after, n = 0; P = 0.03). Although none were clinically incontinent, there was a mean 28 mmHg impairment in resting (P < 0.05) and 42.6 mmHg impairment in maximum squeeze anal pressures (P < 0.05) after operations. There was no other morbidity. CONCLUSION: Transanal rectocele repair effectively improves constipation problems, at the risk of impaired anal sphincter function. Although clinical incontinence was minimum, an alternative approach to rectocele repair should be considered when anal sphincters are lax.

The prediction of common bile duct stones using a neural network.

BACKGROUND: The role of preoperative ERCP and endoscopic sphincterotomy (ES) in the diagnosis and treatment of suspected common bile duct stones (CBDS) in the laparoscopic age is controversial. The preoperative diagnosis of CBDS by ERCP and the removal of CBDS by ES are advantageous because of technical difficulties in performing laparoscopic exploration of the common bile duct. Approximately 50% of preoperative ERCP examinations are normal, however. The noninvasive diagnosis of CBDS has assumed new importance, but it has proved to be an elusive goal. Neural networks are a form of artificial computer intelligence that have been used successfully to interpret ECGs and to diagnose myocardial infarcts. The purpose of this study was to determine whether a neural network could be trained to predict CBDS accurately in patients at high risk of having duct stones. STUDY DESIGN: We trained a back-propagation neural network to predict the presence of CBDS. Retrospective data from patients who had a cholecystectomy and either a preoperative ERCP or intraoperative cholangiogram were used to build the network, and it was tested using unseen data. RESULTS: One hundred forty patients were used to train the network, and 16 patients were used to test it. The trained network was able to predict CBDS in 100% of the patients in both the training and test sets. CONCLUSIONS: Screening of high-risk patients for CBDS by neural network analysis is highly accurate. This promising new, noninvasive, and inexpensive technique can potentially decrease the need for preoperative ERCP by 50%, but additional prospective evaluation is indicated.

Overexpression of ErbB2 blocks Taxol-induced apoptosis by upregulation of p21Cip1, which inhibits p34Cdc2 kinase.

Overexpression of the receptor tyrosine kinase p185ErbB2 confers Taxol resistance in breast cancers. Here, we investigated the underlying mechanisms and found that overexpression of p185ErbB2 inhibits Taxol-induced apoptosis. Taxol activates p34Cdc2 kinase in MDA-MB-435 breast cancer cells, leading to cell cycle arrest at the G2/M phase and, subsequently, apoptosis. A chemical inhibitor of p34Cdc2 and a dominant-negative mutant of p34Cdc2 blocked Taxol-induced apoptosis in these cells. Overexpression of p185ErbB2 in MDA-MB-435 cells by transfection transcriptionally upregulates p21Cip1, which associates with p34Cdc2, inhibits Taxol-mediated p34Cdc2 activation, delays cell entrance to G2/M phase, and thereby inhibits Taxol-induced apoptosis. In p21Cip1 antisense-transfected MDA-MB-435 cells or in p21-/- MEF cells, p185ErbB2 was unable to inhibit Taxol-induced apoptosis. Therefore, p21Cip1 participates in the regulation of a G2/M checkpoint that contributes to resistance to Taxol-induced apoptosis in p185ErbB2-overexpressing breast cancer cells.

Marsupialization of fistulotomy wounds improves healing: a randomized controlled trial.

BACKGROUND: Marsupialization of anal fistulotomy wounds leaves less raw unepithelialized tissue to granulate and may improve wound healing. METHODS: Some 103 consecutive patients with uncomplicated intersphincteric or trans-sphincteric fistula in ano were recruited into a randomized controlled trial. Fistula tracts were identified and laid open under general anaesthesia. The patients were randomized to have either the wounds left open (LO group) or wound edges marsupialized to the fistula tract with interrupted absorbable sutures (MS group). Anal manometry was performed before operation, and 6 weeks and 3 months after surgery. RESULTS: Some 52 patients were randomized to the LO group and 51 to the MS group. There were no differences in the age, sex, fistula type and fistula length distribution between the groups. Mean follow-up times were 9 and 10.2 months respectively. Wounds in the MS group healed faster (mean(s.e.m.) 6.0 (0.4) weeks) than those in the LO group (10.0(0.5) weeks) (P < 0.001). Only one patient (2 per cent) in the MS group was incontinent of liquids after operation compared with six (12 per cent) in the LO group. There was less impairment in maximum anal squeeze pressure at 3 months after marsupialization compared with leaving fistulotomy wounds open (P < 0.05). Apart from a slightly longer operative time required for marsupialization, the hospitalization and complication rates were the same. CONCLUSION: Anal fistulotomy wounds healed faster after marsupialization. Anal squeeze pressures were better preserved and this may improve anal continence.

Coupling of serotonin 5-HT1B receptors to activation of mitogen-activated protein kinase (ERK-2) and p70 S6 kinase signaling systems.

Little is known about the coupling of serotonin 5-HT1B receptors to cellular signals other than cyclic AMP. In the present studies, the activation by 5-HT1B receptors of p70 S6 kinase and the mitogen-activated protein kinase (MAP kinase) ERK-2 was investigated. Studies were performed by using both nontransfected Chinese hamster ovary (CHO) cells, which express endogenous receptors at a very low density, and a stable transfected CHO cell line expressing 5-HT1B receptors at 230 fmol/mg of membrane protein, a density similar to that expressed in cortex. In nontransfected cells, 5-HT was found to stimulate a greater than twofold increase in MAP kinase activity with an EC50 of 20 nM. Reflecting increased density of receptors, 5-HT caused a greater than eightfold activation of ERK-2 in transfected cells with an EC50 of 2 nM. 5-HT was found to also stimulate p70 S6 kinase in both nontransfected and transfected cells. The stimulation was sixfold in both types of cells, but the EC50 for 5-HT was fourfold lower in transfected cells. The coupling of 5-HT1B receptors to ERK-2 and to p70 S6 kinase was inhibited by pertussis toxin, inhibitors of phosphatidylinositol 3-kinase, and by the inhibitor of MAP kinase kinase PD098059. Activation of p70 S6 kinase, but not ERK-2, was also inhibited by rapamycin. These findings demonstrate that 5-HT1B receptors couple to ERK-2 and p70 S6 kinase through overlapping, but nonidentical, pathways.

Prevalence of diabetes mellitus in elderly people in Canada: report from the Canadian Study of Health and Aging.

AIMS: to estimate the age-specific prevalence of diabetes mellitus in elderly people in Canada, and to examine the effect of method of ascertainment on the estimation of prevalence. METHOD: three measures of diabetes were used in a secondary analysis of the Canadian Study of Health and Aging--a 1991 nation-wide cross-sectional study of the prevalence of dementia in a sample of 10,263 elderly subjects (aged 65-106 years). RESULTS: of community-dwelling subjects, 10.3% reported diabetes. Supplementing this information with clinical reports and random plasma glucose measurements increased the prevalence to 12.0% in the community, 17.5% in institutions and 12.4% overall. CONCLUSION: diabetes is common in elderly people, although the prevalence falls in the very elderly. The method of ascertainment influences estimation of prevalence.

Mutational analysis of the Chlamydia trachomatis rRNA P1 promoter defines four regions important for transcription in vitro.

We have characterized the Chlamydia trachomatis ribosomal promoter, rRNA P1, by measuring the effect of substitutions and deletions on in vitro transcription with partially purified C. trachomatis RNA polymerase. Our analyses indicate that rRNA P1 contains potential -10 and -35 elements, analogous to Escherichia coli promoters recognized by E-sigma70. We identified a novel AT-rich region immediately downstream of the -35 region. The effect of this region was specific for C. trachomatis RNA polymerase and strongly attenuated by single G or C substitutions. Upstream of the -35 region was an AT-rich sequence that enhanced transcription by C. trachomatis and E. coli RNA polymerases. We propose that this region functions as an UP element.

Correlation of carpal tunnel size and conduction velocity of the sensory median and ulnar nerves of male and female controls and carpet weavers.

The relation of the carpal tunnel parameters (area, width, and depth) measured by computerized axial tomography to the conduction velocity of the sensory median and ulnar nerves was studied in the right and left hands of the 24 male and 32 female controls and 64 carpet weavers (40 women, 24 men). There were sex- and hand-related differences in these relationships in the two groups. Analysis suggested that carpal tunnel size cannot be responsible for the disorders of sensory nerve conduction in hands; repetitive hand movements, such as carpet weaving, may destroy the normal relation of carpal canal size to velocity of sensory nerve conduction.

The curvelinear correlations between the total testosterone levels and fluid intelligence in men and women.

The relationship between serum total testosterone (T) level and the fluid intelligence (Cattell's Culture Fair Intelligence Test) was studied in men and women. There was no significant difference between IQs of men and women. There was an inverse curvelinear relationship between IQ and T in women. The same was found also in men, but the declining part of the regression line at high T levels was not as pronounced as in women. It is concluded that T may be related to IQ, even in subjects exhibiting no sex difference in IQ tests; too low or too high T levels may be disadvantages for the fluid intelligence, especially in women.

Curvelinear correlations between total testosterone levels and fluid intelligence in men and women.

The relationship between serum total testosterone (T) level and fluid intelligence (Cattell's Culture Fair Intelligence Test) was studied in young men and women. There was no significant difference between IQs of men and women. IQ tended to increase with T in men, except at very high T levels. It was concluded that (i) T may be associated with IQ, even in samples with no sex-related IQ difference; (ii) too low or too high T may be disadvantageous for fluid intelligence in women; (iii) T may be advantageous for this kind of IQ in men, except very high T levels.

The prenatal attenuation of brain/body development through interactions between growth hormone, triiodothyronine and testosterone during prenatal development of female neonates.

The relations of serum growth hormone (GH), free testosterone (T), and free triiodothyronine (T3) concentrations to the head circumference, height, and weight were studied in the human neonates. Blood was taken from the umbilical cord immediately after the birth, to measure the hormone concentrations. GH was found to be inversely correlated with the bodily measures in the female neonates; there were no significant correlations between these variables in males. In females, free T3 negatively linearly correlated with GH, and positively linearly correlated with the neonatal head circumference, height, and weight. Serum free T levels was found to be positively linearly correlated with GH, and negatively linearly correlated with the neonatal body measures in the same subjects. It was suggested that GH may exert growth reducing effects through T3 and T during perinatal development.

Effects of different subtypes of histamine receptors on proliferation and differentiation of murine colony forming unit granulocyte-macrophage and colony forming unit megakaryocyte.

OBJECTIVE: To investigate the function and characteristics of histamine receptors on the hemopoietic progenitor cells. METHODS: BDF1 mice (both male and female), inbred at our university, aged 8-12 weeks, weighing 20-24 g, were used in this study. Bone marrow cells were incubated for 1 hour at 37 degrees C with 2-AT (H1 receptor agonist) or impromidine (H2 receptor agonist) alone, or in combination with the antagonists pyrilamine or cimetidine respectively. Control experiment was performed in Dulbecco's modified Eagle's Medium (DMEM) alone. Cells treated with different drugs were performed by colony forming unit-granulocyte-macrophage (CFU-GM) and colony forming unit-megakaryocyte (CFU-Meg) assay. RESULTS: When bone marrow cells were treated with 10(-8) mol/L to 10(-5) mol/L of 2-thiazolylethylamine (2-AT) which had no influence on CFU-GM and CFU-Meg proliferation, 10(-8) mol/L to 10(-5) mol/L of impromidine could increase the number of CFU-GM and CFU-Meg colonies. The effects of H2 receptor agonists on CFU-GM, CFU-Meg could be antagonized by H1 receptor agonist. CONCLUSIONS: Our findings suggest the existence of histamine H1 and H2 receptors on the hemopoietic progenitor cells and the antagonism between two different histamine receptor subtypes on the proliferation of CFU-GM and CFU-Meg.

[Changes of blood lipids and apolipoproteins in patients with silent myocardial ischemia].

Serum lipid profiles and apolipoproteins were measured in 148 patients with silent myocardial ischemia (SMI) and 30 healthy control subjects comparable in age. The serum lipid profiles and apolipoproteins were abnormal in all types of SMI which were more significant in type II and III. The extent of ST segment depression in ECG were positively correlated with serum TG, TC, LDL, B100, B100/A1, Lp(a); and negatively correlated with A1, HDL1, HDL2, HDL-C/TC. Multiple-factor stepwise regression analysis revealed that the increased concentration of serum TG, LDL, and B100/A1 ratio and decrease of HDL2-C are independent risk factors in SMI.

[Clinical significance of heart rate variability: analysis of silent myocardial ischemia].

To study the potential role of dynamic electrocardiogram(DEG) in silent myocardial ischemia (SMI) patients, we measured the extension of ischemia and heart rate variation-time-domain analysis in 148 patients with SMI and 30 healthy controls by DEG and followed up all patients for 1.5 years. The results were that the extension of myocardial ischemia (the extension of ST segment depression, episodes of SMI attack, and the total ischemia time), the incidence rate of ventricular premature heat increased gradually and the difference of heart rate. Heart rate variant hinder(HRVI), and SDNN decreased gradually in SMI I, III, II types. And the extension of ischemia had positive correlation with the lowest heart rate and the incidence rate of ventricular premature beat, while negative correlation with HRVI and SDNN, indicating that the impairment of cardial autonomic nerves is associated with the extension of ischemia. Also, we found that 23 patients out of 148 patients with SMI died and the levels of HRVI and SDNN of the patients who died were lower than that of the survivals. We conclude that the heart rate variability can serve as a prognosis index of SMI.

[Study on the transformation of mammal cells induced by glycidyl methacrylate in vitro].

Glycidyl methacrylate(GMA) can induce the phenotype transformation of human embryonic lung fibroblasts. The transformed cells prolifeated rapidly with a higher in the ratio of nucleus to cytoplasm, and their life span extended notably. The transformed colonies exhibited in extensive random orientation and the cells crossing-over. The transformed cells could be agglutinated by lower concentration of ConA and could grow in semisolid agar. The chromosome abberations could occur in transformed fibroblasts. These results suggest that GMA is able to induce malignant transformation of mammal cells, and it may be carcinogenic to human.

[Effects of retionoic acid and GM-CSF on apoptosis of HL-60 cell induced by mild heat].

The effects of several factors on the apoptosis of HL-60 cells, a human acute myeloid leukemia cell line, were investigated. The apoptosis model was established with mild heat. Typical changes of cell morphology and characteristic DNA ladder pattern were found. The effect of apoptosis induced by mild heat was enhanced, when HL-60 cells were pretreated with the retinoic acid. By contraries, that of apoptosis was displayed with the slight inhibition while HL-60 cells were pretreated with rhGM-CSF. We also observed the alterations of mRNA expressions of c-myc by the dot hybridization. The results indicate that both the mild heat and the combination of mild heat and retinoic acid obviously reduce the mRNA expression of c-myc compared to the control group.
