Summer-type hypersensitivity pneumonitis. A unique disease in Japan.

A unique form of hypersensitivity pneumonitis in which clinical symptoms appear in the summer and subside spontaneously in the mid-autumn was found in Japan. This disease was named summer-type hypersensitivity pneumonitis and was found the most prevalent form in Japan. This disease has the following characteristic features: 1) initiation in the summer; 2) repeated episodes during subsequent seasons for many years; 3) familial occurrence; 4) no occupational relationship; 5) positive returning-home provocation test; 6) cough, dyspnea and remittent fever as a clinical triad; 7) diffuse nodular shadows on chest x-ray film; 8) leukocytosis with neutrophilia; 9) moderately decreased % VC and markedly decreased Dco and PaO2; 10) skin reactivity to PPD is negative while symptomatic; 11) pulmonary lesions of biopsied specimens show epithelioid cell granulomas without central necrosis (63.3 percent), plus alveolitis and/or pneumonitis; 12) isolation of patients from their home environment diminishes symptoms; 13) corticosteroid is effective; 14) seasonal atmospheric microbiological pollution is speculated upon, but the offending antigen is not defined yet.

A case of calcitonin producing adenoma of the thyroid.

A thyroid tumor about 4 cm in diameter was removed from a 53-year-old female. The clinical features were those of common thyroid adenomas, such as soft, smooth-surfaced and round contour of the tumor, no calcification and no lymph node metastasis. However, microscopically, the tumor was composed of compact cell nests rather similar to paraganglioma without any follicle formation. Histochemical examination revealed its C-cell nature, such as argyrophilia and calcitonin activity, although no amyloid deposit was demonstrated. Extremely high levels of calcitonin were found in the stored serum taken pre-operatively, but serum carcinoembryonic antigen levels were within normal limits. The calcitonin level dropped to the normal range soon after the operation, indicating the absence of residual tumorous tissues. Thus, the tumor behaves as an adenoma with a C-cell nature. Interestingly enough, the benign counterpart of medullary carcinoma of the thyroid seems to be quite rare in humans, and no similar tumors have ever been reported. Variations of C-cell tumors will be discussed in relation to their production and secretion of carcinoembryonic antigen.

An Ia-like antigen positive null cell leukemia cell line (THP-5) lacking in the stimulating capacity in autologous and allogeneic mixed lymphocyte reaction.

A null cell leukemia cell line, THP-5, was established from the peripheral blood of a patient with acute null cell leukemia. THP-5 cells were characterized by the presence of Ia-like antigen and the absence of other cell surface markers examined. Despite the presence of Ia like antigen, THP-5 cells had no stimulating capacity in autologous or allogeneic mixed lymphocyte reaction.

A new indirect method for measuring arteriovenous O2 content difference and cardiac output from O2 and CO2 concentrations by rebreathing air.

A new indirect method for measuring the arteriovenous O2 content difference (avDO2) was developed. The avDO2 was calculated by dividing the gradient of the CO2 dissociation curve by that of a gas exchange ratio against PCO2. The latter slope was obtained from O2 and CO2 concentrations in rebreathing air. The validity of the method was tested preliminarily in human subjects by comparing the cardiac output calculated from avDO2 and O2 uptake (VO2) with that measured hitherto by other authors, and then in dogs by comparing the calculated avDO2 with the measured value. In the dog experiments, the rebreathing was performed 7 times in each of 7 dogs. Immediately after the rebreathing arterial and mixed venous blood were sampled and analyzed for avDO2. For each rebreathing period the avDO2 was calculated by using the CO2 dissociation curve obtained in the individual dogs. The correlation coefficient between the measured and calculated avDO2 was 0.87, demonstrating reasonable validity of the method. The VO2 was further measured from the time interval during which a known amount of pure O2 was consumed. Then, the cardiac output was calculated by dividing the VO2 by the measured and calculated avDO2. The correlation coefficient between the respective cardiac output values was 0.88, indicating the reliability of using the calculated avDO2.

Intracranial germ-cell tumors. Immunohistochemical study of three autopsy cases.

Histological and immunohistochemical findings in three cases with primary intracranial germ-cell tumors are reported. Based on histological examination, they were diagnosed, respectively, as an endodermal sinus tumor with immature teratoid differentiation, teratoma and embryonal carcinoma, and choriocarcinoma and germinoma. The first case was presumed to be a combination of endodermal sinus tumor and germinoma. An immunohistochemical examination showed positive reactions to alpha-fetoprotein (AFP) and carcinoembryonic antigen (CEA) in the endodermal sinus tumor, and to human chorionic gonadotropin (HCG) in the choriocarcinoma, but showed no reaction in the embryonal carcinoma. Human chorionic gonadotropin was demonstrated in syncytiotrophoblastic cells and CEA in a gland-like structure. The value of measuring CEA in cases of germ-cell tumors, in addition to AFP and HCG assays, is stressed, and the characteristics of CEA-positive tissue are discussed.

[Combination chemotherapy with multimodality approaches for small cell carcinoma of the lung].

Small cell carcinoma of the lung (SCCL) is disseminated at the presentation in most patients. Chemotherapy is, therefore, the principal therapeutic modality in this disease. The application of aggressive combination chemotherapy has resulted in an improved survival for patients with SCCL. In an attempt to improve therapeutic results, the current study was undertaken to test a cyclic alternating chemotherapy with the four-drug combination of COMP and a three-drug combination of VP-16, adriamycin and ACNU (VAN), which appeared non-cross-resistant to the former. In this study, also included two randomized clinical trials to test the effectiveness of concomitant chest irradiation in patients with limited disease, and to test the validity of prophylactic cranial irradiation for patients achieved complete remission. In this paper, results of the studies were analysed and discussed.
