Exercise testing of the chronic effects of bunazosin hydrochloride on dilated cardiomyopathy.

Bunazosin hydrochloride (Detantol) was administrated in 3-mg oral doses to 10 patients (6 males and 4 females, average 49.9 years) with dilated cardiomyopathy (DCM) of class II in the NYHA functional classification. The patients were observed for changes in their hemodynamics at rest, chest X-ray, echocardiography, humoral factors and symptom-limited exercise tolerance with treadmill testing before treatment and 4 and 8 weeks after administration. All but the left ventricular end-diastolic diameter after 8 weeks were unchanged. Of the humoral factors, only aldosterone decreased significantly during the treatment. On the other hand, the duration of symptom-limited exercise was prolonged significantly, and for a given exercise load, a reduction of heart rate and systolic blood pressure as well as a drop in the pressure-rate product occurred at 8 weeks after administration. Therefore, bunazosin appears to improve cardiac function and exercise tolerance in the patients with DCM, at dosages which do not affect hemodynamics at rest.

[Immunohistochemical study of T cell subsets in lung tissue and in BALF of patients with farmer's lung disease].

The purpose of this study was to ascertain the immunological abnormality of hypersensitivity pneumonitis. The relationship between T cell subsets of lymphocytes in transbronchial lung biopsy (TBLB) specimens and in bronchoalveolar lavage fluid (BALF) were studied in 3 cases of farmer's lung disease (FLD). The lung specimens were examined by immunoperoxidase staining (ABC method) and the cells in BALF by the immunofluorescence method on the flowcytometry. All cases of FLD were diagnosed according to the following criteria: 1) history of exposure to FLD antigen, 2) clinical symptoms (cough, fever, breathlessness), 3) radiologic feature (diffuse small nodular pattern) and functional pattern of interstitial lung disease and 4) evidence of antibodies against Micropolyspora faeni. Histologically, granulomatous interstitial pneumonitis was revealed in all cases. Immunohistochemically, the number of positive lymphocytes was as follows: for Leu 4 (pan T cell) 8.6 cells/15.6 x 10(-3) mm2; Leu 3a (helper/inducer T cell) 5.1 cells; Leu 2a(suppressor/cytotoxic T cell) 1.2 cells on average, respectively. The Leu 3a+ cells were larger in number than the Leu 2a+ cells and the Leu 3a+/Leu 2a+ ratio was 4.76. In the BALF, the percentage of OKT3+ cells (pan T cell) was increased. The percentage of OKT4+ cells (helper/inducer T cell) was higher than that of OKT8+ cells (suppressor/cytotoxic T cell). The OKT4+/OKT8+ ratio was 6.65 in the BALF. The result of this study revealed the close relationship between the numerical distribution of T cell subsets in the lung tissue and in the BALF. It is suggested that the immuno-reaction in the lung tissue of patients with FLD is a type of helper-T cell alveolitis.

[Prevention of hepatitis B--HLA DR/DQ in HBs Ag nonresponder].

To evaluate the immunological background in HBs Ag nonresponders against hepatitis B vaccine, the lymphocyte surface marker and HLA-DR/DQ antigen were determined on hospital personnel, 70 males and 256 females, injected hepatitis B vaccine for three times. The vaccine made from the plasma of HBs Ag carriers was injected at the first and second vaccination and recombinant hepatitis B vaccine was injected at the third vaccination. A month after the third vaccination, blood was withdrawn for HBs antigen test by RIA. Border line cases (cut off index 1.0-1.9) are included in nonresponder (cut off index less than 0.9). Both nonresponders and low responders (cut off index 2-49) are more often seen in males than females and high responder (cut off index 50 or more) are seen more often in young females than males. Lymphocyte surface markers were studied by flow cytometry using the following monoclonal antibodies; OKT 3, 4, 8, DR, NK, Ia 1 and B7. No differences between lymphocyte surface markers of nonresponders and responders were noted. HLA-DR/DQ antigens were studied by the cytotoxicity test using Locus DR/DQ, Terasaki Second DR W-60 Tray and using following antibodies; DR 1, DR 2, DRW 15, DR 4, DR 5, DR 7, DR 9, DRW 10, DRW 8, DRW 12, DRW 13, DRW 6, DRW 52, DRW 53, DQW 1, DQW 6, DQW 2, DQW 3, DQW 7 and DQW 4. No significant differences between HLA-DR/DQ of nonresponders and responders were noted.

[Histopathologic changes and tumor cell kinetics after hyperthermia and/or radiation therapy in a rat glioma model: bromodeoxyuridine (BUdR) labelling index].

Single and combined treatment of interstitial microwave hyperthermia (HT) and radiation therapy (RT) were delivered to rat glioma models. The animal model tumors were induced by intracerebral inoculation of a small piece of G-XII glioma tissue to 6-8 week-old rats. Heating to about 44 degrees C at the surface of the inserting antenna using a 2450 MHz microwave was carried on for 30 minutes. A single dose of 800 r to the whole head was delivered by deep X-ray apparatus. In combined treatment, heating immediately preceded irradiation. Following treatment, animals were sacrificed at 1, 3, 6, 12, 24, 48, 72 and 96 hours. BUdR was administered intraperitoneally 1 hour before sacrifice. Microscopically, in HT, tumor cells became eosinophilic and separated from each other. Some of them were necrotic. Macrophage infiltration in tumor tissue was recognized after 72 hours. BUdR labelling indices were less than 1% till 24 hours had passed, then became 25% at 48 hours, nearly equal to that of the control animals. In RT, ballooning of the tumor cells was prominent, and some of the tumors became necrotic. Lymphocyte infiltration of the tumor tissue was occasionally seen. BUdR labeling indices decreased slowly; less than 10% in 24 hours, but continued until 96 hours had passed. Combined treatment of HT and RT showed addictive effect of histological changes and suppression of tumor cell growth.

[Acute myocardial infarction probably induced by the oral administration of bromocriptine: a case report].

This is a report of a case of acute myocardial infarction which was probably induced by the oral administration of bromocriptine. A 55-year-old man was admitted because of suspected hypothyroidism and dwarfism. The endocrinological examination revealed primary hypothyroidism, and levothyroxine sodium replacement therapy was started. Nine days later, the bromocriptine test was performed for dwarfism. Two hours after the oral administration of 2.5 mg bromocriptine, he had severe chest pain associated with loss of consciousness and hypotension. His electrocardiogram showed marked ST depression and T wave inversion, but no abnormal Q waves. Serial myocardial enzymes and electrocardiograms confirmed acute non-transmural myocardial infarction. During coronary angiography performed about three months later, coronary arterial spasm was induced by the intravenous administration of ergonovine maleate. This experience suggests that bromocriptine, a derivative of ergot alkaloids, can cause coronary arterial spasm, and subsequent myocardial infarction. To our knowledge, bromocriptine-induced myocardial infarction has not been previously reported in Japan.

Immunofluorescence studies of the cytoskeletal and contractile elements in cultured human trabecular cells.

Characteristics of the distribution of cytoskeletal and contractile elements in cultured human trabecular cells were studied using the immunofluorescence method. Actin microfilaments exhibited stress fiber images which were observed throughout the cell body in a parallel pattern and were concentrated in the cell periphery. Myosin also revealed stress fibers with a distribution corresponding to actin distribution. Vimentin intermediate filaments and microtubules were stained in a filamentous pattern radiating from near the nucleus to the cell periphery. These intermediate filaments and microtubules were more densely and more abundantly observed throughout the cytoplasm in the cultured human trabecular cells than in the cultured human skin fibroblasts. These results suggest that the trabecular cell cytoskeleton plays an important role in maintaining the structural integrity and normal function of the trabecular meshwork.

[A case of nonpenetrating ureteral avulsion].

A 7-year-old boy who had been in a traffic accident was hospitalized in Hamamatsu Red Cross Hospital with diagnosis of head abrasion, contusion of abdomen and multiple fractures. On the 17th day of hospitalization, he complained of left flank pain and high fever. Examinations by IVP and enhanced computed tomographic scan revealed a left ureter avulsed at the level of uretero-pelvic junction. Operative reconstruction by end-to-end anastomosis of the ureter was performed immediately. The function of left ureter was recovered 4 months after operation. This rare case of ureteral avulsion is briefly discussed along with a review of the recent literature.

[The production of denture by preform compression molding method. Part 2. Model material and fittability].

A preform compression molding method to make polysulfone denture was reported previously by paper. Properties of the mold, adaptability of the molded denture to master model and the movement of artificial teeth were examined. It was necessary to raise the temperature of the crystbalite-stone model above 270 degrees C to obtain stable expansion. Mold made using 22 wt%-colloidal silica showed strong compressive strength under the compression molding condition. Displacement of artificial teeth between wax denture and polysulfone denture was independent of the mold expansion, and the displacement was equal to that of heat-curing acrylic denture. Adaptability of the molded denture to the master model was associated with expansion of the model. Good adaptability was obtained under the expansion of the model, such as setting expansion about 0.30% and total expansion 1.05-1.10%, with about 0.2 mm average discrepancy.

[Experimental study of phagocytosis of the cells observed on the surface of intraocular lens].

It has been reported that macrophages adhere to implanted intraocular lens (IOL) in a reaction to the foreign body, and transform themselves into epithelioid cells, fibroblast-like cells, and foreign body giant cells. We attempted to examine the activity of these cells by studying phagocytosis. Extracapsular lens extractions with implantation of posterior chamber IOLs were performed in rabbit eyes. After one week, the implants were removed and incubated in culture medium containing latex beads to investigate phagocytosis of these cells adhering to the implants by light microscopy, scanning and transmission electron microscopy. The cells observed were macrophages, epithelioid cells, fibroblast-like cells, and foreign body giant cells. The round macrophage was most active in phagocytizing latex beads, while the activity was weaker in the epithelioid cells, fibroblast-like cells, and foreign body giant cells. The macrophages on the implants had many ruffles on their surface, and many latex beads adhered to the cell surface. The epithelioid cells and foreign body giant cells had few ruffles with few latex beads on their surface. These results suggested that cellular activity of the macrophages on the implanted IOL might decrease as their shape transformed.

[Production of denture by preform compression molding method. Part 3. Retentive strength of artificial teeth].

A preform compression molding method to make a polysulfone denture has been reported. Retentive strength of artificial teeth to the denture base was examined to select artificial teeth for the compression molding method. Ceramic teeth with metal pins and polysulfone teeth heated at above 140 degrees C were retained to the denture base by useful retentive strength. Acrylic teeth on which the adhesive was used were also retained by useful strength. Each of the teeth, acrylic teeth, ceramic teeth and polysulfone teeth, could be used in the compression molding method.

[Review of 43 autopsy cases of urogenital malignant tumors].

Autopsy findings were reviewed in 43 patients clinically diagnosed in the last 12 years as having urogenital malignant tumors. Clinical diagnoses were 14 bladder carcinoma, 11 prostatic carcinoma, 6 renal cell carcinoma, 5 renal pelvic carcinoma, and 7 other malignant tumors. Autopsy showed that 31 cases died due to carcinoma and 12 because of other causes. The most common ultimate causes of death were DIC and infection, especially pneumonia and sepsis. Autopsy showed 36 of the 43 cases (83.7%) with metastasis. Clinical diagnosis showed 34 cases of metastasis, but the number of metastasized organs and lymph-nodes was much lower than in subsequent autopsy findings. Autopsy proved 5 cases of clinical misdiagnosis (11.6%) and 4 of undiagnosed malignant tumors (9.3%). In 15 cases (32.6%) the ultimate cause of death as revealed by autopsy had not been clinically diagnosed. Five cases diagnosed as having died due to cancer in fact were found to have died due to other causes. Recent diagnostic techniques are greatly advanced, yet many findings are still revealed for the first time by autopsy. Autopsy continues to be a very important final arbiter of progress and the effect of malignant tumors, and serves to remind us of the ongoing need for constant vigilance and improvement of clinical diagnostic techniques.

[A case of retrovesical tumor].

A 53-year-old man was admitted with complaint of left lower abdominal pain. Retrovesical tumor was suggested after digital examination and imaging diagnosis. At operation, the encapsulated tumor occupied the retrovesical space, but was not adhered to the adjacent organs such as prostate, seminal vesicles, bladder and rectum. The entire tumor was successfully resected. Histopathological report revealed a mature teratoma. He is alive without recurrence at 1 year after operation. This is the second case of retrovesical teratoma in the Japanese literature.

Japan: stimulant epidemics past and present.

Japan experienced a serious stimulant epidemic during the period from 1946 to 1956 and has been experiencing a second one since 1970. Over the years, a series of drug control measures have been put into effect by the Japanese Government. In 1953, the Japanese police system was reorganized, centralized and made more efficient. Law enforcement efforts were intensified, supported by the criminalization of stimulant abuse with the enactment of the Stimulant Control Law in 1951 and subsequent amendments to it that were rigorously enforced, resulting in more arrests, indictments and relatively harsh penalties for stimulant offences, as well as an increase in the number and volume of confiscations. In 1951, 26 per cent of those arrested for stimulant offences were under the age of 20. About half of those arrested were stimulant-addicted. The number of arrests increased threefold from 1951 to 1954. The amount of seized stimulants also increased considerably during that period. In 1954, there were about 550,000 chronic stimulant users and 2 million ex-users. From 1980 to 1985, the number of stimulant arrests was relatively stable, levelling off at about 20,000 annually. About half of those arrested were recidivists. In 1985, a record high of nearly 300 kg of stimulants were seized. In 1960, heavy usage of sleeping pills among young persons began in Tokyo; this was considered a foreshadowing of a period of youth drug abuse in Japan. In 1967, there was an outbreak of inhalant use among young people; since 1975, about 40,000 people have been arrested each year for inhalant-related offences.

[A case of invasive thymoma in the left hilus of the lung].

Reported is the case of a 62-year-old man who was admitted to our hospital because of the presence of a left hilar mass found during a routine chest X-ray examination. Shortly thereafter, a left thoracotomy was performed. The tumor was located under the aortic arch in the left hilus of the lung and had invaded the pericardium and pulmonary trunk. There was no connection, however, to the anterior mediastinum. A histological examination revealed an invasive thymoma of epithelial cell of the predominant type. Seen scattered abroad were thymic tissue in the adipose tissue around the tumor of the left hilus. Postoperatively, a distant metastasis of the right lung was observed and the patient died due to respiratory failure.

[Association of sustained ventricular tachycardia and a mid-systolic click: a case report].

A 27-year-old woman had a history of palpitation at the age of nine. Ventricular tachycardia (VT) was detected four years ago and treatment with procainamide was started. Physical examination disclosed nothing abnormal other than an apical mid-systolic click. Electrocardiogram and chest radiograph were normal, and echocardiogram disclosed borderline mitral valve prolapse (MVP). VT was not provoked by exercise, and Holter monitorings of electrocardiogram were always within a normal limit. Angiography was normal. Electrophysiological study disclosed VT by left ventricular stimulation and the earliest activation was at the apex, VT was abolished by early stimulation from the right ventricular apex. The association of a mid-systolic click and VT was interesting in this case. When the patient had a systolic click at the outpatient clinic, VT frequently occurred within a month (six of seven times), while VT did not occur without an audible systolic click (three of four times). Although it is uncertain whether VT of this patient has any relationship to MVP or to the click, these problems remain to be solved in the future.

Multiple enzymes related to differentiation of lymphocyte subpopulations in man.

We studied the relation of various enzymes to subpopulations of lymphocytes in man. T cell-rich fractions were separated with a nylon column from mononuclear cells in the buffy coat. Comparing the enzymatic profiles of the two fractions, we found that the difference between the two groups came from the dominancy of B cells and/or macrophages in the former fraction, and from that of T cells in the latter. The enzymes characterizing T cells included N-Ac-beta-D-glucosaminidase (GlcNAc-ase), prolyl endopeptidase (Post-Pro-Enz), and dipeptidyl aminopeptidase IV (DAP-IV), whereas those characterizing B cells and/or macrophages include poly(ADP-ribose) synthetase, leucine aminopeptidase (Leu-AP), AP-B, cathepsin B, sialidase, and AP-A. Inhibitors of these enzymes may lead to modification of the function of T and B cells.
