Comparison between preload recruitable stroke work and the end-systolic pressure-volume relationship in man.

We compared linearity, inotropic sensitivity (using dobutamine) and afterload dependency (using captopril) of contractile indices derived from the end-systolic pressure-volume relationship (ESPVR) and preload recruitable stroke work (PRSW) in man. We investigated 60 patients, 39 with normal (EF > or = 50%, group 1) and 21 with decreased ventricular function (EF < 50%, group 2). Variably loaded P-V loops were constructed by conductance catheters with vena caval occlusions. The slopes of the ESPVR (Emax) and PRSW (Mw) were assessed by linear regression analysis. The correlation coefficients of the ESPVR (r = 0.964) and PRSW (r = 0.976) were both very high, but the Z-value of the ESPVR was lower than for PRSW. During the control period, normalized Emax and Mw were significantly higher in group 1 than group 2 (Emax: 4.6 +/- 2.3 vs 2.8 +/- 0.8 mm Hg ml-1 m-2, Mw: 109 +/- 31 vs 86 +/- 28 10(3) dyn cm-2. After dobutamine, Emax and Mw increased significantly both in group 1 (128% and 124% vs control respectively) and in group 2 (132% and 130% respectively). After captopril, the ESPVR tended to shift to the right with a decrease in afterload; PRSW was unchanged. We conclude that PRSW may be a more linear and reliable index for evaluation of contractility in man.

Assessment of myocardial oxygen consumption (Vo2) and systolic pressure-volume area (PVA) in human hearts.

Several studies have recently reported that the relationship between myocardial oxygen consumption per beat (Vo2) and left ventricular (LV) systolic pressure-volume area (PVA), which represents total mechanical energy generated by contraction, is linear and independent of loading conditions in excised, supported, and intact hearts. We assessed the Vo2-PVA relationship in nine patients with heart disease. LV volume and pressure were measured simultaneously by conductance catheter and tip-micromanometer. Vo2 was calculated from the difference between arterial and coronary sinus oxygen content, and coronary sinus blood flow measured by the thermodilution method. We obtained the linear relationship between Vo2 and PVA by dextran infusions (median r = 0.917). The slope of the Vo2-PVA relationship was (1.82 +/- 0.66) x 10(-5) mlO2 mmHg-1 ml-1 and the contractile efficiency, the reciprocal of the slope of the Vo2-PVA relationship, was 40 +/- 13%. The Vo2 intercept, which reflects Vo2 for non-mechanical work, was 0.0284 +/- 0.0286 ml O2 beat-1. These results suggest that PVA is a good predictor of myocardial oxygen consumption and a powerful tool to evaluate the coupling of LV mechanical performance to energy use in human hearts.

Characterization of IL-1 inhibitory factor released from human alveolar macrophages as IL-1 receptor antagonist.

IL-1 possesses pleiotropic properties on various cells and its activity may be stringently regulated in several ways. We have previously reported that both IL-1 and its inhibitory factor are concomitantly released from alveolar macrophages in both healthy subjects and patients with chronic inflammatory lung diseases. An increase in IL-1 activities and a decrease in inhibitory activities are characteristics found in both healthy smokers and patients with interstitial lung diseases. In this study, we further examined the biological properties of IL-1 inhibitory factor. The inhibitor exhibited a dose-dependent specific inhibition of an augmentation by IL-1 of PHA-induced murine thymocyte proliferation, while no inhibition of the augmentation by IL-2, IL-4, IL-6, or tumour necrosis factor (TNF) was found. 125I-labelled IL-1 alpha binding on PHA-stimulated murine thymocytes revealed two types of IL-1 binding sites, 44 sites/cell with a Kd of 2.7 x 10(-10) M and 230 sites/cell with a Kd of 2.5 x 10(-9) M. Alveolar macrophage culture supernatants blocked the binding of labelled IL-1 to the IL-1 receptor in a dose-dependent fashion. Scatchard plot analysis revealed that the inhibitory factor in the supernatants blocked the binding competitively. These results indicate that alveolar macrophages produce a specific IL-1 inhibitory factor, functioning as an IL-1 receptor antagonist.

The 72-kDa microtubule-associated protein from porcine brain.

A microtubule-associated protein (MAP) with a molecular mass of 72-kDa that was purified from porcine brain by using its property of heat stability in a low pH buffer was characterized. Low-angle rotary shadowing revealed that the 72-kDa protein was a rodlike protein approximately 55-75 nm long. The 72-kDa protein bound to microtubules polymerized from phosphocellulose column-purified tubulin (PC-tubulin) with taxol and promoted the polymerization of PC-tubulin in the absence of taxol. Microtubules polymerized by the 72-kDa protein showed a tendency to form bundles of several microtubules. Quick-freeze, deep-etch electron microscopy revealed that the 72-kDa protein formed short crossbridges between microtubules. We performed peptide mapping to analyze the relationship of the 72-kDa protein to other heat-stable MAPs, and the results showed some resemblance of the 72-kDa protein to MAP2. Cross-reactivity with a monoclonal anti-MAP2 antibody further suggested that the 72-kDa protein and MAP2 are immunologically related. To study the relationship between the 72-kDa protein and MAP2C, a smaller molecular form of MAP2 identified in juvenile rat brain, we prepared the 72-kDa protein from rat brain by the same method as that used for porcine brain. The fact that the 72-kDa protein from juvenile rat brain was also stained with our monoclonal anti-MAP2 antibody also suggested that the 72-kDa protein is an MAP2C homologue of the porcine brain.

Gomisin A inhibits tumor promotion by 12-O-tetradecanoylphorbol-13-acetate in two-stage carcinogenesis in mouse skin.

Gomisin A, isolated from the fruits of Schisandra chinensis, is one of the dibenzocyclooctadiene lignans. Application of 12-O-tetradecanoylphorbol-13-acetate (TPA, 1 microgram/ear), a tumor-promoting agent, to the ears of mice induces inflammation. Among seven dibenzocyclooctadiene lignans assayed, gomisin A, gomisin J, and wuweizisu C inhibited the inflammatory activity induced by TPA in mice. The ED50 of these compounds for TPA-induced inflammation was 1.4-4.4 mumol. Gomisin A, with an ED50 of 1.4 mumol, showed the strongest inhibitory effect. Furthermore, at 5 mumol/mouse, it markedly suppressed the promotion effect of TPA (2.5 micrograms/mouse) on skin tumor formation in mice following initiation with 7,12-dimethylbenz[a]anthracene (50 micrograms/mouse). It is assumed that the inhibition of tumor promotion by gomisin A is due to its anti-inflammatory activity.

Cytokine-related immunopotentiating activities of paramylon, a beta-(1-->3)-D-glucan from Euglena gracilis.

Paramylon, a beta-(1-->3)-D-glucan, isolated from Euglena gracilis, was tested for its adjuvant activity on the antibody response to sheep red blood cell (SRBC) in mice. Paramylon markedly enhanced anti-SRBC plaque-forming cell production at a dose of 10 mg/kg. It was also found that in vitro addition of lipopolysaccharide in culture to macrophages from paramylon-treated mice produced a large amount of interleukin 1 (IL-1) and there was a significant level of interleukin 6 (IL-6) induced transiently in the blood of these mice. As IL-1 and IL-6 play crucial roles in the immune response to T cell-dependent antigens like SRBC, the immunopotentiating effect of paramylon might be expressed through the action of these cytokines.

Assessment of left ventricular function using a conductance catheter in the human heart.

To validate the usefulness of the conductance catheter in the clinical setting, we first studied the accuracy of human left ventricular (LV) volume measured by conductance catheter in comparison with LV volume measured by biplane angiography in 19 patients with heart disease. Secondly, we made a comparison of end-systolic pressure volume relation (ESPVR) and preload recruitable stroke work (PRSW) relation in 60 patients with heart disease. Thirdly, we studied the myocardial oxygen consumption (VO2)-pressure volume area (PVA) relation to assess contractile efficiency in 22 patients with heart disease. There was a good correlation between the corrected conductance volume (Vcc) and the angiographic volume (V angio) (Vcc = 0.94 Vangio + 5.4, r = 0.94, P less than 0.001). Two relations, ESPVR and PRSW, were well described by straight lines with high correlation coefficients. However, PRSW was a more linear contractile index than ESPVR. The reciprocal of the slope of the VO2-PVA relation was approximately 40% in the control contractile state. We conclude that the conductance catheter accurately measures LV volumes and facilitates the assessment of ESPVR, PRSW and contractile efficiency in human LV.

Effect of soft x-ray irradiation on immunological functions in mice.

Effect of soft x-ray irradiation on immunological functions in mice was investigated. Soft x-ray irradiation with 100R or more induced a significant reduction in the number of plaque-forming cells (PFC). The reduction in the number of PFC depended on the irradiation doses. Irradiation with 600R or more showed a significant reduction in the delayed reaction of footpad swelling. However, soft x-ray irradiation with doses ranging from 100R to 1000R did not exert significant influence on the K values of carbon clearance test. Irradiation with 100R or more of soft x-ray showed a remarkable reduction of response to concanavalinA (ConA) or lipopolysaccharide (LPS) in spleen cells, and the response to ConA was lower than that to LPS. These results suggest that in the soft x-ray-irradiated mice, antibody-producing ability, delayed type hypersensitivity reaction and mitogenic activity are sensitive to soft x-ray irradiation and furthermore, T cell is more sensitive than B cell, but phagocytic activity of reticulo-endothelial system (RES) is resistant to soft x-ray irradiation.

[Effects of self-efficacy and outcome expectation on observational learning of altruistic behavior].

The purpose of the present study was to examined the hypothesis that the effects of observational learning of altruistic behavior are related to personal viewpoint, self-efficacy, outcome expectation, and other factors. One hundred and fifty-five fourth-grade school children were assigned randomly to four conditions; distress viewpoint, outcome expectation, altruistic behavior, or non-observational control. After observational learning, the students were immediately administered the generalization test of altruistic behavior, empathy, reward and punishment expectations, and rating tests of self-efficacy belief. It was found that; (a) altruistic behavior and outcome expectation conditions had significant learning effects, (b) self-efficacy was able to predict altruistic response in the altruistic behavior viewpoint condition, and (c) from an outcome expectation viewpoint, altruistic behavior toward peers and adults related to punishment expectation, whereas social sharing was related to reward expectation. According to these findings, an observational learning model of altruistic behavior was proposed.

Galacardins A and B, new glycopeptide antibiotics.

A strain of actinomycetes identified as Saccharothrix sp. SANK 64289 was found to produce new antibiotics, galacardins A and B. Their physico-chemical properties showed that they were new members of glycopeptide antibiotics. They were structurally related to beta-avoparcin but differed from it only in sugar composition. Though beta-avoparcin does not contain galactose, galacardins A and B did contain two and one moles of galactose, respectively. They showed strong antimicrobial activity against Gram-positive bacteria and also showed excellent in vivo protective activity against Staphylococcus aureus infection in mice.

[Expression of angiotensin II receptor on BALF macrophages from patients with sarcoidosis and healthy individuals].

We previously reported that angiotensin II (A-II) enhanced the accessory function of BALF macrophages in some patients with sarcoidosis. We hypothesized that this finding was mediated by the A-II receptor on BALF macrophages. From this viewpoint, we investigated the expression of A-II receptor on BALF macrophages. The binding of 125I-A-II to BALF macrophages was specifically inhibited by unlabeled A-II when incubated for 1 hour at 37 degrees C. However, the uptake of 3H-inulin to BALF macrophages was not inhibited by unlabeled inulin under the same conditions. These findings indicate that the binding of 125I-A-II to BALF macrophages was mediated by the putative A-II receptor, and was not due to pinocytosis.

[A case of pulmonary atypical mycobacteriosis complicated with aplastic anemia, treated with surgical resection and postoperative sparfloxacin].

A few weeks after treatment with INH, RFP and SM, severe aplastic anemia developed in a 49-year-old man with pulmonary atypical mycobacteriosis due to M. kansasii. All drugs were discontinued immediately after bone marrow examination revealed severely hypoplastic marrow, but pancytopenia continued for several months. Although M. kansasii was sensitive to other drugs including CS and TH, these two drugs were also discontinued because of their respective psychiatric and hepatic adverse effects. Ofloxacin (OFLX), to which M. kansasii was sensitive, was administered without clinical improvement and complete resistance to OFLX developed after several months treatment. Right upper and middle lobectomy and S6 partial lobectomy was performed, and postoperative sparfloxacin (SPFX) administration resulted in cure of the disease. Drug sensitivity testing revealed that the organism had acquired resistance to OFLX, but was still sensitive to SPFX. Thus, SPFX appears to be an useful drug for the treatment of atypical mycobacteriosis.

[The mechanism and specificity of IL-1 inhibitory factor released from human alveolar macrophages].

We have previously reported the presence of interleukin 1 (IL-1) inhibitory factor in the culture supernatants of alveolar macrophages. The activity is decreased in healthy smokers and patients with interstitial lung diseases (sarcoidosis, idiopathic pulmonary fibrosis), compared with healthy non-smokers. In this study, we further examined the mechanism and specificity of IL-1 inhibitory factor. The inhibitory factor exhibited specific inhibition of augmentation by IL-1 of PHA induced murine thymocyte proliferation, whereas there was no inhibition of the augmentation by IL-2, IL-4, IL-6 or TNF. This IL-1 inhibitory factor competitively blocked the binding of IL-1 to the IL-1 receptor on PHA-stimulated murine thymocytes. These results indicate that alveolar macrophages produce a specific IL-1 inhibitory factor which functions as a IL-1 receptor antagonist.

[Breast conservation treatment of early breast cancer--eight-year experience].

In the period between 1983 and 1991, we experienced 80 cases of early breast cancer treated by conservative surgery and postoperative radiotherapy. Our indications for breast conservation treatment are the size of tumor (less than 2 cm), the location (more than 3 cm away from the nipple) and no palpable axillary nodes. Our surgical procedures of quadrantectomy and level I and II axillary dissection are presented. The shape of the skin incision should be elliptical with the major axis radial from the nipple for good cosmetic results. The entire quadrant of the breast containing the primary carcinoma is radically removed with the superficial pectoralis fascia. At present, one breast recurrence and two distant metastases have developed. A further follow up study is needed to prove the safety of breast conservation treatment.

[The oral 1-25 dihydroxyvitamin D3 pulse therapy in hemodialysis patients for the early treatment of secondary hyperparathyroidism].

It is said that maintenance hemodialysis patients are already suffering from secondary hyperparathyroidism (2HPT) from early stage of chronic renal failure. The treatment of 2HPT in this stage is very important for preventing renal osteodystrophy (ROD). But many long-term dialysis patients are still afflicted with ROD although vitamin D have been used for treatment. In this study, an oral administration of 1-25 (OH)2 D3 (4 micrograms) with pulse therapy twice a week at the day before hemodialysis was started for 12 weeks. The concentration of 1-25 (OH)2D3, total calcium (Ca), ionized calcium (Ca++), alkaline phosphatase (ALP) and parathyroid hormone (PHT) in serum were measured not only before and after every 2 hours of administration a day, but also for 12 weeks after that. The peak of serum 1-25 (OH)2D3 could be sufficiently elevated after 8 hours, and the slight peak of Ca++ could be seen after 8 hours as well. But the level of total calcium could not increased. Although the level of only HS-PTH has not increased after 24 hours, a significant reduction in serum level of C-PTH, intact-PTH and HS-PTH could be recognized after 12 weeks finally. This pulse therapy was effective in reducing the serum level of PTH in this early stage from beginning hemodialysis. But, it needs further studies for the standard treatment.

[Secondary sideroblastic anemia caused by long term administration of anti-tuberculous agents including isoniazid].

A 54 year-old man, who had been treated for several years with isoniazid, was transferred to our hospital because of intractable pulmonary tuberculosis. He was given several anti-tuberculous agents including INH, CS and/or PZA for a long period of time without improvement of the pulmonary tuberculosis. After 7 years' treatment with INH, hypochromic microcytic anemia developed. Parenteral administration of iron preparation failed to improve the anemia. The bone marrow findings revealed erythroid hyperplasia and a few erythroblasts showed megaloblastic change. The bone marrow iron staining revealed increased sideroblasts of which 88% were ringed-form. INH was then discontinued and pyridoxal phosphate was administered with rapid complete improvement of the anemia. INH is well known as a causative agent for acquired secondary sideroblastic anemia, however only one case of INH-induced sideroblastic anemia has been reported in Japan. This is the second reported Japanese case of INH-induced secondary sideroblastic anemia, which appears to be a very rare condition because of genetically determined rapid acetylation of INH in Japanese.
