Bacillus subtilis spoVE gene is transcribed by sigma E-associated RNA polymerase.

Expression of the Bacillus subtilis sporulation gene spoVE was examined by runoff transcription assay with an RNA polymerase preparation obtained from vegetative and sporulating cells. Transcripts from tandem promoters (P1 and P2 promoters) located just upstream of the spoVE structure gene were detected. The transcription of spoVE initiated within an hour after the onset of sporulation and coincided with the presence of RNA polymerase associated with a 33-kDa protein. Amino acid sequence analysis of the 33-kDa protein revealed that it is a sigma factor, sigma E. Reconstitution analysis of sigma E purified from the sporulating cell extracts and vegetative core RNA polymerase showed that sigma E recognizes the P2 promoter. SpoVE protein could not be synthesized in the transcription-translation coupled system prepared from vegetative cells (M. Okamoto, S. Fukui, and Y. Kobayashi, Agric. Biol. Chem. 49:1077-1082, 1985). However, addition of sigma E-associated RNA polymerase to the coupled system restored SpoVE protein synthesis. These results indicate that spoVE expression in sporulating cells is controlled essentially by sigma E-associated RNA polymerase.

A solitary renal mass as the only manifest lesion of a B-cell lymphoma.

A patient with a solitary renal mass as the only manifest lesion of B-cell lymphoma was reported. The surgical specimen revealed that the lesion was histologically limited to the renal parenchyma. Liver dysfunction prevented him from receiving systemic chemotherapy. Mediastinal and mesenteric lymph node involvements developed 6 months later. Fortunately, the patient survived without recurrence for 18 months after the surgical removal of these tumors and intensive chemotherapy. Systemic chemotherapy is recommended on patients with renal lymphoma even if the tumor seems to be limited to the organ.

Comparison of hemodynamic determinants for myocardial oxygen consumption under different contractile states in human ventricle.

BACKGROUND: Recently, several indexes such as tension-time index (TTI), tension-time or force-time integral (FTI), rate-pressure product (RPP), pressure-work index (PWI), and systolic pressure-volume area (PVA) have been developed as predictors of myocardial oxygen consumption in experimental and clinical studies. However, it is still unclear whether these indexes are reliable predictors of myocardial oxygen consumption under various contractile states in human hearts. METHODS AND RESULTS: We assessed the relation between TTI, FTI, RPP, PWI, and PVA and myocardial oxygen consumption per beat (VO2) in 13 patients with heart disease during volume loading. Left ventricular (LV) volume and pressure were measured simultaneously by the conductance catheter with the tipped micromanometer technique. VO2 was calculated from arterial coronary sinus oxygen content difference, and coronary sinus blood flow was measured by the thermodilution method. After z transformation of the correlation coefficients, mean z value for the VO2-PVA relation (1.83 +/- 0.60) was greater than those for the VO2-TTI relation (1.22 +/- 0.66; p < 0.005), VO2-FTI relation (1.18 +/- 0.61; p < 0.05), VO2-RPP relation (0.95 +/- 0.65; p < 0.05), and VO2-PWI relation (1.24 +/- 0.58; p < 0.05). During dobutamine infusion (5 micrograms.kg-1.min-1) in five of the 13 patients, VO2 also correlated best with PVA (z = 1.70 +/- 0.89) compared with TTI (z = 1.43 +/- 0.86), FTI (z = 1.48 +/- 0.95), RPP (z = 1.00 +/- 0.53), and PWI (z = 0.88 +/- 0.80). The contractile efficiency (38 +/- 14% to 38 +/- 20%), the reciprocal of the slope of the VO2-PVA relation, remained unchanged, whereas the VO2,PVA 0.8 (VO2 at PVA = 0.8 J per beat/100 g LV) increased from 1.48 +/- 1.16 to 2.06 +/- 1.13 J per beat/100 g LV (p < 0.05). These results show the parallel upward shift of the VO2-PVA relation during dobutamine infusion. Because increases in the VO2-intercept represent the VO2 for the increased excitation-contraction (E-C) coupling associated with the augmented contractile state, the parallelism of the VO2-PVA relation could discriminate between VO2 for mechanical work (PVA-dependent VO2) and VO2 for E-C coupling (PVA-independent VO2). CONCLUSIONS: The results of the present study indicate that PVA is a reliable and valuable predictor of myocardial oxygen consumption under different contractile states in human hearts. The VO2-PVA relation could provide useful information about mechanoenergetics in diseased human hearts.

Studies on novel bone resorption inhibitors. I. Synthesis and pharmacological activities of aminomethylenebisphosphonate derivatives.

A series of aminomethylenebisphosphonate derivatives was synthesized and evaluated for their antiresorptive activities using a parathyroid hormone (PTH)-induced hypercalcemia model in rats (PIH model). Among these compounds, (cycloheptylamino)methylenebis(phosphonic acid) (3j) was finally selected for further investigation, proving 10-fold as potent as pamidronate in the PIH model and an immobilization bone atrophy model in rats (DA model). The structure-activity relationships of this series of bisphosphonates are discussed.

Purification and characterization of serine carboxypeptidases from Absidia zychae.

Among 29 strains of zygomycetes screened for serine carboxypeptidases, Absidia zychae NRIC 1199 showed the highest enzyme production. Two serine carboxypeptidases, CPZ-1 and CPZ-2, were purified to a homogeneous state from an extract of koji culture of A. zychae NRIC 1199. Purified CPZ-1 and CPZ-2 showed similar properties except the isoelectric point (pI); The pI of CPZ-1 and CPZ-2 were 4.50 and 4.65, respectively. The molecular weights of the CPZ-1 and CPZ-2 were 48,000 by SDS-PAGE and gel filtration. Among the proteinase inhibitors tested, phenylmethylsulfonyl fluoride and monoiodoacetic acid strongly inhibited the enzyme activity. The optimum pHs of CPZ-1 and CPZ-2 were 4.2 towards Z-Glu-Tyr. It is shown that the substrate specificities of CPZ-1 and CPZ-2 were dependent on the presence of bulky amino acid residues in the penultimate position (P1) for the small Z-peptides. However, in spite of the presence of Gly, Asp, Arg, or Pro in the P1 position, oligopeptides were hydrolyzed rapidly. CPZ-1 and CPZ-2 had not only carboxypeptidase but also carboxyamidase and amidase activities, and acted preferentially as a carboxyamidase for C-terminal amidated peptides. The hydrophobicity of P2 and P3 positions and the bulkiness of P1 and P'1 positions of the substrate may be important for carboxyamidase and amidase activities.

Overshoot in mixed venous oxygen saturation during recovery from supine bicycle exercise in patients with recent myocardial infarction.

During recovery from dynamic exercise, systemic oxygen extraction rapidly decreases below the resting level in patients with heart failure, which leads to a subsequent increase in mixed venous oxygen saturation (SvO2) above its resting value (postexercise SvO2 overshoot). To evaluate the pathophysiologic basis of this phenomenon, postexercise SvO2 overshoot was evaluated in relation to hemodynamic, metabolic, and neurohumoral responses during recovery from maximal supine bicycle exercise in 22 patients with recent myocardial infarction. Postexercise SvO2 overshoot closely correlated with increased systemic vascular resistance (r = 0.79, p < 0.001) and reduced cardiac output (r = -0.74, p < 0.001), suggesting that SvO2 overshoot is primarily due to increased peripheral arteriovenous shunting caused by an enhanced peripheral vasoconstriction in the setting of reduced cardiac output. Postexercise SvO2 overshoot and systemic vascular resistance were significantly higher and cardiac output was significantly lower in New York Heart Association functional class 3 and 4 (8 patients) compared with class 1 and 2 (14 patients), whereas systemic arterial blood pressure was maintained at normal levels in both groups. Thus, postexercise SvO2 overshoot and, hence, decreased systemic oxygen extraction during recovery represent a compensatory response of an enhanced peripheral vascular tone that maintains systemic arterial blood pressure in the setting of reduced cardiac output by linking central and peripheral blood flow.

[Urodynamic evaluation for bladder dysfunction after radical hysterectomy].

Bladder dysfunction after radical hysterectomy and/or radiotherapy for uterine cancer is a serious problem. Its pathogenesis has not been well elucidated. Urodynamic and clinical evaluations were performed in 53 patients; 24 of them underwent radical hysterectomy and postoperative radiotherapy (RH + RT), 13 had radical hysterectomy alone (RH), 7 had modified radical hysterectomy (mRH), 9 had radiotherapy alone (RT). Nine preoperative patients without micturition disturbance were examined, serving as controls. Patients with more than 50 ml of residual urine were only 3 in RH + RT, 2 in RH and 1 in RT. Bladder volumes at maximum desire to void were significantly lower in RT than in controls. Intravesical pressures at maximum desire to void were significantly higher in RH + RT and RH than in controls, but there were no significant differences between mRH or RT and controls. Detrusor compliances significantly decreased after radical hysterectomy and/or radiotherapy. Maximum urethral closure pressures significantly decreased after radical hysterectomy with or without radiotherapy. In RH + RT, 18 patients (75%) of them mainly complained of urinary incontinence. Their functional profile lengths were significantly shorter than in controls. We conclude that the pelvic plexus injury by radical hysterectomy compromise both urethral closure function and bladder compliance.

Establishment of HPRT-deficient cell lines from mouse neuroblastoma Neuro-2a and rat pheochromocytoma PC12.

Hypoxanthine/guanine phosphoribosyl transferase (HPRT)-deficient cell lines, designated as Neuro-2aTG and PC12TG, were established from mouse neuroblastoma Neuro-2a and rat pheochromocytoma PC12, respectively. Both cell lines stably exhibited HPRT- phenotype, and expressed neuronal properties, i.e., constitutive expression of 200-kD neurofilament protein in Neuro-2aTG and responsiveness to NGF in PC12TG. Therefore, these cell lines will be useful as fusion partners in somatic cell hybridization with neurons.

[Differential detection of IL-1 beta and TNF-alpha mRNA on lung macrophages from patients with pulmonary sarcoidosis].

In pulmonary sarcoidosis or experimental granuloma formation, interleukin-1 beta (IL-1 beta) or tumor necrosis factor alpha (TNF-alpha) are considered to play important roles during inflammatory evolution. In order to examine whether IL-1 beta or TNF-alpha mRNA expression on lung macrophages relates to the disease activity or clinical course, ten cases with pulmonary sarcoidosis were divided into two groups: five cases who had a disease duration of more than 10 years (14.6 +/- 4.4 years; group A), and 5 cases with duration of less than 3 years (1.7 +/- 1.1 years; group B). All cases showed both abnormal radiographs and elevated serum angiotensin converting enzyme activities. We compared the 10 cases with 12 healthy individuals as normal control (6 nonsmokers: NS and 6 current smokers: S), and 5 cases with idiopathic pulmonary fibrosis (IPF) as disease control. Lavage macrophages were purified by rosette forming method and plastic adhesion was then performed for 1 hour. Thereafter mRNA was extracted by AGPC method and amplified by reverse transcriptase-polymerase chain reaction (RT-PCR) (20 cycles). The results showed that IL-1 beta mRNA was detected in all materials studied, but TNF-alpha mRNA expression was different among the groups: 5/5 (100%) in group A, 1/5 (25%) in group B, 5/5 (100%) in IPF, and 12/12 (100%) in normal controls. The absence of detection of TNF-alpha mRNA (rapid down regulation) in pulmonary sarcoidosis may relate to spontaneous regression, because a substantial number of cases in group B showed spontaneous regression in their natural course.(ABSTRACT TRUNCATED AT 250 WORDS)

[Quantitative evaluation of gene expression of IL-1 beta and IL-1 receptor antagonist (IL-1ra) from alveolar macrophages in idiopathic pulmonary fibrosis (IPF) and sarcoidosis].

Our previous studies demonstrated the concomitant release of IL-1 and IL-1ra in the culture supernatants of alveolar macrophages in both healthy subjects and patients with interstitial lung diseases. IL-1ra was decreased in healthy smokers (HS), IPF, and sarcoidosis, compared to healthy nonsmokers (HNS), although an increase in IL-1 release could not be detected. In this study, we examined whether such findings could be found at the gene level, or reflect posttranscriptional regulation in patients with IPF (n = 8), sarcoidosis (n = 7), healthy smokers (n = 6) and healthy nonsmokers (n = 5). The expression of IL-1 beta, IL-1ra and a house keeping gene (glucose 6 phosphate dehydrogenase: G6PD) was investigated using reverse transcription polymerase chain reaction (RT-PCR). After RT-PCR, analysis and quantification of PCR products were performed by high-performance liquid chromatography. We used 26 cycle for IL-1 beta and IL-ra mRNA and 29 for G6PD, because amplification of mRNA was in the exponential phase on that cycles, respectively. The results were expressed as the ratio of each mRNA to G6PD mRNA. IL-1ra/G6PD ratio in patients with IPF and sarcoidosis, was lower than that in HNS. On the other hand, IL-1 beta/G6PD ratio was similar in all groups. IL-1ra/IL-1 beta ratio was decreased in patients with IPF and sarcoidosis, compared to HNS. These results suggest that decreased expression of IL-1ra gene may contribute to the development of chronic low grade inflammation of the lung.

[Invasive diagnostic technique for heart failure].

Invasive techniques have been important to assess the ventricular function. The approach for assessing function is based on measurement of cardiac output which depends on preload, afterload and contractility. Stroke work over a range of end-diastolic pressures was examined and termed the function curve. Clinical application of Frank-Starling principle is useful for characterizing heart failure. The isovolumic phase (positive dp/dt) and ejection phase index (ejection fraction) are responsive to acute change in contractility. The pressure-volume loop can be constructed by measurement of pressure and volume. The slope of end-systolic pressure-volume relation (ESPVR) is an expression of contractility. The linear relationship between stroke work and end-diastolic volume has been also proposed. The arterial system is characterized by the end-systolic pressure-stroke volume relationship and the slope is called as effective arterial elastance (Ea). One can evaluate stroke volume by coupling frame work between ESPVR and Ea. The isovolumic indices during diastole (negative dp/dt, time constant) can show impaired relaxation. The several factors influence the diastolic pressure-volume relation.

[A case of carcinomatous neuropathy with degeneration of posterior spinal column on MRI].

The patient was a 55-year-old man. He was well until eleven months earlier, when he noticed the onset of numbness in his right fingers. Over the next three months, the numbness spread up the right arm and involved the right leg, left arm and left leg in succession. Six months before admission, sensory impairment of the trunk appeared progressively. Mild diffuse muscle weakness occurred gradually. Swelling of the right lateral cervical lymph node was found just before admission. On neurological examination, sensory disturbance involving all modalities of sensation was recognized in the extremities and trunk. The sensory findings were asymmetric and predominant at the lower extremities and distal parts. A positive Romberg's sign and poor coordination was recognized. All the deep tendon reflexes were absent without pathological reflexes. Motor weakness, which was less severe than sensory findings, was present. There was no sphincter disturbance nor orthostatic hypotension. Routine laboratory findings were within normal limits. The protein level of cerebrospinal fluid was slightly increased. The electrophysiological studies and histological evaluation of the biopsied peroneal muscle and sural nerve showed degeneration and reduction of large myelinated fibers of sensory nerves with minimal motor nerve changes. Histological study also revealed the increase of regenerating fibers. Squamous cell carcinoma of epipharynx with metastasis of neck lymph nodes was found. Radiotherapy without chemotherapy resulted in a complete tumor remission and probably suppressed the progression of neurological disturbances. After two years from the onset, diffuse high intensity area was noted in the posterior spinal column on T2 weighted MRI.(ABSTRACT TRUNCATED AT 250 WORDS)

High-risk groups and screening strategies for early detection of hepatocellular carcinoma in patients with chronic liver disease.

Characteristics of high-risk groups for hepatocellular carcinoma (HCC) in Japan were studied to establish screening strategies for early detection of the tumor. Some 93% of patients with HCC were associated with chronic liver disease. On the other hand, 67% of patients with liver cirrhosis had HCC at autopsy. Most were related to current hepatitis virus infection. An analysis of risk factors among 120 patients with chronic hepatitis revealed that age and histological findings were independent risk factors, while HBsAg, anti-HCV, sex, history of heavy drinking, history of blood transfusion were not independent risk factors. Multivariate analysis of 239 patients with liver cirrhosis demonstrated that age, positivity for HBsAg and/or anti-HCV, family history of liver disease, hepatic reserve, and a history of radical resection were independent factors related to the development of HCC. A screening schedule for cirrhotic patients was established in accordance with these results; ultrasonography was done every three months, and tumor markers measured every two months. The screening strategy proved to be effective for the early detection of HCC and improvement of the prognosis.

[A case of mesangioproliferative glomerulonephritis (GN) associated with unique lesions of juxtaglomerular apparatus (JGA) and interstitium].

A case of mesangioproliferative glomerulonephritis (GN) associated with unique lesions of the juxtaglomerular apparatus (JGA) and interstitium is discussed. A 31-year-old Japanese woman who developed eyelid and pretibial edema with nephrotic-range proteinuria (4.8 g/day) and without hematuria, was admitted. Her proteinuria and edema quickly disappeared within 7 days after admission without treatment. Her blood examinations revealed hypocomplementemia on admission, but complement recovered to normal levels after 4 weeks. A renal biopsy specimen obtained on the 5th day of admission revealed moderate mesangioproliferative GN with marked periarteriolar inflammatory cell infiltrations in the JGA and occasionally in the tubular interstitium. Depositions of IgG, IgA, IgM and C3 were observed in the glomerular mesangial regions and some capillary walls, but not in the extraglomerular areas. Titers of GN-related viral antigens were not increased. Although the renal histology of this case was similar to that of experimental acute cytomegalovirus (CMV) GN in mice (described by Smith, R.D.), we could not detect CMV antigen by indirect immunofluorescent method or the virus-like particles by electron microscopy. Clinical cases of nephropathy combining lesions of the glomerulus, JGA, and interstitium are very rare. We herein report a patient with mesangioproliferative GN, who underwent an acute clinical course associated with unique inflammatory lesions of the JGA and/or interstitium.

[Pathology of disseminated intravascular coagulation].

We reviewed the histopathologic characteristics of DIC in autopsy cases. Typical hyaline microthrombi preferentially occurred in renal glomeruli, pulmonary microvasculature, splenic sinuses, adrenocortical capillaries and others, occasionally associated with degenerative or necrotic changes of parenchymal cells of respective organs partly due to ischemic effects of microthrombi and thromboemboli. Investigating the pathogenesis of microthrombi in hepatic sinusoids of rats intravenously injected with LPS (5 mg/kg B.W.) using double immunohistochemical reactions for LPS and fibrinogen, and electron microscopic observations, fibrin thrombi were largely formed around small necrotic foci of hepatocytes 1 hr after injection, which occurred in the very close vicinity to Kupffer cells phagocytizing LPS, and on the cytoplasmic surface of swollen Kupffer cells lading LPS 3 hr after injection. Neutrophils always aggregated in the necrotic foci. Thus, activated Kupffer cells by LPS seemed to play a central role in the development of fibrin thrombi in hepatic sinusoids of endotoxemic rats, through the activation of coagulation system probably via the expression of tissue factor by activated Kupffer cells.

[Changes in the intermediate stage of retinal degeneration after intravitreal injection of ornithine].

We reported previously that intravitreal injection of a small amount of 1-ornithine hydrochloride in monkey eyes damages the retinal pigment epithelium (RPE), selectively. At one year after administration, sharply demarcated chorioretinal atrophy was seen. This paper describes the intermediate stage of retinal degeneration caused by ornithine. Two weeks following intravitreal injection of ornithine, RPE cells at the equator became necrotic and disappeared. However RPE regenerated to cover Bruch's membrane at the border of RPE degenerated area. In 2 months at an area where RPE cells disappeared, the photoreceptors and choriocapillaris degenerated to disappear. However, in the area in where RPE regenerated, they maintained their structure. This report demonstrated that damage of RPE resulted in disappearance of the photoreceptor and choriocapillaris.

[Adverse effect of prolonged formalin fixation on DNA ploidy in paraffin-embedded tissue].

From retrospective analysis of 174 formalin fixed, paraffin-embedded, HCC tissue samples, the possible correlation was investigated between the CV determined by FCM and the duration of formalin fixation. An adverse effect of prolonged formalin fixation on CV and fluorescence intensity in liver tissue was investigated prospectively in samples from 4 patients.
