Experience in posterior lumbar interbody fusion: unicortical versus bicortical autologous grafts.

Over the past seven years, 96 PLIF operations were performed with the autologous bone graft method. In 44 cases, unicortical bone grafts were obtained from the posterior iliac crest area. In the remaining 52 cases, bicortical bone grafts were obtained from the anterior iliac crest. The failure rate in the patients treated by interbody fusion with the unicortical posterior iliac crest graft was 11.8%. The failure rate in patients treated by interbody fusion with bicortical bone grafts was only 1.9%. The patients were ambulatory three to four days after the operation. In cases of spondylolysis and spondylolisthesis, interbody fusion should be coupled with some manner of posterior internal fixation. The extraction of two pieces of bicortical bone from the anterior iliac crest, measuring 1.2 cm or 1.3 cm by 2.5 cm, may cause weakness of the pelvic bone. Therefore, the bicortical grafting in PLIF should be limited to severely disabled patients with complete spinal canal stenosis or very large myelographic defects.

[Studies on incidence and characterization of antibodies to zona pellucida (Z.P.) in human sera].

Incidence and characterization of antibodies to zona pellucida (Z.P.) in human sera were studied and the following results were obtained. Some human sera showed positive immunofluorescent staining of porcine Z.P. after absorption with porcine red blood cells, liver and kidney. The incidence of immunofluorescence positive sera in infertile women was not significantly different from pregnant women and healthy men after absorption with porcine red blood cells, liver and kidney. Among sixteen sera with positive staining of porcine Z.P. after absorption, only two showed positive staining of human oocytes. The gammaglobulin fraction from the above two sera also stained human Z.P. but neither precipitated on the surface of Z.P. nor blocked human spermatozoa to penetrate into Z.P. of human oocytes. There was no correlation between the incidences of antibody to porcine Z.P. and of sperm immobilizing antibody in human sera. Some human sera blocked monoclonal antibodies to porcine Z.P. with different specificities to bind the solubilized porcine Z.P. antigens in an enzyme linked immunosorbent assay. The incidences of the positive sera were not significantly different among the groups of infertile women, pregnant women and healthy men. These results clearly indicate that anti Z.P. antibodies detected by the immunofluorescent staining of porcine Z.P. had no correlation with infertility. The presence of such antibodies, even in the sera of men, suggests that they might be the antibodies produced to some foreign antigens cross-reactive to porcine Z. P. but not autoantibodies produced to human Z.P.

[A case of metastatic renal cell carcinoma with complete remission achieved by interferon therapy].

A case of metastatic renal cell carcinoma in which complete remission was achieved using Interferon therapy is reported. A 67-year-old male was found to have extensive paraaortic lymph node metastasis while undergoing left nephrectomy following diagnosis of 1-renal cancer. Pathological diagnosis of the node was metastasis of clear cell type renal cell carcinoma. Interferon (Hu IFN-alpha 2: Sch 30500; Yamanouchi Pharmaceutical Co.) therapy was started 34 days after surgery. The Interferon was administered i.m. at a dose of 1 X 10(6) units per injection, 3 times a week, and was continued up to a total dose of 236 X 10(6) units. Response of the metastatic lymph nodes was examined using CT scan. Marked response was observed 4 weeks after starting the Interferon therapy, and complete disappearance of the tumors was obtained 5 months after Commencement of therapy. The duration of complete response has lasted 4 months. The toxic manifestations of the Interferon therapy were fever and loss of appetite.

[A clinical survey of 7 cases with primary ureteral tumor: experiences in a year].

Seven cases of primary ureteral tumors treated at Nagaoka Koseiren Chuo General Hospital in 1983 were reviewed retrospectively. The incidence of primary ureteral tumors among the out-patients in the urologic clinic of the hospital was 0.22% and the incidence among the in-patients was 1.65%. The patient's age ranged from 45 to 83 years (average: 59.14 years old). The ratio of male to female was 2.5: 1.0. The left ureter and the lower third of the ureter were involved more frequently than the others. The most common symptom was colicky flank pain (57% of the cases), which was followed by macroscopic hematuria. On IVP, 5 of the 7 cases showed a non-functioning kidney or hydronephrosis, but the others showed filling defects in the ureteral lumen without hydronephrosis. Diagnostic accuracy of CT was 14.3%. Diagnostic accuracy of urine cytology was 42.9% and the false negative rate was the same. Six of the 7 cases underwent total nephroureterectomy with bladder cuff excision. The higher the grade and stage of the tumor, the poorer the prognosis tended to be.

[Penetrating keratoplasty combined with extracapsular lens extraction].

Penetrating keratoplasty combined lens extraction was employed in 4 cases at the Department of Ophthalmology Hospital, University of Occupational and Environmental Health from January 1981 through May 1983. The visual acuity of the treated eye in all of the four cases has improved from before surgery. The operative procedure employed in our cases was very effective in preventing complications that might occur during and after surgery. We performed the extracapsular lens extraction by using the open sky method and A/I tip under a surgical microscope without difficulty. We concluded that the keratoplasty combined with extracapsular lens extraction can be safely and reliably performed by employing the above method and that it is a very effective and useful operation in cases of which visual impairment is due to corneal opacity and cataracts.

Antiallergic agents. 3. N-(1H-tetrazol-5-yl)-2-pyridinecarboxamides.

A series of N-tetrazolylpyridinecarboxamides was prepared and evaluated for antiallergic activity by the passive cutaneous anaphylaxis (PCA) assay. From the structure-activity relationships (SAR) of this class of compounds, it was revealed that the N-tetrazolylcarbamoyl group as an acidic functionality is required to be at the 2-position of the pyridine nucleus and that the phenyl group as a subtituent is not necessarily required for activity. 6-Methyl-N-(1H-tetrazol-5-yl)-2-pyridinecarboxamide (36) showed good oral activity and low toxicity.

Effects of polyamine hydrochlorides on dephosphorylation of phosphorylated H2B histone integrated into nucleosome core particles.

The rate of dephosphorylation of isolated P-H2B histone by pig heart phosphoprotein phosphatase (Mr = 224,000) and its catalytic subunit (Mr = 31,000) was suppressed more than 95% at low ionic strength when the substrate was integrated into nucleosome core particles. The suppressed rate was increased 13-43-fold by polyamine hydrochlorides and Mg(CH3COO)2 at the optimal ionic strength of 0.2-0.38 M. Phosphatase activity toward integrated P-H2B histone was distributed in both rat liver cytosol and nuclear extracts. The phosphatase activities in these fractions showed similar specific activities and were also increased 10-32-fold by 10 mM spermine.4HCl.

Insulin resistance in an infant with leprechaunism.

A Japanese female infant with typical features of leprechaunism exhibited glucose intolerance despite marked hyperinsulinemia, indicating insulin resistance. Circulating insulin appeared to be chemically and biologically normal. There was no evidence of antagonism of insulin action or circulating antibodies to insulin or insulin receptors. Insulin receptor assay of her erythrocytes showed very low affinity despite a six-fold increase in the number of receptors as compared with those of normal adult controls. Our studies indicate that insulin resistance in this patient appears to be due to a qualitative abnormality of insulin receptors or to a postreceptor defect.

Drug absorption from the gastrointestinal tract and immunity: the mechanism of the decreased absorption of salicylic acid during systemic anaphylaxis. I.

Previous studies have demonstrated the decrease of intestinal salicylic acid absorption in ovalbumin-immunized rats during systemic anaphylaxis. In the present study, the mechanism whereby systemic anaphylaxis interferes with the intestinal absorption of salicylic acid was studied. The pH of the luminal solution was not affected by the intravenous challenge with ovalbumin. A significant increase of the intraluminal protein was observed in rats under systemic anaphylaxis. However, there was no significant difference between ovalbumin-treated rats and saline-treated ones on the binding of salicylic acid with intraluminal macromolecular substances. Enhanced mucus release in the perfusate was also observed in sensitized rats but the extent of decrease in absorption of salicylic acid did not correlate with the increase in amount of the intraluminal mucus in the same animals. In addition, no significant effect was observed on the uptake by the intestinal everted sac of rats with systemic anaphylaxis. These findings suggested that mucus as well as protein is not responsible for the decrease of absorption of salicylic acid induced by systemic anaphylaxis. From these observations, it would appear that the circulatory changes in the gastrointestinal tract may play an important role in the decreased absorption of salicylic acid during systemic anaphylaxis.

Drug absorption from the gastrointestinal tract and immunity: the mechanism of the decreased absorption of salicylic acid during systemic anaphylaxis. II.

Rats were intraperitoneally immunized with ovalbumin (egg albumin) with incomplete Freund's adjuvant, and the effect of intravenous challenge with ovalbumin on the intestinal blood flow was measured by means of hydrogen clearance method. The intestinal blood flow was significantly reduced by the antigen challenge in ovalbumin-immunized rats compared to the non-immunized rats. However, no significant change was observed on the intestinal blood flow in rats without challenge of the antigen. Moreover, the blood flow reduction was not found in ovalbumin-immunized rats challenged with bovine gamma-globulin. The effect was maintained for at least 16 weeks after the third immunization, but the reduced blood flow was gradually restored to the control level. The decrease in both blood flow and absorption of salicylic acid was recovered by nearly 70% of the control level when high dose of theophylline or caffeine was administered intravenously. In the case of gastric absorption during systemic anaphylaxis, similar results were also obtained by means of in situ loop technique. The decreased absorption of salicylic acid from the rat stomach also correlate with the reduced gastric blood flow. These findings suggest that the decreased absorption of salicylic acid from the gastrointestinal tract might be affected by the reduced blood flow during systemic anaphylaxis.

A case of chronic pancreatitis successfully treated by endoscopic removal of protein plugs.

A 56 years old male with chronic pancreatitis complained of intractable abdominal pain, anorexia, emaciation and peripheral edema. Medical treatment initiated only partial improvement in the general condition and hypoproteinemia. Endoscopic retrograde cholangiopancreatography revealed multiple filling defects in the dilated main pancreatic duct. Endoscopic aspiration of pure pancreatic juice yielded numerous protein plugs. The endoscopic removal of protein plugs from the pancreatic duct resulted in remarkable improvement in symptoms, laboratory findings and ERCP findings. We consider this procedure to be an important new treatment of chronic pancreatitis.

Changes in histone H1 composition during differentiation of mouse myeloid leukemia cells.

The possible change due to dexamethasone treatment in the relative amounts of two major subtypes of H1 histone, H1A and H1B, in mouse myeloid leukemia cells (M1 cells) during differentiation was examined. The relative amount of H1B decreased after dexamethasone treatment and reached about one-tenth that of H1A at the fully differentiated stage. These results contrast with the results reported for normal fibroblast cells and their neoplastic counterparts. Additional findings are that phosphorylation of H1 occurs almost exclusively in the H1B subtype and fluctuates following dexamethasone treatment.

[Prognosis of long-term survivors of oral cancer treated with radiation].

The prognosis after long-term survival was analyzed for 645 patients with oral squamous cell carcinoma treated primarily with radiotherapy. In contrast to the fact that the main cause of death within five years was related to the primary disease (92%), the estimated causes of death after five years were related to the primary disease in 22% and to other malignant tumors in 15%; this was particularly so in the middle age group (29%). As for the patients who showed local recurrence after long periods the possibility of radiation-induced cancer might be considered; however, it was difficult to distinguish radiation-induced cancer from late recurrence.
