[Aging and exocrine pancreatic function].

To evaluate the effect of aging on exocrine pancreatic function, fecal chymotrypsin activity (FCA) was measured by a photometric method in 62 healthy controls (20 to 87 years old, average 51.0 years), 42 patients with non-pancreatic diseases (31 to 83 years old, average 56.1 years), 40 controls in an old-age home (63 to 92 years old, average 77.6 years), 20 patients with definite chronic pancreatitis (17 to 72 years old, average 53.5 years) and five patients with pancreatic cancer (60 to 76 years old, average 65.4 years). Exocrine pancreatic function showed a significant decrease with aging as indicated by: (a) a significant inverse correlation between aging and FCA in the 62 healthy controls (r = -0.56, p less than 0.001), in the 42 patients with non-pancreatic diseases (r = -0.59, p less than 0.001), and also in the 40 controls in an old-age home (r = -0.52, p less than 0.001); and (b) a significantly lower FCA in the 21 healthy controls aged 65 or more (designated as the B group of healthy controls) than the 41 healthy controls aged less than 65 (designed as the A group of healthy controls). The 40 controls in an old-age home showed significantly lower FCA than the B group of healthy controls. This result was ascribed to the fact that the former group consisted of significantly older subjects than the latter. No significant difference was found in FCA between patients with chronic pancreatitis, those with pancreatic cancer, the B group of healthy controls, and the controls in an old-age home.(ABSTRACT TRUNCATED AT 250 WORDS)

[Changes in intrahepatic localization of hepatitis B core antigen and histological activity in chronic active hepatitis B before and after interferon treatment].

The intrahepatic distribution of HBcAg and evaluation of the histological outcome using a numerical scoring system of Histological Activity Index (HAI) by Knodell et al, were studied in paired liver biopsy specimens from 14 HBeAg-positive patients with chronic active hepatitis B before and after interferon (IFN) treatment. Intrahepatic HBcAg was detected by immunoperoxidase technique and the pattern of HBcAg expression were present in the hepatocyte nucleus (nHBcAg) or cytoplasm (cHBcAg) or in both. Before treatment, intrahepatic HBcAg was distributed both in the nucleus and cytoplasm without significant differences. After treatment, nHBcAg expression however, decreased significantly (p less than 0.001), while cHBcAg remained unchanged or rather increased. These changes of nHBcAg and cHBcAg before and after IFN treatment were observed irrespective of disappearance or presence of DNA-P and/or HBeAg in serum after IFN treatment. The HIA scores decreased significantly (0.05 less than P less than 0.001) after treatment. These findings above suggest that the intrahepatic HBcAg may shift from nucleus to cytoplasm in chronic active hepatitis B during IFN treatment.

Innervation of mouse molars during the early states of tooth germ development.

The topography of nerves and first molar tooth germs in 11-14-day embryos was studied using silver-impregnated serial sections. Nerve fibers growing toward the developing tooth germ became visible on the 12th day, while the first sign of molar tooth differentiation was found as a thickening of the oral epithelium in 11-day embryos. From the 12th to 13th day the nerve fibers spread, forming a plexus close to the base of the tooth bud, and on the 14th day some entered into the dental follicle of the tooth germ at the early cap stage. However, no nerve fiber was found growing into the dental papilla during the observation period. The observations showed that the earliest nerve fibers running toward the tooth forming area appeared after the histogenesis of the tooth germ started, and the timing and pattern of the innervation of the tooth germs revealed that tooth germs are a useful model for investigating the mechanism of nerve growth into developing peripheral organs.

[Clinical and histological features of acute post-transfusion non-A, non-B hepatitis: histological diagnosis in acute stage and clinical outcome].

We have retrospectively studied the clinical and histological features of 55 cases of acute post-transfusion non-A, non-B hepatitis. Histological findings in the liver biopsies taken in the acute stage covered the whole spectrum of acute and chronic hepatitis, the incidence of acute hepatitis or non-specific reactive hepatitis was 12.7% and 16/4%, respectively, while that of chronic active hepatitis was 56.4%. There was no correlation between histological features in acute stage and the mean peak levels in serum alanine aminotransferase (ALT). Post-transfusion hepatitis, non-A, non-B with histological signs of chronic hepatitis in acute stage frequently results in biochemical evidence of chronic liver disease during long-term follow-up periods. Therefore, our results suggest that the histological features of chronic hepatitis in the acute stage are indicative of long-term elevation in serum ALT level.

Induction of asparagine synthetase during lymphocyte activation by phytohemagglutinin.

Asparagine synthetase was increased in cultured mouse spleen lymphocytes after stimulation by phytohemagglutinin. After a lag period of about 24h, the enzyme activity level rose sharply by 48h, reached its maximum at 72h, and decreased thereafter. The time course of the change in the enzyme activity was similar to that of the change in the rate of DNA synthesis. From the results that there was no increase of the activity of asparagine synthetase at the time induction of ornithine decarboxylase would occur (6h), it seems unlikely that asparagine synthesized in the cells contributes to the enhancement of ornithine decarboxylase during the activation of lymphocytes. The increase of asparagine synthetase activity was inhibited by cycloheximide and somewhat by actinomycin D, suggesting de novo enzyme synthesis during the stimulation.

The effects of ciclosporin on the renal histopathological aspects in (NZB x NZW)F1 mice.

Ciclosporin A (CYA) is a newly developed immunosuppressant and has a sparing effect on suppressor cell regulatory mechanism. However, accumulation of experimental animal studies has not been enough for the administration of CYA to human autoimmune diseases such as lupus nephritis. In this study, the histopathological aspects of the kidney in (NZB x NZW)F1 mice after administration of CYA were analysed. A dose of 25 mg/kg/day of CYA was injected intraperitoneally so that serum level of CYA was controlled from 250 to 300 ng/ml with this dosage. Typical histopathological changes of lupus nephritis including wire-loop lesions were observed in the kidney of the non-treated control group. In contrast, only few lesions were demonstrated in the CYA-treated group. These results suggest that CYA prevented the renal deterioration in lupus mice.

[Determination of seven ultraviolet absorbent in cosmetics by high speed liquid chromatography].

A rapid method for the HPLC determination of ultraviolet absorbent (UV abs.) in cosmetics was investigated. This method is based on the technique of solvent extraction of the sample by HPLC with silica-ODS (Develosil ODS-5) as the stationary phase A, B, C, and D as the eluent and the ultraviolet spectrophometer (310, 284 nm) as the detector. The plots of the peak height vs amount of seven UV abs. were linear between 0-20 micrograms/ml. The recoveries of UV abs. to a model sample at the level of 0.1% were 93.2-100.0% by this method. This method is simple and rapid, therefor it is applicable for the determination of UV abs. in cosmetics in routine analysis.

Cardiovascular effects of the new cardiotonic agent 1,2-dihydro-6-methyl-2-oxo-5-(imidazo[1,2-a]pyridin-6-yl)-3-pyridine carbonitrile hydrochloride monohydrate. 2nd communication: studies in dogs.

The cardiovascular effects of 1,2-dihydro-6-methyl-2-oxo-5-(imidazo[1,2-a]pyridin-6-yl)-3-pyridine carbonitrile hydrochloride monohydrate (E-1020), a new nonglycoside, noncatechol cardiotonic agent, were investigated in dogs. In anesthetized dogs, E-1020 (10-100 micrograms/kg i.v.) dose-relatedly increased cardiac contractility (LV dP/dtmax), enhanced cardiac index and decreased systemic vascular resistance accompanying relatively small reduction in mean aortic pressure and a mild increase in heart rate. Coronary and femoral arterial blood flow were increased by either systemic intravenous or topical administration of E-1020. The degree of increase in myocardial oxygen consumption was only slight (10% at 30 micrograms/kg i.v.). The inotropic effect of E-1020 was not markedly affected by pretreatment with beta-adrenoceptor blockade, reserpine or other cardiotonic agents such as dobutamine or ouabain. In two experimental heart failure models induced by an excessive dose of propranolol or by coronary occlusion following volume-loading, E-1020 (30 micrograms/kg i.v.) rapidly reversed the cardiac depression. In chronically instrumented conscious dogs, E-1020 (30-100 micrograms/kg i.v. or 0.3-10 mg/kg p.o.) produced dose-dependent increases in LV dP/dtmax with minor increases in heart rate. E-1020 did not exacerbate arrhythmias of several experimental models in anesthetized dogs even at high dose of 100 micrograms/kg i.v. These results indicate that E-1020 is an intravenously and orally effective cardiotonic agent with vasodilating property, and that it may be beneficial in the treatment of acute and chronic congestive heart failure.

[Clinical evaluation of various tumor markers in pleural effusion and in the serum].

Various tumor markers and enzymes in the pleural effusion and serum have been measured in 47 patients with carcinoma and in 43 patients with benign disease, by means of a radioimmunoassay and biochemical methods. CEA in the pleural effusion showed a high specificity and sensitivity for the detection of the malignancy. In patients with a lung cancer, measurement of the NSE in the serum was more useful than in the pleural effusion. Further, both CA19-9 and SCC in the pleural effusion showed a high specificity in a differential diagnosis of cancer and benign diseases. On the other hand, CA 125, TPA and IAP in the pleural effusion and in the serum showed a high sensitivity, but a low specificity for diagnosing the malignancy. The levels of ADA were significantly higher in tuberculous pleural effusions than in carcinomatous effusions. Therefore, this suggests that measurement of the various tumor markers in both the pleural effusion and in the serum is useful in achieving a differential diagnosis of malignant and benign diseases.

[Quantification of severity and redistribution of ischemic and infarcted myocardium by Thallium-201 polar map: evaluation in patients with single vessel disease].

Quantitative analysis of the polar map obtained from T1-201 single photon emission computed tomogram was employed to assess regional myocardial viability in 66 patients with single vessel coronary artery disease and 16 control subjects. Counts of region of interest in the stenotic and infarcted lesions and normal reference areas were calculated on the polar map. Severity of hypoperfused myocardium was determined as initial percent uptake (%Ui) and delayed percent uptake (%Ud). Redistribution was analyzed as a ratio of %Ud to %Ui (%Ud/%Ui). In the control group, the average %Ui was 85.8 +/- 9.4 and %Ud/%Ui was 1.03 +/- 0.04. Twenty five with reversible defect showed reduced %Ui (51.5 +/- 13.3) and increased %Ud/%Ui (1.37 +/- 0.26). Forty one cases with non-reversible defect were divided into two groups according to the value of %Ui. In the first group with a high %Ui (n = 17), the average %Ui was 74.5 +/- 4.7 and %Ud/Ui was in normal range (1.06 +/- 0.06). In the second group (n = 24), %Ui was most reduced (31.7 +/- 8.7), although %Ud/%Ui was in normal range (1.05 +/- 0.17). No significant correlation was shown between %Ui and %Ud/%Ui in the two groups with non-reversible defect. Correlation between %Ui and %Ud/%Ui was high (r = -0.76) in the group of reversible defect and the group of non-reversible defect with minimal reduced %Ui. It is concluded that the quantitative criteria obtained from polar map is valid to assess regional myocardial viability. The quantitative imaging data were also compared with coronary angiographic and left ventriculographic data.

[A very rare autopsy case of metastatic brain tumor originating from ureteral cancer].

We report here a very rare case of metastatic brain tumor originating from ureteral cancer which was verified at autopsy. An 88-year-old elderly woman was admitted to our department because of recurrent convulsions which started in March, 1987. Following this episode, right hemiparesis and speech disturbance appeared on June 6, 1987. Neurological examinations at admission on August 12, 1987 showed slightly disturbed consciousness, motor dominant aphasia, and dysphasia. Enhanced CT scan demonstrated a large tumor in the left frontal lobe with perifocal edema, and a small mass in the right frontal lobe (Fig. 1). Left carotid angiogram showed downward displacement of the middle cerebral artery complex at the Sylvian portion. After admission, she developed marked distention of the abdomen, with occurrence of hematuria followed by aggravation of respiration and consciousness. She died on September 2, 1987. Removal of metastatic brain tumor had not been performed due to the poor general condition of the patient. An autopsy was permitted. A coronal section of the cerebrum clearly demonstrated a large tumor in the left frontal lobe with small mass in the right frontal lobe (Fig. 2). Kidney and the ureter at the right side showed hydronephrosis and hydroureter because of the presence of ipsilateral ureteral tumor at the distal end (Fig. 3). Microscopical findings verified that the metastatic brain tumor was a transitional cell carcinoma, which was, histologically, completely the same as the ureteral tumor (Fig. 4, 5). No case of metastatic brain tumor originating from ureteral cancer has yet been reported, as far as the authors know.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of marked exercise-induced ST segment depression in anterior precordial leads with inferoposterior left ventricular aneurysm and significant stenosis of left anterior descending artery].

A 71-year-old man was admitted to a community hospital and diagnosed as having acute infero-posterior myocardial infarction. Severe ST segment depression occurred in anterior precordial leads with only 50 m walking, but he had no chest pain. He was then admitted to our hospital. Left ventriculography showed infero-posterior dyskinesis and aneurysm formation. Coronary angiography revealed triple-vessel disease including 90% stenosis at distal site of left anterior descending artery. Exercise thallium-201 myocardial scintigraphy by ergometer showed no filling defect in the anterior segment, though severe ST depression appeared in anterior precordial ECG leads. Infero-posterior segment showed persistent defect. We performed intravenous digital ventriculography at rest and during atrial pacing. Anterior wall motion during pacing was shown to be normal by amplitude and phase analysis. At the same time, the motion of the inferior wall was seen as abnormal and ST segment depression on anterior precordial leads appeared. We considered that in this case the anterior ST depression did not mean anterior myocardial ischemia but might be due to dyskinetic movement of the infero-posterior aneurysm.

Generation of hypophosphatemia in rats by continuous oral administration of cadmium.

To obtain further information on the negative calcium balance caused by cadmium (Cd), the factors associated with serum calcium and phosphorus homeostasis other than inhibition of intestinal calcium absorption were studied by using parathyroid hormone (PTH) and 1 alpha-hydroxycholecalciferol (1 alpha-OH-D3). In rats exposed to Cd for 30 or 90 days, the concentrations of serum calcium after treatment with PTH, parathyroidectomy (PTX) or 1 alpha-OH-D3 showed almost the same patterns as those of control animals. It was considered that the mechanism of regulation of calcium in Cd-exposed rats was normal. The continuous oral administration of Cd generated hypophosphatemia in rats. On the other hand, in 30- and 90-day-treated rats, the low concentration of serum phosphorus caused by Cd was further decreased by administration of PTH, whereas it was increased by PTX. The hypophosphatemia found in rats exposed to Cd for 30 days, but not for 90 days, was reversed by treatment with 1 alpha-OH-D3. From these results, it was concluded that the hypophosphatemia caused by long term oral administration of Cd resulted from secondary hyperparathyroidism, due to inhibited calcium absorption from the intestine, as was demonstrated previously.
