Pancreatic carcinoma associated with chronic pancreatitis.

The incidence of invasive ductal carcinoma of the pancreas was 3.1% (6 cases) in 196 patients with definite chronic pancreatitis. Five patients (3 men and 2 women) had calcific pancreatitis and 1 patient (man) had non-calcific pancreatitis. Large pancreatic stones were recognized in 2 women. Most of the patients complained of continuous intractable abdominal pain and/or back pain together with weight loss and appetite loss. Serum CA19-9 levels and exacerbation of glucose intolerance were retrospectively noted to have been elevated in 1 patient. However, it was difficult to obtain a definitive diagnosis by imaging examinations earlier, due to the presence of chronic pancreatitis. Median survival of the 6 patients was 6.5 months from admission.

Molecular cloning and expression of the SF-1/Ad4BP gene in the frog, Rana rugosa.

SF-1/Ad4BP is a transcriptional factor that was originally found to be a mammalian homologue of the Drosophila Ftz-F1 (fushi tarazu factor 1) (), and transcribed from a gene designated the Ftz-F1 gene (). Ftz-F1 gene-deficient mice lack adrenal glands and gonads. Besides mammals, however, the SF-1/Ad4BP cDNA has only been isolated to date in fish and birds. To understand its role(s) for adrenal and gonadal development in vertebrates, cloning of this gene in animals other than mammals is required. In this study, we succeeded to isolate frog (Rana rugosa) SF-1/Ad4BP cDNA from a testis lambdagt10 cDNA library. It encoded a protein of 468 amino acids, and its open reading frame (ORF) shared 70% similarity with that of chicken OR2.1 (a SF-1/Ad4BP homologue) and 62% with bovine SF-1/Ad4BP. SF-1/Ad4BP mRNA was expressed in the testes, brains, adrenals/kidneys and spleens, but not ovaries, of adult frogs. In addition, we also cloned the 5'-untranslated region (4.6kb) of the SF-1/Ad4BP gene with exons I and II. Genomic structure analysis revealed that frog SF-1/Ad4BP was also transcribed from the same gene as that of mammals. However, many Ftz-F1-related proteins have been reported so far. The Ftz-F1 gene does not encode all of those Ftz-F1-related proteins. Thus, the name of Ftz-F1 is not adequate for the gene coding SF-1/Ad4BP. Here, we propose the use of SF-1/Ad4BP instead of Ftz-F1 for the gene that encodes SF-1/Ad4BP in vertebrates.

Induction of Smad6 mRNA by bone morphogenetic proteins.

Members of the transforming growth factor-beta (TGF-beta) superfamily transduce signals via Smad proteins. Smad2 and Smad3 mediate TGF-beta signaling, whereas Smad1 and Smad5 transduce bone morphogenetic protein (BMP) signals. Smad4 is a common mediator required for both pathways. Smad6 and Smad7 are recently identified members in the Smad family; they inhibit the signaling activity of the other Smad proteins. Here we show that expression of the Smad6 mRNA is dramatically induced by BMP-2 or osteogenic protein-1 (OP-1)/BMP-7 in various cells. BMP-2 induced expression of Smad7 in one cell type, although much less potently than that of Smad6. Smad6 message was induced by TGF-beta 1 in TGF-beta 1-responsive Mv1Lu cells, but the induction was transient in contrast to the induction by BMPs. These results indicate that Smad6 may form a feedback loop to regulate the signaling activity of BMPs.

Differences in genetic backgrounds affecting gonadal differentiation between two local populations of the Japanese wrinkled frog (Rana rugosa).

Gonadal differentiation in two local populations - Hamakita and Hiroshima - of the Japanese wrinkled frog (Rana rugosa) was examined from hatching to the end of metamorphosis. The Hamakita male has X and Y chromosomes distinguishable morphologically from each other, while X and Y chromosomes in the Hiroshima male cannot be distinguished morphologically. In the two populations, primordial gonads differentiated into testes or ovaries by stage I of Taylor and Kollros, and larvae at stage XXV - the end of metamorphosis - possessed definitive testes or ovaries. However, immature testes at stages II-III in the Hiroshima population showed a higher incidence of testes with ovarian characteristics - meiotic figures and gonadal cavities - than those in the Hamakita population. In addition, proliferative activities in the testicular cells of the immature testes in the Hiroshima population were very low. On the other hand, there were no remarkable differences in the histological processes of ovary development between the two populations. Intraspecific hybridization between females of the Hamakita population and males of the Hiroshima population retarded testis differentiation after stage XV, while the reciprocal hybridization showed normal gonadal sex differentiation and development. These observations suggest that R. rugosa, possessing heteromorphic sex chromosomes in the male, establishes firm gonochorism, where the feminization involves the suppression of testis differentiation, and that masculinization is performed by resistance to this suppression as well as active proliferation of testicular cells.

Effects of breathing pathways on tracheal sound spectral features.

The spectra of sounds recorded over the trachea of adults typically reveal peaks near 700 and 1500 Hz. We assessed the anatomical determinants of these peaks and the conditions contributing to their presence. We studied five adult subjects with normal lung function, measuring sounds at the suprasternal notch and on the right cheek. The subjects breathed at target airflows of 15 and at 30 ml sec(-1) kg(-1) both through the mouth with nose clips and then through the mouth and nose using a cushioned face mask. The mouth breathing maneuvers were performed with three lengths (3.6, 21.1 and 38.6 cm) of 2.6 cm diameter tubing between the mouth and the pneumotachograph. The nose breathing maneuver was performed with the longest tube (between the mask and pneumotachograph). The signals occurring at the target flows +/- 20% were used to create averaged, spectral estimates. We found that all subjects had two predominant spectral peaks; a approximately 700 Hz peak loudest over the cheek and a approximately 1500 Hz peak loudest over the trachea. The frequency of both peaks negatively correlated with body height (and presumably, airway length). There was no systematic effect of breathing phase, flow rate or length of the tube connecting the mouth to the pneumotachograph on the spectral peaks. Breathing into the mask and breathing through the nose did markedly alter the spectra. We conclude that the higher tracheal sound peak reflects resonance within the major airways and is relatively independent of extrathoracic influences during mouth breathing through a tube.

Generation of far field potentials from the trigeminal nerve in the cat.

This study provides evidence that far field potentials (FFPs) are generated from the trigeminal nerve in the cat. By stimulating the main mental nerve, three components (component 1, 0.59 +/- 0.06 ms; component 2, 0.81 +/- 0.06 ms; and component 3, 0.98 +/- 0.07 ms) were identified from surface electrodes. These three components were thought to be positive and negative FFPs because each component had a stationary peak and was distributed on the head being divided into positive and negative fields. Results of a study of lesions and recording compound action potentials (CAPs) defined the neural origins of those potentials as follows: component 1, the mandibular nerve at the mandibular foramen; component 2, the mandibular nerve at the foramen ovale; and component 3, somewhere between the gasserian ganglion and the trigeminal root. The amplitude of component 2 decreased when the mandibular nerve at the foramen ovale was immersed in cerebrospinal fluid (CSF) after opening the foramen and recovered to the prior level after closing the foramen with beeswax. We concluded that this transformation resulted in the change in electrical resistance of the volume conductor around the nerve.

Inhibitor and temperature effect on catalase in the liver of adult diploid and haploid Rana rugosa.

The authors succeeded in raising a single mature haploid Rana rugosa female to the age of 2 years from an egg artificially fertilized with ultraviolet-irradiated sperm. In order to discover why this particular haploid individual should survive so long, hydrogen peroxide detoxifying catalase in the liver of this individual and age-matched diploids was examined and compared for total activity, temperature stability, and chemical inhibition. Total activity was found to be significantly higher in the haploid frog than in the diploids, suggesting that this particular haploid had a unique system for hydrogen peroxide detoxification which protected the liver against cell death, preventing hepatic failure, and leading to a prolonged survival. Liver catalase from the haploid proved to be more labile to aminotriazole and urea, losing 60-70% of its original activity after 30 min treatment, whereas diploid catalase lost only 40% under the same conditions. Haploid and diploid catalase responded similarly to heat, however. It seems likely that inhibitor-binding sites differ considerably between the catalase of normal diploids and the catalase of this particular haploid, while overall structure is generally similar.

Long-latency response to transcranial magnetic stimulation in patients with hemifacial spasm.

OBJECTIVE: We studied the long-latency response of the orbicularis oris muscle elicited with transcranial magnetic stimulation in patients with hemifacial spasm (HFS) and evaluated the excitability of the facial nucleus. METHODS: We compared the thresholds on both sides in 8 normal volunteers and 7 patients with hemifacial spasm. The thresholds were determined as the lowest intensity required to produce motor evoked potentials with an amplitude of at least 50 microV in the orbicularis oris muscle. Average values were given as means +/- standard deviation. Wilcoxon's rank sum test was used for comparisons between the sides of normal subjects and of patients with HFS with respect to the threshold stimulus. RESULTS: There was no significant difference between the thresholds on the two sides of the normal subjects (mean 1.88+/-5.30%, P > 0.05). In patients with HFS, there was a significant difference between the thresholds on the spasm side and the normal side (mean 20.7+/-13.0%, P < 0.05) In one patient studied after MVD, the difference between both sides disappeared. CONCLUSION: The difference between the thresholds in patients with HFS and the normalization in threshold after MVD suggested that the mechanism of HFS was hyperexcitability of the facial nucleus.

Interplay of signal mediators of decapentaplegic (Dpp): molecular characterization of mothers against dpp, Medea, and daughters against dpp.

Decapentaplegic (Dpp) plays an essential role in Drosophila development, and analyses of the Dpp signaling pathway have contributed greatly to understanding of the actions of the TGF-beta superfamily. Intracellular signaling of the TGF-beta superfamily is mediated by Smad proteins, which are now grouped into three classes. Two Smads have been identified in Drosophila. Mothers against dpp (Mad) is a pathway-specific Smad, whereas Daughters against dpp (Dad) is an inhibitory Smad genetically shown to antagonize Dpp signaling. Here we report the identification of a common mediator Smad in Drosophila, which is closely related to human Smad4. Mad forms a heteromeric complex with Drosophila Smad4 (Medea) upon phosphorylation by Thick veins (Tkv), a type I receptor for Dpp. Dad stably associates with Tkv and thereby inhibits Tkv-induced Mad phosphorylation. Dad also blocks hetero-oligomerization and nuclear translocation of Mad. We also show that Mad exists as a monomer in the absence of Tkv stimulation. Tkv induces homo-oligomerization of Mad, and Dad inhibits this step. Finally, we propose a model for Dpp signaling by Drosophila Smad proteins.

Transcranial magnetic stimulation of the oculomotor and abducens nerves: determining the site of excitation in the cat.

The authors have attempted to stimulate the feline oculomotor and abducens nerves using a magnetic coil (MC) and to determine the optimal reliable MC position for eliciting motor evoked potentials. The authors have also determined the site of excitation to analyze the validity and potential advantages of this technique as a means of neurophysiologically studying the oculomotor and abducens nerves. The potentials of both of these muscles are evoked by MC placed to be symmetrical to the line connecting the vertex and the center of the eyeball on the side being examined, as the coil center corresponds to the midpoint of this line. The latencies of the motor responses of the superior rectus and lateral rectus were 1.08 +/- 0.22 and 1.02 +/- 0.21 msec, respectively, suggesting that magnetic stimulation excites both the oculomotor and the abducens nerve at the superior orbital fissure.

Sleep-wake rhythm of autistic children.

Using a sleep log for 28 days in the summer vacation, 89 autistic children living in a suburb of Tokyo were examined. Only one girl of 13 years showed a tendency of non-24 h sleep-wake syndrome. Most autistic children showed a large variation of total sleep time. Forty per cent of subjects showed 10% or more on coefficient of variation of total sleep time. In the retiring and rising time, many subjects tended to show late retiring and early rising.

Sleep disorder in children with autism.

Eighty-eight children with autism, living in a suburb of Tokyo, were examined by questionnaire from 21 July to 31 August. Experienced sleep disorders were observed in 56 children; 44 of whom had sleep disorders before 3 years old. The average age when sleep disorders were seen to have stopped was 5 years old. The most common problem was difficulty falling sleep (n=23), followed by frequent awakening during sleep time (n=19), then early morning awakening (n=11). Bed-wetting was observed in 22 children.

Inhaled nitric oxide therapy via nasopharyngeal tube in an infant with end-stage pulmonary hypertension.

The delivery of nitric oxide (NO) via a nasopharyngeal tube is an alternative to endotracheal intubation. A male infant with end-stage pulmonary hypertension (PH) due to a severe hypoplastic lung developed a PH crisis on day 145 and received NO inhalation via a nasopharyngeal tube. Clinical improvement was maintained for 7 days with 18-22 ppm NO inhalation. The patient remained in close physical contact with his parents without the use of sedation. Blood methemoglobin levels remained below 1%. The environmental NO levels were less than 0.06 ppm and NO2 less than 0.3 ppm throughout the treatment, well within the safety margin. On day 152, the patient succumbed to hypoxemia and heart failure. The use of a nasopharyngeal NO delivery system without sedation, as an alternative to endotracheal intubation with sedation, was a practical method in treating a patient with PH while maintaining a certain quality of life for the patient and the family.

Extracellular matrix production regulation by TGF-beta in corneal endothelial cells.

PURPOSE: Production of extracellular matrix (ECM) by corneal endothelial cells is related to physiologic functions and pathologic conditions and is regulated by many cytokines, including transforming growth factor-beta (TGF-beta). In this study, the molecular mechanism of ECM production regulation by TGF-beta was investigated in cultured corneal endothelial cells. METHODS: The production of ECM components (laminin and fibronectin) was detected in cultured corneal endothelial cells by western blot analysis. To determine the signal transduction pathways, mutant TGF-beta type I receptor (TbetaR-I) and/or Smad protein family members (intracellular signal transducers in TGF-beta signaling) were overexpressed by transfecting their cDNA into the cultured cells, and the effects on ECM production were observed. RESULTS: The production of laminin and fibronectin was stimulated by treatment with TGF-beta1 or TGF-beta2. After transient transfection of cDNA of the constitutively active (CA) mutant of TbetaR-I, the production of laminin and fibronectin was stimulated even in the absence of TGF-beta. The transfection of the dominant negative mutant of TbetaR-I counteracted the effects of TGF-beta. These results confirm that TGF-beta directly stimulates ECM production from corneal endothelial cells through TbetaR-I. The ECM production stimulation by TGF-beta or CA TbetaR-I was accelerated by the overexpression of Smad2, Smad3, and/or Smad4 and inhibited by that of Smad7. These results show that TGF-beta signals connected to ECM production are regulated by Smad family members, located downstream of TbetaR-I. CONCLUSIONS: The results of this study show that TGF-beta stimulates ECM production from corneal endothelial cells through TbetaR-I and Smad family transducers.

Smad6 inhibits signalling by the TGF-beta superfamily.

SMAD proteins have been identified as signalling mediators of the TGF-beta superfamily, which is involved in a range of biological activities including cell growth, morphogenesis, development and immune responses. Smad1, Smad2, Smad3 and Smad5 are ligand-specific: Smadl and Smad5 transduce signals from bone morphogenetic proteins, and Smad2 and Smad3 mediate signalling by TGF-beta and activin, whereas Smad4 acts as a common signalling component. For example, Smad2 is phosphorylated by the TGF-beta type I receptor upon ligand binding, forms a heteromer with Smad4, and then translocates into the nucleus where it activates transcription. Here we report the isolation of Smad6 in the mouse. Smad6 is quite different in structure from the other SMAD proteins, and forms stable associations with type I receptors. Smad6 interferes with the phosphorylation of Smad2 and the subsequent heteromerization with Smad4, but does not inhibit the activity of Smad3. Smad6 also inhibits the phosphorylation of Smad1 that is induced by the bone morphogenetic protein type IB receptor. These data indicate that signals of the TGF-beta superfamily are regulated both positively and negatively by members of the SMAD family.

Inhibitory role of prolactin in the downregulation of testicular follicle-stimulating hormone receptors in mice.

Previous studies have shown that follicle-stimulating hormone (FSH) reduced its own receptors (downregulation) in the testis of adult mice. In the present study, we further examined the effect of prolactin (PRL) on the downregulation of testicular FSH receptors using hypophysectomized adult mice. FSH binding per testis increased after hypophysectomy, reaching a peak 5-10 days after the operation, and decreased thereafter. Hypophysectomized mice were given s.c. injections of 100 micrograms ovine PRL (NIADDK-oPRL) and/or 2 micrograms ovine FSH (NIADDK-oFSH) twice daily for 5 or 10 days beginning from 5 or 10 days after hypophysectomy. The 5-day treatment with a combination of oPRL and oFSH maintained the level of testicular FSH binding that was decreased by the treatment with oFSH alone, while the 5-day treatment with oPRL alone did not influence the testicular FSH binding. The change in the FSH binding per Sertoli cell was comparable to that in the binding per testis. A similar result was obtained by the 10-day treatment. Therefore, PRL was suggested to have an inhibitory role in the downregulation of testicular FSH receptors by FSH in adult mice.

A new method for estimating spinal cord function. Refractory period of conductive spinal cord evoked potentials.

STUDY DESIGN: This study was designed to examine the possibility of a new spinal cord monitoring method using measurement of the refractory period to monitor spinal cord function. OBJECTIVES: To determine whether measuring the refractory period and the recovery rate of conductive spinal cord evoked potential is a useful method for estimating spinal cord function. BACKGROUND: Measuring the refractory period and constructing the recovery curve have been used to investigate peripheral nerve function. Spinal cord evoked potential elicited by the single stimulus usually is used to evaluate spinal cord function, and it has been said that 50% attenuation of the amplitude is the critical alarm level. METHODS: In anesthetized cats, amplitude, area, and latency were measured on a personal computer from subtracted data collected with a paired-stimulation technique. The authors constructed recovery curves of ascending and descending conductive spinal cord evoked potentials and measured the refractory period during spinal cord compression. RESULTS: When the amplitude of the ascending spinal cord evoked potential began to decrease during spinal cord compression, the amplitude of the response elicited by the second stimulus with interstimulus intervals of 0.8 msec and 1.0 msec decreased more significantly. When the amplitude of the ascending spinal cord evoked potential decreased to 50% of the precompression amplitude, the mean value of the absolute refractory periods of the ascending and descending spinal cord evoked potentials became prolonged from 0.40 +/- 0.007 msec to 0.53 +/- 0.014 msec, and the mean values of their amplitude and area recovery rates decreased from 75% +/- 1% to 35% +/- 2% (interstimulus interval, 0.8 msec) and from 81% +/- 1% to 46% +/- 2% (insterstimulus interval, 1.0 msec). CONCLUSIONS: The change of the responses elicited by the paired stimuli is more sensitive than those elicited by the single stimulus in the spinal cord evoked potentials. The absolute refractory periods and the recovery rate during 50% attenuation of the precompression amplitude is the critical alarm level in spinal cord monitoring.
