[Studies on the appropriate administration of cisplatin based on pharmacokinetics and toxicity].

Ninety-six patients with non-small cell lung cancer were treated with cisplatin (80-150 mg/m2). The pharmacokinetics of cisplatin were studied in 27 of these patients. Cisplatin was administered intravenously for 30-120 min. Plasma concentrations of total platinum and ultrafilterable (non-protein-bound) platinum were monitored by flameless atomic absorption spectrophotometry. Maximum total platinum levels in plasma were attained at the end of infusion, and thereafter decayed in a biphasic fashion, with an initial phase half-life (t1/2 alpha) of 10.2 to 14.6 min and a secondary phase half-life (t1/2 beta) of 41.7 to 81.3 h. The half-life was prolonged as the dosage was increased. Non-protein-bound platinum was rapidly cleared to below the measurable level within 4 hours at 80 mg/m2 and 120 mg/m2 of cisplatin, but declined in a biphasic manner, with t1/2 alpha of 31.2 min and t1/2 beta of 20.1 h at 150 mg/m2 of cisplatin. Toxicities were increased according to dosage, and decreased with a 50 mg/m2 X 3 days regime. Nephrotoxicity and ototoxicity were the dose-limiting factors in the single-dose escalation scheme used. In conclusion, the optimal dosage of cisplatin appeared to be 120 mg/m2 considering its toxicity.

[A case of spontaneous peripelvic extravasation associated with ureteral stenosis caused by retroperitoneal lymph node metastasis of ascending colon cancer].

A case of spontaneous peripelvic extravasation associated with ureteral stenosis caused by retroperitoneal lymph node metastasis of the ascending colon cancer is reported. A 47-year-old woman complained of colic on right costa-vertebral angle. Excretory urograms showed right peripelvic extravasation and CT-scan showed urinoma formation around right kidney. Subsequent examination of right retrograde pyelo-ureterograms showed ureteral stenosis at sacro-iliac region. Operative findings revealed ureteral stenosis caused by retroperitoneal lymph node metastasis of ascending colon cancer, which was regarded as inoperable. There are a few reports of spontaneous peripelvic extravasation caused by a malignant tumor in Japanese literature. Twenty of them are reviewed.

[Extracorporeal shock wave lithotripsy of urolithiasis].

A total of 159 patients underwent 194 treatments with extracorporeal shock-wave lithotripsy for upper urinary tract calculi between August 4 and November 30, 1986. A single treatment was performed in 76% of the patients with renal stones and 90% of the patients with ureteral stones. Treatment was successful in 153 patients (93%). Seventy percent of the patients with renal stones and 95% of those with ureteral stones were stone-free over the one-month follow-up, while about 15% had small and asymptomatic fragments believed to be passable spontaneously. Only one patient required surgical removal of the calculi due to severe ureteral edema. Adjunctive urological management is required in about 9% of the patients preoperatively or postoperatively. The period of hospitalization averaged about 11 days after treatment and patients usually returned to work within a few days after their discharge. Extracorporeal shock-wave lithotripsy is the preferred form of management for symptomatic ureteral and renal calculi.

[Extracorporeal shock wave lithotripsy--difficulties in ESWL].

At Kanbara Hospital, 187 patients with urolithiasis have been treated by extracorporeal shock-wave lithotripsy (ESWL) since the first ESWL treatment in December, 1986. Some cases in which ESWL could not be performed easily were experienced. These difficulties were analyzed retrospectively and some problems in the ESWL treatment are discussed.

[A case of malignant Sertoli cell tumor of the testis].

A 74-year-old man consulted our hospital about a painless swelling of the right scrotal contents. Laboratory findings showed no abnormalities. A right high orchiectomy was performed. The tumor measured 4.2 X 6.0 cm in size and was yellow-white in color. Histologically, the tumor was diagnosed as a malignant Sertoli cell tumor on the basis of findings of invasion and a high mitosis rate. Combined chemotherapy was performed after the operation and no recurrence was found. The Sertoli cell tumor of the testis is a rare neoplasm, and its malignant type is extremely rare. In Japan, 7 cases have been reported, and only 2 of them have been malignant. This case is the 3rd instance malignant Sertoli cell tumor in this country.

[Two cases of paratesticular rhabdomyosarcoma].

Two cases of paratesticular rhabdomyosarcoma are reported. A 15-year-old boy was admitted in July, 1983, with a 3-week history of gradually increasing painless scrotal swelling on the right side. With the suspicion of testicular cancer, right radical orchiectomy was performed and pathological examination revealed an embryonal rhabdomyosarcoma of the paratesticular region. Chemotherapy and retroperitoneal lymphadenectomy were performed, and the latter revealed lymph node metastasis at the bifurcation of the aorta. Radiotherapy was not done because of adherent ileus. About 8 months after the orchiectomy, he died of recurrence at pelvic cavity and brain metastasis. A 6-year-old boy presented in March, 1985, with a complaint of right scrotal swelling that was painless and gradually increasing for about a month. Transinguinal exploration revealed a paratesticular neoplastic lesion and right radical orchiectomy was performed. Pathological diagnosis was paratesticular embryonal rhabdomyosarcoma. No evidence of metastasis was found by chest X-ray, excretory urogram, abdominal CT-scan or lymphangiography. As post-operative treatment, only chemotherapy was performed, and 9 months after the orchiectomy, the patient was asymptomatic. Ninety four cases of paratesticular rhabdomyosarcoma found in the Japanese literature are reviewed and mainly the policy of treatment is discussed.

Liposome immune lysis assay (LILA). Application of sandwich method to determine a serum protein component with antibody-bearing liposomes.

A complement-dependent liposome immune lysis assay (LILA) using carboxyfluorescein (CF)-entrapped liposomes bearing antibody was developed to measure C-reactive protein (CRP) antigen as a model of protein antigens in human sera. Goat anti-CRP antibody was covalently coupled to liposomes, and a specific lysis of the liposomes could be observed when the liposomes were incubated with both rabbit anti-CRP antibody (secondary antibody) and CRP antigen in sera in the presence of guinea pig complement. In this assay system, so-called sandwich assay, CRP (a multivalent antigen) bound to the liposomes bearing anti-CRP antibody and subsequently secondary antibody, which activated complement efficiently. The amount of CF released by a complement-dependent liposome immune lysis was proportional to CRP concentrations. This sandwich assay was simple, fast, highly sensitive, and covered the ranges 10-300 ng of CRP/ml in a homogeneous mode, that is, one where no separation step was employed. The results correlated well with those obtained by single radial immunodiffusion and enzyme immunoassay. This assay system would be applicable to the measurement of other protein antigens.

Transneuronal transport of WGA-HRP from the ipsi- to contralateral medial habenular nucleus through the interpeduncular nucleus in the rat.

Transneuronal transport of WGA-HRP from the ipsi- to contralateral medial habenular nucleus (mHb) through the interpeduncular nucleus (Ip) in the rat was examined by light and electron microscopy. At 2-3 days after unilateral injection of the tracer into the mHb, retrogradely labeled neurons were observed in the contralateral mHb. The labeling was assumed to be a result of an anterograde-retrograde transneuronal transport of the tracer through the Ip. At the fine structural level, astrocytes and terminal boutons were most readily labeled in the Ip with shorter survival periods, while neuronal somata and major dendrites were prominently labeled with longer survival times. This indicates that astroglia might be involved in the earliest stages of the transneuronal transport of the tracer.

Single pallidal neurons project both to the striatum and thalamus in the rat.

Collateral projections of single small pallidal neurons to the striatum and paraventricular nucleus of the thalamus were observed in the rat using a fluorescent retrograde double labeling technique. This population of pallidal cells was almost separate from the more predominant pallidal cell group projecting to the subthalamic nucleus. We propose that the globus pallidus proper might have the same two functionally distinct populations (limbic and motor), that have been well documented in the striatum and entopeduncular nucleus.

Collateral projections from the substantia nigra to the cingulate cortex and striatum in the rat.

After injecting a retrograde tracer into the posterior cingulate cortex, labeled neurons were found only in the substantia nigra pars compacta, leaving neurons in the ventral tegmental area totally unlabeled. The existence of collateral nigral projections to the cingulate cortex and striatum was clearly established using the fluorescent retrograde double-labeling technique. This constitutes a neuroanatomical substrate for psychotic symptoms expressed by a subpopulation of parkinsonian patients.

Trigeminal nerve branches containing primary afferent fibers to the dorsal division of the principal sensory trigeminal nucleus: a transganglionic horseradish peroxidase study in the cat.

Trigeminal nerve branches containing primary afferent fibers to the dorsal division of the principal sensory trigeminal nucleus (Vp) were examined in the cat by the transganglionic horseradish peroxidase method. The results indicate that the pterygopalatine, superior alveolar, infraorbital, buccal, lingual, inferior alveolar and mental nerves contain primary afferent fibers terminating in the dorsal division of the Vp probably conveying sensory information chiefly from the intraoral structures.

Enhanced formation of mouse hybridomas without hat treatment in a serum-free medium.

A newly developed, serum-free medium (NYSF-404) selects for antibody-producing hybridomas after fusion of antigen-sensitized mouse spleen cells with myeloma cell lines P3-X63-Ag8-U1 (P3-U1), P3-X63-Ag8-6.5.3 (Ag8.653), or P3-NSI/1-Ag4-1 (NS-1). Without the need for hypoxanthine-aminopterin-thymidine (HAT) selection of hybrid cells, frequency of hybridoma formation in medium NYSF-404 is higher (twice) than that in serum- and HAT-containing medium. Colonies developed upon limiting dilution in the presence of the mortal parent myeloma cells in medium NYSF-404 and pure culture of antibody-secreting cells could be subsequently established. The results suggest that fusions can be done in serum-free medium and that the clonal growth of hybridomas is dependent on factors produced by parent myeloma cells under serum-free culture conditions. Such factors seem deficient in serum- and HAT-containing medium or are masked by serum.

The short-term effect of prolactin on the active Na transport system of the tadpole skin during metamorphosis.

Prolactin (PRL) induced a transient increase in active Na transport measured as the short circuit current (SCC) of the tadpole skin. Increase in SCC by PRL accelerated as the stage advanced between stages XXII and XXV. Prolactin caused two types of effect on the electric parameters of the active Na transport. In type I, it increased both the electromotive force of the active Na current (ENa) and the resistance to the active Na current (RNa). In type II, it decreased both ENa and RNa. A second application of PRL had no effect on SCC; that is, desensitization to PRL was observed.
