Acute myocardial ischemia detected in dogs by temporal variation in two-dimensional ultrasound gray level.

This study tests the hypothesis that acute myocardial ischemia induces a characteristic temporal variation in regional ultrasound amplitudes. Myocardial ischemia was created by circumflex coronary artery occlusion in seven closed-chest mongrel dogs. Ultrasound images were acquired prior to occlusion and post-occlusion on a phased-array two-dimensional system. Unprocessed ultrasound data from end-diastolic images were taken digitally for quantitative gray level analysis. Temporal variation in ultrasonic gray level of a nonischemic control region was compared to the temporal variation in gray level of the ischemic area. In the ischemic area, the average gray level for all seven dogs increased from 39.2 +/- 4.2 prior to occlusion to 42.5 +/- 4.9 at 15 minutes after occlusion, and then to 44.4 +/- 5.9, 45.3 +/- 6.2, and 47.0 +/- 6.0 at 30, 60, and 120 minutes, respectively (p less than 0.05 for control vs 15 minutes and 15 minutes vs 120 minutes). No significant changes in the average gray level of all seven dogs occurred in the nonischemic area from pre-occlusion to 2 hours post-occlusion (38.8 +/- 8.8, 38.4 +/- 8.0, 37.7 +/- 8.4, 37.8 +/- 8.5, and 38.0 +/- 8.2 for control, 15, 30, 60, and 120 minutes, respectively. These data show that regions of acute myocardial ischemia can be characterized by temporal variation in intramyocardial ultrasonic gray level, not only from the time before coronary occlusion to 15 minutes after occlusion, but also between 15 and 120 minutes in the post-occlusion period. Gray level values in nonischemic regions of the left ventricle are remarkably constant over time.

Acupressure: an overview of theory and application.

Treatment of pain is common to all nurse practitioners' practices. One treatment method appropriate for use by the nurse practitioner is acupressure. Upon reviewing the professional literature, a paucity of information on acupressure was discovered. This article uses acupuncture theory and research to provide a theoretical basis for the use and study of acupressure. A description of how to locate and stimulate an acupoint useful for the treatment of headaches resulting from shoulder muscle tension is presented.

Antecedents of adherence to antituberculosis therapy.

This correlational study identified antecedents of adherence to antituberculosis (anti-TB) therapy in a convenience sample of 62 English-speaking adults. From a demographic perspective, the study sample was similar to the referent population of TB patients in Georgia. A variety of parametric analyses revealed the following: The mean self-reported adherence score was 92.6% (SD = 3.3). Higher levels of self-reported adherence were associated with an annual income of $11,000 or more, education beyond high school, no current alcohol use, perceived support and absence of barriers to medication taking, strong intentions to adhere, and a high capacity for self-care. Those six variables accounted for 28% of adherence variance, F(6, 44) of 4.3, p = 0.0017. Additionally, belief in the usefulness and benefit of the medications was strongly correlated with intentions to adhere (r = 0.83, p < 0.001), and interpersonal aspects of care was significantly correlated with perceptions of medication utility (r = 0.65, p < 0.001), supports/barriers (r = 0.44, p < 0.001), intentions (r = 0.69, p < 0.001), and self-care (r = -0.42, p < 0.01). Persons who were diagnosed with both TB and human immunodeficiency virus (HIV) reported significantly lower adherence.

Perceived self-care agency: a LISREL factor analysis of Bickel and Hanson's questionnaire.

The psychometric properties of Bickel and Hanson's Perceived Self-Care Agency Questionnaire (PSCAQ) (Hanson, 1981) were examined using a LISREL confirmatory factor analysis approach. Questionnaires were distributed to a convenience sample of 575 noninstitutionalized adults in northwestern Ohio; 462 questionnaires (80%) were returned. Findings indicated that the factor structure of the PSCAQ is significantly different from that described by Bickel (1982) and does not appear to conform to the expected self-care power component structure. These findings call into question the construct validity of the PSCAQ as a measure of self-care agency in non-institutionalized adults. It should be used only with extreme caution, if at all, for testing the self-care deficit theory.
