[The effect of weight reduction on compensation in diabetic obese patients with non-insulin dependent diabetes mellitus].

Non-insulin-dependent diabetes mellitus is associated with obesity in 80 to 90%. The presence of obesity enhances insulin resistance, which increases the demands on insulin secretion and causes deterioration of glucose tolerance. Hyperinsulinism has some metabolic sequelae which increase the risk of the development and progression of atherosclerosis. Reduction of body weight is a basic therapeutic provision in obese diabetics. It results not only in better compensation of diabetes but also other metabolic parameters and blood pressure as it leads to a higher tissue sensitivity for insulin. This can delay the development of long-term complications of diabetes mellitus.

Ventriculomegaly with radial and renal defects: prenatal diagnosis in two consecutive sibs.

We describe two consecutive mid-trimester fetuses of different sexes with identical anomalies of the upper limbs and the kidneys in association with severe ventriculomegaly. We compare this apparently autosomal recessive syndrome to VACTERL-H association, Fanconi anemia, and two other, so far unparalleled syndromes. Taking into account the absence of chromosome breaks, the associated changes of the amniotic fluid, and the renal histology, we conclude that we are dealing with a different entity.

[Biochemical and ultrasonic screening of chromosomal aneuploidies in the second trimester of pregnancy].

For prenatal screening of chromosomal aneuploidies (primarily the most frequent Down syndrome) maternal serum AFP screening in the second trimester of pregnancy has been supplemented by the determination of hCG in Hajdú-Bihar county. In pregnancies at risk on the basis of biochemical tests, a thorough, aimed ultrasound examination for the detection of minor and major anomalies characteristic for chromosomal abnormalities was carried out. If both biochemical and ultrasound examinations were suggestive of high risk prenatal karyotyping was offered. During a two-years prospective study 14328 pregnancies were screened. Authors could detect 38% of Down-syndrome cases prenatally, 5 cases in pregnant women at age under 35 years and at the same time the number of amniocenteses increased only by 70. It was concluded that 66% of all Down syndrome cases could have been prenatally diagnosed if prenatal chromosome test were performed in all cases at high risk on the basis of screening tests and maternal age.

Internal carotid stent implantation with angioscopic control.

Stent implantation is a method, which is being used more and more often, mainly in the field of peripheral arteries, but coronary stent implantation is also well-known. The authors apply this procedure for patients, who besides the carotid bifurcation stenosis also suffer from the internal carotid stenosis in a longer section of the artery. The indication are as follows: 1 in the case of internal carotid stenosis in a longer section we assure the flow with the help of carotid thrombendarterectomy and slove the run-off with balloon catheter dilatation and stent implantation. 2. in the case of a stenosis in a short section we use stent implantation to avoid dissection of the intima. With each intervention we perform the carotid bifurcation thrombendarterectomy. The authors made the first intervention 15 months ago, since then we have performed 17 stent implantations. One of the 17 patients developed a temporary stroke (TIA). This is a new method, further cases are needed for long-term experiences. Intervention under eye-control can be carried out with more confidence. As regards to the post-operative medicinal treatment, the administration of thrombocyte aggregation-blockers or heparinoid preparation (e.g.: PPS-SP 54) after the stent implantation has become a routine therapy. Summarized the foregoing, these cases constitute about 10-15% of all carotid operations, therefore the authors would like to make it clear, that this is not a routine method, but a possibility, strictly respecting the above-mentioned indications.

[Synthesis of hetero-condensed quinazolones: 1,2,4-triazino[6,1-b]- and 1,2,5-triazino[2,3-b]quinazolones].

In the course of the systematic investigation of heterocondensed quinazolones derivatives of a new heterocyclic ring system, [1,2,5]triazepino[2,3-b]quinazolone have been developed as potential biologically active agents, which are new bioisoteric analogues of cholecystokinin antagonist diazepino[2,1-b]quinazolones. The authors worked out an original synthetic route for preparation of ring analogue [1,2,4]triazino[6,1-b]quinazolines from intermediates of the synthesis, too. Starting 2-alkyl-3-amino-quinazolones are easily prepared by reaction of 2-alkyl-benzoxazone with hydrazine hydrate. In the next step bromination reaction of the alpha-methylene group of 2-alkyl-substituents gave a mono-bromo derivative as major product. In the reaction of the bromo compound with N-nucleophyles 2-alkylamino-3-amino-quinazolones were obtained which reacted with alpha-halogeno-carbonyl compounds and led to the tricyclic quinazolones in ring closure reaction. The structural properties of the heterocyclic compounds were characterized by UV-VIS and NMR data and molecular modelling calculation.

Azepines

[Late results of the Palma operation].

Since 1981, 54 femoro-femoral crossover saphenous bypass (Palma-operation) were performed for chronic venous insufficiency caused by unilateral iliac vein occlusion. Long-term patiency rate and clinical results were evaluated by radioisotope venography and venous occlusion plethysmography. Plethysmography demonstrated significant improvement of maximal venous outflow after operation. Radioisotope venography showed remarkable long-term patency rate of Palma-bypass (of 42 operation, 29 patent graft). Long-term clinical results appeared to be excellent after operation of isolated iliac vein occlusion, whereas the results after operations performed with postthrombotic changes in more distal deep veins were not acceptable, therefore a more critical preoperative selection seems to be necessary.

[Screening for congenital anomalies in mid-term pregnancy. Prospective epidemiologic study. Fetal trisomy].

The authors have presented their experiences on prenatal screening of fetal trisomies in this second part of a prospective study between 1988 and 1990. They gained their conclusions by processing 63,496 pregnancies during three years. The results show that maternal age plays the most important role in the prenatal screening of fetal trisomies in Hungary. They recommend fetal karyotyping for every pregnant woman aged 35 years or more. They emphasized that using of a combined screening method (i.e. maternal age, serum alpha-fetoprotein, human chorionic gonadotropin, oestriol) is only permissible if the hormonal and cytogenetic laboratory background are provided under standard circumstances. Since these are not available for the vast majority of pregnant women in Hungary they concluded that, at least for the time being, the main criteria for prenatal screening of fetal trisomies is the maternal age. By applying this recommendation 25-30 percentage of Down syndrome fetuses can be detected.

Impact of prenatal mid-trimester screening on the prevalence of fetal structural anomalies: a prospective epidemiological study.

The objective of this study was to evaluate the effectiveness of the measurement of maternal serum alpha-fetoprotein (MSAFP) at 16 weeks and a subsequent routine ultrasound screening at 18-20 weeks' gestation and the impact on the birth prevalence of congenital structural anomalies in an unselected pregnant population of Hungary in a prospective epidemiological study. A total of 63,794 pregnant women (representing one-sixth of the population of Hungary) were offered this screening program over 3 years (1988-90). Of the pregnant population, 75.7% (48,312) received MSAFP screening and in 81.0% (51,675), at least one ultrasound scan was performed. In the screened pregnancies, 496 craniospinal, thoraco-abdominal, urogenital and other severe major anomalies occurred; 317 were detected at 18-20 weeks (sensitivity 63.1%; specificity 100.0%; positive predictive value 100.0%). The sensitivity of ultrasound scanning was significantly higher (p < 0.05) than that of the MSAFP screening. (At the time of ultrasound scanning the MSAFP value was known.) In this study, the less serious anomalies such as hydrocele, hypospadias and undescended testicle were not systematically searched for, but the birth prevalences were calculated. The overall mid-trimester prevalence of severe plus less severe major anomalies was 2.26%. The birth prevalences of severe major anomalies were 0.57 (craniospinal), 4.36 (thoracoabdominal and urogenital) and 1.21 (other severe) per 1000. These values were lower than the mid-trimester prevalences which were 2.94, 5.20 and 2.06 per 1000, respectively. The prevalence values at the age of 1 year were also calculated (0.36, 2.21, 0.54 per 1000, respectively). We conclude that our screening program with availability of termination of pregnancy could significantly (p < 0.05) reduce the prevalence of severe major abnormalities at birth. Training programs in cardiac scanning are required.

[Synthesis of potential CCK antagonist quinazolone derivatives. II].

An alternative route has been developed for the synthesis of [1,4]diazepino[3,4-b]quinazolones (2), a new ring system of heterocondensed quinazolones. 2,3-Bifunctional quinazolones (4) were synthesized by halogenation of 2-alkyl-quinazolone-3-propionic acids, yield from the reaction of 2-alkyl-benzoxazone (3) with beta-alanine. The reaction of haloalkyl compounds (5,6) with N-nucleophiles produces [1,4]oxazepino-[3,4-b]quinazolones (8), a new heterocyclic ring system, and 3-[2'-hydroxi-ethyl-, or 3-[2'-chloro-ethyl-4' oxo(3'H)-quinazoline-3'-yl]propionamides (7, 9) in consecutive reaction. The cyclisation of 2-aminoalkyl-3-propionates (13) resulted in the title compounds (14). Physical data and properties of the newly synthesized compounds such as IR, UV and NMR spectra were obtained for all compounds reported.

[Synthesis of potential CCK antagonist quinazolone derivatives].

An original route has been found for the synthesis of [1,4]diazepino-quinazolones, a new ring system of heterocondensed quinazolones. These anthranilicacid-alanin-beta-alanin cyclopeptide derivatives constitute a structural moiety of asperlicin, the first natural cholecystokinin antagonist alkaloid. These compounds are therefore potential CCK antagonists. The new compounds were prepared via condensation of 2-amino-alkyl-quinazolones, obtained from 2-alkyl-quinazolones by side-chain substitution, with 1,3-bifunctional-reagents. We studied the cyclisation process under basic, acidic and phase-transfer catalyzed conditions. The structures of the synthesized compounds were characterized by IR, UV and NMR spectroscopy.

Cholecystokinin

[Fetal and neonatal endocarditis].

Two infants with endocarditis were diagnosed at our Institute within a six month period. One infant died due to severe aortic valve destruction and ruptured sinus Valsalvae aneurysm both of which developed during fetal life. The other one with tricuspid valve endocarditis was successfully treated after the early echocardiographic diagnosis. To our knowledge this patient is the only survivor with neonatal endocarditis in our country.

[Screening of congenital anomalies in the mid-trimester of pregnancy. Prospective epidemiologic study. Developmental anomalies].

Authors report about the data of 60,000 pregnant women from three district counties of Eastern-Hungary between 1988 and 1990, on whom both ultrasound and alfa-fetoprotein screenings were performed. They demonstrate the value of this screening for the detection of fetal malformations and for the birth prevalence rate as well. This effective screening-protocol is proposed for a nation-wide application.

[Retrospective survey of nursery-age children 5 years after Chernobyl].

In their paper, the authors are dealing with the effect of the Chernobyl's nuclear catastrophe (1986) for the outcome of pregnancy and some parameters of the status of health. Their empirical study is based on a sample (sample size is 1168) of pregnant of the critical year 1986, and also based on a control sample (sample size is 1068) of the year 1985. They compared the five old year children's parameters of status of health between the critical and control groups. Their retrospective epidemiologic survey is extended to study some prenatal, perinatal and childhood parameters also. Finally, they summarise the biometric evaluation of their questionnaire survey. No significant difference of health conditions of the two groups defined above was found.

[In vitro study and clinical transplantation of human fetal islands of Langerhans].

A simple and reliable procedure was developed for the long-term culturing and cryopreservation of human fetal pancreatic islets. The methods involved can ensure the survival and the growth capacity of fetal tissue. The long-term cultivation or cryoculturing of fetal islet tissue was applied in 23 clinical transplantatious on the basis of the experimental study. All of the diabetic patients suffered progressive retinopathy and three of them had nephropathy as well. After grafting the insulin requirement reduced significantly (39.4%) correlated with the higher serum C-peptide level (mean: 0.41 ng/ml). During the long-term follow-up (9 yrs), the graft function failed in 5 cases, while in 18 successful cases the long term graft function depended on the volume of the grafted islet mass. The results suggest that the transplantation of fetal pancreatic islet has a beneficial effect in the early stage of diabetic complications.
