Treatment of idiopathic pituitary dwarfism with methionyl human growth hormone.

Ten patients with idiopathic pituitary dwarfism were treated with methionyl human growth hormone (met-hGH) produced by recombinant DNA technology. They were given 0.5 IU/kg/week of met-hGH for three months. There were no significant changes in physical, blood, urine examinations. Their height increased between 1.5 and 2.7 cm during the 3 months of treatment, which was calculated to be equivalent 6.0 and 10.8 cm per year with a mean increase of 8.7 cm. Anti-hGH antibody was observed in all patients between one and two months of treatment with a titer between 10(1)X and 10(4)X. In spite of the formation of antibodies, the growth rate did not decrease in 7 of the 10 cases.

Inhibitory effect of the CA2+ antagonist nifedipine on histamine release from rat peritoneal mast cells.

45Ca uptake and histamine release was examined in mast cells from rats sensitized with ovalbumin and Bordetella Bertussis as an adjuvant. The uptake of 45Ca by the mast cells was significantly increased by stimulation with ovalbumin as was the release of histamine from the mast cells. Nifedipine, a calcium antagonist, inhibited the increase in both 45Ca uptake and histamine release stimulated by ovalbumin, though the effect on 45Ca uptake was stronger than that on histamine release.

Individualized fluid resuscitation based on haemodynamic monitoring in the management of extensive burns.

An evaluation has been made on the effectiveness and feasibility of the individualized fluid replacement programme based on intensive haemodynamic monitoring using a Swan-Ganz catheter. Twenty-one extensively burned patients with an average burn of 60.8 per cent BSA were resuscitated with lactated Ringer's and colloid solutions. The rate of fluid administration was adjusted to maintain the optimal ranges of the various haemodynamic parameters including cardiac index and left ventricular stroke work index. Two patients failed to respond to fluid resuscitation possibly due to inadequate emergency procedures given before arrival at our institute. The remaining patients survived the shock phase, with the amount of fluid given to the 18 adult patients being (3.38 /+- 1.02 ml/kg) x (percentage burn) for the first 24 hours. A negative correlation existed between the amounts of lactated Ringer's solution and colloid solution used for the fluid resuscitation. Dopamine was effective in 4 out of 5 patients who showed depressed myocardial function. The individualized fluid programme was shown to be effective and reliable for the management of critically burned patients.

Renal handling of salt and water in the early stage of obstructive jaundice in rabbits.

Renal handling of salt and water in the early stage of obstructive jaundice was studied in rabbits 10 days after the ligation of the common bile duct (BDL). Sham-operated (SO) animals served as controls. No sodium retention was found in BDL rabbits, despite reduced renal perfusion and elevated plasma aldosterone level. A redistribution of intrarenal blood flow was not found. The filtration fraction did not change. A saline load resulted in decreases in arterial hematocrit and total serum proteins, and increases in urine output, urinary sodium excretion and osmolal clearance. Blood pressure, glomerular filtration rate (GFR), RPF, the filtration fraction and the intrarenal flow distribution were not significantly affected by the saline load. No significant difference was found in the natriuretic response to the saline load between the BDL and SO groups. After 60 h of water deprivation, there was no significant difference in urine-to-plasma osmolality ratios or renal tissue fluid osmolality between the BDL and SO animals. The findings indicate that renal handling of salt and water was well maintained in the early phase of obstructive jaundice in rabbits. The data also suggest the critical role of the redistribution of intrarenal blood flow rather than of GFR or aldosterone in determining sodium retention in obstructive jaundice.

Changing plasma and urinary organic acid levels in a patient with isovaleric acidemia during an attack.

Organic acids in plasma and urine of a patient with isovaleric acidemia were measured serially during a severe ketoacidotic attack. Urinary ketone bodies, lactic acid and 2-hydroxy-n-butyric acid changed in parallel with the plasma isovaleric acid concentration, which was correlated closely with the severity of the clinical symptoms. A 2-day lag was observed between the time of the peak plasma isovaleric acid level and that of the highest urinary excretion of 3-hydroxyisovaleric acid, methylsuccinic acid, mesaconic acid and 2-hydroxyisobutyric acid, the total quantity of which amounted to one-third of that of isovalerylglycine on the 4th day of the attack. Urinary isovalerylglycine excretion remained at a plateau for 4 days after the highest level of plasma isovaleric acid was observed. The decrease of urinary hippuric acid excretion during the attack continued until the patient had recovered enough to be able to eat. Benzoic acid administration concomitant with leucine and glycine canceled the effect of glycine in lessening the rise of plasma isovaleric acid concentration after a leucine load.

Diurnal variation of urinary potassium excretion in chronic renal insufficiency.

Diurnal variation of urinary potassium excretion was examined on 10 adult patients with chronic renal insufficiency and 24 control subjects without cardiorenal functional disturbances. The daily cycle of potassium excretion, similar to that in controls, was present but less obvious in chronic renal insufficiency. In the controls, urinary potassium excretion reached a peak in the early morning (5.30 to 7.30) with a minimum in the night (21.00 to 5.30). The morning rise in potassium excretion was associated with increases in urinary sodium excretion, arterial blood pH, and plasma potassium and aldosterone concentrations. Creatinine clearance rate did not change significantly. In renal insufficiency, plasma potassium level and potassium excretion increased in the morning as compared with the night, whereas sodium excretion, the creatinine clearance rate, arterial blood pH and plasma aldosterone concentration did not change. 9 alpha-Fluorohydrocortisone gave rise to markedly enhanced potassium excretion. The findings suggest that the blunted cycle of potassium excretion in renal insufficiency is due, in part, to suppressed diurnal variations of sodium delivery to the distal tubule, acid-base balance and/or aldosterone secretion.

Two siblings, including a fetus, with Tay-Sachs disease.

This report consists of a morphological study on two siblings of Tay-Sachs disease. One is a girl of 3 years of age and the other is a 23-week fetus. In the first case, the nerve cells in the cerebral cortex showed histochemically abnormal accumulation of glycolipids and electron microscopically numerous membranous cytoplasmic bodies characteristic to gangliosidosis. In the fetal case, the nerve cells in the brain stem were well differentiated and here abnormal inclusions were found by histochemical study. The cortical neurons were immature in structure and inclusions were not demonstrated by histochemical study. In electron microscopic study, the cortical neurons also showed a small number of intracytoplasmic inclusions which contained electron dense granules and/or membranous structures. Causes of the differences in amount of abnormal lipid accumulation in the brain stem and the cerebral cortex were discussed.

(omega-1)-Hydroxymonocarboxylic acids in urine of infants fed medium-chain triglycerides.

Organic acids in the urine of infants fed a formula containing medium-chain triglycerides (octanoic acid 49% and decanoic acid 26% of total fatty acids) were determined by gas chromatography-mass spectrometry. We found a considerable amount of 5-hydroxyhexanoic acid and 7-hydroxyoctanoic acid, but no detectable 9-hydroxydecanoic acid, as well as a large amount of C6-C10-dicarboxylic acid. We believe that such acids are derived, via (omega-1)-hydroxylation, from medium-chain monocarboxylic acid or medium-chain acyl-CoA in the liver cell. The degree of (omega-1)-hydroxylation of medium-chain fatty acids relative to omega-hydroxylation apparently increases as the chain length of the fatty acids decreases.

Mechanisms of altered renal perfusion in the early stage of obstructive jaundice.

Mechanisms responsible for renal hemodynamic alterations were studied in jaundiced (by bile-duct ligation) rabbits (BDL) 10 days after ligation of the biliary tract, in comparison with sham-operated rabbits (SO). Arterial hematocrit, plasma volume, blood pressure, abdominal inferior vena cava pressure, and heart rate were not significantly different between the BDL and SO groups. Cardiac output in BDL rabbits decreased to approximately 73% of the value for SO rabbits. Renal blood flow and GFR were reduced to 64 and 61%, respectively. Reductions in blood pressure and renal blood flow, caused by bleeding (8 ml/kg of body wt), were more marked in the BDL group than they were in the SO group. In the BDL group, the recovery of blood pressure following blood infusion was slower and the mortality was higher, There was no significant increase in the renovascular sensitivity to exogenous noradrenaline or angiotensin II in the BDL group. The findings indicate that the early stage of obstructive jaundice in rabbits was characterized by altered renal perfusion partly due to reduced cardiac output and by incrased liability to hemorrhagic hypotension.

Pituitary hyperplasia due to hypothyroidism.

Two cases of hypothyroidism with computed tomographic evidence of enlargement of the pituitary gland are reported; in both the enlargement disappeared after treatment. These findings are in agreement with the previously reported observation that an increase of TSH production in primary hypothyroidism may lead to pituitary hyperplasia.
