Acute infantile hemiplegia in a patient with propionic acidaemia.

A 10-month-old girl with mild developmental delay became hemiplegic after seizures. Cranial CT scan and magnetic resonance imaging (MRI) revealed no lesions related to vascular diseases, but brain atrophy on the right side was remarkable. Digital subtraction angiography showed slightly decreased visualization of peripheral branches of the right medial cerebral artery. Propionic acidaemia was diagnosed on the basis of high plasma levels of propionic acid and its metabolites and the elevated urinary excretion of these acids. With therapy, the levels of these acids fell, and her left hemiplegia disappeared 3 months later.

Juvenile nephronophthisis diagnosed from glucosuria detected by urine screening at school.

We describe the case of an 11-year-old girl in whom glucosuria detected by urine screening at school was the first clue to the diagnosis of juvenile nephronophthisis (JN). On admission, she showed renal failure with combined proximal and distal tubular defects and progressive deterioration of renal function. JN should be considered in children with glucosuria.

Diagnosis of iliopsoas pyomyositis by ultrasonography and CT.

A patient with iliopsoas muscle pyomyositis was successfully treated with intravenous antibiotics alone. The diagnosis was made by ultrasonography and X-ray computed tomography after five days of spiking fevers. Ultrasonography and X-ray computed tomography revealed swelling of the muscle body but no abscess formation. We emphasize the usefulness of early diagnostic imaging to allow conservative treatment of iliopsoas muscle pyomyositis.

Growth patterns of the normal pituitary gland and in pituitary adenoma.

The growth pattern of the pituitary gland of 86 infants, children and adolescents without pituitary or hypothalamic dysfunction was studied with sagittal magnetic resonance imaging. Postnatally the gland grew rapidly, reaching a plateau at almost three years after birth, and growing rapidly again between the age of 10 to 13 years. Pituitary gland area showed a similar growth pattern to gland height. The pituitary glands of seven patients with pituitary adenomata were also examined. All had convex upper-gland margins, with height and area significantly greater than those of normal controls of the same age.

Auditory brainstem response in hyperbilirubinemic rat: Part I.

The effect of hyperbilirubinemia on the auditory brainstem response (ABR) of rats and its change by hyperosmolality were examined. The ABR wave I did not change significantly. In bilirubin-loaded rats, the latencies of wave II and IV were prolonged and the amplitudes decreased. Bilirubin injection damaged the central auditory pathway but the peripheral auditory pathway was spared. These ABR changes were reversible in the acute phase of hyperbilirubinemia. Mannitol administration hastened the appearance of the ABR peaks and the recovery of the changed ABR. This tendency was more remarkable in latency changes than in amplitude changes.

Neurohypophyseal function of an ectopic posterior lobe in patients with growth hormone deficiency.

We studied the neurohypophyseal function of 20 patients with complete GH deficiency owing to pituitary stalk transection by means of the water deprivation and the hypertonic saline infusion test. In T1-weighted magnetic resonance images, high-signal intensity of the posterior lobe of the hypophysis was missing in all the patients. An ectopic posterior lobe was observed at the proximal stump of the transected stalk in 17 patients, whereas the 3 patients without ectopic posterior lobe had overt diabetes insipidus or intractable nocturnal enuresis. After water deprivation, 3 patients with large ectopic posterior lobes (length along the pituitary stalk axis exceeding 5 mm) showed urinary osmolality and plasma AVP levels as high as those of 13 patients with partial GH deficiency in whom magnetic resonance images revealed no abnormalities. On the other hand, 14 patients with small ectopic posterior lobes (length less than 5 mm) showed significantly lower urinary osmolality and plasma AVP levels than the patients with large ectopic posterior lobes (p less than 0.01 and p less than 0.01, respectively) and the patients with partial GH deficiency (p less than 0.01 and p less than 0.01, respectively). Urinary osmolality in the patients with small ectopic posterior lobes, however, was higher than that in 3 patients without ectopic posterior lobes (p less than 0.01). During the hypertonic saline infusion test, peak plasma AVP levels in the patients with small ectopic posterior lobes were significantly lower than those in the patients with partial GH deficiency (p less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical characteristics of bronchial asthma in the elderly in relation to cell component in the airways].

Cellular responses in the airways of bronchial asthma, which might be affected with aging, were compared between 28 elderly (60 years old or more) and 28 younger (40 years old or less) patients with bronchial asthma. The cellular responses were observed to differentiate 500 cells in bronchoalveolar lavage fluid (BALF), excluding epithelial cells. The average frequency of each type of cell in the BALF of the elderly asthmatics was 65.7% for macrophages, 15.5% for lymphocytes, 8.9% for neutrophils and 9.5% for eosinophils. In the younger asthmatics the average frequency was 61.8% for macrophages, 17.1% for lymphocytes, 4.1% for neutrophils and 16.1% for eosinophils. A significant increase in the frequency of neutrophils in BALF of the elderly cases was observed in steroid-dependent intractable asthmatics compared to the younger cases (p less than 0.05). An increase in frequency of eosinophils in the younger cases was also observed, although the difference in the frequency was not significant between the elderly and the younger intractable cases. A significant difference in the frequency of each type of cell in the clinical type of bronchial asthma was not present between the elderly and the younger cases. However, the frequency of neutrophils in elderly cases was significantly increased in Ib. bronchospasm + hypersecretion type compared to Ia. bronchospasm type (p less than 0.05), and was more elevated in II. bronchiolar obstruction type (p less than 0.01). The frequency of neutrophils was markedly more increased in the elderly cases with poor prognosis than in the younger cases (p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Delayed auditory brainstem response in thiamin-deficient rats.

We recorded the auditory brainstem responses of rats fed a thiamin-deficient diet. The interpeak latencies between waves I and III, as well as those between waves I and IV, were significantly prolonged from day 24, while the latency of wave I was prolonged on day 26 of the thiamin-deficient diet. These delayed responses were corrected in 2 to 4 days after the initiation of daily intraperitoneal thiamin injections from day 32. The rats that were fed the thiamin-deficient diet, and then sacrificed on day 32, showed a decrease of total thiamin levels in the brain (26% of the level in control rat brains). Based on these results, we emphasize the value of the auditory brainstem response to detect thiamin deficiency.

[High deposition rate of aluminum in tissues in diabetic hemodialysis patients].

Aluminum (Al) accumulation in bone is a serious problem in patients on hemodialysis. We studied deferoxamine infusion test (DFO test) in 14 diabetic patients on hemodialysis (HDDM) and 23 hemodialysis patients originated from glomerulo nephritis (HDCGN) to determine whether Al accumulation is different between the two groups or not. There was no difference in hemodialysis duration and total oral intake of Al containing drugs between two groups. Serum C-terminal parathyroid hormone (C-PTH) in HDDM was lower than that in HDCGN group (1.82 +/- 1.30 vs. 3.80 +/- 1.82 ng/ml; P less than 0.01). However serum Al (s-Al) levels were comparable (61.9 +/- 53.0 vs, 45.0 +/- 32.3 micrograms/l). A significant correlation was observed between duration of dialysis period and s-Al in HDDM (r = 0.806, p less than 0.01), but in HDCGN, the relation was not significant. The patients in HDDM whose cumulative aluminum intake was less than 2.0 kg showed the higher serum A1 concentrations before DFO and greater increases in s-Al after DFO test, as compared with those in HDCGN with matched aluminum intake (93.8 +/- 67.6 vs. 35.9 +/- 23.6 micrograms/l; p less than 0.001 and 141.2 +/- 81.8 vs. 70.3 +/- 41.1 micrograms/l; p = 0.035). These results indicate that in uremic diabetic patients with lower intake of Al containing drugs, an early accumulation of Al in the whole body occurs possibly because of the enhanced absorption rate of Al at an intestine and/or the low PTH level.

[Characteristic of cell components in bronchoalveolar lavage fluid (BALF) in patients with bronchial asthma classified by clinical symptoms].

The characteristics of cell components in BALF were examined in three groups of 104 patients with bronchial asthma, classified by clinical symptoms. 1. The frequency of neutrophils in BALF was significantly higher in patients with hypersecretion type (Ib) and in cases with bronchiolar obstruction type (II) than in cases with simple bronchoconstriction type (Ia). 2. A significantly higher proportions of neutrophils in BALF was observed in atopic asthmatics with type Ib than in non-atopic asthmatics with type Ib. 3. More frequency with moderate or high grade of eosinophilia in BALF were observed in cases with type Ib, whether they were atopic or non-atopic type. 4. More frequency with moderate or high grade of neutrophilia in BALF were observed in atopic cases with Ib and in both atopic and non-atopic cases with II. 5. Cases with moderate and high grade of both eosinophilia and neutrophilia in BALF were more frequent in atopic cases with Ib. The results suggest followings--1) both eosinophils and neutrophils participate in hypersecretion of type Ib in atopic cases, and only eosinophils in non-atopic cases. 2) neutrophils participate in bronchiolar obstruction of type II, whether they are atopic or non-atopic cases.

[Occlusion of basilar artery in a 7-year-old boy].

A case of occlusion of basilar artery in a boy aged 7 years was reported. Compared with CT, MR imaging was more valuable for the early diagnosis of this disease. SPECT with IMP was thought to be useful in evaluating cerebellar circulation. ABR revealed brain stem abnormalities that could not be detected by neuroradiological techniques.

Mean cellular volume of urinary red blood cells in investigation of hematuria.

The mean cellular volume (MCV) of urinary red blood cells (RBCs) of pediatric patients with glomerular (group I; n = 52) and nonglomerular (group II; n = 21) hematuria was determined using an automated blood-cell analyzer. Group I patients had a significantly lower MCV than group II (61 +/- 8 vs. 88 +/- 15 microns 3, mean +/- SD; p less than 0.001). With a MCV of 75 microns 3 taken as the dividing line between glomerular and nonglomerular hematuria, correct assessment of the site of bleeding was made in 67 of the 73 (92%) patients studied. Determination of the MCV of urinary RBCs is a simple noninvasive test for localizing the site of hematuria in pediatric patients.

Ectopic posterior lobe of the pituitary gland and intractable nocturnal enuresis in a case with pituitary dwarfism.

An 11 5/12-year-old girl with pituitary dwarfism had intractable nocturnal enuresis. Magnetic resonance imaging revealed pituitary stalk transection and the formation of an ectopic posterior lobe. The results of responses to dehydration, infusion of hyperosmolar NaCl solution, and 1-desamino-8-D-arginine vasopressin showed that the nocturnal enuresis in the present case was due to a partial vasopressin deficiency. We suggest that the ectopic posterior lobe is one cause of nocturnal enuresis.

Branched-chain alpha-ketoacids and related acids in thiamin-deprived rats.

We showed elevated plasma levels of branched-chain amino acids, alpha-ketoacids, alpha-hydroxyacids in thiamin-deprived rats, which had decreased liver thiamin levels (2.4% of control) after 4 weeks of feeding of thiamin-deficient diet. The ratios of mean levels of these acids in thiamin-deficient rats to those in control rats were as follows: total branched-chain amino acids 1.6; total branched-chain alpha-ketoacids 2.7; total branched-chain alpha-hydroxyacids 3.6; pyruvic acid 1.6. The plasma level of total branched-chain alpha-hydroxyacids was correlated to liver thiamin concentration (r = -0.67, p less than 0.01), but those of the other two acids were not correlated. Although the increased levels of branched-chain acids in thiamin-deprived rats were less remarkable than those of ill patients with maple syrup urine disease, they reflect the reportedly decreased branched-chain alpha-ketoacid dehydrogenase activities (about 40% of control), and we discussed whether the toxic effects of individual branched-chain acid could be demonstrated by its administration to the thiamin-deprived rats.

[Clinical studies on steroid-dependent intractable asthma. Comparison between early and late onset of asthma].

The various components making for severe intractable asthma were clinically and allergo-immunologically studied in 90 patients with bronchial asthma, by comparison between early onset and late onset groups. 1. In the early onset asthma group, cases with low serum IgE levels showed a stronger tendency toward severe intractable asthma. 2. Late onset asthma cases with negative skin tests and negative specific IgE antibodies to house dust tended more often to be severe intractable cases. 3. There was no correlation between sensitization by specific antigens (house dust and Candida), especially Candida, and a tendency toward severe intractable asthma. 4. Severe intractable asthma might be caused by bronchospasm in cases under 30 years of age, by bronchospasm plus hypersecretion in cases between 31 and 40 years of age, and by bronchospasm plus bronchiolar obstruction in cases over 40 years of age.
