50 Norwegian doctors selected at random from the population of general practitioners who were neither recognized specialists nor enrolled in the specialist training programme, were interviewed by telephone about their own postgraduate training and their attitude towards the system of specialist training for general practitioners. Their educational activities and needs differed very little from those described in previous surveys among Norwegian practitioners. All the younger doctors were determined to enroll in the specialist training programme, but many thought that this might be difficult for family reasons. Most of the doctors above the age of 40 lacked the motivation for specialist training. The doctors expressed a variety of different attitudes towards the system of specialist training, including certain conflicts of loyalties and values.

Most of the doctors expressed a strong need for the education provided by such training. The most important obstacles to hospital training were personal domestic reasons, concern for the functioning of the practice and concern for the emotional problems the hospital work might cause. Strategies are suggested to make it easier, in practice and emotionally, to do hospital training.

The authors report a case of ulnar nerve compression caused by an abductor digiti minimi longus. After resection of the muscle the patient recovered well.
Trypanosomes were isolated by culture from 2 out of 50 blood samples collected from reindeer (Rangifer tarandus L.) in northern Sweden and from a blood sample from a moose (Alces alces) from Southern Sweden. The parasites were indistinguishable morphologically from other trypanosomes reported from cervids, both as epimastigotes in axenic culture at 27 degrees C and as bloodstream-like trypomastigotes cultured on mammalian fibroblasts at 37 degrees C. Surface carbohydrate and isoenzyme comparison of these isolates with four American isolates, from reindeer (Rangifer tarandus), mule deer (Odocoileus hemionus), moose (Alces alces) and elk (Cervus canadensis), are reported. The isoenzyme patterns of the isolate from the Swedish moose were similar to those of the isolates from America while the patterns of the 2 isolates from the Swedish reindeer were considerably different from the others; these 2 isolates may represent a new species. Lectins can be used to distinguish between the trypanosomes of cervids.

Hypertrophic scars and tissue defects are the most common cause of functional and aesthetic problems in the head and neck of burn patients. For reconstructive surgery full-thickness or split thickness skin grafts are preferred. Pedicled flaps or free flaps are mostly used to correct contractures in the neck. The transplantation of bone or cartilage and tissue expansion are also used for special indications.

In regard to the good regeneration of the skin in the facial area surgical therapy in burns can be postponed up to the 2nd week. By then deep 2nd degree burns are healed. The flap procedures are rarely indicated, they are necessary only if bone is exposed. Meshgraft is contraindicated. It is mandatory not to delay escharotomy of the ears in regard to the problem of chondritis. The same approach should be followed in 3rd degree damage of the eye lids.

Deep burns of the skull are uncommon. In fire burned victims we sometimes found a necrosis of the outer table of the skull. Grafting with split thickness skin is generally successful, when the outer table is removed. Complete destruction of the skull was found in 2% of electrical injuries. The treatment with local rotation flaps gives good cover for the dura. The trapezius muscle flap or free flaps with microvascular anastomoses are alternative methods. Skull defects should be replaced with bone and we used autologous split grafts in three cases successfully. In order to replace hair loss, hair pieces and wigs were used. In some cases local flaps can introduce hair bearing skin.

The authors report on a case of subungual malignant melanoma, which developed beneath a split-skin graft. The patient had an accident which caused a pathologic growth of the nail. Eleven years later the patient removed the nail, because it became moist, and a surgeon grafted the nail-bed with a split-thickness skin. Six years later we saw a 2 X 2 cm tumor under the skin graft and histologic examination showed a nodular malignant melanoma. Diagnostic and therapeutic guidelines for subungual and acral melanomas are discussed.

Under exposure treatment of 33 burn patients with povidone-iodine (polyvinylpyrrolidon-iodine) preparations (Betaisodona ointment and solution) massive absorption of the iodine by the body and temporary changes in the thyroid hormones (T3 and T4 lowered, TSH raised) were found. No other clinical or laboratory-chemical side effects were observed in the patients. TRH stimulation test was carried out on 18 patients 3-6 months after ceasing povidone-iodine application. The results showed normoreaction of the hypothalamus-pituitary regulation. Povidone-iodine topical treatment of the burn patients involves them in substantial exposure to iodine, but offers no additional risk to the patients.
