[In-vitro-fertilization: chromosome analysis of unfertilized human oocytes].

Chromosomal abnormalities and abnormal embryonic development have been observed after human in vitro fertilisation (IVF). Such investigations are not permitted according to the ethical guidelines approved by the IVF team in Berlin ("Berliner Modell"). However, the chromosomal status was studied in oocytes which remained unfertilised in a human IVF programme. In 50 fertilisation attempts the chromosomes of 62 oocytes could be analysed. 45 of the oocytes were in the process of meiotic maturation. One of these showed abnormalities in the first meiotic division and seven during the second meiotic division. After fertilisation in addition to normal embryos, all of which were transferred to the mother, three embryos were triploid and six oocytes were fragmented. The results prove that in human IVF chromosome abnormalities can occur in oocytes already before fertilisation. The importance of the results for later stages of embryonic development is discussed.

[Pregnancy, labor and the puerperium in women prisoners].

In 43 imprisoned women in Berlin, the authors investigated the situation in which imprisoned women find themselves when they are pregnant, with particular reference to pregnancy, delivery and puerperium. A control group of 172 women was examined parallel to this study. It was evident that the imprisoned women had significantly more complications with regard to the course of their pregnancy and delivery, including the condition of the newborn. An analysis of the individual data yielded, for the majority of the women, a worsening of their primarily poor social and mental condition, i.e. an aggravation caused by the imprisonment. It was the aim of this study to formulate, first of all, appropriate suggestions for lowering infantile and maternal morbidity in this risk group. One of the factors examined in this connection was a possible overall change in law enforcement (imprisonment) practice (for example, suspension of imprisonment according to paragraph 455/455a, 3 of the Federal German Code of Criminal Procedure, 1981, or rescission of imprisonment). Secondly, the authors recommend more intensive care during pregnancy and delivery, taking into account the experiences collected and presented in this article.

[Pregnancy, labour, and puerperium in heroin addicted women, with reference to experience and the present state of knowledge (author's transl)].

The mounting problems of drug addiction now also confront the obstetrician and pediatrician. 1/3 of the 60,000 addicts to opiates in the Federal Republic of Germany are women. Of these 80% are in the reproductive age group. Despite endocrine problems pregnancies do occur and are high risk pregnancies because of hepatitis, venereal disease, malnutrition, phlebitis, abscesses, premature deliveries, premature rupture of the membranes, malpresentations, dysmaturity, pre-eclampsia, and numerous other social and psychological problems. Because of the varying content of heroin on the black market the fetus runs the risk of acute overdose or withdrawal. Withdrawal results in extremely marked fetal movements, with increased oxygen consumption and a danger of intra-uterine asphyxia sometimes resulting in intra-uterine fetal death. Most newborns develop a withdrawal syndrome with irritability, high pitched cries, shivering, tachycardia, perspirations, fever and generalized seizures. The experience of the Department for Women for the Free University in Berlin-Charlottenburg are presented and compared to a review of the literature. The medical, social, and psychological aspects of the problem are discussed.

[The psychosomatic foundations for the development of ambulatory deliveries (author's transl)].

The psychosomatic aspects of obstetrics received a scientific foundation by new investigations. This had a positive effect in obstetric departments by the establishment of family centered rooming-in services and the early post-partum contact between mother and infant. The psychology demand for a return to domiciliary deliveries appears to be exaggerated. The realistic evaluation of home deliveries shows that the physical risks of home deliveries are to high. A practical intermediary method of delivery is the ambulatory delivery with delivery in the hospital under an emotional home type environment in a birthing room.

Unoccupied binding sites for oestradiol in nuclei from human breast carcinomatous tissue.

The binding of oestradiol to a nuclear fraction extracted from human breast carcinomatous tissue was demonstrated. The material, which was extracted with KCl, sedimented at 3--4S and bound oestradiol with high affinity (dissociation constant approximately 2 X 10(-10) mol/l). Oestriol, diethylstilboestrol and 5 alpha-dihydrotestosterone (100-fold excesses) competed with [3H]oestradiol for the binding sites (binding inhibited by 89 +/- 8 (S.D.), 92 +/- 6 and 57 +/- 8% respectively), whereas progesterone and cortisol (100-fold excesses) did not (binding suppressed by 5 +/- 5 and 2 +/- 3% respectively). Similar competition patterns were found for cytoplasmic material which bound oestradiol. The binding occurred at 4 degrees C and was therefore considered to be a measure of the amount of binding material unoccupied by endogenous oestrogen, Unoccupied binding sites for oestradiol in the nucleus and cytoplasm were measured in 35 samples of breast carcinomatous tissue using sucrose gradient centrifugation. In 17 out of 35 tumorus, unoccupied nuclear and cytoplasmic 8S and 4S binding sites could be detected. Three out of 35 tumours contained unoccupied nuclear binding sites and 4S cytoplasmic binding sites. Nuclear binding sites only were found in two out of 35 tumours. Unoccupied nuclear binding sites were not detected in 13 out of 35 tumours and ten of these tumours also did not contain unoccupied cytoplasmic binding sites.

[Experimental study concerning the psychological preparation to parturition (author's transl)].

The following study reports an application to primae gravidae of two different psychological treatments aiming at reducing the fear of childbearing and facilitating the delivery. The treatments under comparison were systematic desensitization and modification of self instruction, a more cognitive approach. The effects of the two treatments proved to be identical; nevertheless there were significant differences between the results of the women who underwent treatment and those of the control group. Moreover, in our study a certain number of socioeconomic factors turned out to be correlated with the treatment effects.
