[What helps in back pain? Guideline for symptomatic therapy].

Both in acute and chronic, unspecific back pain, the myofascial pain syndrome resulting in muscular dysbalance is a major factor. For the differential diagnosis, however, consideration must always be given to concomitant symptoms (neurological deficits, general symptoms, signs of osteopathy). Pathophysiologically, the active trigger point corresponds to a contraction in the muscle fibers that forms in the region of the a neuromuscular endplate, and leads, via biochemical processes, to the stimulation of mesochymal nociceptors. Symptomatic treatment of acute and chronic back pain may be broken down into a) physical measures, b) local therapeutic regimens, c) systemic pharmacotherapy. As medication, non-steroidal antiinflammatory drugs, non-opioid analgesics, opioid analgesics, muscle relaxants and antidepressives are available, and are dose-matched to the severity and stage of the condition. The spectrum of therapeutic options is outlined.

[Fibromyalgia].

The classification of fibromyalgia is based on the criteria of the American College of Rheumatology. For diagnostic reasons autonomic disturbances and mental features have to be considered. The distinction between fibromyalgia (tender points) and myofascial pain syndrome (trigger points) is essential. Internal and neurological disorders as a primary cause of fibromyalgia have to be excluded. The aetiology and pathogenesis of fibromyalgia still remain uncertain. The myopathological patterns in fibromyalgia are non-specific: type-II-fiber-atrophy, a slight increase in lipid droplets, a proliferation of mitochondria and a slightly elevated incidence of ragged red fibers. Biochemically alterations of the serotonin system and high levels of substance P in the cerebrospinal fluid of fibromyalgia patients are important. Animal experiments showed that the central stimulation by nociceptor input from muscles is exaggerated in skeletal muscle pain conditions, suggesting central hyperexcitability. The diagnosis of fibromyalgia requires a thorough exclusion of other rheumatologic and neurologic disorders. The differential diagnosis is complicated by an overlap to other chronic somatoform pain disorders.

[Color duplex ultrasound imaging of renal arteries and detection of hemodynamically relevant renal artery stenoses].

AIM: Although colour coded Doppler ultrasound (CCDU) is established as a non-invasive diagnostic tool for detecting renal artery stenoses (RAS), no uniform criterion for defining a hemodynamically relevant stenosis (i.e. angiographic stenosis of > or = 70% of the diameter) exists. We therefore investigated the predictive value of the renal-aortic flow-velocity ratio (RAR) > 3.5 m/s and a difference of < 0.05 between the left and right side for the resistance index according to Pourcelot (dRI) in the detection of a relevant RAS. PATIENTS AND METHODS: We analysed 500 consecutive CCDU examinations of patients with hypertension retrospectively. An RAR > 3.5 and/or a lateral inequality of the RI < 0.05 were used as stenosis criterion. RESULTS: In 448 patients (90%) both renal arteries could be found, in 11 patients (2%) only the right artery, in 6 patients (1%) the left artery, and in 35 patients (7%) no renal artery was detectable. In 98 patients (19.6%), RAS was diagnosed, 69 (71%) of them underwent angiography. 38 patients presented an RAR > 3.5 plus dRI > 0.05. In 29 of these, angiography was performed. 96% of them presented with an RAS of > or = 70% and 4% showed an RAS of 40-69% (specificity 97%, sensitivity 76%). In 54 patients the RAR was > 3.5, but dRI < 0.05. 24% of the patients undergoing angiography (n = 37) presented with an RAS of > or = 70%, 68% with an RAS of 40-69%, and 8% with an RAS of < 40% (specificity 60%, sensitivity 100%). 44 hypertensive patients who underwent angiography after a CCDU examination not suggesting the presence of RAS were used as control group. CONCLUSION: An experienced physician using a high quality colour-coded duplex-machine can reliably detect the renal arteries. The presence of RAS can be diagnosed with certainty by CCDU applying the criterion of RAR > 3.5, but the diagnosis of a one-sided haemodynamically relevant RAS can only be certain if the criterion of dRI > 0.05 is used in addition.

Short-term treatment of primary fibromyalgia with the 5-HT3-receptor antagonist tropisetron. Results of a randomized, double-blind, placebo-controlled multicenter trial in 418 patients.

We investigated the efficacy and tolerability of short-term treatment with tropisetron, a selective, competitive 5-HT3-receptor antagonist in fibromyalgia. The trial was designed as a prospective, multicenter, double-blind, parallel-group, dose-finding study. We randomly assigned 418 patients suffering from primary fibromyalgia to receive either placebo, 5 mg, 10 mg or 15 mg tropisetron once daily for 10 days. Clinical response was measured by changes in pain score, visual analog scale, tender point count and ancillary symptoms. Responders were prospectively defined as patients showing a 35% or higher reduction in pain score. Treatment with 5 mg tropisetron resulted in a significantly higher response rate (39.2%) than placebo (26.2%) (p < 0.05). In the visual analog scale, the group administered 5 mg tropisetron showed a significant improvement (p < 0.05) and the group administered 10 mg tropisetron showed a nonsignificant clinical benefit. The number of painful tender points was significantly reduced (p = 0.002) in the 5 mg tropisetron group. Regarding ancillary symptoms, the 5 mg tropisetron group showed a significant improvement (p < 0.05) in sleep and dizziness. The patients' overall assessment of efficacy was significantly higher for 5 mg (p = 0.016) and 10 mg (p = 0.002) tropisetron than for placebo. The safety and tolerability of tropisetron was good; gastrointestinal tract symptoms were the most frequently reported adverse events. Short-term treatment of fibromyalgia patients with 5 mg tropisetron for 10 days proved to be efficacious and well tolerated. In this study a bell-shaped dose-response curve was seen.

Efficacy and tolerability of tropisetron in primary fibromyalgia--a highly selective and competitive 5-HT3 receptor antagonist. German Fibromyalgia Study Group.

OBJECTIVE: Based on a potential role for serotonin receptors in fibromyalgia, we investigated the efficacy and tolerability of treatment with tropisetron, a highly selective, competitive inhibitor of the 5-HT3 receptor. METHODS: In this prospective, multicenter, double-blind, parallel-group, dose-finding study, 418 patients suffering from primary fibromyalgia (ACR criteria) were randomly assigned to receive either placebo, 5 mg, 10 mg or 15 mg tropisetron once daily, respectively. The duration of treatment was 10 days. The clinical response was measured by changes in pain-score, visual analog scale (VAS), and the number of painful tender-points. RESULTS: Treatment with 5 mg tropisetron resulted in a significantly higher response rate (39.2%) when compared with placebo (26.2%) (p=0.033). The absolute reduction in pain-score was -13.5% for 5 mg tropisetron, -13.0% for 10 mg tropisetron, and -6.3% for placebo (p<0.05). The effects of 15 mg tropisetron were similar to placebo, thus suggesting a bell-shaped dose-response curve. Compared with placebo, treatment with 5 mg tropisetron led to a significant improvement (p<0.05) in VAS, while a clear trend in terms of clinical benefit was seen with 10 mg tropisetron. The number of painful tender-points was also reduced significantly (p=0.002) in the 5 mg tropisetron group. Of interest, during the 12-month follow-up period, pain intensity of responders on 5 mg and 10 mg tropisetron was still markedly below baseline. The treatment was well tolerated, with gastro-intestinal complaints being the most frequently reported side effects, in keeping with the known safety profile for 5-HT3 receptor antagonists. CONCLUSIONS: This study demonstrates the efficacy of short-term treatment with 5 mg tropisetron once daily in primary fibromyalgia. Treatment was well tolerated and prolonged clinical benefits were seen.

Oral treatment of fibromyalgia with tropisetron given over 28 days: influence on functional and vegetative symptoms, psychometric parameters and pain.

The 5-HT3 receptor antagonists are a novel therapy for patients suffering from fibromyalgia, although the optimal duration of treatment is still unclear. The objective of this phase II study was to evaluate whether prolonging treatment with tropisetron to 4 weeks is tolerable and correlated with an improved clinical benefit. Thirty female patients with fibromyalgia received oral tropisetron (5 mg) daily for 28 days in an open-label fashion. Treatment resulted in significantly decreased pain as measured by visual analog scale (VAS), with a mean reduction of 59.7% and an absolute median change of -25.0 from baseline to day 28 (p<0.0001). A similar, significant reduction of 55.7% and absolute median change of -31.0 was observed in the painscore (p<0.0001). The response rate with patients showing a > or = 35% reduction in individual pain scores was 72.4% at day 28. The pressure tolerance of tender-points was slightly increased at the end of the treatment period. In addition, significant improvements were observed in the State-Trait-Anxiety-Inventory (STAI), scales of von Zerssen (Bf-S) and Beck Depression Index (BDI). Functional symptoms were compared with the results from a 10-day, randomized, double-blind phase III study of tropisetron in 418 fibromyalgia patients. In both studies several functional symptoms such as sleep disturbances and dizziness improved significantly (p<0.05). In the 28 days study, the number and extent of improvement in functional symptoms was increased compared with the shorter trial. Tolerability and safety of tropisetron was good, and typically for 5-HT3-receptor antagonists, gastrointestinal symptoms and headache were the most frequently reported events. In conclusion, 28 days treatment of fibromyalgia patients with 5 mg tropisetron resulted in significant pain reduction, which was most pronounced after 10 days with a further reduction up to day 28. Psychometric tests showed significant improvements in depression and anxiety state scores, while functional symptoms improved with extended tropisetron treatment.

Treatment of chronic fatigue syndrome with 5-HT3 receptor antagonists--preliminary results.

OBJECTIVE: The serotonin system presumably is involved in the pathogenesis of chronic fatigue syndrome (CFS). Results from a few studies led to the hypothesis of a "postsynaptic hyperresponsiveness" in CFS. Therefore we intended to evaluate the efficacy of 5-HT3 receptor antagonists in the treatment of CFS. PATIENTS AND METHODS: 2 patient groups (10 patients each; CFS according to the CDC classification criteria) received either oral tropisetron (5 mg once daily) or oral ondansetron (2 x 8 mg daily), open-labelled. Treatment duration was 15 days. Treatment response was evaluated by visual analog scales (VAS) for fatigue and capability. RESULTS: 19 patients finished their respective study. In the tropisetron group 6/9 (VAS fatigue) and 7/9 (VAS capability) patients documented benefit, 8/10 rsp. 8/10 patients in the ondansetron group. The score changes (VAS before and after treatment) in case of response were more pronounced in the tropisetron group. The frequency of concomitant symptoms did not differ significantly in the treatment groups. The overall analysis of both studies showed a remarkable improvement (> or = 35%) of approximately one third of the patients in both VAS. Treatment was well tolerated. CONCLUSION: Our preliminary results encourage to perform placebo-controlled, double-blind studies to further evaluate the efficacy of 5-HT3 receptor antagonists in the treatment of CFS.

Relationship of substance P, 5-hydroxyindole acetic acid and tryptophan in serum of fibromyalgia patients.

The serotonergic system has repeatedly been discussed to be involved in the pathophysiology of fibromyalgia (FM), which is a syndrome of widespread pain and sleep disturbance. Elevated levels of substance P (SP), a mediator of nociception, have been described in FM. In this study the possible relationship between SP and serotonin (5-HT) together with its precursor tryptophan (TRP) and its metabolite 5-hydroxyindoleacetic acid (5-HIAA) was evaluated in 51 serum samples of fibromyalgia patients. These parameters were compared with clinical data such as pain intensity or sleep quality. A strong negative correlation between SP and 5-HIAA (P = .000) as well as between SP and TRP (P = .009) could be demonstrated. High serum concentrations of 5-HIAA and TRP showed a significant relation to low pain scores (5-HIAA: P = .030; TRP: P = .014). Moreover, 5-HIAA was strongly related to good quality of sleep (P = .000), while SP was related to sleep disturbance (P = .005). These data are valid to support the hypothesis of a systemic involvement of 5-HT and SP in fibromyalgia.

Quantitative immunohistochemical analysis of the glutathione S-transferase GSTM1: in situ phenotyping in archival material.

1. GSTM1 is present in only approximately 50% of Caucasian individuals and deficiency of GSTM1 is associated with susceptibility to a growing number of diseases, especially cancer. Thus, a method that would allow accurate, retrospective determination of the GSTM1 phenotype in different patient populations would have many applications. 2. Developed, therefore, is a quantitative, image-analysis-based immunohistochemical technique for the analysis of GSTM1 protein in paraffin-embedded tissue samples. It was applied to the determination of the GSTM1 phenotype using liver biopsies taken from 70 patients. 3. Of the 70 cases (depending on the cut-off point), 51-54% were deficient in GSTM1. A single 27 kD band characteristic for GSTM1 was found in seven of 16 cases analysed by Western blotting using the same GSTM1 antibody as in the immunohistochemical analysis. There was a good correlation (r = 0.87) between the staining intensity of the GSTM1 band and the staining intensity evaluated by immunohistochemistry. 4. It is concluded that this quantitative immunohistochemical method permits accurate determination of the GSTM1 phenotype and is well suited for retrospective analysis of GSTM1 expression in specific tissues in situ.

[Myofascial pain syndrome--frequent occurrence and often misdiagnosed].

Myofascial pain syndrome (MPS) is a very common localized--sometimes also polytopic--painful musculoskeletal condition associated with trigger points, for which, however, diagnostic criteria established in well-designed studies are still lacking. These two facts form the basis for differentiating between MPS and the fibromyalgia syndrome. The difference between trigger points (MPS) and tender points (fibromyalgia) is of central importance--not merely in a linguistic sense. A knowledge of the signs and symptoms typically associated with a trigger point often obviates the need for time-consuming and expensive technical diagnostic measures. The assumption that many cases of unspecific complaints affecting the musculoskeletal system may be ascribed to MPS makes clear the scope for the saving of costs.

Morphologic aspects of fibromyalgia.

The most common morphological finding in muscle biopsies in longstanding fibromyalgia is type II fiber atrophy. This can be found in many other conditions such as disuse atrophy, affections of the corticospinal tracts, steroid atrophy, and other different neuromuscular disorders. An increase in lipid droplets and a slight proliferation of mitochondria in type I muscle fibers are correlated with the duration of fibromyalgia. In some cases we could find some ragged red fibers (RRF) which histochemically show a pronounced accumulation of lipids and mitochondria and single fiber defects of cytochrome-c-oxidase. In some fibromyalgia patients with RRF, we could find deletions of the mitochondrial genoma.

Sjoegren's syndrome: MR imaging of the parotid gland.

To date, myoepithelial sialoadenitis (Sjoegren's syndrome) has been diagnosed with sialography and other techniques. First results of MR imaging offer new possibilities in the diagnostic imaging of this disease. Thirty-six patients with immunohistologically and serologically confirmed Sjoegren's syndrome and 25 patients suffering from other diseases, included as a control group, were examined by MR in transverse and coronal orientation. T-2-weighted sequences (TR/TE 1600/25/90) and T1-weighted sequences (TR/TE 500/25 ms) plain and after Gd-DTPA administration were obtained. In all patients the parotid gland showed characteristic internal patterns and abnormalities in gland size. There was a nonhomogeneous internal pattern with a characteristic speckled, honeycomb-like appearance visible especially on T2-weighted sequences. Enhancement with Gd-DTPA yielded no additional information. A staging system with four stages of Sjoegren's syndrome (no characteristic changes to a nodular and swollen gland) was developed. Magnetic resonance has become an important new tool in assessing parotid gland changes in patients suffering from Sjoegren's syndrome, and could well replace the more invasive and unpleasant diagnostic methods in the near future.

Arginine vasopressin and oxytocin increase intracellular calcium and cAMP in human glomerular epithelial cells in culture.

The signal transduction linkages of arginine vasopressin (AVP) and oxytocin receptors were investigated in human glomerular epithelial cells (GEC) in culture. AVP (ED50, 10(-7) mol/l) and oxytocin (ED50, 3 x 10(-8) mol/l) induced a rapid, transient and dose-dependent increase in [Ca2+]i as detected by fura-2 microfluorimetry. The baseline of [Ca2+]i in human GEC was 109 +/- 2.8 nmol/l (n = 60). The V1a receptor antagonist [d(CH2)5(1), Tyr(Me)2, Arg8]-vasopressin inhibited the AVP-(IC50, 5 x 10(-9) mol/l) and oxytocin-induced (IC50, 3 x 10(-8) mol/l) increase in [Ca2+]i in a dose-dependent manner. Both, AVP and oxytocin caused accumulation of cAMP. The AVP-stimulated cAMP increase was blocked by pretreatment of human GEC with the V1a receptor antagonist (10(-7) mol/l), whereas the oxytocin-induced cAMP accumulation remained uninfluenced. In conclusion the present results indicate that: (1) V1a receptor activation, AVP and oxytocin induce a transient elevation in [Ca2+]i in human GEC; (2) AVP and oxytocin cause cAMP accumulation; (3) the AVP-induced cAMP accumulation is inhibited by a V1a receptor antagonist, whereas (4) the oxytocin response showed no effect. In addition, a different receptor might be possible, at least in oxytocin-induced-cAMP accumulation.

[T-cell lymphoma under immunosuppressive treatment in minimal change glomerulopathy with nephrotic syndrome].

HISTORY AND CLINICAL FINDINGS: A 43-year-old woman was admitted for treatment of a nephrotic syndrome with increasing oedema, ascites, dyspnoea and nausea during the preceding 2 weeks. INVESTIGATIONS: There was a proteinuria of 25 g daily. Renal biopsy (seven glomeruli) showed a minimal-change glomerulopathy. TREATMENT AND COURSE: Prednisone administration (1 mg/kg daily) achieved complete remission. In the course of this treatment the patient contracted listeriosis with encephalitis, which quickly responded to ampicillin and netilmicin. When the glucocorticoid dosage was reduced or the drug discontinued, the nephrotic syndrome recurred. Cyclosporin treatment was begun (5 mg/kg daily) and quickly led to another remission. 4 weeks after start of the treatment a reddish-blue nodule, about 2 cm in diameter, was noted on the right upper arm. Histologically it showed cutaneous infiltration of a medium-sized T-cell lymphoma. Other efflorescences were found on the trunk and the limbs. Cyclosporin treatment was at once discontinued and the cutaneous changes gradually regressed. There were no other manifestations of a lymphoma. A further recurrence of the nephrotic syndrome responded well to a short course of chlorambucil (cumulative dose 420 mg) and of prednisone with slow dose reduction. There has been no recurrence in the last year.

Regulation of phosphoinositide hydrolysis and cytosolic free calcium induced by endothelin in human glomerular epithelial cells.

The regulation of the inositide signalling pathway and [Ca2+]i by endothelin (ET) peptides was investigated in human glomerular epithelial cells in culture. Endothelin-1 and -2 induced an accumulation of inositol phosphates in a time- and dose-dependent manner. The baseline of [Ca2+]i in glomerular epithelial cells was 109 +/- 2.8 nmol/l, n = 60. Endothelin-1 (ED50: approx. 3 x 10(-9) mol/l) caused a rapid and transient rise in [Ca2+]i as detected by fura-2 microfluorimetry studies. The endothelin-1-induced inositol phosphate accumulation was inhibited by the selective ETA receptor antagonist BQ123. Endothelin-3 and BQ3020, a selective ETB receptor agonist, showed no effect. The results suggest an ETA-mediated pathway. This study demonstrates an ETA-mediated transmembrane signalling via phospholipase C with consecutive elevation of inositol phosphates and intracellular calcium. Since endothelin peptides contribute to both normal renal function and renal dysfunction, this study adds further knowledge on glomerular cell regulation.

Impaired function of platelet membrane glycoprotein IIb-IIIa in end-stage renal disease.

Impaired platelet function and a bleeding tendency are well-recognized complications of chronic renal failure. Because the fibrinogen receptor GPIIb-IIIa plays a central role in platelet aggregation and adhesion to the subendothelium, it was reasoned that a defect in this receptor may underlie the impaired platelet function in uremia. To test this hypothesis, the function of this receptor in the platelets of 11 uremic patients was studied. Aggregation studies were performed with flow cytometric techniques with anti-GPIIb-IIIa conformation-specific monoclonal antibodies (mAb) (anti-LIBS1 and anti-PMI-1). Antifibrinogen and antithrombospondin mAb were used to characterize fibrinogen binding to GPIIb-IIIa and the release of alpha-granules, respectively. Platelets from patients with chronic renal failure showed significantly decreased binding of conformation-dependent anti-LIBS1 mAb after ADP, phorbol myristate acetate, or RGD-peptide stimulation compared with normal controls, suggesting a defect related to the ability of the fibrinogen receptor to undergo a conformational change. Moreover, antifibrinogen and antithrombospondin binding to activated platelets were reduced in uremic patients, implying impairment of both ligand-binding and alpha-granule release. Hemodialysis partially restored GPIIb-IIIa function, which may account for the observed effects of this therapy in restoring platelet aggregation. These findings indicate that platelets of patients with chronic renal failure reveal an aggregation defect at least partially due to an intrinsic GPIIb-IIIa dysfunction and the presence of a putative uremic toxin that inhibits fibrinogen binding to GPIIb-IIIa.
