[Mucinous adenocarcinoma of the urachus and peritoneal pseudomyxoma].

The adenocarcinoma of the urachus is very rare tumor, with an incidence of 1/5.000.000 inhabitants, represents less than 0.001 of all types of bladder cancer. Peritoneal pseudomixoma is a rare neoplasm characterized by mucinous acites that involvement the peritoneal surface and omentum. Usually is associated with benign o malignant mucinous tumor of the appendix or ovary. In this paper, we present a case of peritoneal pseudomixoma caused by a mucinous adenocarcinoma of the urachus.

[Adrenal ganglioneuroma. Report of a new case].

Ganglioneuroma is a benign neoplasm arising from neural crest tissue and is composed of mature ganglion cells and Schwann's cells. Most ganglioneuromas arise in the posterior mediastinum followed by the retroperitoneum. Only a small proportion of ganglioneuromas are adrenal in origin and occur most commonly in children and young adults. Characteristically, ganglioneuromas do not secrete excess catecholamines or steroid hormones, and they are usually clinically silent lesions incidentally detected in imaging studies for unrelated reasons. Because it is an uncommon lesion, adrenal ganglioneuromas is not included in the differential diagnosis of adrenal incidentalomas in adults. We presents a new case of a 6.5 cm adrenal ganglioneuroma incidentally diagnosed in a 50-year old male patient. The exeresis of the adrenal mass and pathohistological study confirmed the diagnosis.

[Lumbar eventration as complication of the lumbotomy in the flank: review of our series].

OBJECTIVES: To study the prevalence and risk factors of the lumbar eventration after lumbotomy in the flank. MATERIAL AND METHODS: Survey of 230 patients that underwent lumbotomy in the flank, with a least followup of two years. Statistical study: contingency 2 x 2 tables (Pearson's chi 2 or Fisher exact test) to compare qualitative variables, Student's test to compare quantitative variables, Kaplan-Meier test and comparison of Log-rank curves. For the study of clinical variables with more influence in postoperative lumbar eventration, a logistic regression model was used. RESULTS: The prevalence of postoperative lumbar eventration was 31.3%. Clinical variables more influence for postoperative lumbar eventration are: age, deficit of proteins, wound infection and haematoma of the abdominal wall. CONCLUSIONS: Eventration is one the most frequent complications in urologic surgery and is of primary importance due to its laboral, economic and social implications.

[Ureteroscopy as diagnostic and treatment method in ureteral processes: our experience].

Endoscopic approach to the ureter both as a diagnostic procedure in various diseases and treatment of low ureteral lithiasis is a widely used technique. Review of case reports where this technique was used over the last four years. A total of 73 ureteroscopies were conducted, 64 as therapeutic approach (89.04%) and 9 for diagnosis (10.96%) achieving an overall 94.4% success rate. Multiple variables such as type and location of the stones, patient's age and sex, diagnostic methods, length of stay etc. All the above data were used for a descriptive analysis of the situation in our centre. In our experience, ureteroscopy is a good therapeutic option for low ureteral lithiasis, mainly when no extracorporeal lithotrity is available, and an excellent diagnostic tool in other ureteral processes when more traditional methods fail.

[Transrectal ultrasonography in the assessment of patients after radical prostatectomy. Normal and pathologic ultrasonography anatomy of vesico-urethral anastomosis].

OBJECTIVE: To determine the ultrasound characteristics of the vesico-urethral anastomotic complex in patients with radical prostatectomy to separate normal prostate fossae from those affected by local recurrence or residual tumour. MATERIAL AND METHODS: Between January 1998 and June 1999 15 ultrasound guided transrectal biopsies of vesico-urethral anastomosis or prostate fossae were performed in patients with radical prostatectomy. The selection criteria to perform puncture in these patients were a negative extension study (abdominal-pelvic CT and bone scan), PSA higher than 0.4 ng/mL and/or suspicious DRE. The study was completed with 10 transrectal ultrasound (TRU) after radical prostatectomy in patients with normal PSA and DRE. TRU parameters in patients with tumour-positive biopsies were compare to those from patients with negative biopsies and those obtained from TRU in patients with both normal PSA and normal DRE. RESULTS: The ultrasound parameters that best described prostate fossa abnormalities were the presence of hypoechoic masses or nodes and the integrity or not of the retro-anastomotic fat layer. When a hypoechoic mass or node was found at the anastomosis level, 80% biopsies were positive for tumour local recurrence. The percentage increases when changes are seen at the retro-anastomotic fat layer. CONCLUSIONS: In spite of a good correlation between ultrasound abnormalities and positive biopsies, ultrasound findings from the vesico-urethral anastomosis in patients with radical prostatectomy are not well defined. Nodes or ultrasound irregularities can exist in normal prostate fossae as well as normal anastomosis ultrasound in the presence of tumoral relapses. We believe TRU of vesico-urethral anastomosis to be the best method available to evaluate local recurrence or tumoral persistence after radical prostatectomy.

[Congenital bladder diverticulum and Ehlers-Danlos syndrome: an unusual association].

The existence of a vesical diverticulum in the context of a congenital connective tissue disorder such as Ehlers-Danlos syndrome led us to consider the possibility of a relationship. Four types of diverticula can be found in the literature: congenital, acquired, iatrogenic and syndrome-associated. Within the later, Ehlers-Danlos syndromes type IV and IX, even type V, are associated to the existence of vesical diverticula. The potential spontaneous rupture of the diverticulum is a typical feature, as well as post-surgery relapse. The attitude towards such diverticula should be one of watchful waiting, and simple, plasty-free diverticulectomy on the bladder's neck is indicated when performing a surgical procedure.

[Mullerian-type papillary serous tumor: exceptional pathology of the testis. Report of a case and discussion].

Within the exceptional tumoral pathology of the testis and paratesticular region are the common epithelial type tumors. Although, its histogenesis is under discussion, it has been interpreted as arising from the remnants of the müllerian duct, or from the mesothelium of the tunica vaginalis differentiating in a müllerian direction. Similarities with homologous ovarian tumors are well recognized and in generally we accept a good prognostic due to its low malignant potential or borderline type, more experience with these uncommon lesions is necessary to evaluate their biologic potential.

[Transitional carcinoma of the prostate].

Case report of prostate transitional carcinoma diagnosed by transurethral resection, an infrequent tumour with incidence ranging from 1% to 4% of all prostate neoplasias which appears in 2.8% of all radical cystoprostatectomies specimens carried out at the Mayo Clinic. Most commonly, when it appears in the prostate this tumour is simultaneous or subsequent to other transitional carcinoma arisen in other organs mostly the bladder. Very rarely it occurs as an early form within the ducts and even less often within the prostate acinus. Review of the neoplasia histology, signs and symptoms, diagnostic procedures and management, emphasising the aggressiveness of its behaviour (such as in our case report) when the prostate gland stroma is infiltrated by the tumour.

[Testicular tumors: our experience].

OBJECTIVE: To report our series of testicular tumors diagnosed from June 1990 to June 1998. The diagnostic protocol, treatment and follow-up are described and data on patient survival over the 8-year period are presented. METHODS: Our series comprised 21 testicular tumors; 19 germ cell and two non-germ cell tumors. There were 5 seminoma, three pure embryonary carcinoma, two mature cystic teratoma, 9 mixed germ cell tumor and two lymphoma. Treatment was by orchidectomy plus radiotherapy, chemotherapy and some cases were closely followed. RESULTS: All 5 patients with seminoma are currently tumor-free, for a mean survival of 38.6 months. Of the 14 non-seminomatous tumors, there were two deaths; the remaining patients are in complete remission, for a mean survival of 37.5 months. The overall survival rate was more than 95% at a mean follow-up of 48 months. CONCLUSIONS: Testicular tumor is one of the most common tumors in young male patients; germ cell tumors are the most frequent type. The different therapeutic regimens currently available have achieved a 5-year survival rate of more than 95%.

[Melanoma of the female urethra: a very infrequent tumor].

Case report of a melanoma of the urethra in a 75-year old female patient. A few months after diagnosis the patient died of multiple lung metastasis. This is a review of the clinical and diagnostic aspects of the neoplasia highlighting the therapeutical options that albeit debatable seem to be quite clear as to the master lines to follow.

[C.F.C.-type ureterosigmoidostomy. Results].

This paper presents the results obtained with a C.F.C. type ureterosygmoidostomy technique described by us. A total of 25 patients aged between 52 and 74 years (mean age, 65 years), 23 (23/25) male and 2 (2/25) female were evaluated. 24 of the total 25 patients had infiltrant neoplasia of the bladder (pT2: 8; pT3a: 12; pT3b: 4) which was graded as moderately differentiated (G2: 10) or undifferentiated (G3: 14). The remaining patient, a female, had tuberculous microbladder. Male patients underwent radical cystoprostatectomy (23/25); while in two females traditional cystectomy was performed (2/25). Patients with neoplasia of the bladder (24/25) were performed bilateral lymphadenectomy prior to radical surgery. All 25 patients were performed C.F.C. type ureterosygmoidostomy (Actas Urol Esp 20: 324, 1996). Follow-up of patients was carried out over a mean period of 27 months (July 1994-October 1997). The complications reported were 4 cases of ureterointestinal stricture and one stercoral fistula. The strictures were resolved with endoscopic techniques and the fistula through discharge colostomy. Death (6/25) occurred as a result of the tumour progression, and in no case was related to the surgical technique. All patients showed daytime continence (100%), and 22/25 were also continent during the nighttime (88%): there were occasional leaks in 5/25 (20%). No metabolic disorders were seen in any of the patients. (All patients were given drug therapy to prevent metabolic acidosis).

[Renal adenocarcinoma in a girl 13 years old].

Case report of a renal adenocarcinoma in a 13 year-old girl found during the study of a painless, gross haematuria of several months evolution. Apart from a review of signs and symptoms, diagnosis, therapeutical approach and other issues related to this tumour, the authors insist on the significance gross haematuria has in children presenting symptoms that should alert the medical profession about a possible neoplasia of this features, in spite of its rare occurrence.

[Bladder hemangioma: a clinical case].

Contribution of a new case of vesical haemangioma in a 65-year old male. This is a benign and very rare tumour with not even 90 cases described in the urology literature to date. Haematuria was reported in all cases. This paper presents a review of the literature and a discussion of clinical and pathological features as well as diagnostic methods and recent changes in management approach.

[Primary lymphoma of the testicle].

Case report of a testicular lymphoma. A diagnosis of primary neoplasia of the testis was made since no involvement to the lymphoreticular system in any other organ was demonstrated. We review the clinical, diagnostic and therapeutical aspects and make a note on the rarity of our case, a type T tumour, due to the exceptional nature of this variety within primary lymphomas of the testis.

[Percutaneous nephrostomy as management technique of obstructive uropathy: review of 105 cases].

105 percutaneous nephrostomies in 73 patients. Indication for nephrostomy was obstruction in the drainage system in all cases. Nephrostomy was bilateral in 24 cases. Reasons for nephrostomy were: lithiasis in 35 cases, carcinoma of the bladder in 17, post-surgical iatrogenic stenosis in 13, prostate carcinoma in 9, cancer of the rectum in 9, infection in 5, neoplasia of the upper urinary tract in 5, retroperitoneal fibrosis in 3, glandular cystitis cystica in 3, ovarian cancer in 3, congenital in 2, lymphoma in 1. Six case were single-kidney patients. Renal puncture through the lower calyceal group was the preferred approach. Purulent urine was extracted in 10 cases. Complications of nephrostomy included haematuria in 34 cases, contrast extravasation in 8, fever in 6, 1 case of death due to septic shock, 2 perirenal haematoma (nephrectomy was required in 1 case), and catheter detachment in 5 cases. All patients showed improvement of both clinical signs and symptoms, and lab results.

[Glandular cystic cystitis].

Cystic-glandular cystitis is considered as part of the urothelial pre-neoplastic proliferative abnormalities. This group includes atypical hyperplasia. Von Brunn's nidus, and cystitis cystica. They are a consequence of the changes experienced at the urothelium level in response to inflammation, irritation or carcinogens. Diagnosis is mainly based in the pathoanatomical study of the biopsy obtained following endoscopic resection. The signs and symptoms it presents are varied and show a clear relationship to distribution and extension of cysts. Treatment is based in the removal of irritative factors. Cystectomy with urinary by-pass may be necessary if required by clinical evolution.

[Penile cancer. Review of 18 cases].

INTRODUCTION: Squamous cell carcinoma is the most common tumor of the penis. Nodal metastases are relatively common, but distant disemmination is very rare. MATERIAL AND METHODS: From 1990 to 2002, we diagnosed and treated 18 cases of carcinoma of the penis. The minimum follow-up was 2 years (range 2-11 years; mean 49 months). Treatment of the primary lesion is usually by partial amputation of the penis, which enables us to determine the pathological stage and the histological grade of the tumor and, consequently, our approach to the regional lymph nodes. RESULTS: Palpable inguinal nodes after antibiotherapy remained in six out of seven patients. Inguinal lymphadenectomy was performed early in 4 cases. Postoperative complications were present in the 4 cases, lymphedema being the most frequent one (100%). CONCLUSIONS: The T and the histological grade of the primary lesion must be considered when deciding the approach in the management of the lymph nodes as unneccesary lymphadenectomy can be avoided and those at high risk of lymph node invasion can be treated radically and timely.
