Melanoma: latest diagnosis methods, surgical treatments and follow-up procedures.

Melanoma is a disease that is estimated will affect approximately 1 in 80 Americans. It is a potentially lethal cutaneous malignancy that requires initial diagnosis by full thickness incisional or excisional biopsy followed by appropriate surgical care. Prospective randomized studies for adjuvant therapy with intermediate (0.76-4 mm. Breslow depth), and deeper (4 mm. or greater Breslow depth) lesions will be discussed. This paper outlines an algorithm for lifelong postoperative clinical follow-up.

Suprasellar monomorphous pilomyxoid neoplasm: an ultastructural analysis.

The authors report 3 patients, 2 children and 1 adult, each of whom presented with an unusual myxoid lesion reminiscent of pilocytic astrocytoma but with other features of myxopapillary ependymoma. The magnetic resonance imaging findings in all cases showed a diffusely contrast-enhancing suprasellar mass focally extending into the third ventricle. Involvement of adjacent structures was more extensive in both infants. By light microscopy, all were composed of a monotonous population of cells with delicate piloid-like processes, loosely arranged within a prominent myxoid background. Focally, the neoplastic cells converged upon small blood vessels in pseudorosette-like formations. These histomorphologic features are identical to those of the recently described astrocytoma with monomorphous pilomyxoid features. In addition, the individual tumor cells showed strong cytoplasmic immunoreactivity with antibodies to glial fibrillary acidic protein (GFAP) and vimentin, as well as nuclear and cytoplasmic staining with S-100. All stained positive for synaptophysin and negative for chromogranin. By electron microscopy, the tumor cells were bipolar with elongated processes and apical surfaces displaying microvilli, cytoplasmic blebs and rare cilia. Vesicles and coated pits were seen, as were occasional synaptoid complexes. The current study serves to expand our clincopathologic experience with this rare and enigmatic lesion, with particular attention given to the ultrastructural characteristics.

Mapping of the tuberous sclerosis genes.

Tuberous sclerosis is a dominantly inherited genetic disorder, with a high frequency of new mutations, which has been shown through genetic linkage studies to be genetically heterogeneous. In this paper we summarize recent progress in linkage studies which indicate that there is one TSC gene on chromosome 9q34 which accounts for approximately 40% of cases and another on chromosome 16p13.3 which accounts for approximately 50% of cases. We discuss the map position of these TSC genes on chromosome 9q34 and 16p13.3. We discuss physical mapping data in these two regions. We postulate on the pathogenesis of TSC lesions and on the cause of the high frequency of new mutations. We present a strategy to progress from the mapping of the TSC genes to isolation of these genes.

Development and implementation of an instrument measuring CNCs' activities.

This report describes the development and introduction of a 'Clinical Nurse Consultant Activity Report' for use within the nursing division at a major metropolitan teaching hospital. The project involved the designing of an appropriate reporting format and definitions that could encompass all clinical nurse consultants' professional activities. Following the Report's development, an intensive education program was conducted, the Report was successfully trialled and evaluated and some minor modifications were made.

Cost reductions. Part 1: An operations improvement process.

Today's nursing leaders are commonly expected to reduce labor costs. This two-part series provides a framework for achieving significant cost reductions in the nursing labor budget. Part One describes a process that has been used to identify and implement cost savings in varied hospital settings. Part Two will address organizational culture issues that can hamper or support cost savings.

Cost reductions Part 2: an organizational culture perspective.

Part One of this series described how an Operations Improvement process, computer simulation, and a focus on the dollars per patient day or unit of service ($PPD) can enhance a hospital's cost-reduction efforts. Part Two will address the organizational culture issues that can hamper or support nursing's cost-reduction efforts.
