Effects of topical mitomycin C on primary trabeculectomies and combined procedures.

The charts were reviewed of all patients who underwent primary trabeculectomies (group 1) or combined procedures (phacoemulsification + intraocular lens implantation + trabeculectomy--group 2) and received intraoperative mitomycin C (0.4 mg/ml) between 1991 and 1992 at Wills Eye Hospital. A total of 54 eyes of 54 patients were included: 39 in group 1 and 15 in group 2. Intraocular pressure was controlled successfully in 97.4% of eyes in group 1 (mean follow up 6.7 months), and in 93.3% in group 2 (mean follow up 6.8 months). Main complications included choroidal detachment (n = 15), shallow anterior chamber (n = 9), cataract formation (n = 8), and hypotony maculopathy (n = 3). The use of mitomycin C in primary trabeculectomies and combined procedures is associated with high success rates. However, it may be associated with unacceptable risks of vision threatening complications related to excessive filtration.

Bronchial lavage and bronchoalveolar lavage in allergen-induced single early and dual asthmatic responders.

The phenotypic cellular profile of bronchial lavage (BL) and bronchoalveolar lavage (BAL) was studied in 7 single early (SR) and 10 dual asthmatic responders (DR). Lavage was performed, after previously having determined bronchial hyperresponsiveness to histamine and the response to house dust mite (HDM) challenge. The recovered lavage fluid was separated in two fractions, BL and BAL. Total fluid recovery and cell number from the BL and BAL were comparable in both patient groups. Differential cell counting and immunocytochemistry were performed. DR had a significantly higher number of eosinophils and EG2+ cells in BL but not in their BAL. No differences could be found in CD4+, CD8+, and HLA-DR+ cells. A strong correlation was found between eosinophils in the BL+ and EG2+ cells in the BL (r = 0.79, p < 0.001) and between eosinophils in the BL and peripheral blood eosinophils (r = 0.70, p < 0.0025). The number of EG2+ cells and the number of epithelial cells in both BL and BAL showed a correlation (r = 0.55, p < 0.05). Dual responders had a higher total IgE (p < 0.01), and total serum IgE correlated well with the eosinophils in the BL (r = 0.85, p < 0.0001). Our observations demonstrate cellular differences in the lung on mainly a bronchial level between single early and dual asthmatic responders. A bronchial lavage eosinophil and EG2+ cell count and higher blood total IgE level are associated with the tendency to develop a dual asthmatic response.

Limb congestion and sympathoexcitation during exercise. Implications for congestive heart failure.

During static exercise, heart failure (HF) subjects activate the sympathetic nervous system differently than normal controls. HF causes metaboreceptor desensitization with either enhanced mechanoreceptor activity or central command. In this report, we examined whether increased muscle interstitial pressure, as seen in HF, augments other neural systems. We measured muscle sympathetic nerve activity (MSNA; peroneal nerve) in 10 normals during static exercise (40% maximal voluntary grip) and posthandgrip circulatory arrest (PHG-CA). This was repeated after venous congestion (VC; cuff inflation to 90 mmHg). VC increased forearm volume (plethysmography) by 4.7%. MSNA responses to exercise were greater after VC (150.5 +/- 41.8 vs. 317.3 +/- 69.9 arbitrary units; P < 0.01). However, MSNA responses during PHG-CA were not affected by VC, and 31P nuclear magnetic resonance (n = 5) demonstrated no effect of VC on pH or H2PO4-. Similar effects of VC on MSNA were noted after ischemic exercise (n = 7), excluding flow alterations as the explantation. VC probably sensitized mechanically sensitive afferents since MSNA during involuntary biceps contractions increased after VC (n = 6), and skin sympathetic nerve responses during handgrip, an index of central command, were not increased by VC (n = 6).

Multiple spinal meningiomas: a case of 47 distinct lesions in the absence of neurofibromatosis or identified chromosomal abnormality.

The authors present a unique case of multiple spinal meningiomas with the late development of intracranial lesions. The patient had 47 distinctly separate, yet histologically identical, lesions excised, with many others noted at the time of surgery and by radiographic studies. He was evaluated for neurofibromatosis and was found to have neither Type I nor Type II. Genetic analysis, including restriction fragment length polymorphism analysis, was performed and detected no abnormalities.

Effect of enzyme supplementation on the performance and digestive tract size of broiler chickens fed wheat- and barley-based diets.

Three experiments were conducted to study the effects of crude enzyme preparations on the performance and gastrointestinal tract size of chicks fed wheat and barley diets. In the first experiment, enzyme addition (100 and 200 mg/kg of Roxazyme G and 1,000 mg/kg of Avizyme SX) to diets containing Bedford barley improved weight gain (6%) and the feed to gain ratio (5%) over a 6-wk period for both male and female broilers. In Experiment 2, enzyme addition to diets containing Scout (hulless) and Bedford (hulled) barley improved (P < or = .05) weight gains of Leghorn chicks by 25 and 11% and the feed to gain ratios by 10 and 6%, respectively. Feed consumption increased significantly (16%) only in the case of birds fed enzyme with Scout barley. Corresponding reductions in the relative weights of the crop and gizzards were 15 and 17% for birds fed Scout barley and 7 and 8% for those fed Bedford barley. Enzyme treatment of the diet containing Scout barley also reduced the relative length of the duodenum, jejunum, and ileum and the relative weight of the proventriculus, whereas a similar treatment of Bedford barley resulted in changes in the relative length of the duodenum and jejunum (P < .05). In the final broiler experiment (42 days), crude enzyme addition (100 mg/kg) to wheat and barley diets improved weight gains by 13 and 9% and feed to gain ratios by 7 and 10%, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Mental healthcare for rural seniors. An outreach program in Cedar Rapids, IA, uses an integrated approach to break down barriers to care.

Several barriers prevent elderly persons, especially those living in rural areas, from receiving mental health services. The Abbe Center for Community Mental Health is breaking down some of these barriers in the Cedar Rapids, IA, area. The center's Elderly Outreach Project identifies and provides mental health services to the area's rural elderly. A multidisciplinary team (psychiatrist, nurse, and social worker) assesses and treats home-bound clients. Four major barriers prevent seniors from using traditional mental healthcare services: A lack of trained professionals. Because many professionals have not received training in geriatrics, those working with elderly clients should be encouraged to attend educational conferences to fill gaps in their knowledge. Organizational barriers. Transportation and cost may prohibit elderly persons from seeking mental healthcare. Facilities must revise policies detrimental to clients' well-being. Ageism. Many elderly persons have internalized negative and incorrect beliefs about what aging is or should be. Education about "normal" aging is essential. Stigma. The stigma of mental illness is particularly troublesome. Services such as in-home counseling allow clients to get the help they need while keeping their mental illness confidential. To eliminate the barriers to mental healthcare, increased financial resources are necessary to develop, implement, and maintain innovative programs that can reach frail, isolated, hard-to-find persons in need of mental health, medical, and social services.

Growth factors interact with estradiol and gonadotropins in the regulation of ovarian cancer cell growth and growth factor receptors.

The factors controlling cell growth in epithelial ovarian cancer (OV Ca) remain poorly defined. Epidemiological evidence suggests that gonadotropins and estrogens may be important in the development of OV Ca. We have established permanent cultures of OV Ca cells from a mesenteric metastasis of an ovarian papillary adenocarcinoma. The growth of these cells was increased 20-60% (p < 0.01) by picomolar concentrations of 17 beta-E2 (but not by 17 alpha-E2) and 0.1 to 1 microgram/mL of hCG (but not hFSH). Both EGF and IGF-1 increased cell growth, and a maximal effect of 3- to 5-fold increase in cell number was observed. Under conditions in which the growth responses to EGF and IGF-1 were submaximal, E2 (0.1 to 100 nM) produced a concentration related increase in the growth response to IGF-1 and EGF. Ten nM E2 increased the responses to EGF and IGF-1 more than 2-fold. In combination, E2/hCG enhanced the growth response to EGF but not to IGF-1. Estradiol increased the Ka of the EGF receptor approximately 2-fold (p < 0.01) and increased the number of receptors/cell for IGF-1 approximately 50% (p < 0.01). While hCG had no effect on EGF receptor binding parameters, it increased the Ka of the IGF-1 receptor and enhanced the effect of E2 on IGF-1 receptor number. These studies suggest that E2 and hCG may regulate cell growth responses of OV Ca cells to IGF-1 and EGF. These changes may be exerted at least partly through modulation of properties of receptors for the two growth factors.

The influence of primary open-angle glaucoma upon the retrobulbar circulation: baseline, postoperative and reproducibility analysis.

CDI is showing continued promise as a reliable, reproducible method to assess the hemodynamics of a variety of ophthalmic diseases in which vascular perfusion abnormalities are suspect. Moreover, some patients with advanced POAG appears to have abnormalities of the vasculature of the retrobulbar circulation that is corrected by trabeculectomy. Continued CDI clinical correlations with longitudinal studies are now required to elucidate the significance of this technology and these results for individual patients.

[Risk factors for low birth weight and intrauterine growth retardation in Santiago, Chile].

An epidemiologic case-control study to ascertain the determinants of low birthweight was carried out in Santiago, Chile, from January to December 1989. The cases were defined as livebirths < 2500 g. The controls were livebirths > or = 2500 g of birthweight. All cases and a random sample (1:1) of controls were selected among 8,254 singleton births occurring at the El Salvador Hospital in the Eastern area of Santiago. These deliveries represented 50% of institutional deliveries in the area. Home deliveries (2%) and private hospital deliveries were not included in the study. Information was obtained from hospital medical records by six trained medical students. Some information could not be obtained from the hospital medical records. Thus the second step in data collection was the tracking of all the selected subjects to their referring neighborhood health centers. For the analysis, the data were divided into 3 case (outcome) categories: 453 subjects were the total case group. From these, 153 were the IUGR case group and 300 were the LBW preterm case group. The general control group consisted of 605 normal birthweight infants. 565 were the IUGR control group and 40 were the preterm control group. A total of 25 risk factors showed a significant crude odds ratio for at least one of the groups. In the multivariate logistic regression analysis eight variables: No. of pregnancies, previous adverse outcomes, previous LBW, pregnancy maternal weight, No. of visits, month of first prenatal care visit, maternal smoking and intrahepatic cholestasis of pregnancy, were significantly associated with LBW after adjustment by confounding. Eight risk factors: IUGR in previous pregnancies, Previous adverse outcome, Maternal smoking, intrahepatic cholestasis, maternal pregnancy weight, maternal height, month first prenatal visit, No. of visit, were significant to IUGR. Only two variables: pregnancy weight, divorced mother, were significantly associated with low birth weight in the preterm group. The most relevant risk factors were included in stepwise logistic regression models carried out for the outcome LBW for the general group, term group and preterm group, in order to adjust by confounding. Adjusted odds ratios were then obtained. Prenatal care related factors and maternal adverse obstetric factors were at higher significance for LBW in the general and IUGR groups. Only nutritional factors were related to LBW in preterm group. Women who delivered a LBW or IUGR infant were more likely to have fewer pregnancies, a history of previous LBW, lower prepregnancy weight and lower gestational weight gain. ICP was associated with an elevated risk of LBW that was independent of gestational age.
