[Diverticula and pseudodiverticula of the gallbladder in chronic calculous cholecystitis].

The study covers twenty-two patients with chronic calculous cholecystitis (CCC) subjected to operative treatment. Diverticula are discovered in three of them (1.4 per cent), and pseudodiverticula of the gallbladder--in eighteen (8.2 per cent). In all patients presenting diverticula and pseudodiverticula of the gallbladder, the clinical course of the disease is typical of CCC, and in three it is characterized by the clinical picture of obstructive jaundice. Concrements are documented echographically in all cases. Intraoperatively, numerous adhesions, gallbladder kinking at infundibulum level with inflated gallbladder type "Hartmann's pouch" are established in all 18 patients presenting pseudodiverticula. Two of them have empyema of the gallbladder, and in one a colonic-vesical fistula is detected. All patients are subjected to operative management. The immediate postoperative and long-term results are estimated as very good.

[The premenstrual syndrome in adolescents].

The premenstrual syndrome is associated with the appearance of distressing somatic, physiological and behavioural symptoms at a certain phase of the menstrual cycle and has not been sufficiently studies with adolescents. The aim of this study is to determine the frequency, severity and period of appearance of the more important somatic and psychological symptoms characteristic of adolescents with PMS. The study includes 186 girls at an age between 16 and 18 years monitored for the period of one year and four months. The most frequently met somatic symptom is headache (39%), the most frequent psychological one--irritability (46%). Sufficient attention is paid to the influence of the menarche and cigarette smoking in adolescents with PMS.

[Sexual activity, contraception and abortions among teenagers in Bulgaria].

Sexual activity in adolescence, the pregnancies, abortions and deliveries, STDs and the decisive role of the information about sex and sexuality are questions with great importance nowadays. The aim of present study is to find the changes in the sexual activity among the adolescents, in their motivation in beginning the sexual life, information that they have about sex and sexuality, in unwanted pregnancies, deliveries and about abortions in the last five years, following our former study. We present the data obtained from an inquire of 432 schoolboys and girls (aged 17-18), from 263 girls, aged 13 to 18, admitted for interruption of unwanted pregnancy in the Univ. Ob. & Gyn. Hospital in Sofia, Bulgaria, as well as from 132 girls after delivery in adolescence.

[The combination of chronic calculous cholecystitis and diabetes mellitus].

A total of 206 patients with chronic calculous cholecystitis (CCC) are subjected to operation in the surgical clinic of the Military Hospital--Plovdiv over the period 1991-1994 = Diabetes mellitus as a concomitant disease is discovered in 21 cases (10.20 per cent). All of them have pain syndrome complaints, and during cholecystectomy concrements in the gallbladder are found. As shown by the results, the combination CCC and DM leads to aggravation of the disease. Destructive changes in the gallbladder in this combination are detected in 28.5 per cent, whereas in patients free of DM they amount to 10.5 per cent. Presumably, removal of the gallbladder contributes to eliminate the persisting stress factor in terms of the insular apparatus of the pancreas. Assessment of the long-term results shows that the positive therapeutic effect of the operation is permanent.

[Phlegmon-exacerbated chronic calculous cholecystitis and its surgical treatment].

A series of 165 patients presenting chronic calculous cholecystitis (CCC), operated in the surgical clinic of the Military Hospital--Plovdiv over the period 1991 through 1994, are analyzed. In twenty patients (10 men and 10 women) the disease a clinical course characterized by phlegmonous-purulent cholecystitis. All patients are subjected to operative management, and in all the clinical diagnosis is confirmed intraoperatively. Empyema of the gallbladder with concrements and purulent secretion is discovered in nine cases, and fragility of the gallbladder wall-in four. The histomorphological study reveals phlegmonously exacerbated CCC in 12 patients, walls totally replaced by granulation tissue--in three, and necrotic mucosa--in five. The early results of operative treatment are estimated as good. The long-term results are likewise very good. The positive effect of treatment is lasting, and persists for many years.

[Chronic recurrent acalculous cholecystitis--apropos 6 clinical cases].

Over the period 1991 through 1994, a total of 120 patients with chronic cholecystitis are operated on. In six of them (5 per cent) no concrements in the gallbladder and biliary ducts are discovered. In 83.3 per cent the echographic study fails to detect concrements in the gallbladder and biliary ducts. All patients are subjected to cholecystectomy, and in all of them markedly expressed adhesions are found. The early and long-term operative results are estimated as very good. The positive therapeutic effect is durable and persists for many years.

[4 cases of botryoid sarcoma].

The authors describe the rare botryoid sarcoma--histomorphology, clinic, diagnosis, treatment and prognosis with a contribution of 4 own cases at the age up to 18 years. They indicate the difficulties in the establishment of the diagnosis-early clinical and histological-in three of the cases the histological examination did not establish immediately the exact diagnosis--the material was considered as "granulation tissue" or "fibroma mole". The results of the treatment are summarized as well as the choice of therapeutic methods and drugs. The applied therapeutic scheme consists of radical hysterectomy with systemic pelvic lymph dissection and postoperative chemotherapy. The described four cases are alive at the present moment and without data for tumourous relapse--respectively 11 years and 2 months, 3 years and 4 months, 2 years and 8 months after establishment of the diagnosis.

[Sexual activity, contraception and unwanted pregnancy at puberty and adolescence].

The sexual activity during the pubertal-adolescent age and the closely connected with it questions about psychological consequences, extramarital conceptions, deliveries and abortions is a problem with more and more increasing significance in the world. The present study includes an inquiry on 468 girls and boys at the age of 17-18 years, and 120 girls at the age of 14 to 17 years with interrupted pregnancy 214 girls with interrupted pregnancy at the age of 13 to 17 years and 120 girls at the age of 14 to 17 years, who delivered extramaritally at the Higher Research Institute of Obstetrics and Gynecology. It is established that 48.6% of girls have sexual life, but 92.1% of girls, aged 17-18 years, have sexual life. The question was studied when, why and with whom the first sexual contact was made. The questions about contraception, sexual education, abortions, deliveries and the fate of the newborn are discussed as well. Inferences for prophylactic work among the coming generation are made.

[A report of 2 cases of the Herlyn-Werner-Wunderlich syndrome].

Two cases are described, illustrating two varieties of comparatively little known, but not rarely encountered syndrome--the syndrome of Herlyn-Werner-Wunderlich. The characteristic and the symptoms of the syndrome are presented and various concepts on therapeutic management are discussed. The question about future fertile capability of the patients is discussed as well.

[Chronic stress and the degree of anxiety in girls with menstrual cycle disorders].

The present study was made with the aim to clarify the role of personality attitude as well as of stress actions found in three elements of the environment-family, school and social encirclement in wider aspect in the occurrence of menstrual disturbances. Anxiety as a personality characteristic was examined by a clinical inquiry as well as by the test for anxiety of Taylor and acute and chronic stress actions were search for. 228 girls at the age of 15 and 18 years were investigated. Diagnosis "psychogenic amenorrhea" was established in 34 out of 228 girls. In regard to physical and sexual development there was not significant difference with the girls of the control group. There was direct connection between stress event and the disturbance in the menstrual cycle in 41.2% of the examined girls. The fourth (elevated anxiety) and fifth degree (highly elevated anxiety) according to Taylor was established altogether in 70% of the cases. Polymorphous anxiety was found in structural respect. Third degree according to Taylor-mean manifested anxiety was established in 26% of the girls of the control group. Inferences are made in connection with the therapy.

[Vaginoscopy--a modern method in pediatric-adolescent gynecology].

Inspection of the vagina is a compulsory part of the examination of sexually mature woman. However, the intact hymen is assumed as a barrier, which could not be surmount without physical or psychic traumatization of a girl at the infant-adolescent age. Fear of hymen traumatization in practice is stronger than fear to miss some gynecological diseases. The necessary instruments as well as the basic indications for vaginoscopy are described. A review is made on the cases admitted in 1987 to the ward of the infant-adolescent gynecology at Institute of Obstetric and Gynecology, in whom vaginoscopy has been performed. The findings and the succeeding therapy are discussed. An inference is made for necessity of knowing and usage of the method as well as for its possibility to establish exact diagnosis and application of adequate therapy.
