OBJECTIVE: Assessment of efficacy and side effects of emergency contraception for teenagers with levonorgestrel (LNG) and the level of users' informedness about possibilities and practical application. METHODS: The subjects are healthy girls (n = 49) with regular menstrual cycles at the age between 15 and 19, having had one unprotected or faultily protected sexual intercourse. All of them have administered 0.75 mg LNG within the 72nd hour, repeated after 12 hours. The data have been processed by variational analysis. RESULTS: One pregnancy was registered of a girl with firstintake at the 67th hour - pregnancy rate - 2,0%. The most frequent side effect was nausea - 26,5%, folowed by breast tenderness - 22,4% and fatigue - 20,4%. An up to 7th day delay in menstrual cycle is non significantly more frequent - 14,3%, followed by a delay of more than 7 days breakthrough bleeding - 8,2%. No significant changes were established in the lenght of the menstrual cycle. Emergency contraception is sought for after unprotected sexual intercourse in 69,4%, and condom failure problems in 30,6%. Only 18,4% have sufficient information about the possibilities and practical use of emergency contraception. CONCLUSION: LNG provides effective, highly tolerable contraception with a small number of side effects. Need is felt for serious popularization of the application of emergency contraception with teenagers.
In this work I would like to speak about a group of substances which invisible are present in our environment. We come across with them every day. We eat them, drink them, breathe them and use them at work, at home and in the garden. They are present in soil, water, air and food, household products, packing materials, probably in the breast milk and during development in our mother's womb. These substances are "xenoestrogens". "Xeno" literally means foreign. So xenoestrogens means foreign estrogens. They are also known as: endocrine or hormone disrupters, environmental estrogens, hormonally active agents (EPA), estrogenic substances, estrogenic xenobiotics, and bioactive chemicals. The human health risks, that may be associated with these low-level but constant exposures are still largely unknown and highly controversial. May be, more knowledge in this area could give us an answer to the questions which every one of us asks many times: what is the reason of appearance of this or that disease or condition, why suddenly, without visible reason childs' breasts start to grow, the menstrual cycle break off, why prescribed by me therapy does not work? Etc....

UNLABELLED: The aim of the study is to determine the effectiveness of the single per oral application of 200 mg Fungolon (Fluconazolum) in adolescent girls with proven Candida spp. Vaginal infection. MATERIAL AND METHODS: A prospective, case control study in 50 girls, aged 9 to 18 years, was performed. Patients were recruited in the outpatient Clinic of Paediatric and Adolescent Gynecology at the University Hospital "Majchin dom", Sofia, Bulgaria. They suffered from vaginal discharge and/or pruritus of the external genitalia. Candida spp. Was proven in all of the microbiological investigation performed. The treatment with Fungolon was performed per os, in single dose of 200 mg. RESULTS: The complaints and Candida in the vaginal smear (controlled 1 week after the treatment) disappeared in 43 of 50 patients (86%) treated. No effect was observed in 7 girls (14%). A reinfection with Candida was observed in 6 of the 38 patients (16%) at the second week after the treatment. The typisation of the resistant to Fungolon Candida shown predominance of Candida glabrata. There were no side effects registered during the Fungolon treatment. CONCLUSION: The treatment of Candida spp. Vaginal infection in adolescent girls by a single per oral application of 200 mg Fungolon (Fluconazolum) is very convenient and efficient. It gives a prompt effect (the complaints disappear in one or two days), high percentage of healed patients (86%), with absence of side effects. The usage of Fungolon should be highly recommended for treatment of vaginal mycoses in puberty and adolescence.

The authors have done a biliary retrospective analysis of 10 deceased patients with a diagnosis of biliary peritonitis (BP). It is reported that the patients who died of BP are 9.5% of all who died of diffuse purulent peritonitis. All deceased patients were advanced and well advanced in years with prolonged complaints--in over 50% of them over 5 years. The complications set in 50% of the cases are recorded in the postoperative period. The perforation of the gallbladder and the biliopancreatitis are in the second place. Intrabiliary fistulas with 4 patients and 1 with a vesicocolon fistula; tumor of the pancreas with 4 patients; empyema of the gallbladder--3; diabetes with 3 of the patients and others are recorded as accompanying troubles which complicate the operation and the outcome of it. The time between the beginning of the complication and the operation with all the deceased patients is over 72 hours.

Assessment is made of the long-term results in 75/232 patients with chronic cholecystitis, operated in the surgical clinic of the Military hospital--Plovdiv over the period 1991 through 1995. To analyze the questionnaire study results, the patients are divided up into three groups, as follows: Group one--61 cases (81/3%) where the complaints are completely eliminated. Group two--13 cases (17/1%) where the condition shows improvement, but during physical atress and nonobservance of the dietary regimen, heaviness and blunt pain in the right subcostal area occur. Group three--one patient with persisting complaints and lethal outcome. Evaluation of the long-term results shows that the positive effect attained is persistent and durable. Cholecystectomy is taken to be as the most rational and often used operative intervention.
