Management of giardiasis by an immuno-modulatory herbal drug Pippali rasayana.

Pippali rasayana (PR), an Ayurvedic herbal medicine, prepared from Piper longum (Pippali) and Butea monosperma (Palash), and prescribed for the treatment of chronic dysentery and worm infestations was tested for anti-giardial and immuno-stimulatory activity in mice, infected with Giardia lamblia trophozoites. It produced up to 98% recovery from the infection. The rasayana had no killing effect on the parasite in vitro. It induced significant activation of macrophages as evidenced by increased macrophage migration index (MMI) and phagocytic activity. Enhancement of host resistance could be one of the possible mechanisms contributing towards the recovery of animals from the giardial infection.

A shortened dengue IgM capture ELISA using simultaneous incubation of antigen and peroxidase-labeled monoclonal antibody.

A shortened IgM capture ELISA for the detection of dengue IgM antibodies using simultaneous incubation of antigen and peroxidase-labeled monoclonal antibody was described. The shortened two-step assay was compared with the four-step IgM capture ELISA on sera from dengue patients confirmed by the hemagglutination inhibition (HI) test. When paired acute and convalescent sera were tested, the shortened ELISA showed 100% agreement with HI results. It detected dengue IgM antibodies in the acute sera of 66% of patients with a primary dengue infection, 60% of patients with a secondary infection, and 98% of patients with a presumptive secondary infection. When the results of 151 dengue patients were combined, 75% of the acute sera were positive by the shortened IgM capture ELISA.

Ferrous sulphate does not directly affect pteroylmonoglutamic acid absorption in rats.

A variety of compounds which bind to Fe have substantial reductions in absorption when co-administered with Fe compounds. The binding of both Fe2+ and Fe3+ ions to pteroylmonoglutamic acid and the pteroylmonoglutamate dianion was examined in vitro. In dimethylsulphoxide (DMSO) alone, pteroylmonoglutamate formed a 2:1 (pteroylmonoglutamate:Fe3+ ion) complex. However, in DMSO-aqueous Bis-Tris buffer (4:1, v/v; pH 6.0) no evidence of complex formation could be seen. Likewise spectroscopic evidence was obtained for complex formation with Fe2+ ion and pteroylmonoglutamate in DMSO alone but not in the aqueous DMSO buffer. In vivo studies examined the effect of FeSO4 on pteroylmonoglutamic acid absorption in an isolated perfused rat jejunal model of nutrient absorption. The dose of pteroylmonoglutamic acid approximated a human dose of 1 mg for the rat, while the FeSO4 doses were chosen to represent 6.4 mg, 64 mg and 300 mg human doses. There was no significant effect of FeSO4 on pteroylmonoglutamic acid absorption or instability of pteroylmonoglutamic acid in vivo in the presence of FeSO4 in the rat. Although 2:1 binding of pteroylmonoglutamic acid to Fe ions could be demonstrated in DMSO alone, no binding could be demonstrated in DMSO-Bis-Tris buffer (4:1, v/v; pH 6.0). It is unlikely that there will be a significant reduction in pteroylmonoglutamic acid absorption during concurrent ingestion of Fe preparations.

Comparison of the effects of an organic and an inorganic form of selenium on a mammary carcinoma cell line.

The development of new compounds with greater cancer inhibitory activity and that are well tolerated continues to be a priority in chemoprevention research involving selenium. One compound, 1,-4-phenylene-bis(methylene)selenocyanate (p-XSC), is representative of a series of organoselenium compounds with these characteristics. In this study, the effects of p-XSC on a mouse mammary carcinoma cell line were compared to those of sodium selenite, which has been shown to be growth inhibitory. Treatment with p-XSC caused a 3- to 6-fold greater accumulation of selenium within cells than did treatment with equivalent amounts of selenite and cells were able to better tolerate higher cellular levels of selenium derived from p-XSC. Both compounds resulted in a dose-dependent reduction in cell number after 24 h of exposure. Selenite and p-XSC also caused a dose-dependent increase in cell death by apoptosis. This effect was observed within 5 h of treatment. The effect of p-XSC on apoptosis was more pronounced than that of selenite, especially at the 20 microM level of exposure. The induction of apoptosis by selenium compounds may partially account for their chemopreventive activity.

Effect of excess dietary iron on the promotion stage of 1-methyl-1-nitrosourea-induced mammary carcinogenesis: pathogenetic characteristics and distribution of iron.

The effect of feeding a 10-fold excess of dietary iron on the promotion stage of MNU-induced mammary carcinogenesis was investigated. Rats fed excess iron in the diet had more mammary carcinomas than rats fed the recommended level of iron. A significantly greater proportion of carcinomas in rats fed the excess iron diet had the normal Ha-ras gene rather than the mutated form (G-->A transition mutation in codon 12). In non-tumor bearing rats, mammary epithelial cells in lobules were the primary site of iron accumulation. However, in mammary carcinomas, a shift in the distribution of iron from the epithelial cells to the stroma was noted. Iron was predominantly found in tumor stroma; malignant epithelial cells failed to accumulate comparable levels of iron. These observations indicate that in the presence of excess iron there is an increase in the number of mammary carcinomas that do not bear the mutant Ha-ras gene. Whether changes in the distribution of iron within the mammary gland contribute to the altered pathogenetic characteristics of these tumors is being investigated.

Evaluation of transvaginal sonography combined with a urinary luteinizing hormone monitor in timing donor insemination.

There remains controversy as to the most accurate method of predicting ovulation for the timing of donor insemination. In a prospective study based at a tertiary referral donor insemination clinic we have assessed the ability of a urinary luteinizing hormone (LH) kit combined with vaginal ultrasonography to predict ovulation for donor insemination. A total of 25 natural cycles were monitored from the first day of the LH surge (detected by the urinary kit). Daily transvaginal sonographic measurement of follicular size and endometrial thickness were used to predict ovulation; monitoring continued until post-ovulatory changes were seen. Transvaginal sonography combined with a urinary LH kit successfully predicted all those women who ovulated (20/25) and detected unfavourable conditions for insemination in the remaining 20% (5/25). In conclusion, where available transvaginal sonography combined with a urinary LH kit should be the method of choice for timing donor insemination.

Predictive perinatal score in the diagnosis of neonatal sepsis.

A scoring system for prediction of neonatal sepsis was evolved after determining the interdependence of perinatal risk factors for infection. Records of 100 babies with a history of one or more perinatal risk factors were analysed for incidence of infection within 4 hours of birth and followed for 1 week thereafter for appearance of any clinical or laboratory signs of infection. The incidence of sepsis was compared amongst various risk factors. Since majority of perinatal risk factors occur in combinations interdependence of factors was determined using actuarial analysis and score assignment was done whether the factor was dependent or independent. No definite infection was seen in the control group of 100 babies having no history of high risk factors. The scoring system thus elucidated is recommended as a screening procedure for selecting of neonates for laboratory evaluation.

Characterization, expression, and immunohistochemical localization of 5 alpha-reductase in human skin.

Human skin has been shown to contain a high level of 5 alpha-reductase activity, the enzyme that catalyses the conversion of the weak androgen testosterone into dihydrotestosterone, the most potent androgen. Because two types of 5 alpha-reductase genes have been characterized in humans, we have cloned 5 alpha-reductase cDNAs from adult human keratinocyte and skin fibroblast cDNA libraries to identify and gain better knowledge of the 5 alpha-reductase expressed in normal human skin. Nucleotide sequence analysis shows that the clones obtained correspond to the type I 5 alpha-reductase. RNase protection analysis using (poly A)+ RNA obtained from human skin and prostate also confirms that type I 5 alpha-reductase is the predominant type expressed in normal skin, whereas type II 5 alpha-reductase is the major form found in the prostate. Following polymerase chain reaction amplification of human keratinocyte and skin fibroblast cDNA, a low level of type II 5 alpha-reductase cDNA has been detected. Using antipeptide antibodies raised in rabbits against the peptide sequence covering amino acids 227 -240 to perform immunohistochemical localization of 5 alpha-reductase, we have found that 5 alpha-reductase is distributed in sweat and sebaceous glands, as well as in the epidermal cell layers, thus providing the basis for the important role of androgens in human skin and its appendages.

Isolation and characterization of novel plasmid-encoded umuC mutants.

Most inducible mutagenesis in Escherichia coli is dependent upon the activity of the UmuDC proteins. The role of UmuC in this process is poorly understood, possibly because of the limited number of genetically characterized umuC mutants. To better understand the function of the UmuC protein in mutagenic DNA repair, we have isolated several novel plasmid-encoded umuC mutants. A multicopy plasmid that expressed UmuC at physiological levels was constructed and randomly mutagenized in vitro by exposure to hydroxylamine. Mutated plasmids were introduced into the umu tester strain RW126, and 16 plasmids that were unable to promote umuC-dependent spontaneous mutator activity were identified by a colorimetric papillation assay. Interestingly, these plasmid mutants fell into two classes: (i) 5 were expression mutants that produced either too little or too much wild-type UmuC protein, and (ii) 11 were plasmids with structural changes in the UmuC protein. Although hydroxylamine mutagenesis was random, most of the structural mutants identified in the screen were localized to two regions of the UmuC protein; four mutations were found in a stretch of 30 amino acids (residues 133 to 162) in the middle of the protein, while four other mutations (three of which resulted in a truncated UmuC protein) were localized in the last 50 carboxyl-terminal amino acid residues. These new plasmid umuC mutants, together with the previously identified chromosomal umuC25, umuC36, and umuC104 mutations that we have also cloned, should prove extremely useful in dissecting the genetic and biochemical activities of UmuC in mutagenic DNA repair.

Hand preference and approval among Hindus and Muslims in India.

Two forms (hand-preference and hand-approval) of a 87-item handedness questionnaire were administered to eighty subjects including both Hindus and Muslims. Both male and female subjects were included in the sample. Factor analysis for both forms of the questionnaires revealed different factor structure for hand preference and hand approval patterns. These patterns were found to be similar for right- and left-handed but differed in case of weak-versus strong-handed subjects. For hand preference scores there were minimal differences between Hindus and Muslims. However, female subjects (more in the case of Hindus) showed higher preference for right hand use than their male counterparts. The analyses of hand approval scores, on the other hand, indicated that Muslims were more insistent upon using the right hand, especially activities related to eating behavior than Hindus. These results are discussed in terms of cultural influences on hand preference and hand approval.

Metabolism of the cardioprotective agents dexrazoxane (ICRF-187) and levrazoxane (ICRF-186) by the isolated hepatocyte.

1. The metabolism of dexrazoxane (ICRF-187) and its optical isomer levrazoxane (ICRF-186) by the isolated rat hepatocyte was studied by hplc. 2. 4-Chlorobenzenesulphonamide, which is a strong inhibitor of dihydropyrimidine amidohydrolase (DHPase), caused 82% inhibition of the loss of dexrazoxane from the hepatocyte suspension. 3. Dexrazoxane was metabolized at an initial rate by isolated hepatocytes that was 1.8 times faster than levrazoxane. This ratio is close to that found for purified DHPase, suggesting that DHPase present in the hepatocyte catalyses the ring-opening hydrolysis of these drugs. 4. The ratios of the rates at which each of the one-ring open intermediates of dexrazoxane and levrazoxane were produced in the hepatocyte suspension are also consistent with DHPase being primarily responsible for the metabolism of dexrazoxane and levrazoxane. 5. Thus, the DHPase-catalysed formation of the one-ring opened intermediates enhances the rate at which the presumably active metal-ion binding forms of dexrazoxane are produced in the hepatocyte. 6. The DHPase content of the hepatocyte was estimated to be 1.2 nmol/kg of total hepatocyte mass, or equivalently 5700 molecules of DHPase per hepatocyte.

Multiple fractures following camel bite of the face (a case report).

A rare case of multiple fractures following camel bite of the face in a young male, producing facial deformity and open bite is described. Fractures were confirmed by X-rays and CT scan. Treatment consisted of an open reduction and interosseous wiring of fractures to attain normal occlusion which was maintained by intermaxillary and suspension wire fixation followed by soft tissue repair. This corrected the open bite, collapse of maxillary arches and facial deformity. The behaviour of camels during rutting is briefly discussed.

The intraosseous route is a suitable alternative to intravenous route for fluid resuscitation in severely dehydrated children.

It is sometimes difficult to gain a rapid intravenous access in hypovolemic states. The suitability of intraosseous (IO) route for fluid infusion as an effective, safe and reliable alternative to intravenous (IV) route was explored. Sixty children (age range 3 months to 2 years) with severe dehydration were assigned alternately to receive resuscitating fluid through either IO or IV routes. The IO route was successfully secured in all cases within the first 5 minutes of attempt. On the other hand, the IV line could not be secured in 33% (10 out of 30) patients within 5 minutes. The time taken for IV cannulation when it was successful (129 +/- 13 seconds, 95% confidence interval 103-156 seconds) was significantly longer than the time taken for IO cannulation (67 +/- 7 seconds, 95% confidence interval 55-80 seconds). Fluid infusion through either routes was equally effective in stabilizing vital signs and normalizing laboratory abnormalities. No significant complication of IO route was noted on short term follow-up. We conclude that IO route is a safe, effective alternative for emergency fluid administration in severe dehydration when intravenous line cannot be secured rapidly.

Complications of percutaneous renal biopsy.

Three hundred twenty renal biopsies were done in 305 patients over a period of 5 years. Adequate tissue for pathologic diagnosis was obtained in about 79% of biopsy attempts. The overall morbidity of procedure was 6.8%. Haematuria in 11.8% cases was the commonest complication. Haematuria resolved spontaneously in 63.8% of patients within 4-12 hours. Hypotension due to blood loss occurred in 3 patients (0.98%) who required blood transfusion. Five patients (1.6%) developed perirenal haematoma. Urinary retention requiring single catheterisation was seen in 12 cases (3.9%). Perirenal abscess occurred in one case. Minor complications improving with symptomatic medication included vomiting, abdominal pain and vasovagal attack in 3.6%, 2.6% and 1.6% cases respectively.

Potassium channel blockade amplifies cardiac instability numerical studies of torsades de pointes.

Suppression of responses to premature stimulation has been the guiding principle in managing many cardiac arrhythmias. Recent clinical trails revealed that sodium channel blockade increased the incidence of re-entrant cardiac arrhythmias resulting in sudden cardiac death, although the physiologic mechanism remains uncertain. Potassium channel blockade offers an alternative mechanism for suppressing responses to premature stimuli. We have developed a simple model of a 2D sheet of excitable cells. We can initiate re-entrant activation with stimuli timed to occur within a period of vulnerability (VP). Reducing the Na conductance increases the VP while reducing the K conductance increases the collective instability of the array, and arrhythmias similar to torsades de pointes seen in patients subjected to K channel blocked can be readily initiated. Thus, while K channel blockade may suppress excitability by prolonging the action potential duration, it appears to simultaneously exhibit proarrhythmic properties that result in complex re-entrant arrhythmias.
