A case of malignant fibrous histiocytoma of the ilium, evaluation by blood-pool scintigraphy.

A rare case of malignant fibrous histiocytoma (MFH) of the ilium was presented and blood-pool scintigraphic images and angiographic images were correlated. Hypervascular tumor and lacking of contrast material with arteriovenous shunts were shown by angiography. Intensive tracer accumulation was shown by blood-pool scintigraphy. After radiation therapy and transcatheter arterial embolization of the tumor, tracer accumulation was noticeable reduced. These findings suggested a vascular tumor such as angiosarcoma, but surgery revealed MFH of the bone. Blood-pool scintigraphy was useful in the evaluation of the vascular characteristics of the tumor.

99mTc-DTPA-HSA uptake in a case of splenic hamartoma.

Primary splenic tumors are very rare. The authors present a case of splenic hamartoma in which positive 99mTc-DTPA-HSA uptake was recognized with ultrasound, CT, MR and angiographic images. This case showed the need to consider hamartoma as well as hemangioma in the differential diagnosis of 99mTc-DTPA-HSA uptake in splenic tumors.

"Neuritic conglomerates" in the cerebral cortex of a patient with Creutzfeld-Jakob disease.

In the cerebral cortex of a patient with Creutzfeld-Jakob disease (CJD), we found peculiar glomeruloid or skein-like structures which have not previously been described. The patient was a 67-year-old man, whose clinical features and neuropathological findings were consistent with CJD. The glomeruloid or skein-like structures were distributed in the deep layers of the cortex and consisted of intricately entangled masses of thick argyrophilic fibers. These structures were immunostained with anti-neurofilament antibodies and were considered to have originated from neuronal cytoplasmic processes, most likely axons. The pathogenesis and pathological significance of these structures, which were tentatively termed "neuritic conglomerates," remains unclear. However they probably represent an overgrowth of the distal portion of axons and indicate the plasticity of the injured neurons.

Lipoleiomyomatous tumors of the uterus: a heterogeneous group? Histopathological study of five cases.

Five surgical cases of lipoleiomyomatous tumor of the uterus were studied histopathologically. All the patients were postmenopausal. Three tumors had arisen in the corpus, and the remaining two in the cervix. In four of five cases, tumors consisted of smooth muscle tissue admixed with varying amounts of mature adipose tissue, which did not show cytological atypism. In the remaining single case, the tumor contained, in addition to smooth muscle tissue and fibroadipose tissue, many anomalous arterial blood vessels, which showed an irregular, tortuous appearance resembling that found in renal angiomyolipoma. From the histopathological findings and a review of the relevant literature, it was considered that lipoleiomyomatous tumor of the uterus is not a single histopathological entity but is composed of two distinct neoplasms with different histogenesis; that is, 'lipoleiomyoma' and 'anglolipoleiomyoma'. While the latter tumor is most likely of a choristomatous nature, the former probably arises from 'lipomatous' metaplasia of a pre-existing leiomyoma.

Immune reactions associated with cerebral amyloid angiopathy.

BACKGROUND AND PURPOSE: Cerebral amyloid angiopathy (CAA) occasionally coexists with cerebral vasculitis. An immune system may influence deposition or degradation of the amyloid in cerebral blood vessels. The purpose of this study was to elucidate immune reactions associated with CAA. METHODS: In 11 elderly patients with sporadic CAA, 2 patients with Icelandic familial CAA, and 2 patients with CAA and granulomatous angiitis, the cerebrovascular amyloid proteins and infiltrating inflammatory cells were analyzed immunohistochemically. RESULTS: In both sporadic CAA (beta-protein amyloid angiopathy) and Icelandic familial CAA (cystatin C amyloid angiopathy), leptomeningeal and cortical vessels were associated with an increase or activation of monocyte/macrophage lineage cells. In the cases of CAA with granulomatous angiitis, the vascular amyloid was of beta-protein and associated with infiltration of many monocyte/macrophage lineage cells, which included multinucleated giant cells containing the amyloid in the cytoplasm as well as T cells composed of CD4+ and CD8+ subsets. Amyloid P component, which was reported to be a common component of amyloid deposits and to prevent phagocytic proteolysis of amyloid fibrils of beta-protein, was negative for the vascular amyloid in a case of CAA with granulomatous angiitis but positive in the others. CONCLUSIONS: In both the beta-protein and cystatin C amyloid angiopathies, cerebrovascular amyloid deposition was associated with an increase or activation of monocyte/macrophage lineage cells. Prominent reactions of monocyte/macrophage lineage cells admixed with CD4+ and CD8+ T cells (granulomatous angiitis) were occasionally associated with beta-protein angiopathy. In some of these cases, the absence of amyloid P component might be related to pathogenesis of the granulomatous reaction.

Ga-67 and Tc-99m (V) DMSA imaging in non-Hodgkin's lymphoma with amyloid tumor.

This report describes the localization and categorization of tumors using Ga-67 and Tc-99m (V) DMSA imaging in a rare case of non-Hodgkin's lymphoma associated with an amyloid tumor. Ga-67 uptake was found in both the lymphoma and the amyloid tumor. Significant Tc-99m (V) DMSA uptake was found only in the amyloid tumor. In this case, Ga-67 uptake was nonspecific, but Tc-99m (V) DMSA uptake gave additional information before treatment.

Immunohistochemical staining patterns of keratins in normal oesophageal epithelium and carcinoma of the oesophagus.

To clarify the keratin staining patterns of invasive carcinoma of the oesophagus, 22 cases of formalin-fixed paraffin-embedded surgical specimens were examined immunohistochemically with the labelled streptavidin biotin method using a panel of six different monoclonal anti-keratin antibodies. The antibody reacted adequately when antigen was retrieved in a microwave oven, and the relationship between morphological characteristics and keratin reaction patterns was analyzed in carcinomas and compared with adjacent histologically normal epithelium. In the normal oesophageal epithelium, AE3 and CK8.12 labelled all layer of cells, KS-1A3, E3 and KL1 labelled suprabasal cells, and LL002 selectively labelled the basal cells. In squamous cell carcinomas, AE3, CK8.12, KL1 and LL002 labelled almost all the tumour cells regardless of their differentiation, E3 only labelled keratinized cells, while marked decrease or loss of KS-1A3 staining was seen in all cases examined. Therefore, the characteristic profile of squamous cell carcinoma was a strong and diffuse expression of keratin 14 and 16, strong but localized expression of keratin 17, and loss of keratin 13 expression. Undifferentiated carcinoma totally lacked all keratin reactivity. The findings suggested that the neoplastic epithelial cells showed different keratin reactivity and distribution compared to normal oesophageal epithelium. In addition, histologically normal epithelium, dysplasia and carcinoma-in-situ adjacent to or overlying carcinoma expressed keratin 14.

Intra-abdominal desmoplastic small cell tumour in a patient with Peutz-Jeghers syndrome.

A surgical case of intra-abdominal desmoplastic small cell tumour with divergent differentiation (IDS-CT) in a patient with Peutz-Jeghers syndrome (PJS) is reported. The patient was a 23-year-old woman who underwent surgery for a tumour in the left paraovarian region. Widespread peritoneal metastases were noted. Histopathological examination revealed solid cell nests composed of medium-sized cells demarcated by desmoplastic stroma. Gland-like spaces were found within many cell nests. Tumour cells were positive for cytokeratin and smooth muscle actin, but negative for desmin. Both ovaries contained minute foci of a sex cord tumour with annular tubules. This is the first documentation of the association of IDSCT with PJS. Since patients with PJS have an increased risk of cancer in various organs, the occurrence of IDSCT in the present patient might not be fortuitous.

Liver scintigraphy in a patient with Gaucher disease.

Liver scintigraphy including SPECT was performed in a patient with Gaucher disease and compared with other methods. Multiple photon-deficient areas in the liver and spleen were recognized, and in evaluating the reticuloendothelial system of the liver, liver SPECT image was superior to US, CT and MR images.

Coat protein gene sequences of two cucumber mosaic virus strains reveal a single amino acid change correlating with chlorosis induction.

The coat protein genes of two chlorosis-inducing strains of cucumber mosaic virus (CMV) were compared by nucleotide sequence analysis. The predicted amino acid sequences of the encoded coat proteins were compared with those of two other chlorosis-inducing and four mosaic-inducing CMV strains. Overall, the sequences were highly conserved, with more than 95% amino acid sequence identity between any two strains. However, a proline is present at amino acid 129 of all the mosaic-inducing strains, whereas that position is occupied by either a serine or a leucine in the coat proteins of all the chlorosis-inducing strains. The correlation of chlorosis induction and a substitution for proline with leucine or serine at amino acid 129 suggests that this residue is the determinant of chlorosis induction.

Ceroid-containing histiocytic granuloma of the endometrium.

In an endometrial curettage specimen, taken from a 67-year-old woman with post-menopausal bleeding, the endometrium was replaced by aggregates of swollen histiocytes filled with yellowish-brown pigment. Histochemical and ultrastructural examination of the specimen revealed that the pigment was of ceroid nature. Previously reported cases with similar findings are reviewed, and the distinction of this type of endometrial lesion from 'foam cell reaction' is briefly discussed.
