Phase I--II study of N4-behenoyl-1-beta-D-arabinofuranosylcytosine.

A phase I-II study of N4-behenoyl-1-beta-D-arabinofuranosyl-cytosine (BH-AC) was conducted by a cooperative study group. In phase I study, a total of 126 patients, 64 of whom had metastatic solid tumors and 62 of whom had leukemia, were administered BH-AC in a single IV dose at day 1 only or in daily IV doses for 3 to 21 days, with dose ranges of 1.5--10.0 mg/kg. Side effects included nausea and vomiting, which were significantly less in incidence and severity than those observed with ara-C. Myelosuppressive toxicity became severe with doses 3.6--5.0 mg/kg per day x 10 days. In phase II study, a total of 37 adult patients with acute leukemia were entered in the study. Responses were noted, with an overall rate of 35% complete remission. Of th 26 patients with AML, there were 13 CR. The recommended schedule of treatment for BH-AC, based on our data, is daily infusion of 4--5 mg/kg over 3 h for approximately 3 weeks. The results with BH-AC in patients with acute leukemia are superior to those which have been reported for ara-C.

[Reconstructive operation of Moyamoya disease and its problems (author's transl)].

Cerebrovascular moyamoya disease is a chronic occlusive disease of unknown etiology, 90% of which are found among Japanese. At the 3rd International Symposium on Microsurgical Anastomosis held in 1976, Kikuchi and Kawasawa reported an preventing effect of STA-MCA bypass surgery for ischemic episode of this disease. Thereafter, they proposed a new operation to place temporal muscle on the brain surface, namely Encephalo-Myo Synangiosis (EMS), for the patients lacking suitable recipient artery of middle cerebral artery territory. The authors report clinical experience of eight STA-MCA anastomosis, two STA-MCA anastomosis with EMS and one EMS performed on six cases with moyamoya disease. The follow up period after operation ranged from seven to 18 months. Four patients had excellent results, and one good. One died of subdural hematoma of non-operated side seven months after contralateral bypass procedure. STA-MCA bypass surgery is an effective treatment for moyamoya disease. Also, EMS is expected to be a replaceable surgical procedure for STA-MCA bypass under absence of suitable recipient artery.

Adult T cell lymphoma with hypergammaglobulinemia.

Five cases of adult T cell lymphoma with hyperglobulinemia were reported. Two patients died of extension of the disease despite intensive chemotherapy. Three others are still under treatment. Histologically, focal proliferation of monotonous pale cells and foci composed of polymorphic large, medium, and small cells were characteristic of all five cases. Infiltration by eosinophilic leukocytes and/or small histiocytic aggregates were also recognized. In addition, proliferation of branching vessels with hyaline deposits, perivascular proliferation of "immunoblasts" and plasma cells, absence of residual follicles, and depletion of small lymphocytes were the findings, which were consistent with immunoblastic lymphoadenopathy (IBL). Varying degrees of polyclonal hypergammaglobulinemia also corresponded with those of IBL. Rosette assay of the lymph node cells revealed a mixed population of E receptor-positive large, medium, and small cells with atypical nuclei. They also had T cell antigens on the surface and revealed no terminal deoxynucleotidyl transferase activity. These findings suggested their peripheral T cell origin. The cellular characteristics of these five cases are discussed in relation to other adult T cell lymphomas and to the helper function of neoplastic T cells.

Effect of polyamines on ADP-ribosylation by chick-embryo-liver nuclei.

Effects of polyamines on poly(ADP-ribose) formation and DNA synthesis in the chick-embryo-liver nuclei were investigated. When 14-day chick-embryo-liver nuclei were incubated with [3H]NAD in the presence of 1 mM spermine, 2.5 mM spermidine, or 3.5 mM putrescine, a 9-fold increase in poly)ADP-ribose) formation was observed. Nuclei treated with nuclease showed high poly(ADP-ribose) synthetase activity as spermine-treated nuclei. However, no further increase in the polymer formation by polyamines was detected in the nuclease-treated nuclei. We found that an increase in the polymer formation by spermine was the result of an increase in both chain length and chain number of the polymer at 2.3- and 6-fold, respectively. The major ADP-ribosylated proteins were determined as two non-histone proteins of Mr 130 000 and 70 000. The experiment of DNA synthesis with nuclei ADP-ribosylated in the presence of spermine showed a 7-fold increase in [3H]dTMP incorporation into the acid-inaoluble fraction. A similar stimulation was also found with nuclei treated with other polysmines, spermidine and putrescine, in the presence of NAD. These results indicate that DNA synthesis in growing tissues containing polyamines at high levels, such as is the case with tumors and the fetus, is stimulated by polyamine-mediated ADP-ribosylation of the nuclear proteins.
