Cellular differentiation, secretory and proliferative activities of craniopharyngiomas.

The cellular differentiation of craniopharyngiomas was studied immunohistochemically by determining the expression of cytokeratins. Alcian-Blue staining was used to assess secretory activity and expression of AgNORs and PCNA as indicators of proliferative activity. Both, tumors of the adamantinomatous type and of the squamous type expressed complex type cytokeratins, with skin type differentiation being noted in some cases. In addition, epidermal differentiation of cytokeratins was found in the squamous types. Intracellular Alcian-Blue staining was limited to apical cells of tumors of the squamous type. Neither AgNORs staining nor PCNA staining proved statistically significant results with respect to the histological type of tumor or patient's age. However, PCNA staining was predominantly noted in basal cells of the squamous type tumors. Although their ultimate significance remains to be established, the results presented provide additional evidence with regard to the possible origin of both craniopharyngioma types, and suggest the potential value of some of the parameters studied as prognostic indicators.

Effects of CD4+ and CD8+ T cells in tumor-bearing mice on antibody production.

The function of T cell subsets in tumor-bearing mice was examined using an in vitro culture system of anti-(sheep red blood cell) antibody production, which is known to be dependent on T cells. The helper function of T cells of fibrosarcoma-MethA-bearing mice in antibody production decreased with the tumor stage of the mice. T cells were separated into CD4+ and CD8+ cells for further analysis of T cell subsets by the panning method using monoclonal antibodies. The helper function of CD4+ T cells in antibody production began to decrease significantly in tumor-bearing mice 1 week after the tumor transplantation. On the other hand, the suppressive function of CD8+ T cells was retained and had not decreased in the mice even 3 weeks after the transplantation. The same changes in function of CD4+ and CD8+ T cells were also observed in Meth1-bearing mice. These results suggested that this tumor-associated immunosuppression in antibody production is attributable to the decrease in helper activity of CD4+ T cells and the maintenance of the suppressive activity of CD8+ T cells.

Assessment of residual tumor viability in thymic carcinoma by sequential thallium-201 SPECT: comparison with CT and biopsy findings.

We present a case of 201TI accumulating thymic carcinoma, in which sequential CT scans demonstrated a steady decrease in tumor volume. The presence of a residual mass on CT scans after the completion of therapy presented the clinical dilemma of whether or not a viable tumor remained. Sequential 201TI SPECT images demonstrated a marked decrease in tumor uptake. At 2 wk after therapy, no significant accumulation of 201TI in the region of the residual mass was observed, indicating a lack of viable tumor. A biopsy specimen revealed no tumor cells. Sequential histopathologic findings were correlated well with the findings of 201TI SPECT rather than those of CT. Thallium-201 SPECT is of great clinical value in assessing tumor viability in the course of therapy.

[Effects of repeated treatment with electroconvulsive shock or imipramine on [3H]prazosin binding sites in the rat frontal cortex].

Two subtypes (alpha 1A and alpha 1B) of alpha 1-adrenoceptors have been defined on the basis of radioligand binding studies with 2-(2,6-dimethoxyphenoxyethyl)-1,4-benzodioxane (WB4101). In contrast, functional studies with blood vessels have proposed another subclassification, where the alpha 1-adrenoceptors can be classified into putative two (alpha 1H and alpha 1L) or three (alpha 1H, alpha 1L and alpha 1N) subtypes. The present study was performed to elucidate the effects of repeated treatment with electroconvulsive shock (ECS) or imipramine on the subtypes of alpha 1-adrenoceptors. The density of the alpha 1H binding sites (including alpha 1A and alpha 1B binding sites) in the frontal cortex was increased by repeated treatment with ECS but decreased by imipramine. These treatments did not change the density of alpha 1L binding sites. These results indicated that repeated treatment with ECS and imipramine had different effects on the alpha 1H binding sites in the rat frontal cortex. In addition, the effects of antipsychotic drugs, antidepressant drugs and calcium channel antagonists on [3H]prazosin binding were studied. The order of inhibition of [3H]prazosin binding was: antipsychotic drugs (1.4-4.2 nM) > antidepressant drugs (69-116 nM) > calcium channel antagonists. The calcium channel antagonists, verapamil (720 nM), diltiazem (4.3 microM) and nicardipine (75 microM) were weakly bound. These results suggested that the antimanic effects of calcium channel antagonists were not associated with inhibition of alpha 1-adrenoceptors.

Mucin-producing pancreatic tumors: historical review of its nosological concept.

A brief historic outline of the problem of mucin-producing pancreatic tumors is presented. Based on the authors' observations, clinical aspects and pathomorphology of these tumors have been described. The authors propose their own classification of this tumor type which is based on the literature published so far. Their classification also takes into account the localization of lesions. Reference is made to the concept described by the term "mucinous ductetatic/cystic lesions" (MDCL) and it is also pointed out that mucinous carcinoma may develop on the background of MDCL. Since the appearance of the term "mucus secreting pancreatic cancer" or "mucin-producing pancreatic tumor", many similar and/or related conditions have been described especially in Japan under the same or different names. However, there seems to be some confusion about the concept of this condition not only among clinicians but also among pathologists. In addition, another entity, mucinous cystic neoplasms of the pancreas, was proposed and may have provided some overlap with the former conditions in its concept. Furthermore, definitions of these two conditions varied according to the authors. In this paper, therefore, we review and critically analyze cases of mucin-producing pancreatic tumor (MPPT) as well as mucinous cystic neoplasm (MCN) of the pancreas, and intend to classify them under a generic term, i.e. "mucinous ductectatic/cystic lesions (MDCL) of the pancreas".

Mucinous cystadenocarcinoma of the pancreas with mucinous biliary obstruction due to uncommon complication of fistulas to the common bile duct and main pancreatic duct.

We report a case of biliary obstruction by a large amount of mucinous material produced by a cystadenocarcinoma of the pancreas invading the common bile duct. This diagnosis was made preoperatively by various imaging methods. Endoscopic retrograde pancreatography (ERP) revealed a mucus-containing cyst with a communication to the main pancreatic and common bile ducts due to a fistulization. Mucinous biliary obstruction was observed. This case indicates that mucinous biliary obstructions should be considered in patients with a mucinous cystic neoplasm of the pancreatic head in the presence of jaundice and that mucinous cystadenocarcinoma may develop into mucinous carcinoma, especially in cases with fistula formations. ERP is useful for detecting the presence of a fistula to the common bile duct or other neighboring organs in such cases. Surgical treatment is recommended because the prognosis of mucinous cystadenocarcinoma is better than that of the usual pancreatic cancer, even though the tumor shows a communication with neighboring organs.

Left ventricular functional reserve in nonobstructive hypertrophic cardiomyopathy: evaluation by continuous left ventricular function monitoring.

UNLABELLED: The cardiac functional response to exercise in patients with nonobstructive hypertrophic cardiomyopathy (HCM) was evaluated using a continuous ventricular function monitor with a cadmium telluride detector (CdTe-VEST). METHODS: Supine ergometric exercise was performed under CdTe-VEST monitoring in 41 patients with nonobstructive HCM (34 men and 7 women, age 18-72 yr, mean 51 yr) and 15 patients without cardiac disease (9 men and 6 women, age 36-56 yr, mean 49 yr). RESULTS: Although 20 of 41 patients with HCM maintained a LVEF above baseline at peak exercise (Group A), 21 did not show an EF increase at peak exercise (Group B). Exercise duration and work load in Group A were longer and higher, respectively, than in Group B. Resting EF in Group B (72 +/- 7.7%) was significantly higher than that in Group A (65 +/- 8.2%) and the control group (62 +/- 5.9%). The EF increase from baseline to EF overshoot during recovery and the time to EF overshoot were lower and longer, respectively, in Group B than in Group A and the control group. Septal wall thickness and the septum-to-posterior-wall-thickness ratio between Groups A and B were not different. ST-segment depression was observed in all 21 Group B patients and in 8 of the Group A patients. CONCLUSION: In patients with nonobstructive HCM, left ventricular dysfunction during exercise and during recovery was frequently observed but was not related to the degree of septal wall hypertrophy. The CdTe-VEST is a useful means to evaluate left ventricular functional reserve to exercise in patients with HCM.

Basilar artery migraine. Reversible ischemia demonstrated by Tc-99m HMPAO brain SPECT.

The authors present a case of basilar artery migraine in a 33-year-old woman who was initially misdiagnosed as having a cerebrovascular or mental disorder and subjected to MRI and cerebral angiography, which, however, did not show any pathologic findings. During admission to the university hospital, she lost consciousness. An emergency Tc-99m HMPAO brain SPECT showed a significant decrease of regional cerebral blood flow in the right temporal and occipital cortices, and right cerebellar hemisphere, where regional cerebral blood flow was decreased by 10-24% as compared to the left side. The second brain SPECT during a symptom-free phase showed the reversion of regional cerebral blood flow to normal in these areas. Basilar artery migraine was diagnosed by the finding of reversible ischemia in the territory of the right basilar artery on brain SPECT images and the clinical picture.

[Relationships between serum vitamin-A and subjective health, blood and urinary biochemical parameters, dietary intake and food preferences in students].

Students, 60 females (mean age of 19 years) and 54 male (mean age of 23.8 years), were studied for relationships between serum vitamin A concentration (V-A level) and subjective health (by CMI method), birth order, blood and urinary parameters, dietary intake and food preferences. V-A level for females was significantly lower than for males. V-A level significantly and positively correlated to the parameters of hepatic enzymes (gamma-GTP, GOT, GPT), lipid (T-chol, TG) and urinary Na/K. The V-A level showed independent significant correlation to gamma-GTP in multiple regression analysis. V-A level of those born second in birth order showed a tendency to be lower than those first and third in birth order. In females, subjects without subjective CMI complaints had higher level of V-A than those with complaints. This tendency was not seen in males except for items related to the respiratory system. V-A level showed an apparent increase corresponding to daily intake of soybean and vegetables in females, and with amounts of drinking or smoking in male. In males, V-A levels showed a significant positive correlation to serum oxygen peroxide measured by malondialdehyde and did not show an anti-peroxidation effect. In addition positive correlation to total cholesterol was observed in males.

[A case of perforated mitral valve aneurysm following aortic valve replacement associated with infective endocarditis].

A case of perforated mitral valve aneurysm following aortic valve replacement associated with infective endocarditis was reported. The patient was a 29-year-old man, who was suffering from high fever, Osler's nodules and headache. A brain abscess was recognized in a computed tomography and 3rd grade aortic regurgitation was recognized in echocardiogram and aortography. Hematological studies suggested the inflammation and gram-positive cocci was incubated from his arterial blood. Then infective endocarditis with aortic regurgitation was diagnosed. AVR was performed following 8 weeks treatment with antibiotics, when he had negative CRP and his blood culture. After the operation, he was received the intravenous antibiotic therapy for 6 weeks and oral antibiotic drugs was given following his hospital discharge. At 6 months after AVR, mitral valve aneurysm was recognized in his echocardiogram. At 30 months after AVR, the perforation of it was revealed and mitral valve replacement was performed with his negative blood culture. The patient was discharged 28th day after MVR. There has been no active inflammation from his first hospital discharge and following days, the mitral valve aneurysm and the perforation was caused by weakened tissue of the anterior mitral leaflet due to sibilant inflammatory change.

[Successful repair of right ventricular rupture due to blunt trauma].

Successful repair of blunt traumatic rupture of ventricle due to traffic accident is reported. Patient was a 73-year-old woman who was driving and had seated with seatbelts at the traffic accident. Arriving at our hospital, her blood pressure was under 50 mmHg and she had clouding of consciousness, although no brain injury was recognized. No heart beating was visible in X-ray television and pericardial effusion was recognized in the echocardiogram. The diagnosis of acute cardiac tamponade was established within 7 minutes of arrival. The pericardiectomy was performed immediately, then blood pressure was improved to 70 mmHg. The ruptures of right ventricle were repaired without cardiopulmonary bypass via median sternotomy. Post-operative course was without trouble and patient was discharged within 2 weeks.

[A study of diagnostic scale in borderline personality disorder that was synthesis of symptomatic and personality structural element].

The diagnostic criteria for BPD such as the DSM-III(-R) and DCR, based on polythetic format for prototypal categories, has not always been able to result in accurate clinical diagnosis of BPD. Reasons posited for this were that DSM-III(-R) criteria consist of symptom items based on descriptive phenomenology, even for Axis II personality diagnosis, and that descriptions of the criteria were vague because of the standardization of vocabulary which aimed at improved inter-rater reliability. With the polythetic "yes/no" format, diagnosis was influenced by the determination of only one item; furthermore, different combinations of items yielding a heterogeneous membership might lead to the same diagnosis. It was, therefore, considered difficult to perform an accurate diagnosis of BPD in clinical practices using the existing general diagnostic criteria, and it was realized that the development of new diagnostic criteria for BPD was necessary. For those reasons, we generated a Clinical BPD Scale (CBS) as diagnostic evaluation scale of BPD with considering advantages for clinical use. This reconstituted some features of BPD which have been reported by many researchers, synthesizing symptomatic elements and personality structural elements. CBS is composed of four clusters, Two of these four clusters are evaluated by four grade anchor points which are provided for the rating of the severity of symptoms, and that are illustrated on the radarchart for comprehensive evaluation of the severity of the disorder. CBS was confirmed to have a high degree of validity, and achieved a satisfactory degree of inter-rater reliability by ANOVA ICC, and it has been found in application of CBS to several cases in the clinical practice that the degree of severity of each symptom could be clarified and differential diagnosis was possible with this diagnostic scale. Furthermore, the outcome of treatment could be confirmed at any time during the clinical follow up. In addition, through the dimensions, it is possible to grasp the severity, and further, through spectrum and hierarchy. We considered diagnostic area of BPD and its position by CBS correlating the other closely related disorders.

[Advantages of combined antegrade/retrograde GIK cardioplegia for coronary artery bypass surgery].

We evaluated the advantages of combined antegrade and retrograde GIK cardioplegia in 65 patients undergoing CABG. Thirty seven patients were administered antegrade cardioplegia (Group A), whereas 28 patients were administered combined antegrade and retrograde cardioplegia (Group C). Enzyme release and hemodynamic data were obtained before the onset of CPB and at 1, 3, 6, 9, 12 and 24 hours after operation. Both groups were similar in age and incidence of diabetes, previous MI, PTCA, severity of coronary artery disease and emergent operation. In group A, antegrade cardioplegia produced poor anterior septal cooling in 17 patients (45%). To ensure adequate myocardial protection in these cases 9, patients were anastomosed saphenous vein graft first to LAD instead of IMA and perfused cardioplegic solution through the grafts. In group C, although adequate anterior septal cooling also could not be obtained with antegrade cardioplegia in 16 patients (57%), after retrograde cardioplegia, anterior septal temperature fell below 10 degrees C. The use of IMA graft was more practiced in group C. (79% in group C versus 32% in group A, p < 0.05) Enzyme release (CPK-MB, %CPK-MB, LDH) and hemodynamic data (CI, LVSWI, RVSWI, RVEF, RVEDVI) were similar in both group. Furthermore, no significant difference were noted in the incidence of post-operative LOS, PMI and ventricular arrhythmia. We concluded that the use of combined antegrade and retrograde cardioplegia is more effective than antegrade cardioplegia, because of adequate anterior septal cooling, and it will allow patients with severe and extent coronary artery disease to undergo safe IMA grafting.

A multicenter study on the prognosis of fulminant viral hepatitis: early prediction for liver transplantation.

To determine the risk of death at an early stage of fulminant viral hepatitis, we created severity indexes drawn from clinical data on the day of development of encephalopathy in 128 patients with fulminant hepatitis B and 103 with fulminant hepatitis non-A, non-B. In fulminant hepatitis B, the risk score was 2.75 x BL + 2.75 x BR + 2.7 x AG + 2.3 x WB + 1.67 x CD + 1.56 x AL - 0.098 x PR - 0.88, where BL is 1 if total bilirubin is higher than 20 mg/dl, BR is 1 if the ratio of total to direct bilirubin exceeds 2.2, AG is 1 if age is above 40 yr, WB is 1 if white blood cell count is less than 4,000 cells/mm3 or more than 18,000 cells/mm3, CD is 1 if a hazardous disease coexists and AL is 1 if ALT is less than 100 times the upper limit of normal (otherwise all are 0), and PR is prothrombin time (percentage of normal value). Using a cutoff score of 0, we found the positive predictive value, negative predictive value and predictive accuracy to be 0.90, 0.86 and 0.89, respectively. Sensitivity and specificity were 0.94 and 0.77, respectively. In fulminant non-A, non-B hepatitis, the risk score was 2.66 x BR + 2.25 x BL + 2.24 x DI + 2.05 x AL +/- 1.38 x AG + 0.00021 x WB - 6.33.(ABSTRACT TRUNCATED AT 250 WORDS)
