Development of corticospinal tract fibers and their plasticity. II. Neonatal unilateral cortical damage and subsequent development of the corticospinal tract in mice.

In this study, the right cerebral cortices of mice on postnatal day 0 (P0) were cryocoagulated with dry ice. Subsequent development of the corticospinal tract (CST) was studied morphologically and quantitatively, and was compared with that in age-matched controls. When the pyramidal tract was traced anterogradely by injecting HRP into the sensorimotor area of the left cerebral cortex of adult operated mice, the right CST originating from the healthy left hemisphere showed remarkable hypertrophy. The number of axons in the CST at the C4-C6 level became maximum on P14 in the control mice and rapidly decreased thereafter. In the operated mice, the axonal number in the right CST also was maximal on P14 and then rapidly decreased. However, the decrease in axonal number after P21 was less in the operated mice than in the controls. Moreover, the number of axons showed a slight increase after P56. These results indicate that the physiological elimination of the parent axons and their collaterals is much lower in the operated mice than in the controls, and that the increase in axon collaterals from parent axons in the hypertrophic right CST persists a long time in the operated mice.

Asymptomatic cerebral infarction in Kawasaki disease.

Meningitis, facial palsy, subdural effusion, and cerebral infarction have been reported to be some of the nervous system complications of Kawasaki disease which usually are clinically obvious. A 22-month-old boy with Kawasaki disease, who developed asymptomatic cerebral infarction, is reported. Hyperpyrexia persisted for 57 days; echocardiography and magnetic resonance imaging revealed the presence of aneurysms in the coronary, axillary, and internal iliac arteries. This study indicates that patients with Kawasaki disease may have silent cerebral infarction.

Ectasis and pallor of peripheral airways during mechanical ventilation.

STUDY OBJECTIVE: To evaluate morphologic changes in peripheral airways associated with positive-pressure ventilation (PPV). DESIGN: Single-blind study. SETTING: Intensive care unit of a university hospital. PATIENTS: Ten patients who required mechanical ventilation due to acute respiratory failure but had no history of prior pulmonary disease. INTERVENTIONS: Seventeen fiberoptic bronchoscopy procedures were performed using a fine fiberoptic bronchoscope of 1.8 mm outside diameter. MEASUREMENTS AND MAIN RESULTS: The main morphologic findings were pallor and ectasis. Increased secretions, pigmentation, and stenosis or collapse in peripheral airways also were observed. Patients with ectasis were more likely to have had prolonged mechanical ventilation (17 +/- 9 days) as compared with those patients without ectasis (9 +/- 5 days). CONCLUSION: The structural alterations in peripheral airways noted in patients on prolonged PPV suggest that barotrauma may be more widespread than previously recognized.

Prevalence of senile dementia in a rural community in Japan: the Tajiri project.

Knowledge of the prevalence of dementia in different age groups is needed for the planning of a health policy. This study shows the prevalence of dementia and magnetic resonance imaging (MRI) findings in elderly people aged 65 years and over, living in the town of Tajiri in the northern part of Japan. They were shown by two cognitive screening tests, the Mini-Mental State examination (MMS) and the Dementia Screening Test (DST) and medical diagnosis. Two subject groups were assessed, those who completed both tests (Subjects I, n=2066) and those from among the 200 'MRI-administered subjects' who were interviewed and diagnosed (Subjects II, n=170). For Subjects I, there were 6.3 and 10.2% 'dementia range' according to the severe and mild criteria, respectively. As for Subjects II, 9.4% were clinically diagnosed as having dementia. They met the National Institute of Neurological and Communicative Disorders and Stroke and the Alzheimer's Disease and Related Disorders Association (NINCDS-ADRDA) criteria of probable Alzheimer's disease (AD) or possible AD with cerebrovascular disease. The estimated prevalence rate of dementia was 8.0%. Visual ratings of brain atrophy using MRI disclosed two distribution patterns. The 'continuous' pattern of the frontal and temporal lobes atrophy suggest that both are affected by the aging process, while a 'discontinuous' pattern of the hippocampal atrophy could indicate a pathologic background such as early changes of AD.

Efficacy of three-dimensional fast imaging with steady precession dynamic MR imaging in evaluating pancreatic ductal adenocarcinoma.

To compare the diagnostic efficacy of three-dimensional fast imaging with steady precession (3D-FISP) dynamic MR imaging in assessing pancreatic ductal adenocarcinoma with conventional MR imaging and helical CT, 15 patients with surgically proven pancreatic ductal adenocarcinoma were studied. Contrast-to-noise ratio (CNR) of the tumor, tumor detectability, local tumor extension, and vascular involvement were evaluated for all techniques. The results indicate that 3D-FISP dynamic MR imaging may improve the imaging assessment of pancreatic ductal adenocarcinoma.

A simple method for detecting breakthroughs in used chemical cartridges.

This study was intended to develop a simple method using a gas detector tube to detect the end of the service lives of chemical cartridges for organic solvent vapors. The practical usefulness of this method was confirmed by two series of experimental observations of cartridge service lives: (1) The breakthrough times for six organic solvents were determined by passing a test airflow through a cartridge, and the concentration at each breakthrough was measured with a gas chromatograph (GC). These GC-obtained concentrations were compared with the migrated vapor concentrations through each broken cartridge measured with a gas detector tube. (2) CS2 breakthrough of the cartridges used on 32 viscose rayon workers were measured with detector tubes, after which the residual service life of each cartridge was determined. In laboratory experiments the released concentrations detected by the gas detector tube were approximately equal to the final leak concentrations measured by GC. In a field survey the used cartridges exhibiting a stain of CS2 from detector tubes for concentrations greater than 4 ppm were found not to have enough residual adsorption capacity for CS2. Migrated concentrations of CS2 measured with detector tubes were found to be a reasonable predictor of remaining service life.

Modulation of immunologic reactions between cultured porcine hepatocytes and human sera.

In the clinical application of a hybrid artificial liver system using porcine hepatocytes, some immunologic reactions occur between human serum and porcine hepatocytes. In this study, we investigated the immunologic mechanisms of the cytotoxic reactions, and we tried to inactivate the human serum cytotoxicity by heating the serum or the addition of nafamostat mesilate (NM). Immunologic reaction between human serum and porcine hepatocytes by evaluating the immunochemical response against human IgM, IgG, and C3 was investigated. The immunochemical analysis of inactivation by heated human serum (56 degrees C, 30 min) and adding NM were performed. The evaluation of serum cytotoxicity was as follows: when porcine hepatocytes were cultured with heating the human serum or the addition of NM, the survival ratio was observed. Immunochemical reactions against human C3 were all positive, but positive reaction against human IgM occurred in only one case (5%); those against human IgG were all negative. Both heating the serum and adding NM inhibited the immunochemical reaction of human C3. The inhibition of human C3 with NM was dependent on that concentration. Both heating of the serum and adding NM to the medium decreased damage of porcine hepatocytes. An immunologic reaction between human serum and porcine hepatocytes in a porcine bioartificial liver clearly occurred, and this reaction was controlled by heating the serum and adding NM. We believe that NM is useful in the clinical application of our hybrid artificial liver system.

Hepatic angiomyolipoma: report of changing size and internal composition on follow-up examination in two cases.

We present two cases of hepatic angiomyolipoma in which the size and internal composition of the tumor changed during the course of follow-up study. The tissue elements composing the tumor are thought to grow or regress independently during the disease's clinical course. Radiologists should be aware that hepatic angiomyolipoma can change in size and internal composition during its natural course.

Radiologic features of intrahepatic bile duct adenoma: a look at the surface of the liver.

We report the radiological features of intrahepatic bile duct adenoma (BDA) in three patients. BDA was shown as a small mass located in the peripheral region of the liver with each imaging modality: a hypervascular mass on angiography and a mass appearing as early nodular enhancement found disproportionately evident compared with their small size and distinct delayed or prolonged enhancement on CT. BDA should be included in the diseases to be differentiated from hypervascular hepatic tumors.

Chronic hepatitis associated with Epstein-Barr virus infection in an infant.

A 1-year-old infant girl had a chronic illness characterized by persistent intermittent fever, thrombocytopenia, granulocytopenia, and marked hepatosplenomegaly. Histological findings on liver biopsy 307 days after the onset of her illness disclosed marked mononuclear cell infiltration and moderate fibrosis in the portal and perilobular areas. Interferon-therapy induced no clinical and laboratory improvement, whereas the patient's clinical and biochemical findings responded to prednisone. Although these findings suggested a possible autoimmune mechanism, there were no significant findings in extensive immunological and serological tests consistent with autoimmune disease. The profile of antibodies to Epstein-Barr virus (EBV)-specific antigens in the patient demonstrated that this prolonged atypical illness was the result of persistent EBV infection.

An infant with neuroblastoma and MYCN amplification found through mass screening.

PURPOSE: This report describes an extremely rare case of an infant with neuroblastoma involving MYCN amplification found through mass screening (MS). PATIENT: An 8-month-old boy was referred to our hospital after screening for neuroblastoma showed positive results. A firm tumor was found above his left kidney and completely resected. RESULTS: The tumor arose from the left adrenal gland. The disease was classified as stage I according to the International Neuroblastoma Staging System. Although the tumor showed a favorable histology, MYCN was amplified to 20 copies. After surgery, the infant underwent chemotherapy, but a new mass in the right adrenal gland was found. After chemotherapy, the tumor was excised. Despite several courses of chemotherapy, multiple metastases in both lung fields appeared. More intensive chemotherapy and allogenic peripheral blood stem cell transplantation were performed, but the infant died from pulmonary hemorrhage after the transplantation. CONCLUSIONS: The majority of patients with neuroblastoma found through MS have a good outcome. However, some patients have poor prognostic factors, like our patient with MYCN amplification. There is a need to develop a suitable protocol for babies with high risk features younger than 1 year of age found through MS.

Outcome of patients with early ovarian cancer undergoing three courses of adjuvant chemotherapy following complete surgical staging.

We conducted the present study to determine the outcome of patients with early ovarian cancer who underwent three courses of adjuvant chemotherapy after complete surgical staging. One hundred consecutive patients with stage I-II epithelial ovarian cancer who had undergone complete surgical staging and received three courses of platinum-based chemotherapy were entered in this study. Twenty-one patients were low risk, defined as stage IA-B, grade 1 and histologic types except for clear cell adenocarcinoma, and remaining 79 were high risk. All patients with stage IA or IB, whatever histologic type and histopathologic grade, were alive without disease. The 5-year survival rate was 89.4% for patients with stage IC and 76.2% for those with stage II. The 5-year survival rate for low- and high-risk patients was 100% and 89.4%, respectively. The survival rate for grade 1 was significantly better than that for grade 2 or 3. Multivariate analysis revealed that histologic grade was an independent prognostic factor in stage IC-II ovarian cancer. The outcome of patients with early ovarian cancer undergoing three courses of chemotherapy after complete surgical staging was favorable even in high-risk patients.

Mucosal squamous cell carcinoma of the esophagus: a clinicopathologic study of 30 cases.

A clinicopathologic study was carried out on 30 patients with mucosal esophageal cancer (MEC). The depth of cancer invasion was subdivided histologically into three categories: m1 = carcinoma in situ (intraepithelial carcinoma) or carcinoma with questionable invasion beyond the basal membrane; m2 = cancer invasion confined to the lamina propria, and m3 = cancer reaching to or infiltrating into the muscularis mucosae. Lymph node metastases and lymphatic invasion were found only in the tumors reaching or infiltrating the muscularis mucosae (m3). The maximum histologic vertical extent of the tumors was more than 1 mm in 4 of 5 patients with lymph node metastasis or lymphatic invasion. None of the patients died of recurrent esophageal disease, and 3 of the 6 patients who had a second primary tumor died of this other malignancy. It is critical to distinguish between m1, m2 and m3 tumors to plan a treatment strategy, including an endoscopic mucosal resection.

Childhood form of adrenoleukodystrophy in the sucking infant.

The childhood form of adrenoleukodystrophy (ALD) in a sucking infant was discovered before the appearance of clinical symptoms. C24:0/C22:0 and C26:0/C22:0 ratios in umbilical cord plasma were significantly higher so that very long-chain saturated fatty acid-restricted milk was given. Ultrastructural study on biopsied rectal mucosa revealed rectilinear and/or curved electron-lucent clefts in the histiocytes at 3 months of age, which were also recognized in the elder brother and another boy with symptoms of ALD. However, magnetic resonance imaging and T1 and T2 values were within the normal range in cerebral gray matter, cerebral white matter, caudate nucleus, internal capsule, and thalamus.

Overexpression of low density lipoprotein receptor on Chinese hamster ovary cells generates foam cells.

The atherosclerotic lesion is characterized by the presence of cholesterol-loaded foam cells. Chinese hamster ovary (CHO) cells do not normally store cholesteryl esters because low density lipoprotein (LDL) receptors are suppressed by exposure of these cells to LDL cholesterol. We transfected LDL receptor cDNA linked to the simian virus 40 early promoter into CHO cells (CHO 29) and found that LDL receptor binding in these cells was not suppressed by an excess amount of LDL cholesterol, indicating no regulation of the LDL receptor in CHO 29 cells. Furthermore, CHO 29 cells showed a high activity of LDL uptake and intracellular accumulation of cholesteryl esters. Light-microscopic examination demonstrated the resulting formation of foam cells in CHO 29 cells in the presence of 5 micrograms LDL/ml. These results demonstrated that foam cell changes in atherosclerotic lesions can be reproduced in CHO cells, whose LDL receptor activity is overexpressed, through the mechanism of LDL receptor-mediated endocytosis of native LDL.

A personal computer network system for equitable allocation of cadaver organs.

We developed a personal computer network system for the equitable allocation of cadaveric organs. This network consists of a host computer (IBM PS55 model 5570 T) and various kinds of personal computers manufactured by many different computer makers in Japan. The merits of our personal computer network include lower cost and an easy access to the host computer from all the centres participating in this network while using their own favourite personal computers. Among the programs made for allocating cadaveric organs, we present in this paper the program for livers. This program was developed with a modified version of the logic developed by Starzl et al. The grade modification for the United Network for Organ Sharing (UNOS) in the United States was used as the basis for classification of medical urgency. Our program weighed the factors of medical urgency, compatibility of blood group and waiting time. Distance factors were omitted because of the smaller area of the network compared to that of UNOS. This computer network would be linked to other computer networks in creating a national organ procurement and transplant network in Japan, in order to help them to catch up with other advanced transplant countries. Such an equal and objective computer system should allow organ transplantation to become more widely accepted.

Interactive statistical analysis system for clinical investigators.

We have developed an interactive statistical analysis system (ISAS-Q) with which clinical investigators with little experience in computers and programming can easily perform statistical analyses. ISAS-Q can perform most of the frequently used statistical methods, including multivariate analysis, in an interactive mode. Furthermore, ISAS-Q has self-consistent and extensive help functions.
