A disposable patient identification card made of a paper.

This paper describes a patient identification system with a disposable paper card. In general, total costs of cards themselves, equipments and personnel are remarkable, not negligible for the hospital management. Therefore, a disposable identification cards made of a paper were issued to out-patients in our hospital. Many order forms were integrated into only one sheet. Patient identification data were printed on this sheet by a computer system when a patient came to the reception desk, and quickly transmitted to physicians. We could save the hospital costs and printing works by physicians, and also shorten the waiting time of patients at reception desks.

Enzymatic production of L-tryptophan from DL-serine and indole by a coupled reaction of tryptophan synthase and amino acid racemase.

Enzymatic production of L-tryptophan from DL-serine and indole by a coupled reaction of tryptophan synthase and amino acid racemase was studied. The tryptophan synthase (EC 4.2.1.20) of Escherichia coli catalyzed beta-substitution reaction of L-serine into L-tryptophan and the amino acid racemase (EC 5.1.1.10) of Pseudomonas putida catalyzed the racemization of D-serine simultaneously in one reactor. Under optimal conditions established for L-tryptophan production, a large-scale production of L-tryptophan was carried out in a 200-liter reactor using intact cells of E. coli and P. putida. After 24 h of incubation with intermittent indole feeding, 110 g liter-1 of L-tryptophan was formed in molar yields of 91 and 100% for added DL-serine and indole, respectively. Continuous production of L-tryptophan was also carried out using immobilized cells of E. coli and P. putida. The maximum concentration of L-tryptophan formed was 5.2 g liter-1 (99% molar yield for indole), and the concentration decreased to 4.2 g liter-1 after continuous operation for 20 days.

Praziquantel-induced secretion of proteolytic enzyme from Schistosoma mansoni cercariae.

The effect of praziquantel (PZQ) on secretion of proteolytic enzyme by Schistosoma mansoni cercariae was examined using an azocoll assay. The cercariae secreted proteolytic enzyme in various concentrations of PZQ (0.1, 1, and 10 micrograms/ml), but secretion of enzyme was highest at the lowest concentration. PZQ-induced secretion of proteolytic enzyme was partially inhibited by treatment with verapamil and ethylene glycol-bis(beta-aminoethyl ether)N,N,N',N'-tetra-acetic acid but not by calmodulin antagonist W-7 and protein kinase C inhibitor H-7.

[Hepatocellular carcinoma with a complete remission of bone metastasis achieved by arterial chemotherapy].

Reported is the case of a hepatocellular carcinoma with a complete remission of the bone metastasis by arterial chemotherapy. The patient was 60 year old male, with the chief complaint being a tumor at the right side of the chest. The diagnosis was a hepatocellular carcinoma with a bone metastasis at the right 10th rib. The bone tumor showed no decrease in size after an intra-arterial injection of adriamycin, and radiation and or hyperthermia. Therefore, intra-arterial injections of mitomycin C mixed with lipiodol were instituted through the intercostal artery, and no bone tumor was noted after start of this therapy. Although the patient subsequently died, microscopic examination of a specimen obtained at autopsy revealed no malignant cells at the right 10th rib. Intra-arterial injections of anticancer agents mixed with lipiodol therefore are thought to be useful for the treatment of a bone metastasis.

[Influence of intrathecal morphine and buprenorphine on EEG and their analgesic effect].

This study was performed on fifty patients undergoing gynecological operations of lower abdomen. Their mean age and body weight were 43 +/- 5yrs and 55 +/- 8kg (mean +/- SD) respectively. All patients were given spinal anesthesia for surgery. Patients were divided into four groups of ten patients at random as follows: Group I (control) received 7.5mg of dibucaine only, Group II received 7.5mg of dibucaine with 0.05mg of buprenorphine (B), Group III received 7.5mg of dibucaine with 0.25mg of morphine (M), Group IV was the same as G-III but received 0.2mg of naloxone after operation. We observed EEG during operation and assessed intrathecal B and M for postoperative analgesic effect and side effects. EEG finding was as follows. In G-I, alpha and beta waves appeared dominantly during operation. In G-II, G-III and G-IV, EEG showed specific beta wave which is characterized with low voltage and rapid activity. This specific EEG change emerged at 10 minutes after the intrathecal injection of M in G-III and G-IV, and at 60 minutes after the intrathecal injection of B in G-II. This finding showed that intrathecal M with less lipid solubility reached more rapidly to CSF in the fourth ventricle than intrathecal B. The result suggests that the earlier respiratory depression may be caused by large doses of intrathecal M. Postoperative analgesic effect was recognized remarkably in G-II, G-III and G-IV (P less than 0.01). The duration for pain relief was 6 hrs in G-II and 12 hrs in G-III and G-IV.(ABSTRACT TRUNCATED AT 250 WORDS)

[Mixed type total anomalous pulmonary venous connection with the left upper pulmonary vein draining into the innominate vein and the other pulmonary veins into the coronary sinus--surgical correction in 2 infants].

Surgical repair of the mixed type total anomalous pulmonary venous connection (TAPVC) in infancy is known to be difficult. In this report, experience of two infants with the mixed type TAPVC with the left upper pulmonary vein (PV) draining into the innominate vein and the other PVs into the coronary sinus (IIa + Ia (left upper PV) type TAPVC) is presented. In such cases, it has been said that the left upper PV should be left uncorrected, but we anastomosed the left upper PV, that was thin and fragile, to the left atrium and corrected all the other anomalous PVs. The results were satisfactory. Recently, Extra-Corporeal circulation, microsurgery, and other techniques have significantly progressed, so we consider that the anastomosis is possible in most infants with IIa + Ia (left upper PV) type TAPVC. In the future, careful clinical follow-up and repeat catheterizations should be done, and we'd like to examine the long term patency of the left upper PV.

[Long-term (10 to 23 years) clinical evaluation of quality of life in patients following open heart repair of tetralogy of Fallot].

The quality of life of 78 patients out of 79 (99% follow-up rate) was evaluated 10 to 23 years following intracardiac repair of tetralogy of Fallot. Mean age at operation was 6 years of age. Their present ages ranged from 12 to 35, averaging 21 years old. Eighty three percent of patients are assessed to be in good quality of life so far in terms of physical development, life style, CTR values, Ekg findings and so on. Pregnancy and delivery in 5 patients were uneventful. On the other hand, 17% showed various degree of morbidity including significant residua necessitating reoperation in 3 patients, complications such as complete A-V Block requiring pacemaker implantation in 7 and sequela like postoperative life threatening cardiomyopathy in 3. There were 2 deaths due to this sequela comprising 2.6% of mortality rate. Although excellent results are reported in recent years, further efforts will be necessary to minimize residuum, complication and sequela after surgery especially for this particular disease with broad morphological spectrum.

[Experimental study on pathogenesis of neonatal hypoxic-ischemic encephalopathy. I. Histopathological study].

This experiment was undertaken to study the effect of hypoxic and/or ischemic condition on developing brain and also to investigate the pathogenesis of hypoxic-ischemic encephalopathy. A unilateral common carotid artery of 7-day-old Sprague-Dawlay rats was ligated and cut under ether inhalation. Three hours after the operation, these rats were exposed to hypoxia (8% oxygen and 92% nitrogen mixture) for two hours. Three days after manipulation, neuronal necrosis on the ligated hemisphere were recognized in 50% in the cortex, 66.7% in the hippocampus, 66.7% in the corpus striatum and 33.3% in the thalamus. Various sizes and degrees of necrotic regions were found in the cerebral cortex. In some cases, cuffs of radially arranged viable neurons alternating with stripes of full-depth necrosis of the cortex were noticed. In the hippocampus, neuronal necrosis was found segmentally at the pyramidal cell layer. In the corpus striatum and the thalamus, the damage consisted of small focal or multifocal neuronal necrosis. After four and eight weeks of the manipulation, a large porencephalic cavity in the cerebral cortex of the ligated side was noticed as the severest manifestation in 16% of the cases. In some cases, multiple lobulation similar to poly-microgyrus was noticed. Various sizes of gliosis were also numerous in the cerebral cortex, hippocampus, corpus striatum and thalamus of the ligated side hemisphere. Calcified lesions were also occasional in the basal ganglia.

[Experimental study on pathogenesis of neonatal hypoxic-ischemic encephalopathy. II. Energy metabolism in the brain].

In a preceding paper, we showed that the exposure to hypoxia after unilateral carotid artery ligation of sucking rat on 7 days of age caused various pathological changes in the ipsilateral forebrain, which had close similarity to human perinatal hypoxic-ischemic encephalopathy. To evaluate the energy metabolism in various regions of the brain during hypoxia after unilateral carotid artery ligation, tissue pH was monitored in the cortex by direct insertion of an electrode. The contents of the lactate, pyruvate, ADP and ATP were also measured during hypoxia. The tissue pH in the cortex of ligated side declined rapidly after initiation of hypoxia as compared to non-ligated side. The contents of pyruvate and ADP showed no particular changes through various regions. Only in the cortex and hippocampus of ligated side, ATP was decreased, and elevation of lactate was more salient than in non-ligated side. In the striatum and thalamus, only elevation of lactate was indicated. However, the levels of lactate, pyruvate, ADP and ATP disclosed no significant difference between the ligated side and non-ligated side, although neuropathological changes were observed only in their ligated side. These results suggest that pathogenesis of the cerebral damage caused by hypoxic-ischemic condition may be different between the cerebral cortex and subcortical structures.

[A case of tricuspid atresia with unroofed coronary sinus].

Tricuspid atresia with unroofed coronary sinus is an extremely rare cardiac anomaly. A 15-year-old boy who was successfully operated was suspected this disease preoperatively. We speculated this disease by right atrial angiography and underwent the direct suture closure of a coronary sinus defect and Fontan's operation. When we perform Fontan's operation on tricuspid atresia, we should take the existence of unroofed coronary sinus into our consideration. The echocardiography, cardiac catheterization and angiography, especially, right atrial angiography had to be carried out preoperatively. During the operation, it is important to check over the influence of cardioplegic solution from the ostium of coronary sinus. If not, elaborate inspection have to be done on the back wall of the left atrium through ASD to search for abnormal communication directly or by inserting sound from coronary sinus.

[Treatment of chronic retinal artery obstruction with stellate ganglion block and electric acupuncture].

A 56-year-old man (160 cm, 64 kg) having right visual field defect due to obstruction of the central retinal artery for 6 months was treated with stellate ganglion block and electric acupuncture. His visual field improved slightly immediately after preliminary treatment with stellate ganglion block and electric acupuncture judging from perimeter recording and his complaints. Stellate ganglion block on every day and electric acupuncture on every other day were performed for 10 days and there after these treatments were done two times a week. Twenty days after starting the treatment, his visual field improved on his perimeter examination. We discussed that the cause of improvement was activation of slept cone cells which needed more energy for activation than rod cells by increasing retinal blood flow with stellate ganglion block and electric acupuncture. We recommend vasodilating treatments such as stellate ganglion block and electric acupuncture for visual field defect due to obstruction of central retinal artery even in chronic state.

Clinical and EEG studies on complex partial seizure in children.

In 18 (40%) of 45 patients with CPS, paroxysmal discharges were hard to be found on their interictal EEG examinations. These patients were, however, not different from the other 27 patients with paroxysmal discharges on EEG as for the onset, prognosis for attack, complication of generalized tonic-clonic seizure and febrile convulsion, family history and adaptation to society.

[Impairment of erythrocyte anti-oxidant defense mechanisms in hypoxemic patients].

Defense mechanisms against lipid peroxidation were studied in erythrocytes from hypoxemic patients. The levels of lipid peroxides in patients erythrocytes were higher than those of control subjects (15.3 +/- 10.9 vs 7.5 +/- 3.0, p less than 0.05). The levels of reduced glutathione (GSH) decreased in erythrocytes from the patients (426 +/- 108 vs 756 +/- 201, p less than 0.01). Activities of glutathione peroxidase (GPX) and superoxide dismutase (SOD) were lower than those of control subjects (GPX; 27.6 +/- 7.1 vs 35.1 +/- 3.8, SOD; 76.6 +/- 42.9 vs 125.7 +/- 40.1, p less than 0.01). Positive correlations were observed between the levels of arterial oxygen pressures (PaO2) and the GSH concentrations, the activities of GPX and SOD, respectively. These data suggest that decreased anti-oxidant activity enhances lipid peroxidation in erythrocytes from hypoxemic patients.
