Thrombomodulin inhibits intrahepatic spread in human hepatocellular carcinoma.

Thrombomodulin (TM) converts thrombin from procoagulant into anticoagulant protein to activate protein C. Thrombin also plays an important role in the metastatic process of cancer cells. We performed an immunohistochemical and clinicopathological study of TM in 141 cases with resected hepatocellular carcinoma (HCC) measuring less than 6 cm in diameter. Twenty-five specimens (17.73%) stained positive for TM. TM was found in the cytoplasm and surface of cancer cells. The clinicopathological findings according to the positive of TM are examined in HCC. The preoperative plasma TM level of the patients with tissue that stained positive for TM was significantly higher than that of the patients with negative results; for the postoperative TM level, there were no differences between them. In addition, the frequencies of intrahepatic metastasis, tumor thrombus in the portal vein, and capsular infiltration were significantly lower in patients whose tissue stained positive for TM than in patients whose tissue stained negative for TM. The recurrence freedom rate was significantly higher in patients whose tissue stained positive for TM than patients whose tissue stained negative for TM. Thus, TM-producing HCC shows a slow intrahepatic spread. Therefore, these findings suggest that TM may inhibit the adhesion of tumor cells to the portal vein because of anticoagulant activity and thus prevent the spread of intrahepatic metastasis.

[Experimental study on cisplatin microspheres incorporated in polylactic acid and polyethylene glycol acid].

Cisplatin incorporated into polylactic acid/polyethylene glycol acid blend polymeric microspheres was prepared as a dosage (CDDP-MS) by the solvent evaporation method in an oil-in-oil emulsion system. When CDDP-MS was preserved in phosphate-buffer saline, the dissolution rate of cisplatin from CDDP-MS was 14% after one day, 25% after 5 days, 33% after 7 days, 66% after 21 days and 85% after 30 days. CDDP-MS and CDDP aqueous solution (CDDP-SOL) were intraperitoneally administered to compare the tissue distribution of cisplatin in 42 rats each. On days 0.5, 1, 5, 7, 14 and 21, omentum, lung, liver and kidney were removed, and the CDDP concentration was measured. The CDDP concentration of the CDDP-MS group was maintained at a high level in the omentum for a long time. On the other hand, the CDDP level of CDDP-MS group was low in the lung, liver and kidney, compared with the CDDP-SOL group. Consequently, it was suggested that CDDP-MS is useful as a carrier in a drug delivery system, since it improves the burst effect and releases CDDP for a long time without serious side effects.

[Left atrial ball thrombus in the SLE patient without organic mitral lesion].

A case of 77-year-old female patient with the left atrial ball thrombus without organic mitral valve lesion is reported. She had a history of SLE for one year and experienced pericarditis 2 months before this admission. A 2-D echo examination at that time did not show any abnormal shadow in the left atrium. The repeated 2-D echo on this admission demonstrated peduncular left atrial thrombus of 2 x 1.5 cm in size. Surgical removal of the thrombus was performed successfully. Her cardiac rhythm was atrial fibrillation, but other possible factors related to the intracardiac thrombus formation such as bradycardia, left atrial dilatation or low cardiac output were not noted. Although lupus anticoagulant and anticardiolipin antibody were negative before the operation, participation of SLE in formation of the left atrial thrombus was highly suspected.

[Air pollution by sevoflurane in operating room and serum and urine inorganic fluoride of anesthetists].

Since lower blood-gas partition coefficient of sevoflurane provides rapid induction and emergence from anesthesia, sevoflurane has been used widely for inhalational anesthesia. However, because higher minimum alveolar concentration of sevoflurane requires a large dosage of anesthetic than other volatile anesthetics, air pollution with sevoflurane in the operating room might be of great concern. Anesthetists may keep inhaling the low concentration of anesthetics every day, even though scavenging system is equipped in the operating room. The purpose of this study is to evaluate the effects on anesthetists of the low concentration of anesthetics by measuring the inorganic fluoride concentration in the urine and serum of anesthetists and operating room nurses. Healthy 29 anesthesiologists and two operating room nurses were studied. Informed consent was obtained. Inorganic fluoride ions in the serum and urine were measured. Simultaneously sevoflurane concentration in operating room was measured in three operating rooms, at two places in the corridor and in the recovery room. Sevoflurane concentrations in three operating rooms were 1.22 ppm, 2.13 ppm and 6.05 ppm respectively. Concentration in the recovery room was 0.544 ppm. Serum and urine concentrations of inorganic fluoride were 1.1 +/- 0.1 mumol.l-1 and 36.2 +/- 17.1 mumol.l-1, respectively (mean +/- SD). Serum concentration of inorganic fluoride was within normal ranges. Although it is possible that fluoride concentration in urine is influenced by urine volume and a half of fluoride deposits in bone, no abnormal values in urine were found in this study. These results suggest that long term exposure to low concentration of sevoflurane and isoflurane causes no significant increase in their metabolites in operating room staffs.

[Surgical repair of complete atrioventricular septal defect: annuloplasty by using a smaller VSD patch].

Recent policy for the intracardiac repair of complete atrioventricular septal defect (CAVSD) is demonstrated on the basis of the following theoretical background. In the normal heart, the originating portion of the ascending aorta and the crux intrude between the mitral and tricuspid orifices each from the anterior and from the posterior, separating both the orifices together with a short fibrous septal junction. In CAVSD, however, the common A-V orifice takes an elliptic form without any septal junction. It is considered beneficial, therefore, to surgically produce a short septal junction by using a smaller patch to close the ventricular portion of the AVSD. When the left lateral leaflet is larger, two leaflets valve plasty is suggested and when it is smaller, three leaflets valve plasty is suggested to reconstruct the left-sided A-V valve, although competence of the valve leaflets must be finally secured by doing repeated water injection tests. The size of the orifices of the reconstructed left-sided as well as right-sided A-V valve should also be confirmed not to be stenotic by using Hegar sizers. Since 1991, we have applied this surgical concept in 7 patients with CAVSD. Postoperatively, there was no left A-V valve regurgitation in one and slight regurgitation remained in 6 patients. The result is considered to be satisfactory. Although one patient died due to PH crisis, the cause of death was not related to valve regurgitation.(ABSTRACT TRUNCATED AT 250 WORDS)

[Measurement of total body and leg bone mineral densities by dual energy X-ray absorptiometry in severely handicapped children and adults].

Total body and leg bone mineral densities (BMD) were measured in 42 severely handicapped children and adults using a dual energy X-ray absorptiometry (DEXA) (LUNAR Radiation Corp., DPX). Despite the differences in motor ability and nursing history, about 95% of patients, except for 2 cases, were diagnosed as having osteopenia. The degree of osteopenia was dependent on the motor disabilities of their original disease. Therefore we should bear in mind a precaution and therapy from early period. Because BMD of the legs in females were less than -3 SD of the age-matched control values, we should be careful for a possible fracture of femur. Since X-ray hazard of DEXA for patients is considerable to be negligible, this will provide an effective means for quantifying bone mineral in severely handicapped patients.

[A case of spontaneous rupture of the esophagus].

Since cases of spontaneous rupture of the esophagus are extremely rare and early symptoms of the disease are similar to those of emergency diseases of the chest and abdomen, the diagnosis and treatments are often delayed, resulting in an unfavorable outcome in some cases. A 49-year-old man had suffered from severe chest pain and postemetic hematemesis after consuming some alcohol. Because the patient was diagnosed as having a spontaneous rupture of the esophagus, the patient was referred to our hospital and underwent surgery 25 hours after the onset. There was a 3-cm vertical perforation in the left wall of the lower esophagus. The ruptured esophageal wall was directly closed by the layer to layer method and reinforced with fundic patch by Thal-Hatafuku. The post-operative course was uneventful and the patient was discharged on the 46th day after operation. This surgical procedure was very useful in the patient. We reviewed 59 cases, including our case, which occurred recently in Japan.

Use of confocal laser scanning microscopy in radioautographic study.

Confocal laser scanning microscopy was used to visualize the radioautograph at the light-microscope level. Confocal reflectance images from developed silver grains were overlaid with the differential interference contrast image of the tissue. With this technique, a well-focused radioautograph can be taken of both tissue sections and all of the silver grains dispersed in the thick emulsion layer. Further application of this microscopy is discussed.

Heterogeneous distribution of 125I-insulin binding sites in the liver of fed and fasted mice.

A microradioautography made three min. after intravenous injection of 125I-insulin was used to study the distribution of insulin binding sites in the liver of fed and fasted mice. A substantial specific binding of 125I-insulin to liver parenchymal cells could be seen in these mice, whereas radioautographs from animals injected with 125I-insulin plus an excess of unlabelled insulin showed only trace amounts of silver grains. In both fed and fasted mice, a density gradient of the binding from the periportal zone to the perivenous zone was evident, and the binding in each zone was significantly higher in the fasted mice than in the fed mice.

Recent progress in whole-body radioautography.

We review whole-body radioautography for water-soluble substances and its applications studied to date in our laboratory. To transfer the whole-body section onto a glass slide, Japanese paper was inserted between frozen block and adhesive tape before sectioning. Then the section was dried together with the paper and adhesive tape in a cryotome and applied onto the glass slide coated with a mixture of egg-albumin and glycerin. The whole-body section was transferred to slide and contacted with the film in vacuum. By using these techniques, we review the glucose and taurine metabolism, the glucose-6-phosphatase activity and the insulin receptor.

Distribution of [1,2-3H]taurine in the skin of adult and newborn mice studied by microradioautography.

In vivo light microscopic radioautography was used to demonstrate the distribution of taurine in adult and newborn mice at 30 min. and 3 hrs. after the injection of 3H-taurine. This study showed that the injected taurine was present in free form in the skin. Radioautographically, it was found that in the adult a very high density of taurine was found over the epidermis and external root sheaths near the opening of the hair follicles. The external root sheath just below the insertion of the sebaceous glands and the peripheral part of the sebaceous glands showed a high taurine distribution. Connective tissue in the dermis, however, showed low uptake of taurine. In the neonate mouse, the density of injected taurine in both the epidermis and the external root sheath was lower than that the density in the connective tissue in the dermis. We report these findings in detail and discuss some of the functional roles of taurine in the skin.

[Neoadjuvant chemotherapy of liver tumors metastasized by sigmoid colon cancer: a case report of CDDP/5-FU intraarterial infusion therapy followed by hepatectomy].

A case of liver tumors metastasized by Sigmoid colon cancer was treated with intrahepatic arterial infusion of CDDP (10 mg) and 5-FU (250 mg) via subcutaneously implanted reservoir. After 20 administrations of CDDP and 5-FU, the tumors in the right lobe of the liver disappeared, but those in the left lobe remained. Therefore left hepatectomy was carried out. Six months after the hepatectomy, a nodule recurrence was disclosed in the right lobe of the liver. With re-elevated serum CEA level, subsequent intrahepatic arterial infusion of CDDP and 5-FU the nodule size reduced with normalization of the serum CEA level.

[Mechanism on formation of new ipsilateral corticospinal tract following neonatal unilateral cortical ablation in rats].

The corticospinal tract in the rat after neonatal ablation of the unilateral cerebral cortex was studied morphologically using the antegrade horse-radish peroxidase (HRP) tracing method. An aberrant ipsilateral tract was observed 7 days after the operation. Formation of the aberrant neuronal pathway has been confirmed to be contributed mainly by new axons, which ramified at the level of the pyramidal decussation from healthy corticospinal fibers. This ramified axon ran toward the ipsilateral dorsal funiculus. A few fibers also contributed to the aberrant pathway by changing their direction on the way of extension at the level of the pyramidal decussation. These results indicate that the ramification and change of the direction of extending axons play an important role for formation of a new ipsilateral corticospinal tract.

[Clear skeletal visualization on whole body 201Tl-chloride scintigraphy: a case of prostatic cancer with diffuse bone metastases].

A 70-yr-old man who was diagnosed as early gastric cancer showed leukocytopenia after total gastrectomy. Osteosclerotic findings on radiography were not remarkable. 99mTc-HMDP bone scintigraphy showed diffusely increased uptake in the axial skeleton, but visualization of the kidney and urinary bladder was apparent. However, whole body 201Tl-chloride scintigraphy showed diffuse abnormal visualization of axial skeleton. Physical and ultrasonographic examination indicated no abnormality in prostate. Afterward, further investigation, including bone marrow biopsy and immunohistochemical study, confirmed the diagnosis of bone metastasis from prostatic cancer. Microscopically, metastatic tumor cells were located in the intertrabecular space. Furthermore, no osteoclastic bone resorption or new trabecular bone formation was seen in this biopsy specimen. These findings suggest that whole body 201Tl-chloride scintigraphy can be a useful non-invasive diagnostic tool to investigate patients with suspicious malignancy in the bone marrow.
