[Effects of a rehabilitation program for alcoholic inpatients based on the MMPI].

The Minnesota Multiphasic Personality Inventory (MMPI) was administered to 53 male inpatients of alcohol dependence syndrome (ICD-10) at the time of their admission to the Alcoholism Rehabilitation Program (ARP) of Shiga Mental Health Center Hospital and again at the time of their discharge. The pretreatment profile had high peaks above 60 on the scale F, Hs, D, Hy, Pd, Pa, Pt, Sc and MAS. The 20 scales in the 27 examined scales changed significantly after the 84 days' treatment in the direction of reduced symptoms (Table 3). The changes in scores between pre and post treatment objectively showed that the ARP was effective as the treatment of alcoholism. It might be the base of all the recovery that the subjects regained their intelligence through the sobriety. Their changes in scores failed to predict their prognoses.

Regional differences in age-related decrements of the cutaneous vascular and sweating responses to passive heating.

Ten older (aged 64-76 years) and ten younger (aged 20-24 years) healthy men were exposed to a standard heat stress [by placing the lower legs and feet in a water bath at 42 degrees C while sitting in a controlled environment (ambient temperature 35 degrees C and 45% relative humidity) for 60 min]. During passive heating, the rectal temperature of the older men was significantly greater (P < 0.05) and mean skin temperature was lower (P < 0.001), compared to the younger men. Skin blood flow by laser Doppler flowmetry (LDF) was significantly lower on the chest and thigh for the older men (P < 0.001), but forehead LDF did not differ between the groups. The percentages of total LDF in the older men to total LDF in the younger men for the last 30 min were 99%, 58% and 50% on the forehead, chest and thigh, respectively. The age-related differences in LDF responses mirrored cutaneous vascular conductances (CVC), since no group and time effects were observed in mean arterial blood pressure during the test. During the last 30 min the local sweat rates (msw) on the back and thigh were significantly lower for the older men (P < 0.02), but not on forehead, chest and forearm, although the older men had lower msw during the first 30 min exposure regardless of site (P < 0.03). The percentages of total msw in the older men to total msw in the younger men during the last 30 min were 105%, 99%, 63%, 106% and 88% on the forehead, chest, thigh, forearm, and back, respectively. During the latter half of the exposure, the older men had similar LDF, CVC and msw on the forehead, lower LDF and CVC and a similar msw on the chest, and lower LDF, CVC and msw on the thigh, compared to the younger men. These results suggest firstly that regional differences exist in the age-related decrement of cutaneous vasodilatation as well as sweat gland function, secondly that the age-related decrement in cutaneous vascular function may precede a decrement in sweat gland function, and thirdly that the successive decrements may develop sequentially from the lower limbs to the upper body, and head.

Rapidly progressive hepatitis C in a patient with common variable immunodeficiency.

UNLABELLED: A 15-year-old girl with common variable immunodeficiency contracted hepatitis C, which progressed to liver cirrhosis and finally to hepatic failure 5 years later. Since she was agammaglobulinaemic, hepatitis C virus (HCV) infection was diagnosed on the basis of HCV-RNA detection. Quantification of her sera showed high levels of HCV-RNA (more than 10(7) copies of RNA/mL), which implied active viral replication. There were no other hepatotoxic factors except HCV infection. The initial liver biopsy at 16 years of age and the autopsy confirmed a rapid progression in liver histopathological change over 4 years. CONCLUSION: Contrary to the widely held view of a benign short-term prognosis in paediatric hepatitis C, progressive fatal liver disease can develop in some patients with HCV infection. Such a rapid progression of liver injury provides the rationale for antiviral therapy in at least certain high risk groups of these children. Hepatitis C may progress rapidly in an immune deficiency condition.

Kinetics of oxygen uptake and cardiac output at onset of arm exercise.

Pulmonary oxygen uptake (V O2) kinetics at onset of exercise is reported to be slower for arm than for leg exercise. This could be attributed to reduced cardiac output (Q) or reduced arteriovenous O2 content difference or both. To test this, V O2 mean tissue oxygen consumption (V O2T) and Q kinetics in arm cranking were compared with corresponding values found in leg cycling. The increase in V O2 during phase 1 (abrupt increase after onset of exercise) was less in arm than in leg exercise, suggesting that immediate Q adjustments to arm exercise were less pronounced. Mean response times (MRT, the relative rates at which a steady state was attained) for V O2, V O2T, and Q were prolonged during arm exercise. The MRT of VO 2 in arm exercise at a given blood lactate increase was higher than in leg exercise. The delayed V O2 kinetics in arm exercise might be due to delayed Q kinetics and higher anaerobic glycolysis occurring early during arm exercise.

An artificial CuII complex with intriguing oxygen radical-quenching profile. X-ray structure, cytochrome c assay, and ESR study.

A novel artificial peptide named HPH-Pep, comprising a pyridine and two histidine units, was synthesized. The HPH-Pep-CuII complex had unique pentacoordinated structure as shown by X-ray crystallography and exhibited superoxide-scavenging activity as indicated by ESR spectroscopy. The superoxide-quenching profile of HPH-Pep-CuII was studied in detail by cytochrome c assay and ESR spin trapping and it was found that (1) HPH-Pep-CuII did not scavenge hydrogen peroxide or hydroxyl radical and hence the scavenging activity was specific to superoxide, and (2) HPH-Pep-CuII did not generate hydrogen peroxide upon scavenging superoxide.

Treatment of whole houses with liquid nitrogen for control of dust mites.

A large volume of liquid nitrogen, (120-150 L) was applied to the houses of 4 asthmatic patients, and the mite densities of the floor dust from these houses were monitored every 2 wk for 4 mo from July to November 1989. The 1st liquid nitrogen treatment was applied in early August and decreased the mite densities to 20-44% of the pretreatment level in all cases, but they returned to the pretreatment level 3-4 wk later (on 3 occasions). The 2nd liquid nitrogen treatment was applied in early September and decreased the mite densities to 6-27% of the pretreatment level within 3 wk in all cases. However, mite numbers again increased to the pretreatment level 4 wk after treatment. The 3rd treatment was applied in mid-October and reduced the mite densities to < 6% of the pretreatment level within 2 wk, and mite numbers remained low thereafter. The results suggested that using liquid nitrogen to freeze houses reduces mite numbers, but that mite recolonization of the houses is an important problem.

Comparisons of IgE, IgG, and IgG4 responsiveness to Dermatophagoides farinae in children by immunoblotting.

Although asthmatic children are often sensitized to the house-dust mite (HDM) during early childhood, it is not clear what antigenic components are associated with the early immune response of these children. To investigate this problem, we evaluated IgE, IgG, and IgG4 antibody responses to Dermatophagoides farinae (Df) by immunoblotting among three groups of children: group I aged 0-4 years, group II aged 5-9 years, and group III aged 10-14 years. In the group I subjects, positive IgE-binding reactions to 15- and 25-kDa components were found in 76% and 44% of sera, respectively. Those to other components were generally low in frequency. In contrast, positive IgG-binding reactions to 15- and 25-kDa components were found in 44% and 24% of sera, and those to 30- and 110-kDa components in 48% and 88% of sera, respectively. Positive IgG4 reactions to 15- and 25-kDa components were found in 48% and 24%, respectively, and those to other components were very low. Positive IgE-binding reactions to these components gradually became more frequent with increasing age in groups II and III, while IgG and IgG4 reactivities were not markedly different in these age groups. These results suggest that the 15- and 25-kDa proteins of Df are important antigens associated with the initial IgE response to HDM in early childhood, while the 30- and 110-kDa proteins are associated with IgG and 15-kDa components with IgG4.

IgE responsiveness to Dermatophagoides farinae in young asthmatic children: IgE binding study using recombinant allergens of Der f1, Der f2 and mutant proteins of Der f2.

IgE reactivities to two recombinant (r) allergens, rDer f1 and rDer f2, synthetized by cDNA clones from Dermatophagoides farinae (Df) were analyzed using sera of Df-sensitive asthmatic patients of varying ages. Positive RAST responses to rDer f1 and rDer f2 were found in 88 and 80%, respectively, with sera from young asthmatic children aged 0-1, positive RAST rates to these antigens increased to 100% up to 5 years of age. There was a significant correlation between RAST levels of rDer f1 and rDer f2 in these young children aged 0-1. IgE reactivities to four mutant proteins of rDer f2, which have at least one amino acid substitution, were similarly examined in the asthmatic sera. IgE reactivity to D7A, in which Asp7 was replaced by Ala, was reduced to 30-60% compared to the rDer f2. IgE binding to C8/119S, in which both Cys8 and Cys119 were replaced by Ser, lacking a disulfide bond between Cys8 and Cys119, was reduced almost to a background level at all ages. In contrast, A72L and A120L, in which Ala72 and Ala120 were substituted for Leu, respectively, almost retained the same IgE binding activity as the rDer f2 at all ages. These results suggest that Der f1 and Der f2 are important antigens associated with early sensitization to house dust mite in young asthmatic children. In addition, the disulfide bond between Cys8 and Cys119 and the N-terminal region including the 7th amino acid residue are considered to maintain IgE epitopes of the Der f2 allergen.

Novel phorbol analogs which bind to protein kinase C (PKC) without activation.

13-Deacetoxy-11-demethyl-phorbol derivatives with acyl groups of various lengths (from hexanoyl to tetradecanoyl) at the C-12 position were synthesized in an optically active form. Although considerable binding affinities to PKC were observed by analogs 3-7, no activation of PKC was seen even at 10 microM.

The influence of work loads on regional differences in sweating rates.

The influence of work loads (20, 40 and 60% VO2max) on regional differences in sweating rates was investigated in six healthy male students. The ratios of local sweating rate (msw) on the chest and back to estimated total sweating rate (mean msw of four sites examined x body surface area) at 20% work load were significantly greater than those on the forearm and forehead, whereas the ratio on the forehead at 60% work load was greater than at the other sites. The ratios on the forearm at 40 and 60% work loads were significantly greater than at 20% work load. The regional differences in sweating rates change with the increase in work load as seen by the greater variation in the ratios of msw on each site at 20% work load compared with 40 and 60% work loads. The results indicate that redistribution of this sweat activity to whole body or head with the rise in work load induces favorable conditions for evaporation and controlling brain temperature.

Comparison of basophil histamine release, eosinophil cationic protein and non-specific airway responsiveness between mite-sensitive asthmatic and non-asthmatic children and non-allergic controls.

To understand the relevance of allergy to the development of asthma in children, we examined basophil histamine release (HR) with Df antigen, blood eosinophil counts, serum eosinophil cationic protein (ECP) levels, and bronchial responsiveness to methacholine (PC20) in three groups of children, including 36 asthmatics with high RAST titre for Df (group 1), 36 non-asthmatics with similarly high RAST titre for Df (group 2) and 21 non-asthmatics with negative RAST titre for Df (group 3). The amount of Df antigen inducing 50% HR from basophils did not vary significantly between group 1 and 2 (P > 0.05), while none of the cells responded to higher concentrations of Df in group 3. The mean number of blood eosinophils and level of serum ECP were highest in group 1, and lowest in group 3, with group 2 being intermediate, and the differences were significant between all three groups (P < 0.01). The mean PC20 value was the lowest in group 1, intermediate in group 2, and the highest in group 3, and the differences were significant between all three groups (P < 0.01). While correlation studies showed that PC20 values of group 2 subjects significantly correlated with their eosinophil numbers (r = -0.48, P < 0.01) and ECP levels (r = -0.49, P < 0.01), such correlations were not found in group 1 subjects. These results suggest that the degree of the eosinophilic inflammation caused by the allergic reaction to mites is an important factor in determining the clinical expression of asthma in atopic subjects.

[Clinical evaluation of urinary leukotriene E4 levels in asymptomatic bronchial asthma].

To evaluate the significance of peptide leukotrienes (LTC4, D4, E4) in asymptomatic asthmatic patients, we measured urinary LTE4 levels which is thought to reflect in vivo production of peptide LTs. Urinary LTE4, was extracted using C18 solid phase column and measured by radioimmunoassay. There was no significant difference in urinary LTE4 levels among asthmatics with different severity or between atopic and non-atopic asthmatics. Urinary LTE4 levels were significantly elevated in asthmatics compared with normal controls (p < 0.05). When compared with normal controls, urinary LTE4 levels were significantly elevated in moderate to severe asthma (p < 0.05), and non-atopic asthmatics (p < 0.001). Urinary LTE4 levels were significantly elevated in aspirin-sensitive asthmatics compared with aspirin-tolerant asthmatics (p < 0.05). There was no significant difference in urinary LTE4 levels among aspirin-sensitive asthmatics with different severity. These results suggest that increased production of peptide LTs is a characteristic in aspirin-sensitive asthma, and that the severity and type of asthma and the presence of aspirin-sensitive asthma should be taken into consideration in the analysis of urinary LTE4 levels.

Radiolabeling of catalytic subunits of PI 3-kinases with 17 beta-hydroxy-16 alpha(-)[125I]iodowortmannin: identification of the G beta gamma-sensitive isoform as a complex composed of 46-kDa and 100-kDa subunits.

A fungal metabolite, wortmannin, is a potent inhibitor of phosphatidylinositol (PI) 3-kinases. In the present study, we prepared a radiolabeled derivative of wortmannin, 17 beta-hydroxy-16 alpha(-)[125I]iodowortmannin. The compound bound tightly to a 110-kDa subunit in the previously identified isoform of PI 3-kinase (p85/p110), and also to a 100-kDa peptide in a partially purified preparation of another isoform of PI 3-kinase whose activity was markedly stimulated by the beta gamma subunits of GTP-binding proteins (G beta gamma). The binding to both peptides was inhibited by non-radiolabeled wortmannin and also by LY294002, another inhibitor of PI 3-kinases. An antibody against p85 recognized a 46-kDa peptide in the G beta gamma-sensitive isozyme and precipitated the 100-kDa peptide specifically labeled with 17 beta-hydroxy-16 alpha(-)[125I]iodowortmannin. These results suggested that the newly found isozyme was a complex composed of 46-kDa and 100-kDa peptides.

Successful treatment of invasive thoracopulmonary mucormycosis in a patient with acute lymphoblastic leukemia.

BACKGROUND: Pulmonary mucormycosis associated with hematologic malignancy is an uncommon, but important opportunistic fungal pneumonia that is usually a fatal infection. Only a few survivors of pulmonary mucormycosis have been reported. METHODS: A case report of invasive thoracopulmonary mucormycosis during remission-induction therapy for acute lymphoblastic leukemia and a review of the literature are presented. RESULTS: The fungal lesions extended to both lungs, the left ribs, and intercostal muscles. Percutaneous needle biopsy and immunostaining of the fungal hyphae established the diagnosis of thoracopulmonary mucormycosis. The patient was treated with granulocyte-colony stimulating factor (G-CSF) and intravenous amphotericin B for 9 weeks and the lesions in the right lung disappeared. Left pneumonectomy and partial resection of the chest wall were later performed. The left lung was grossly necrotic and contained a large cavity and bronchopulmonary fistula. Thereafter, antileukemic therapy was resumed and completed without recurrence of mucormycosis or leukemia. CONCLUSIONS: In the management of mucormycosis, the addition of G-CSF to the conventional treatment may substantially improve outcome.

Cloning, properties, site-directed mutagenesis analysis of the subunit structure, tissue distribution and regulation of expression of the type-C eel natriuretic peptide receptor.

Eel natriuretic peptide receptor C (NPR-C) was cloned, characterized and found to have a unique interchain disulfide linkage when compared to that of mammalian NPR-C. The NPR-C cDNA was obtained from an eel gill cDNA library; the open reading frame codes for a polypeptide of 502 amino acids exhibiting the known features of NPR-C, including a weak ligand specificity and a disulfide-linked homodimeric structure. The deduced amino acid sequence shares approximately 60% similarity with the mammalian NPR-C but it lacks the Gly-rich prosequence present in the mammalian counterparts. Site-directed mutagenesis revealed that eel and mammalian NPR-C are quite different in their interchain disulfide-bonding pattern; eel uses the second Cys residue and mammals the fifth Cys residue for the covalent dimerization. The ligand-binding activity of the extracellular domain is not independent of the short cytoplasmic tail. RNase protection analysis revealed that the eel receptor is highly expressed in the gill and heart and, to a much lesser extent, in other tissues including the brain and intestine. The NPR-C mRNA levels were found to be down-regulated in most tissues when eels were transferred from fresh water to seawater; however, in the anterior intestine, the levels were up-regulated, suggesting that NPR-C plays a role in the adaptation to salinity changes in the euryhaline eel.

The effect of diurnal variation on the regional differences in sweating and skin blood flow during exercise.

The aim of the present study was to examine changes in the control of heat-dissipation responses to exercise associated with the diurnal variation in core temperature from the viewpoint of the regional response patterns. We studied seven men during exercise on a cycle ergometer at 100 W for 40 min at 25 degrees C at 0630 (morning) 1630 (evening) hours on 2 separate days. Oesophageal temperature (T(oes)), local skin temperature, local sweating rate (msw) on the forehead, back, forearm and thigh, and skin blood flow by laser Doppler flowmeter (LDF) on the back and forearm were measured continuously. The T(oes) at rest was significantly higher in the evening than in the morning, the difference averaging approximately 0.4 degrees C (P < 0.05). The T(oes) thresholds for each site in msw and that for back in LDF were significantly different between the two times of day (P < 0.05). The change in T(oes) thresholds for sweating and vasodilatation for morning and evening were similar to T(oes) at rest. Although msw on the forehead was significantly higher in the morning than in the evening, msw on the back was significantly higher in the evening than in the morning (P < 0.05). Total local sweating rate (msw, tot) for each site during exercise was significantly higher on the forehead than on the forearm in the morning, and on the back than on the forearm in the evening, respectively (P < 0.05). The results would suggest that the diurnal variation of heat-dissipation responses to exercise is influenced not only by a central controlling mechanism but also by changes in the regional differences.

Metabolism of clomipramine in a Japanese psychiatric population: hydroxylation, desmethylation, and glucuronidation.

We measured the concentrations of clomipramine and its metabolites, N-desmethylclomipramine, 8-hydroxy-N-desmethylclomipramine, 8-hydroxyclomipramine by high-performance liquid chromatography in 108 Japanese psychiatric patients receiving clomipramine hydrochloride PO. The concentrations of the glucuronide conjugates of 8-hydroxyclomipramine and 8-hydroxy-N-desmethylclomipramine were assayed via enzymatic hydrolysis. Although there were large interindividual variations of concentrations of parent, intermediate metabolic compounds, and glucuronide conjugates, significant positive correlations were observed between these drug concentrations and daily doses of clomipramine hydrochloride (mg/kg body weight). Although the metabolic ratios for desmethylation, hydroxylation, and glucuronidation that were calculated from steady-state drug concentrations varied substantially with 36-, 14-, and 28-fold interindividual variations, respectively, apparent poor desmethylators, poor hydroxylators, or poor glucuronidators were not found.
