Bilateral pleural thickening: a manifestation of asbestos dust exposure.

Bilateral pleural thickening in a relatively asymptomatic patient without evidence of pulmonary disease is frequently a manifestation of asbestos dust exposure, particularly when it is localized or in the form of a noncalcified pleural plaque. Bilateral pleural thickening will be discovered with increasing frequency if specifically searched for on every radiograph. This radiographic finding is suggested as a more realistic indication of asbestos dust exposure, particularly when correlated with a complete occupational history.

Chronic ectopic pregnancy: ultrasonic diagnosis.

The classical ultrasonic description of a chronic ectopic pregnancy is a slightly enlarged uterus with uniform internal echoes and no evidence of an intrauterine pregnancy, combined with an extrauterine semicystic mass, gestational sac, or fetal structures. It is our experience that the ultrasonographer more commonly finds a midline pelvic mass containing irregular echo patterns and cystic areas. The mass may also be associated with abdominal or pelvic fluid and obliteration of normal anatomical structures. We wish to emphasize these variations from the classical description with illustrations that demonstrate the differences in the ultrasonic presentation of chronic ectopic pregnancy.

High frequency oscillations of the pulmonary valve leaflet in double-chamber right ventricle.

The abnormal pulmonary valve echocardiographic tracing in a patient with a double-chambered right ventricle is presented. Preoperatively, the posterior pulmonary valve leaflet demonstrated a partial closure in systole, followed by coarse fluttering of the leaflet. The postoperative echocardiographic tracing of the pulmonary valve was normal. It is suggested that an obstructive process proximal to either semilunar valve which converts laminar flow to turbulent flow is capable of producing a similar pattern in the leaflets during ventricular ejection.

Fetal cystic lymphangioma (cystic hygroma): prepartum ultrasonic findings.

The abnormal prepartum ultrasonic scan in a case of fetal cystic lymphangioma is presented. A large intrauterine cystic mass adjacent to a 23-week-old fetus was identified by ultrasound. After delivery, this was proved to be a cystic lymphangioma extending from the fetal neck and supraclavicular region.

The ultrasonic evaluation of nonfunctioning thyroid nodules.

Thyroid echography was carried out on 100 patients with thyroid abnormalities to evaluate the diagnostic accuracy of ultrasound in differentiating simple thyroid cysts from solid thyroid nodules. In all 46 proven cases, the ultrasonic diagnosis of the solid or cystic nature of thyroid nodules was correct.A proposed diagnostic workup of a hypofunctioning (cold) thyroid nodule is suggested with primary needle aspiration and cytologic examination of the cyst fluid being recommended if the nodule is shown to be entirely cystic by ultrasound.
