HL-A and cadaver kidney transplantation. 7-year experience at Johannesburg General Hospital.

The role of HL-A matching in the clinical outcome of 159 consecutive first cadaver renal allografts at the Johannesburg General Hospital has been analysed over a 7-year period, April 1968-May 1975. Over-all actuarial graft survival was 69% at 1 year and 57% at 7 years. When all cases were considered together there was a trend towards improved graft survival with better grades of matching, but this was not statistically significant. Presensitisation had an adverse effect on graft survival. Donor-recipient combinations with no demonstrable FOUR locus mismatches provided a significantly superior graft survival rate of 82% 1-6 years after transplantation. The majority of these compatible for HL-A 7, 8 OR 12, and therefore may have been matched indirectly for lymphocyte activating determinants. Relatively inferior graft survival was observed with FOUR locus incompatibility in patients who had been presensitised, and when donor-recipient combinations had no serologically determined antigens in common, particularly in the donor "full house" situation. Despite these unfavourable immunogenetic circumstances, approximately 50% of grafts functioned well for long periods, and not infrequently clinical considerations took precedence over immunological considerations in the selection of recipients. This policy is justified by over-all results, the critical shortage of cadaver organs in relation to clinical requirements and the rare occurrence of combinations with superior HL-A matching. Specific allograft tolerance remains the goal of clinical organ transplantation.

Blood transfusion practice: facts and fallacies.

Practical problems and potential dangers associated with blood transfusion have led to the introduction of modifications in transfusion practice and the search for blood substitutes. Many of these changes, promoted by the pharmaceutical industry, have been of questionable value.

Mechanical precipitation of hemoglobin köln.

Hb Köln (beta 98 Val leads to Met) was found to precipitate rapidly during mechanical shaking. The rate of precipitation of Hb Köln is 5-6 times faster than that of Hb S. The kinetics of precipitation of the patient's hemolysate, which is a mixture of Hb Köln and Hb A, showed a biphasic curve indicating that Hb Köln precipitates independently from Hb A. The instability of Hb Köln may be attributed to the conformational change in the vicinity of heme. The mechanical shaking may be used as a new method for detection and quantitation of hemoglobin Köln and other unstable hemoglobins.

Haemophilia home infusions with dried fresh plasma.

A group of 26 haemophilic patients was successfully managed on a home infusion programme over a one-year period. Single donor units of dried fresh plasma, the coagulant activity of which was adequate to ensure haemostasis of most of the haemorrhagic episodes treated, were used. The transmission of hepatitis was kept to a minimum by careful selection of regular, known donors and by the utilisation of single donor units of plasma in preference to plasma pools. Although the programme was costly, it was less expensive than it would have been had alternate antihaemophilic products been used. The advantages and disadvantages of self-administration are discussed. No serious mishaps occurred, and the programme was considered an outstanding success by both laymen and medical personnel. Minor alterations in liver function tests were demonstrated in several of the recipients. No factor VIII inhibitors were detected.

Applications of artificial intelligence: relationships between mass spectra and pharmacological activity of drugs.

The possibility that the mass spectrum and pharmacological activity of a compound may be directly related has been explored with the help of various computer-based pattern-recognition techniques. The relationship appears to hold at least for tranquilizers and sedatives, and compounds with one or the other of these two pharmacological activities can thus be classified from their mass spectra with a high degree of accuracy.
