Angiographic demonstration of recanalization of the basilar artery.

This report details the clinical course and angiographic findings of a patient with intermittent vertebrobasilar insufficiency. Recanalization of the basilar artery was demonstrated during anticoagulant therapy. A review of the literature failed to disclose a previous report of this occurrence.

Unusual metastases from a medulloblastoma: case report.

Calvarial and extracranial metastases by hematogenous spread of primary intracranial neoplasms have been considered a rare occurrence. This report deals with the clinicopathological and radiological findings of a child with metastatic medulloblastoma. Correlation of serial postoperative computed tomography (CT) scans and the autopsy examination indicated hematogenous calvarial and intracranial spread of the primary lesion.

A cross-national comparison of the institutionalized elderly in the cities of New York and London.

Two random samples, each constituting 0.5% of the institutionalized elderly (over 65) in the cities of New York and London have been compared. This preliminary report of a collaborative study demonstrates the similarity of the 2 samples. Institutions in the 2 cities are shown to differ in size, type of ownership and the frequency that skilled nursing is provided. Some implications of these 2 findings are raised now, and will be discussed later when the full data for the study are presented.

A measure of the fulfillment of health needs.

Female patients, sixty-five of age and over, admitted to acute medical and psychiatric wards of a municipal hospital were observed directly and unobtrusively for a three hour time period every other day during the first two weeks of hospitalization. The behaviors of the patients and of those who interacted with them were recorded and later categorized according to fifteen activities defined as representing basic health needs. Behaviors were compared according to the outcome of hospitalization, discharge, transfer to another ward or institution, or death. Differences which distinguished patients who were discharged from those who died were found in the activities of movement, comunication, and learning. Behaviors observed at different points in time during hospitalization also were compared. This study is the first step in the development of an instrument to measure the perceived satisfaction of health needs of geriatric patients in institutions.

Neuroepithelial (colloid) cysts. Pathogenesis and unusual features.

A brief history of the theories of pathogenesis of the origin of neuroepithelial cysts is presented. An origin more diffuse than the paraphysis seems most likely in our opinion, and we attempt to substantiate this theory with several of the cases from our series. One posterior third ventricular neuroepithelial cyst and one in the fourth ventricle are demonstrated roentgenographically. Also included in our series are a case showing suprasellar calcification on plain skull roentgenograms, not previously reported in neuroepithelial cysts, and a case with a cyst measuring 9 cm, the largest neuroepithelial cyst yet reported.

The cervical aortic arch.

Cervical aortic arch anomaly is a rare congenital entity. The aortic arch extends into the soft tissues of the neck before turning downward on itself to become the descending aorta. Nineteen cases have been previously reported in the world literature with one more presently in publication. We have collected six additional cases, in four of which the cervical arch was located on the left. Formerly the left cervical aortic arch was felt to be much less common than the right. It now appears that they have equal incidence. Four of our cases were also associated with cardiac anomalies, and this coincidence has not been previously reported.

Recurrent methemoglobinemia after acute dapsone intoxication in a child.

Dapsone is a synthetic sulfone increasingly used in the treatment of a wide variety of dermatologic disorders. The case of a child with dapsone-induced recurrent methemoglobinemia is presented with a discussion of dapsone toxicity and its treatment. In addition, the diagnostic value of pulse oximetry in the presence of dysfunctional hemoglobins is discussed.

Clinical profile of apparently healthy neonates with in utero drug exposure.

OBJECTIVE: To identify the characteristics of neonates exposed to drugs in utero but admitted to the normal newborn nursery because of apparent good health. DESIGN: Retrospective evaluation with chart review of toxicology screening tests sent from a normal newborn nursery. SETTING: Newborn service in an urban hospital. PARTICIPANTS: Fifty newborns with positive drug screening results. MAIN OUTCOMES: Frequency of positive results and assessment of demographic features and neonates' clinical features. RESULTS: 43 (86%) of the apparently healthy newborns tested positive for cocaine. The results of simultaneous or proximate drug testing of mothers and their newborns were discordant in 11 (22%) cases. CONCLUSIONS: Neonates with in utero drug exposure often are born with few or no signs of abnormality and are admitted to the normal newborn nursery.
