Serum IgE levels in patients with liver disease.

Serum IgE level was determined in patients with chronic and acute liver diseases. In the latter it was correlated with that of other immunoglobulin classes and with disease activity. IgE level was found to be within normal range in patients with chronic active hepatitis and in those with non-alcoholic cirrhosis, as well as in asymptomatic HBsAg carriers. In contrast, the prevalence of patients with high serum IgE level was significantly increased during the active stage of both type A an type B hepatitis. Serum concentration of IgE decreased to normal following complete recovery from the disease. The increase in serum IgE level during type A hepatitis was accompanied by a very marked increase in IgM level while that of IgG remained unchanged. The changes in the pattern of serum immunoglobulins during acute hepatitis might be attributed to selective depletion of suppressor T-cells.

Second primary lung carcinoma.

Long-term survivors with bronchogenic carcinoma represent only a small fraction of the overall population affected with this disease. Therefore, the occurrence of a second primary lung carcinoma is a rare event and can be diagnosed either simultaneously, or, more frequently, sequentially. A review of the literature disclosed a median interval of five years between metachronous lesions with a range of 0 to 16 years. We thus consider it of interest to report a case in which 17 years had elapsed between the first tumor, successfully treated by a lobectomy, and the diagnosis of a second primary tumor located in the contralateral lung. The literature on second primary lung tumors is reviewed and the histopathological aspects, as well as the therapeutic implications, of this relatively rare condition are discussed.

Hyperglobulinemic purpura in the course of multiple myeloma.

Secondary hyperglobulinemic purpura of Waldenström is characterized by polyclonal gammopathy associated mainly with autoimmune diseases. Its occurrence with multiple myeloma is very rare. We described a patient who developed characteristic lesions of hyperglobulinemic purpura in the course of IgA myeloma. Skin biopsy revealed deposition of IgA in the blood vessels.

Superiority of bromocriptine over pyridoxine in the treatment of patients with acromegaly or galactorrhea.

Sixteen patients, eight with acromegaly and eight with galactorrhea, were treated with pyridoxine (600 mg/day) for a three-month period. The clinical and biochemical changes induced by the drug were compared with those induced by bromocriptine (2.5 to 50 mg/day) administered for the same length of time to the same patients. Pyridoxine induced lowering of growth hormone and prolactin levels in two acromegalic patients. In only one patient with galactorrhea after irradiation of the hypophysis for active acromegaly did pyridoxine treatment stop and galactorrhea and induce renewal of menstrual bleeding without significantly changing the basal prolactin levels. In another patient with a chromophobe adenoma of the hypophysis, it reduced the excessively elevated prolactin levels. On bromocriptine therapy, all the patients reported signifcant clinical improvement paralleled by significant drops in plasma growth hormone and prolactin. It appears that at the dosages administered bromocriptine is preferable to pyridoxine in the treatment of patients with acromegaly or galactorrhea.

Congenital vascular malformation of spinal cord simulating diastematomyelia.

Diastematomyelia is usually caused by a midline bony, cartilaginous or fibrous spur. A case is presented with splitting of the spinal cord by a large arteriovenous malformation at the thoracolumbar junction. A hairy skin patch, kyphoscoliosis, vertebral anomalies and an arachnoidal cyst were found adjacent to the vascular and neural malformations.

Brain abcess following dental infection.

A 48-year-old woman underwent root canal treatment of the upper left lateral incisor and lower right second premolar. Close to the conclusion of the endodontic treatment she complained about headaches. Later on, because of aggravation of her condition, with headaches, fever, malaise, weakness, and numbness of the right limbs, she was admitted to the hospital. The disease progressed to an epileptic state, with appearance of a right hemiparesis. A brain scan and carotid arteriogram revealed the presence of a mass occupying the left parietal space. Craniotomy disclosed an abscess containing yellow pus from which Streptococcus viridans was cultured. After thorough surgical cleansing of the area, removal of the bone for decompression, and treatment with ampicillin the patient improved gradually and slowly regained the mobility of her right side.

Sinus histiocytosis with massive lymphadenopathy and spinal epidural involvement: a case report and review of the literature.

A case of "sinus histiocytosis with massive lymphoadenopathy" (SHML) in which spinal epidural involvement caused paraplegia is described. The literature pertaining to this newly recognized entity is reviewed; this review indicates that the process is not necessarily restricted to the lymph nodes and may be progressive in nature. The case we report is the first with histologically proved bone involvement.

Nedocromil sodium modulates the function of long-term rat peritoneal mast cell cultures.

Long-term rat peritoneal mast cell (MC) cultures consisting of MC co-cultured with 3T3 fibroblasts (MC/3T3) were employed to study the effects of repeated and prolonged incubation with nedocromil sodium (10(-3) and 10(-5) M) on MC activation. When nedocromil sodium was added simultaneously with compound 48/80 to MC/3T3 on the first day of the experiment, it inhibited histamine release to the same extent as when added to the cultures a week later upon rechallenge. In other experiments activated MC/3T3 were incubated with nedocromil sodium for a week and on the last day of the experiment fresh nedocromil sodium was added simultaneously with compound 48/80. Under these conditions the drug was as potent in inhibiting histamine release from the activated MC as it was in cultures incubated for a week with medium alone. In addition, preincubation of naive MC with nedocromil sodium for two days inhibited histamine release from these cells when activated for the first time. Salbutamol (10(-3) and 10(-5) M) exhibited a similar inhibitory effect to nedocromil sodium upon simultaneous incubation with MC/3T3. However, prolonged incubation of salbutamol with MC/3T3 cultures resulted in tachyphylaxis. We conclude that nedocromil sodium is an effective MC stabilizing drug since it similarly inhibits histamine release from both primarily and secondarily challenged MC. Moreover this drug does not lose its efficacy upon prolonged incubation with MC.

Successful intubation with the Combitube in acute asthmatic respiratory distress by a Parkmedic.

The Combitube is a relatively new device used for blind insertion emergency intubation. We report a case of successful Combitube treatment of an acute respiratory arrest secondary to an acute asthma exacerbation. An advanced EMT-II (National Park Service Parkmedic) utilized this device. Our review of the literature revealed no reported cases of an advanced EMT-II, nor any other cases, using the Combitube in asthma-related respiratory failure.

The Mt. Tyndall incident.

The authors describe the 53-hour rescue of a 6-foot, 1-inch tall, 250-pound hiker in the face of harsh environmental conditions in Sequoia National Park. This 43-year-old man fell 25 feet, injured his leg, and was noted to be hypothermic and hypovolemic. Weather, altitude, and the patient's size delayed and complicated his evacuation. After being carried down 1,500 vertical feet, he was hoisted into a hovering helicopter and flown to University Medical Center in Fresno, California. On arrival, the patient was determined to have a comminuted subtrochanteric right femur fracture, which was ultimately repaired surgically. The authors also discuss some of the unique aspects of wilderness and National Park Service EMS.

Intellectual and psychosocial functions of children, adolescents and young adults before and after operation for craniopharyngioma.

An evaluation of intellectual, personal and social functioning was made in 16 young patients with craniopharyngioma. No changes were found in the overall IQ verbal and performances scores before and after operation, however, discrepancies between verbal and performance scores suggest that one of the manifestations of craniopharyngioma in children are disturbance in cognitive functions. The decrease in the quality of life of the patients and their families manifested through the deterioration of familial and social relationships and the decrease in scholastic achievements lead to the conclusion that children with craniopharyngioma and their families require close psychological supervision and counselling both before and after surgery.
