[Renal cell carcinoma in a patient with malignant lymphoma: a case report].

A 65-year-old woman was admitted for the treatment of malignant lymphoma. Computed tomography revealed a right renal tumor. After 3 cycles of CHOP (cyclophosphamide, adriamycin, vincristine, prednisone) chemotherapy, we performed right radical nephrectomy. The histopathological diagnosis was renal cell carcinoma. After nephrectomy she was treated with 3 cycles of CHOP chemotherapy and radiation therapy. She received no adjuvant therapy for renal cell carcinoma and had no recurrence after 8 months from the nephrectomy.

Noninvasive estimation of FDG input function for quantification of cerebral metabolic rate of glucose: optimization and multicenter evaluation.

UNLABELLED: The aims of this study were to determine whether body weight or body surface area (BSA) should be used for noninvasive measurement of the cerebral metabolic rate of glucose (CMRGlc) by FDG PET with a standardized input function (SIF) and an autoradiographic method and to validate the procedure in a large population from different PET centers. METHODS: Plasma input functions measured by intermittent arterial blood sampling after intravenous injection of FDG, in 101 patients from 1 institution who were fasting for at least 4 h, were used to generate the SIF. The SIF was generated by averaging over 101 patients the input function normalized with the net injected dose and initial distribution volume (DV) of FDG estimated by the formula c x Ha x Wb, where H is body height and W is body weight. To evaluate the estimation of DV by BSA or body weight, the coefficient of variation (CV) of the ratio of Ha x Wb to the measured DV was calculated by changing a and b independently. Estimation of the CMRGlc with SIF based on the formula for DV was validated with an additional 192 subjects from 3 institutions who underwent FDG PET while fasting. The result of simulation was compared with the results of 4 previously published formulas for BSA and body weight. RESULTS: The optimal set of parameters, in which a was 0.80 and b was 0.35, minimized the CV. The averaged percentage error of the CMRGlc based on the optimal set of parameters for DV estimation and SIF was 8.9% for gray matter and 10.6% for white matter. Four BSA formulas brought about a similar error, which was significantly smaller than that based on body weight (P < 0.001, ANOVA). CONCLUSION: Noninvasive estimation of CMRGlc is made possible by careful measurement of the net injected dose and BSA.

Behaviour of NMDA and AMPA receptor-mediated miniature EPSCs at rat cortical neuron synapses identified by calcium imaging.

1. Simultaneous recording of intracellular calcium concentration at a synapse and synaptic currents from the cell body allows mapping of miniature excitatory postsynaptic currents (mEPSCs) to single synapses. 2. In the absence of extracellular Mg2+, 77 % of synapses had mEPSCs with fast and slow components, attributed to AMPA- and NMDA-type glutamate receptors, respectively. The remainder of synapses (23 %) had mEPSCs that lacked a fast component; these responses were attributed to NMDA receptors. 3. A strong positive correlation between the amplitude of the calcium transient and the NMDA receptor-mediated mEPSC was observed, indicating that the mEPSCs originate from an identified synapse. 4. At synapses that had both mEPSC components, the AMPA receptor component was positively correlated with charge influx mediated by NMDA receptors during repeated synaptic events. No periodic failure in the AMPA receptor mEPSC was observed at synapses expressing both receptor components. 5. A significant positive correlation between the mean amplitudes of NMDA and AMPA receptor components of mEPSCs is observed across different synapses. 6. We suggest that factors effecting both receptor classes, such as the amount of transmitter in synaptic vesicles, might contribute to the variation in mEPSC amplitude during repeated miniature events at a single synapse. Although the average postsynaptic response at different synapses can vary in amplitude, there appears to be a mechanism to keep the ratio of each receptor subtype within a narrow range.

Recovery from exercise-induced desaturation in the paraspinal muscles in idiopathic scoliosis.

STUDY DESIGN: A study using near-infrared light spectroscopy to measure recovery from exercise-induced desaturation in the paraspinal muscles of patients with idiopathic scoliosis. OBJECTIVES: To measure oxygenation of the paraspinal muscles and obtain differences between the convex and concave sides. SUMMARY OF BACKGROUND DATA: Authors of previous studies have reported that some patients experience pain on the convex side of the paraspinal muscles. The muscles on the convex side are more stretched and stressed than those on the concave side. The current authors investigated the degree of stress by measuring oxygenation and blood volume changes. METHODS: Paraspinal muscle spectral properties at L3 were investigated using near-infrared light spectroscopy. Thirty-six patients (8 men and 28 women) underwent this procedure. To assess a level of peripheral adaptations to exercise, the half-time of Oxyhemoglobin/Myoglobin recovery was measured, which indicates the recovery from energy deficit after exercise. RESULTS: The average half-time recovery on the convex side was 3.38 seconds (range, 1.5-5.5 seconds), whereas that on the concave side was 1.51 seconds (range, 0.7-4.0 seconds). The average difference between the convex side and the concave side was 1.87 seconds (range, 0.9-3.5 seconds). CONCLUSIONS: Half-time recovery on the convex side in the patients with idiopathic scoliosis was slower than that in healthy adults (P < 0.05). Half-time recovery on the concave side in such patients was faster than that on the convex side (P < 0.01). The authors consider half-time recovery an indication of back muscle stress.

Visualization of mitochondrial oxygen fixation in brain slices by gas-tissue autoradiography.

We have developed a novel autoradiographic method of visualizing oxygen fixation with sufficient delivery of [(15)O]O(2)/O(2). Brain slices (400 microm) were preincubated in Krebs-Ringer phosphate buffer and exposed to [(15)O]O(2) in a chamber. Fixation of [(15)O]O(2) correlated with the polarographically measured oxygen consumption among tissue slices from various organs (r=0.84). The fixation of [(15)O]O(2) by brain slices was significantly reduced (7. 2% of the control) by heat-treatment or dose dependently by NaCN (18. 2% of the control on 50 mM NaCN pretreatment). The (15)O radioactivity in the brain slices prepared from rotenone injected rats was also reduced compared to the control (56.8% of the control side). In an autoradiographic study, (15)O radioactivity showed a heterogeneous distribution both in coronal and sagittal sections. Autoradiography of young and senescent rat brain sections showed reduction of oxygen uptake with aging in the cerebrum, the senescent being 77.4% of the young. This method provides information regarding basic oxygen consumption of tissue slices under condition of sufficient O(2) delivery, which reflects mitochondrial electron transport.

Verb generation in Japanese--A multicenter PET activation study.

Cerebral blood flow (CBF) during silent verb generation was measured at four Japanese PET centers. To minimize the variance of the measurement, speakers of a single language (Japanese) served as subjects and experimental conditions at the four PET centers were controlled as much as possible. Two types of activation patterns were observed: activations in the left dorsolateral prefrontal cortices and the medial frontal cortex (at the two centers with a 2D PET scanner) and additional activation in the left posterior temporal cortex (at the two centers with a 3D scanner). This suggests either a difference in the sensitivity of the two types of PET scanners (viz., a 3D scanner is generally more sensitive than a 2D scanner) and/or subject bias due to the small number of subjects at the individual centers. The pooled activation pattern was fundamentally similar to activation patterns obtained in the previous studies for verb generation in English and other European languages, suggesting that regions for verb generation are independent of particular languages. Regions relevant to verb generation are discussed.

Improved signal-to-noise ratio in parametric images by cluster analysis.

Parametric images are formed by analyzing the concentration history of every voxel in PET data sets. Because PET concentration data at the voxel level are rather noisy, noise propagation into the parametric image is often quite noticeable. To address this problem, a model-based clustering method has been developed to generate parametric images. The basic idea of the clustering method is to average over voxels whose concentration histories have the same shape. We applied the method to a two-parameter (K1, k2) compartment model of local cerebral blood flow. The statistic R = integral tC(t) dt/integral C(t) dt= integral te-k2t multiply sign in circle Ca(t) dt/integral e-k2t multiply sign in circle Ca(t) dt classifies curves in terms of k2, where C(t) and Ca(t) denote the tissue and blood concentration histories, respectively, and multiply sign in circle is the convolution operator. Simulation studies of noise propagation in the clustering statistic showed that 30% voxel noise yielded a 2% standard deviation in R. Parametric images of blood flow and partition coefficient were computed for an O15 study, with and without clustering. Cluster size affected bias, statistical precision, and computation time. With clusters of 400 voxels, the variance of the flow parameter was around 1/50 smaller with clustering, with negligible bias and a computation time of 30 s on a 64-MHz workstation for 15 x 128 x 128 images with MATLAB 5.1.

Comparison of three PET dopamine D2-like receptor ligands, [11C]raclopride, [11C]nemonapride and [11C]N-methylspiperone, in rats.

We studied the tracer kinetics of three dopamine D2-like receptor ligands, [11C]raclopride ([11C]RAC), [11C]nemonapride ([11C]NEM) and [11C]N-methylspiperone ([11C]MSP), in anesthetized rats by tissue dissection, ex vivo ARG and PET in order to clarify their characteristics for PET imaging. The in vivo affinity of the three ligands for the striatum ([11C]MSP > [11C]NEM > [11C]RAC) obeyed the in vitro affinity for dopamine D2 receptors. The affinity of [11C]RAC and [11C]MSP for the cerebellum was very low, but the affinity of [11C]NEM for the cerebellum was compatible to that for the cortex and was not to be ignored. Also the affinity of [11C]MSP for the cortex was relatively high. [11C]RAC showed the highest selectivity. The striatal PET image with [11C]RAC was clearer than that with [11C]NEM or [11C]MSP, but the activity decreased much faster than that measured by tissue dissection because of the partial volume effect. The striatal activity with [11C]NEM remained high and that with [11C]MSP gradually increased. [11C]RAC and [11C]MSP, but not [11C]NEM, showed a high accumulation in the periorbital region.

An alternative synthesis of [11C]raclopride for routine use.

The standard method of [11C]raclopride synthesis requires a large amount of its desmethyl precursor. We prepared [11C]raclopride by methylation of a small amount of desmethyl derivative (0.3-0.5 mg) with [11C]methyl iodide in a DMF solution containing NaH, with a decay-corrected radiochemical yield of 11-14% based on [11C]methyl iodide and with a specific activity of 48 TBq/mmol for 25 min from EOB. The reaction was reproducible and reliable.

Preserved benzodiazepine receptors in Alzheimer's disease measured with C-11 flumazenil PET and I-123 iomazenil SPECT in comparison with CBF.

This study evaluates the regional cerebral blood flow (CBF) with H2(15)O-PET and the distribution of central benzodiazepine receptor (BZR) with C-11 flumazenil (FMZ) by PET and I-123 iomazenil (IMZ) by SPECT in Alzheimer's disease (AD). In AD, whereas the CBF was diminished in the frontal, temporal, parietal, and occipital cortex, the distribution volume of FMZ and delayed activity of IMZ were relatively preserved in these cortices, suggesting that the BZR reduction, reflecting neuronal loss, is less prominent than the CBF suppression. The mini-mental state examination score (MMS) was weakly correlated with the CBF in the parietal cortex but not with BZR. It is speculated that the neuronal density reflected by BZR is less impaired than the neuronal function assessed with blood flow in the association cortex of AD. High correlation was found between the uptake of FMZ and the delayed activity of IMZ. The delayed image of IMZ-SPECT is clinically useful to evaluate the preservation of neuronal density in the affected temoporoparietal association cortex in AD.

Electromyographic evaluation after endoscopic carpal tunnel release in idiopathic carpal tunnel syndrome.

The purpose of this study was to electromyographically evaluate results in patients with carpal tunnel syndrome (CTS) who underwent endoscopic carpal tunnel release (ECTR). The subjects were 26 patients with idiopathic CTS (37 hands) who were followed for at least 6 months after ECTR. To compare results informatively, hands were classified into four groups: those with normal distal motor latency (DML) and sensory conduction velocity (SCV) were classified as group A, those with normal DML and abnormal SCV as group B, those with an abnormal DML and normal SCV as group C, and those with abnormal DML and SCV as group D. All but one of the hands were classified as group D on the basis of preoperative electromyographic evaluation, while one was classified as group C. The mean preoperative obtainable DML and SCV values were 7.2 m and 27.3 m/s, respectively. Postoperatively, 12 hands were in group A, 8 hands in group B, 2 hands in group C, and 15 hands in group D. The mean DML and SCV values at final follow-up were 4.3 ms and 40.8 m/s, respectively. Of the 25 hands with muscle atrophy before surgery, 6 hands were in group A, 5 hands were in group B, 1 hand was in group C, and 13 hands were in group D at final follow-up. Thenar muscle atrophy and denervation potentials were present before surgery in 13 of the 15 hands classified as group D at the final follow-up.

Chronic compartment syndrome of the lower leg: a new diagnostic method using near-infrared spectroscopy and a new technique of endoscopic fasciotomy.

A 19-year-old female basketball player had chronic compartment syndrome. During basketball playing, she complained of bilateral lower leg pain that disappeared after several minutes of rest. The intracompartmental pressure in the anterior compartment was 41 mm Hg on the right side and 29 mm Hg on the left side immediately after playing. Prolonged ischemia of the anterior compartment was observed in comparison with four normal controls using near-infrared spectroscopy. Magnetic resonance imaging also revealed that the anterior compartment was mainly affected. Endoscopic fasciotomy was performed using an arthroscope, a transparent outer tube, and a retrograde blade. After the operation, her symptoms disappeared. Three months postoperatively, the anterior compartment pressure decreased and prolonged tissue ischemia improved. Endoscopic fasciotomy allowed us to cut the fascia safely and less invasively. We concluded that this technique is useful in treating chronic compartment syndrome in the anterior compartment of the lower leg.

Synthesis of [11C]salicylic acid and related compounds and their biodistribution in mice.

For in vivo measurement of the hydroxyl radical (.OH), we synthesized [11C]salicylic acid, [11C]O-acetylsalicylic acid and [11C]2-methoxybenzoic acid by carboxylation of 2-bromomagnesiumanisol using [11C]CO2. The radiochemical yield of [11C]salicylic acid, [11C]O-acetylsalicylic acid and [11C]2-methoxybenzoic acid calculated from trapped [11C]CO2 in a liquid argon cooled stainless tube was 7.3 +/- 1.6, 5.2 and 10.2 +/- 1.7% (decay corrected), respectively. The uptake of 11C tracers by mouse brain was 0.46, 0.32, and 0.46% dose/g tissue, respectively, at 10 min post injection and presented washout patterns thereafter.

Enzymatic synthesis of no-carrier-added 6-[18F]fluoro-L-dopa with beta-tyrosinase.

An enzymatic synthesis of nca 6-[18F]fluoro-L-dopa has been developed. The process consists of a chemical synthesis of 4-[18F]fluorocatechol and its enzymatic reaction with beta-tyrosinase. The 4-[18F]fluorocatechol was prepared by nucleophilic aromatic substitution of the NO2 group on 6-nitroveratoraldehyde with [K/222]+18F-, followed by decarbonylation with tris(triphenylphosphine) rhodium(I) chloride and hydrolysis with hydroiodic acid. By C18 Sep-Pak purification, 4-[18F]fluorocatechol was obtained in ethanol with a radiochemical yield of 9.2%. An enzymatic reaction of 4-[18F]fluorocatechol, ammonium and pyruvate catalyzed by beta-tyrosinase in an ethanolic Tris-HCl buffer (pH 9.0) containing ascorbate gave within 5 min 6-[18F]fluoro-L-dopa with an approximate radiochemical yield of 60% without any isomers. The deproteinized reaction mixture was applied to a preparative reverse phase column, and the radiochemically and enantiomerically pure 6-[18F]fluoro-L-dopa was obtained with a radiochemical yield of 2.0% based on [18F]F- (decay-corrected). The synthesis time was 150 min from the EOB and the specific activity was > 200 GBq/micromol.

Synthesis of [11C] coenzyme Q-related compounds for in vivo estimation of mitochondrial electron transduction and redox state in brain.

We have studied the synthesis of [11C]2,3-dimethoxy-5-methyl-6-(10-hydroxy)-decyl-1,4-benzoquinone (idebenone) and [11C]2,3-dimethoxy-5-methyl-1,4-benzoquinone (CoQo) by methylation of their respective desmethyl precursors using [11C]CH3I for in vivo measurement of mitochondrial electron transfer and redox state. The [11C]idebenone was more lipophilic than [11C]CoQo; the latter became hydrophilic by reduction. Clearance of [11C]idebenone from mouse brain was more rapid than that of [11C]CoQo. The results indicated that modification of the isoprenoid side chain in [11C]CoQ is necessary to develop more suitable radiopharmaceuticals.

Quantitative ex vivo and in vitro receptor autoradiography using 11C-labeled ligands and an imaging plate: a study with a dopamine D2-like receptor ligand [11C]nemonapride.

Ex vivo and in vitro autoradiography (ARG) with radioluminography is a useful technique to characterize newly developed 11C-labeled positron emission tomography (PET) tracers and to apply them to biological and pharmacological studies. In this report, we have described a method of evaluating the radioactivity distribution quantitatively in ex vivo and in vitro ARG using imaging plates and a dopamine D2-like receptor ligand [11C]nemonapride as a model compound. The photo-stimulated luminescence (PSL) values of the rat brain section provided by the imaging plates showed an excellent linear relationship with the radioactivity in a wide range under constant slice-thickness, although the PSL values slightly decreased with increasing slice-thickness both in ex vivo and in vitro ARG. The injection dose of 11C-tracers for ex vivo ARG was also discussed. We found saturable binding sites of [11C]nemonapride in the cortex besides the striatum both ex vivo and in vitro.
