[Early detection of non-palpable (T0) breast cancer: the diagnostic procedure for pathological discharge from the nipple].

Detection of non-palpable (T0) breast cancer by pathological nipple discharge is possible 3 years or more earlier than tumorous breast cancer. However, the definite diagnosis of T0 breast cancer has been considered to be very difficult because the standard diagnostic method such as exfoliative cytology and ductography were not totally reliable. In 1985 we first demonstrated the significance of CEA measurement in nipple discharge for diagnosis of T0 breast cancer. Since then CEA activity in nipple discharge was estimated in 60 patients with breast diseases by means of enzyme immunoassay using monoclonal anti CEA antibody. They include 17 with T0 breast cancer, 9 with borderline lesion, 20 with intraductal papilloma and 14 with fibrocystic diseases. When the cut off value of CEA concentration was set at 600 ng/ml, the sensitivity, specificity and accuracy were 76.5%, 100% and 92.2%, respectively. These levels were higher than those for mammography or cytology. In the past 7 years, 13 cases of T0 breast cancer were detected in our hospital. They accounted for 2.5% of total breast cancer cases. In conclusion, CEA measurement in nipple discharge is a useful method for the diagnosis of non-palpable breast cancer.

[Rapid growth of eyelid squamous cell carcinoma in elderly patients with multiple cancers].

Two cases of squamous cell carcinoma of the eyelid are reported. Both patients had multiple cancers. Case 1 was an 81-year-old man who had a squamous cell carcinoma on his right upper eyelid and on his right palm, and Bowen's disease in his left thumb. Case 2 was an 87-year-old woman who had a squamous cell carcinoma on her right lower eyelid, an adenocarcinoma in her colon, and a uterus cancer of unknown histology. In spite of their advanced age, the eyelid tumors showed rapid growth in both cases. The reason for this rapid growth was unclear. However, the presence of cancers in other organs may provide clues, because the immunological ability in patients with multiple cancers is considered to be weakened.

Psychiatric complications after ocular surgery.

Delirium was demonstrated after cataract surgery in a 64-year-old man (case 1). The patient struck his head against the floor; thereafter an extradural hematoma was found and extracted. Postoperative delirium was also noted in a 58-year-old woman (case 2) and in a 26-year-old woman (case 3) with rhegmatogenous retinal detachment. All 3 patients recovered from their psychiatric complications within 1 month.

Pseudoretinitis pigmentosa in patients with systemic lupus erythematosus.

A 42-year-old woman and a 62-year-old man with systemic lupus erythematosus (SLE) had bilaterally mottled retinas with spots or clumps of pigment. Their electroretinographic responses were subnormal. Their fundi resembled those of patients with retinitis pigmentosa. The fundus lesions of our patients may have resulted from vascular occlusion associated with SLE.

[The threshold lowering effects of chronic treatment with haloperidol on beta-carboline derivative-induced tonic convulsion].

To clarify a relationship between dopamine neuron and purine, GABA or benzodiazepine system, we have studied the changes in the threshold of tonic convulsion induced by each antagonist after chronic treatment with haloperidol in mice. Mice were given haloperidol (1 mg/kg, sc) once a day for 19 d and challenged with caffeine (an adenosine receptor antagonist), beta-DMCM (beta-carboline derivative: as a benzodiazepine receptor antagonist), picrotoxin (a Cl- channel blocker) or bicuculline (a GABAa receptor antagonist) 30 min, 24 h and 48 h after the last injection of haloperidol. Only the threshold of beta-DMCM-induced tonic convulsion was lowered and it was reversed 7 d after the last injection. The beta-DMCM-induced convulsions on 2 d withdrawal were reversed by diazepam (2.5 mg/kg, ip; a benzodiazepine receptor agonist), Ro15-1788 (5.0 mg/kg, ip; as like a benzodiazepine receptor partial agonist), muscimol (2.0 mg/kg, ip; a GABAa receptor agonist) or apomorphine (0.25 and 2.0 mg/kg, ip; a dopamine receptor agonist). These results suggest that the lowering effect of chronic haloperidol on seizure threshold may be involved in the development of tolerance to haloperidol. It may implicate in direct interactions between benzodiazepine and dopamine or GABA systems but may not between dopamine and GABA neurons in development of lowering seizure threshold following chronic haloperidol treatment.

Ophthalmic complications in patients with malignant tumors of the nose and paranasal sinuses.

We examined 34 Japanese patients (16 men and 18 women) with malignant tumors of the nose and paranasal sinuses. Squamous cell carcinoma of the maxillary sinus was the most common tumor. During the patients' initial visits, several ophthalmic symptoms were noticed, including proptosis, epiphora, lid swelling, displacement of the globe, orbital mass, and sudden visual loss. In addition, several ocular and orbital symptoms and signs resulted from treatment with radiotherapy, chemotherapy, and surgery and from local recurrence of the tumor. Four patients underwent orbital exenteration, and one underwent evisceration. Five eyes had no light perception.

Type of arteriovenous crossing at site of branch retinal vein occlusion.

A retrospective study of 63 patients (64 eyes) with symptomatic branch retinal vein occlusion, 30 men and 33 women, was carried out in this study. The ages of the subjects ranged from 40 to 82 years. Systemic hypertension was present in 41 patients and diabetes mellitus in 19. The occlusion involved the superotemporal vein in 39 eyes, inferotemporal vein in 20, superonasal vein in 2 or macular vein in 3. The site of the occlusion could be examined in detail in 57 eyes, and an arterial overcrossing was involved in 53 of these eyes (93%). This incidence of arterial overcrossing was significantly higher in the branch retinal vein occlusion eyes than in control eyes (P less than 0.05).

Presumed iris metastasis from abdominal neuroblastoma.

A 2-month-old infant had a retroperitoneal tumor and increased levels of vanillylmandelic acid and homovanillic acid in the urine, indicating an abdominal neuroblastoma. Two whitish masses that were noted on the left iris regressed in synchrony with the primary tumor mass as therapy was administered. We believe that our patient represents the third reported case of iris metastasis from abdominal neuroblastoma.

Ophthalmic complications in patients with paranasal sinus mucopyoceles.

We examined 45 (23 male and 22 female) Japanese patients with paranasal sinus mucopyoceles associated with ophthalmic complications. The patients ranged in age from 31 to 87 years. The patient distribution by the affected sinus was as follows: maxillary sinus alone, 17 patients; ethmoidal sinus alone, 9 patients; frontoethmoidal sinuses, 9 patients; frontal sinus alone, 7 patients, and other multiple sinuses, 3 patients. Thirty-nine mucoceles and 6 pyoceles were identified. Ophthalmic signs and symptoms included lid swelling, ocular pain, pseudoptosis, proptosis, displacement of the eyeball, epiphora, slight blurring of vision, severely decreased visual acuity, visual field abnormality, diplopia, disk edema, retinoichoroidal folds and increased intraocular pressure. Of the 45 patients, 1 had a history of sinus trauma and 29 had a history of sinus surgery.

Effects of indeloxazine hydrochloride on sleep in normal humans.

The effects of 40 mg of indeloxazine hydrochloride, a cerebral metabolic enhancer, on sleep were investigated in 6 healthy male students. Polygraphic recordings were made for 8 consecutive nights on each subject. An inert placebo was given on the first 3 nights and on the 7th and 8th nights, and 40 mg of indeloxazine was administered on the 4th, 5th and 6th nights. Drug and placebo were administered orally at 21:30 h, and the recording of polysomnograms started at 22:00 h and ended at 8:00 h the next morning. The polysomnograms were evaluated by computerized automatic analysis using the interval histogram method. Indeloxazine prolonged both sleep latency and REM latency, and increased stage 1 sleep, but decreased stages 3, 4 and REM sleep. A significant increase of REM sleep was observed on the 7th and 8th recovery nights. No obvious changes were shown in the subjective assessments after administration of the drug. These results suggest that the effects of indeloxazine on sleep are similar to those of the tricyclic antidepressants. Furthermore, polysomnographic examinations might be useful in evaluating the pharmacological properties of cerebral metabolic enhancers.

Herpes zoster ophthalmicus complicated by hyphema and hemorrhagic glaucoma.

We treated two patients with herpes zoster ophthalmicus in whom hyphema and hemorrhagic glaucoma occurred. Case 1 complained of facial skin eruption, and was given intravenous acyclovir for 7 days. Hyphema and high intraocular pressure occurred in the left eye 10 days after the onset of the skin eruption. Case 2 had severe pain and blisters on her face, and was given intravenous acyclovir for 7 days. An intracameral hemorrhage and glaucoma developed in the right eye 15 days after the onset of the skin lesion. Intravenous acyclovir may be necessary for longer than 7-day periods if the iridocyclitis remains.

Production of tumor necrosis factor (TNF) by monocytes from cancer patients and healthy subjects induced by OK-432 in vitro, and its augmentation by human interferon gamma.

A cytostatic factor was induced in the supernatants of human blood monocytes cultured in vitro with OK-432 at 0.01 KE/ml or more. This cytostatic activity was almost completely abolished by anti-human TNF antibody, and so was thought to be that of TNF. Recombinant human gamma interferon (IFN-gamma) alone did not induce TNF as described in other reports, but enhanced the induction of TNF by OK-432 when added to monocytes before or with OK-432. Monocytes of 20 patients with cancer of the digestive tract and 10 healthy donors were tested for ability to produce cytostatic activity. The frequencies of positive responses to OK-432 and the mean cytostatic activities of the two groups were similar. Moreover both groups included high responders whose monocytes produced potent cytostatic activity with OK-432 alone, and also low responders. Interestingly, marked synergistic effects of IFN-gamma and OK-432 were observed with monocytes that did not produce potent cytostatic activity with OK-432 alone. These results suggested that OK-432 may have a stronger immunomodulatory effect clinically when used in combination with IFN-gamma.

Lysosomal enzymes in tear fluids from patients with Terrien's marginal corneal degeneration.

We examined biochemically the lysosomal enzyme activities in tear fluids from patients with mild myopia, senile cataract, and Terrien's marginal corneal degeneration. Tear acid phosphatase activities in Terrien's degeneration were almost the same as those in mild myopia and senile cataract, while those of N-acetyl-beta-D-glucosaminidase in Terrien's degeneration were higher. The high activity of tear N-acetyl-beta-D-glucosaminidase may be derived from the lacrimal gland and infiltrate histiocytelike cells in Terrien's marginal corneal degeneration.
