Aplastic anemia during ethosuximide medication. Treatment with bolus-methylprednisolone.

A girl, born in 1975, developed severe aplastic anemia in November 1982 while on ethosuximide monotherapy for petit mal epilepsy, about 3 months after starting the ethosuximide medication. She was treated with bolus-methylprednisolone as described by Bacigalupo et al. (13), transfusions and antibiotics. During the third week on this regimen slight improvements in granulocyte and reticulocyte counts were demonstrated, and after approximately 6 weeks she was in stable condition without further need of transfusions. Blood status should be checked regularly during ethosuximide therapy, particularly during the first 6 months.

Erythrocyte pyrimidine 5'-nucleotidase deficiency. Report of 2 new cases, with a review of the literature.

We present 2 Norwegian siblings who we believe to be the first cases of pyrimidine 5'-nucleotidase deficiency diagnosed in the Scandinavian countries. In addition to the features presented by previously published cases, our patients exhibited signs of intravascular haemolysis with haemoglobinuria and increased loss of iron in the urine. The loss of iron has made iron medication necessary from time to time. A kidney biopsy from the oldest patient revealed accumulation of iron in the epithelium of the proximal tubuli. The kidneys were considerably enlarged in both patients, but with normal function tests.

Cytosine and uracil nucleotides in erythrocytes from two patients with pyrimidine 5'-nucleotidase deficiency.

The purine and pyrimidine nucleotides in the erythrocytes from two children with 5'-nucleotidase deficiency have been studied using HPLC-technique. The children belonged to the same Norwegian family. In addition to the conventional uracil and cytosine nucleotides relatively high concentrations of UDP-glucose, UDP-N-Ac-glucosamine, CDP-choline and CDP-ethanolamine were found.

Intermediate dose methotrexate (IDM) in childhood acute lymphocytic leukaemia in Norway. Report on a national treatment program.

Three courses of intermediate dose methotrexate, 500 mg/m2 at three weekly intervals have been used as part of sanctuary therapy to 95 out of 112 children with acute lymphocytic leukaemia in Norway in the period August 1975 to December 1979. Eight doses of MTX IT were also used for CNS prophylaxis, the first dose was given on the day of admission. As of July 1, 1980, 75 out of 95 (79%) were still in CCR for a median of 36 months (6-57 months).

Serum lipoproteins in children with anaemia.

Plasma and erythrocyte lipids were estimated in 9 children with different types of anaemia and in 9 healthy control children. The anaemic children had rather high plasma triglcerides, but low total cholesterol and phospholipids. The levels of high density lipoprotein (HDL) cholesterol were particularly low, with a mean of only 70% of the controls. A positive correlation was found for haematocrit with HDL cholesterol, but not with low density lipoprotein (LDL) cholesterol. Plasma albumin was at the same level in both groups of children, however, suggesting that the diluting effect of increase in plasma volume could only partly explain the lowering of HDL cholesterol. A striking, inverse correlation between HDL cholesterol and very low density lipoprotein (VLDL) triglycerides was present in the anaemic children. Anaemia in children adds to previously recognized pathophysiological condditions which induce inversely interrelated changes in HDL and VLDL.

High dose methotrexate in acute lymphocytic leukemia in childhood.

Gonadal and other types of leukemic "sanctuaries" are probably the main causes of hematological relapse in the treatment of acute leukemia. The introduction of high-dose Methotrexate (HDM) in a consolidation phase is based on theoretical considerations and the use of HDM in malignant tumors. Three courses of Methotrexate, 500 mg/sq.m. at 3-weekly intervals, has been used as part of a consolidation therapy in Norway during the last two years to 59 children with ALL and one with AML. One child died following HDM. Postmortem examination showed that she was not in complete remission at the time. Among 154 courses of HDM in the 60 patients were eight severe reactions, including six cases of allergic-toxic skin reactions. Two patients developed a Stevens-Johnson's like syndrome. Stomatitis was common in those with toxic reactions. The risk of HDM in patients who are not in complete remission is stressed and the use of rescue therapy with two doses of Leukovorin instead of one is recommended. Forty of forty-two children in 1st complete remission have been in sustained primary remission for 4 to 28 months. Two of these 40 children died after about a year from infections. Only two patients so far have relapsed.

Fat cell size and lipid content of subcutaneous tissue in congenital generalized lipodystrophy.

Fat cell size and lipid composition of subcutaneous tissue from 3 patients with congenital generalized lipodystrophy have been measured before and after treatment with either pimozide, fenfluramine or hypophysectomy. The fat cell volume before treatment ranged between 9.8 x 10(4) and 17.7 x 10(4) micron 3, compared to 9.0 x 10(4) and 85.3 x 10(4) micron 3 in 15 controls. The amount of lipids was only 1/10 to 1/50 of normal, triglycerides being the most heavily reduced lipid component. Neither the fat cell size nor the lipid content were affected by treatment with hypophysectomy, pimozide or fenfluramine.

Serum cholesterol and triglycerides in children with anaemia.

Serum cholesterol and triglyceride levels were determined in 17 children with various forms of anaemia and varying rates of red cell production. Anaemic patients had low serum cholesterol values, and a good correlation between Hb and serum cholesterol levels was found. The serum triglyceride levels were also low in most patients, but the results were less closely related to Hb. No relationship could be demonstrated between the rate of red cell production as judged by reticulocyte counts, and serum cholesterol or triglycerides. Apparently all lipoprotein classes are reduced in individuals with uncomplicated anaemias. This hypolipidaemia can only partly be explained by the diluting effect of the increase in plasma volume accompanying anaemia.

Lipodystrophic diabetes treated with fenfluramine.

Congenital generalized lipodystrophy is considered to be a diencephalic syndrome with disturbance of hypothalamic transmitters. After puberty and arrest of growth the patients develop a serious untreatable diabetes mellitus. One of our patients, a girl 15 years of age, developed a lipodystrophic diabetes with fasting blood glucose levels above 300 mg/100 ml, increased serum insulin with insulin resistance, and hyperlipidaemia. Daily administration of fenfluramine gave a dramatic improvement. The voracious hunger and profuse perspiration were reduced, the patient's serum lipids became normal, her blood glucose fell, and her sensitivity to exogenous insulin increased. A normalization of the urinary excretion of the serotonin metabolite, 5-OH-indole acetic acid, was observed.

Insecticides in human breast milk.

Fifty samples of human breast milk were analysed by gas chromatography and thin layer chromatography for DDT (dichlordiphenyltrichloraethan), hexachlorobenzol, benezene-hexachlorides, dieldrin, aldrin and heptachlorepoxide. The three first substances were found in all samples in amounts varying from quite small up to eleven times the WHO recommended maximum for cow milk. The other substances were found in fewer milk samples, however, in some of these samples they were found in relatively high amounts. In Norway, only DDT has been investigated earlier (4), and compared to that study, there has been no significant change in the mean concentration of DDT in human breast milk during the last five years. The insecticide content was highest in colostrum and decreased with increasing duration of lactation. Considerable fluctuations in the content of insecticides were recorded in repeated milk samples collected from the same woman a few days apart. The percentage of samples with insecticide content higher than that permitted for cow's milk was greater in May/June (79%) than in early April (54%). The significance of these findings is discussed.
