Opioid-induced respiratory depression and analgesia may be mediated by different subreceptors.

Use of selective delta opioid antagonists provide evidence that the delta receptor within the brain seems an integrated part in the mediation of respiratory depression induced by a potent analgesic like fentanyl. Low doses of the delta antagonists RX-8008M (3-6 micrograms/kg) as well as ICI 174,864 (3-6 micrograms/kg) reversed fentanyl-related respiratory depression (arterial blood gases) in the unanesthetized canine. Opioid-induced blockade of afferent sensory nerve volleys (amplitude height of the somatosensory-evoked potential) could be reversed only by a high dose (9 micrograms/kg) of RX-8008M. Depression of amplitude height of the SEP could not be reversed by ICI 174,864 over the whole dose range (3-6-9 micrograms/kg). In comparison, naloxone (1-5-10 micrograms/kg) not only reversed depression of PaO2, it also reversed the blockade of afferent sensory nerve impulses in the low (5-micrograms/kg)-dose range. A highly selective delta antagonist may have a therapeutic value in reversing opioid-related respiratory depression, resulting in little or no attenuation of analgesia.

[Epicondylitis humeri (tennis elbow)].

Epicondylitis is mostly related to a tendinosis found in the region of the radial or ulnar epicondylus, where the common origin of the extensor or flexor muscles is anchored. Several causes are associated with the etiopatho-genesis, most important is mechanical stress. The working hand is involved in the majority of cases. The age of the patients ranges mainly from 40-60 years. In 90% of the cases conservative measures are successful; if surgical treatment is necessary we recommend a modified version of Hohmann's operation.

[Fixation casts in skiing-traūmatology].

Extensive experiences have shown that bandages of ultraviolet sensitive synthetic resin (LightCast II) offer important advantages compared to the conventional plaster cast. The special technique and details of procedure are described. The most important advantages are the light weight, water resistance (the patients can wash, shower and swim,), absolut stability is gained after 5--6 minutes under a ultraviolet lamp. Indications for the use of the fiberglass-cast are discussed.

Effect of epidural analgesia on colorectal anastomotic healing and colonic motility.

BACKGROUND AND OBJECTIVES: To examine the effect of epidural local anesthetic and narcotic agents on colonic anastomotic healing. METHODS: A prospective randomized study was conducted in a porcine model. Twenty-one pigs undergoing colorectal resection and anastomosis were randomized to receive either bupivacaine (Group 1), morphine (Group 2), or normal saline (Group 3) by intraoperative and postoperative epidural infusion. Colonic blood flow was measured using laser doppler velocimetry and colonic motility assessed with radio-opaque markers and daily x-rays postoperatively. Seven days postoperatively, the anastomoses were resected and analysis of bursting pressure and hydroxyproline content performed. RESULTS: In this porcine model, epidural anesthesia accelerated colonic transit time. Group 1 and 2 animals had significantly faster colonic transit time (3.9 and 4 days, respectively) when compared with Group 3 animals (6 days; p less than 0.05, chi-square analysis). There was no statistically significant difference in blood flow, bursting pressure, and hydroxyproline content between the three groups, and no anastomotic complications occurred in any animal. CONCLUSIONS: These findings suggest in this model that postoperative epidural analgesia is a safe technique after colorectal resection and anastomosis.

Bioequivalence of two commercially available tamoxifen tablet formulations in healthy male volunteers.

Tamoxifen, a non-steroidal antiestrogen, is used in the palliative treatment of advanced breast cancer and as an adjuvant therapy after mastectomy. The bioequivalence of tamoxifen following single oral doses of Zemide 20 and a commercially available tablet formulation (reference formulation), each containing 30.4 mg tamoxifen dihydrogencitrate corresponding to 20 mg tamoxifen was investigated in 12 healthy male subjects. In a randomized, crossover study with a washout period of 13 weeks the volunteers received one pharmaceutical unit of both formulations after an overnight fast on an empty stomach. Plasma concentrations of tamoxifen were determined at predose and predetermined time points up to 20 days after administration using a validated HPLC method. The two tamoxifen formulations were proven to be bioequivalent for tamoxifen regarding rate and extent of absorption. The bioequivalence decision was based on Cmax, tmax and AUC0-t.
