Heterotopia of the macula (ectopic macula): an unusual presentation.

Heterotopia of the macula is a condition in which the macula is displaced markedly from its normal position. Only 40 cases have been reported in the literature. Of those cases more than 90% have occurred in patients presenting with an apparent exotropia (macula displaced temporally), a large positive angle lambda, and an actual esotropia. We report a unique case of heterotopia of the macula in which the patient presented with an apparent esotropia and a large negative angle lambda. This is the only reported case of ectopic macula in which the macula has been dragged nasally toward the disc in association with a suspected retinopathy of prematurity. There have been three other cases of heterotopia of the macula in which the macula was displaced nasally, but these were associated with chorioretinitis.

Divergence insufficiency: characteristics, diagnosis, and treatment.

Although the entity of divergence insufficiency was described as early as 1886 by Duane, it has received relatively little attention in the literature. Patients presenting with a greater eso deviation at distance than at near, a concomitant deviation, and diplopia pose a challenging and critical diagnostic and therapeutic puzzle for the clinician. Divergence insufficiency, a benign condition, must be differentiated from divergence paralysis and sixth nerve palsy, two conditions that present with somewhat similar findings yet have underlying etiologies of a serious nature. This paper reviews the literature and clarifies the etiology, diagnosis, and treatment of divergence insufficiency and related conditions.

The effect of protective sports eyewear on peripheral visual field and a peripheral visual performance task.

Seven different types of protective sports eyewear were tested to determine whether peripheral awareness as measured with a visual performance task were affected. Goldmann perimetry and the Wayne Saccadic Fixator with Stik-Up attachments were utilized in a repeated measures research design. The sports glasses did restrict visual field, but no one pair of glasses were least restrictive. The restrictions did not translate into significant differences on the performance task. Peripheral visual factors should be taken into account when prescribing protective sports eyewear. The importance of proper eye protection in sports is also discussed.

Anisotropic resolution in children's vision.

Grating acuity was measured for 4 orientations in 111 children aged 3-8 yr. Results showed that 73% of the children had better acuity for horizontal and vertical compared to oblique gratings, 14% showed no anisotropy, and 13% had better acuity for oblique gratings. The magnitude of the oblique effect was 0.2 octaves, similar to that found in both younger children and adults.

Astigmatism in children: changes in axis and amount from birth to six years.

Noncycloplegic refractions of 1,000 children aged 0-6 years revealed a high incidence of astigmatism, especially in the first 2 years of life. Before age 41/2 years, most of the astigmatism was against-the-rule and after that age most was with-the-rule. Of 19 children who did not show astigmatism in the first year, only one acquired it by 4 years. Of 29 children who had large amounts of astigmatism in the first year, all showed elimination or a large reduction in the amount of the cylindrical error by 4 years. These results are relevant to the etiology of astigmatism.

Report on the status of vision care in Israel.

Recognition of optometry in Israel has not been achieved due to various factors: the lack of recognition by the Ministry of Health in proposing a law of optometry to the Knesset (Parliament); the opposition of organized ophthalmology; the internal conflict between the various associations of opticians and optometrists within the country which lead to a lack of unification for propagation of a law of optometry; the absence of an academic institution of learning in optometry. The establishment of the Optometric Centre by the American Friends of Israel Optometry has instituted low vision clinics and post-graduate courses which have helped to advance optometry. Optometry must become organized in order to apply pressure on the Ministry of Health in order to recognize optometry as an independent profession. There is a need for the establishment of operating courses to equalize the standard of eye-care in the profession, and for the founding of an academic school of optometry to maintain a constant supply of qualified optometrists.
