Keratoconus associated with Terrien's marginal degeneration. A clinical and ultrastructural study.

A 59-year-old man presented with Terrien's marginal degeneration associated with keratoconus of his left eye. Penetrating keratoplasty of the left eye permitted an evaluation under optical microscopy, as well as transmission and scanning electron microscopy. An anatomical-pathological evaluation confirmed the co-existence of a central lesion due to keratoconus and a peripheral lesion due to Terrien's marginal degeneration. An absence of the knowledge of the etiology of either affliction does not allow one to define whether there exists one or two active degenerative processes.

Effect of cefazolin on the corneal endothelium. An experimental study.

Cefazolin sodium was injected into the anterior chamber of the rabbit eye in order to evaluate its effect upon the corneal endothelium. Endothelial toxicity was evaluated by contact specular microscopy and transmission electron microscopy 48 h after injection. The results suggest that doses of 1 and 2 mg of cefazolin are nontoxic to the corneal endothelium. Endothelial toxicity occurred when 5 and 10 mg of cefazolin were injected.

[Terrien's disease, apropos of 6 cases. Ultrastructural study].

Terrien's disease is characterized by slowly progressive thinning of the pre-limbal cornea in middle-aged patients. It usually leads to peripheral ectasia with marked astigmatism. It is rarely accompagnied by inflammation and its cause is unknown. Very few histopathologic examinations have been published, in particular those using electron microscopy We present detailed results concerning 6 patients in whom perforating keratoplasty permitted histopathologic and electron microscopic examination. Clinical and histological features reveal marked degeneration of the peripheral corneal stroma, with lipid accumulation unaccompagnied by significant inflammatory cellular infiltration in each case. Of interest is the association in one patient with keratoconus. The generally accepted pathogenic theories are fully discussed. The wealth of electron microscopic data provides a complete review of stromal degenerative changes.

Excimer laser trephination in penetrating keratoplasty. Morphologic features and wound healing.

The imprecision of trephination of donor and recipient corneas is a major factor in post-keratoplasty astigmatism. In order to improve the quality of trephination, the authors developed a rotating slit delivery system for noncontact penetrating keratoplasty trephination using the excimer laser at 193 nm. Scanning electron microscopy (SEM), transmission electron microscopy (TEM), and light microscopy (LM) demonstrated the superior quality of excimer-cut buttons and recipient beds as compared with those obtained by free hand and suction trephines in human cadaver and rabbit eyes. The laser trephined more regularly and precisely without distortion of corneal topography and with less damage to adjacent corneal tissue. The authors morphologically examined wound healing at 6 hours, 12 hours, 3 days, 5 days, 2 weeks, 2 months, and 3 months after penetrating keratoplasty with laser and mechanical trephination in an animal autograft model. The laser did not cause any adverse alteration of wound healing processes including cellular migration, proliferation, and production of new tissue.

[Tissue colonization of expanded polytetrafluoroethylene in healthy cornea in the view of its use as a support in keratoprosthesis].

The biocompatibility of polytetrafluoroethylene (E. PTFE) keratoprosthetic skirts was studied for periods of observation from 4 to 14 weeks. In a first experimental study, the authors showed that E. PTFE was well tolerated but without fibrovascular ingrowth. In this study, eight perforating keratoplasties were performed and a disk of E. PTFE with a diameter of 4.5 mm was sutured in and covered with a conjunctival flap. The results show that E. PTFE allows fibrovascular ingrowth without a significant foreign body response or encapsulation. Coverage of E. PTFE with conjunctival was essential for fibrovascular ingrowth and to prevent extrusion and rejection.

Acute corneal edema in pellucid marginal degeneration or acute marginal keratoconus.

This article reports a case of bilateral corneal pellucid marginal degeneration. The right cornea had an acute hydrops. Both eyes underwent penetrating keratoplasty. A histopathological study of the corneal specimens was performed by light and electron microscopy. The histological changes observed on the right cornea showed breaks on Bowman's layer, edema and disorganization of the stromal collagen, and break of Descemet's membrane. The ultrastructural changes were similar to those observed in acute keratoconus, leading to the belief that these two corneal diseases are closely related.

[Unilateral progressive annular opacity of the cornea. Apropos of a case. Clinical, histological and ultrastructural study].

A case of unilateral corneal annular opacification with a slowly progressive thickening of the cornea is reported; clinical, optical and electron microscopic studies have made possible to eliminate oedematous pathological process. The central zone of the cornea was histologically within normal aspect. In the peripheral zone intracellular mucopolysaccharides (MPS) and lipids were found in the stroma. Their presence suggest a collagen degeneration, thus likely revealing a forgotten initial stromal pathology. Its consequence, the unilateral annular thickened opacified cornea, seemed to us confusing enough to be presented and discussed.

[Acanthamoeba keratitis. Clinical, histological and ultrastructural study].

Acanthamoeba keratitis is a rare but very severe necrotizing stromal keratitis due to a non-parasitic free-living soil and freshwater amoeba. It affects more readily soft and hard lens wearers. It is often clinically mistaken for herpetic keratitis because of the similarity of the signs and the difficulty in performing the specific microbiological tests. This is the reason why the diagnosis is most often made at the time of penetrating keratoplasty and histological study of the buttons with special stains. Medical treatment is effective only when starting at the beginning of the disease.

[Orbital localization of clear-cell sarcoma of tendons and aponeuroses. Apropos of a case report].

Clear cell sarcoma of tendons and aponeuroses actually is a well identified soft tissue tumor. A primary orbital involvement is presented, with an optical and electron microscopic study. The orbital topography of such a tumor seems to be an exceptional opportunity. The histological study showed the typical pattern of this clear cell tumor. Ultrastructurally, there are not absolute specific features to establish the diagnosis, except the finding of intra-cellular melanosomes. Histogenesis has been debated for a long time between a melanotic or a synovial origin. According to present knowledge, this tumor may be attributed to a neural crest origin.

[Eyelid localization of a Merkel cell tumor (trabecular carcinoma)].

The authors present four cases of Merkel cells tumor of the eyelid. These cases conform to the standard clinical and histological criteria specified in the dermatological literature. Immuno-histochemical studies have been performed (specific neuron enolase, cytokeratin KL 1). Il the last case an ultrastructural study has made evident the characteristic neurosecretory granulations and intermediate filaments. Various hypothesis concerning the origin of this tumor are discussed in light of anatomical evidence.

Drug-induced cicatricial pemphigoid affecting the conjunctiva. Light and electron microscopic features.

Cicatricial pemphigoid (CP) is a chronic inflammatory disease which can affect the conjunctiva. It is a slowly progressive disorder of unknown but presumed autoimmune etiology. Pseudopemphigoid, or CP associated with ocular drug administration, has also been described. These cases, which appear clinically indistinguishable from unilateral idiopathic CP affecting the conjunctiva, have been related to the use of echothiophate iodide, pilocarpine, idoxuridine, and epinephrine. We report the histopathologic and ultrastructural characteristics of 22 biopsies from ten patients with iatrogenic CP following the use of various ocular drugs. All patients presented with clinically obvious, active CP affecting the conjunctiva. Light microscopy and electron microscopy revealed findings identical to those previously reported for idiopathic CP of the conjunctiva: squamous metaplasia, increased numbers of desmosomes, basal lamina modifications suggestive of damage and attempted repair, subepithelial inflammatory cell infiltration, and diminished intravascular space within the stroma. These patients responded to immunosuppressive therapy. The authors wonder if it is possible that these patients were destined to develop CP, but that with topical ocular drug use, a more rapid emergence of the chronic cicatrizing feature of CP developed.

[Breaks in Descemet's membrane. Association with secondary warts. Histologic and ultrastructural study].

We analysed the histological and ultrastructural aspects of corneal buttons obtained by keratoplasty in two patients presenting breaks in Descemet's membrane. The edges of the breaks appear clinically as glassy ridges or ledges and are also called Haab's lines. One patient had a birth trauma and the other had congenital glaucoma. The edges of the breaks coiled anteriorly towards the stroma and were dramatically thickened by a multilaminar Descemet's like membrane that exhibited excrescences within its outer part and that was secreted by regenerated endothelial cells. Only one case was analysed by SEM. Secondary guttuta could not be detected by this technique because it was completely covered by a sheet of newly secreted fibrous tissue that prevented the warts from indenting the endothelial cells. The part of cornea corresponding to the break was also covered by a new thick Descemet's like membrane exhibiting guttate warts. This study is thus a good opportunity to discuss the mechanisms of endothelial and Descemet's membrane regeneration.

[The vernal plaque, a complication of so-called spring conjunctivitis].

The vernal ulcer is a peculiar corneal complication of vernal conjunctivitis. Clinically, the ulcer is oval, grey, and resistant to medical treatment. Histological study by light and electron microscopy reveal the lesion to be composed of fibrin deposits adherent to a well-preserved Bowman's membrane. These deposits impede lateral progression of the epithelium, thus, opposing spontaneous healing. Resolution of the ulcer can be obtained, however, by superficial keratectomy.
