[Is papilloma of the esophagus a preneoplastic lesion? Study of 33 cases].

The etiology of esophagus papilloma is much debated: some authors attach greater importance to irritation factors, while others give preference to the viral hypothesis and suggest that this disease could eventually lead to squamous cell carcinoma of the esophagus. To verify the viral hypothesis, we reviewed the histological slides of the 33 cases of esophageal papilloma diagnosed in our Institute of Pathology between 1973 and 1988. We evaluated the histological diagnosis using Winckler's criteria. HPV typing was done using probes of HPV DNA types 6-11, 16-18, 31-33-35 applied to paraffin sections according to the in-situ hybridization method. Clinical and endoscopic data of 15 cases (from the CHUV) are reviewed. Oncological data was provided by the Vaud Cantonal Tumor Register. No patient in our series fulfilled all the histological criteria set by Winckler to diagnose an HPV condition. Viral DNA 31-33-35 was found in a small minority of the papillomas. The clinical impact of esophageal papilloma on epidermoid carcinogenesis is nil.

[The place of total aerodigestive upper endoscopy in oncology].

The epidemiology of upper digestive and bronchogenic carcinoma shows a large overlap between the high risk groups. Thus, a candidate for bronchogenic carcinoma is also a possible candidate for upper digestive cancer. Furthermore, the high frequency of multiple carcinoma in the upper aero-digestive tract, the esophagus and the tracheo-bronchial tree warrants a pan-endoscopic (buccal-pharyngo-esophagoscopy and laryngo-tracheo-bronchoscopy) approach to the problem. Using this type of endoscopy, 16 multiple carcinomas have so far been simultaneously discovered among 200 pan-endoscopies.

A case of oesophageal cancer limited to the mucosa and submucosa.

The authors report a case of an oesophageal cancer limited to the mucosa and the submucosa. This case is interesting because of its long history for more than 18 months, the difficulty of fiberoptic diagnosis and the diagnostic value of rigid instruments completed via vital staining by o-toluidine.

[Fungal maxillary sinusitis].

Since 1969, 600 patients suffering from maxillary sinusitis have been systematically controlled clinically as well as mycologically. We discovered 81 cases of maxillary sinusitis (13,4%). Aspergillus fumigatus and Candida albicans were the most frequently isolated fungi. Three clinical forms could be distinguished: (1) a muco-purulent form (35 cases); (2) a budding and/or caseous form (22 cases), and (3) a pseudotumoral form (24 cases). 42 patients treated with a 1% econazol (base) solution were healed.

[Otomycoses].

Otomycosis is a relatively frequent disorder (21.7% of the cases of external otitis) of fungi origin. Otomycoses can be discriminated from bacterial otitis by the following symptoms: itching, absence of otalgy, discharge and negative response to different local antibiotic therapies. Our study involves 148 cases of otomycosis and their therapy. Recovery was obtained in approximately 10 days.

Amyloid pseudotumor treated by tracheal resection.

Primary amyloidosis is a rare disease, and even more so when it appears as a tumorlike formation in the lung or tracheobronchial tree. On rare occasions it has been treated by pulmonary resection but never before by tracheal or bronchial sleeve resection. A case of tracheal sleeve resection for tumorlike amyloidosis is reported. If the lesion is well circumscribed to a relatively short tracheal or bronchial segment, sleeve resection appears to be a satisfactory and economical solution. However, the problem of a mucosa undermined by amyloid deposits above and below the tumorlike lesion entails the risk of anastomotic stenosis. For this reason in future cases, particularly those with extensive involvement, piecemeal endoscopic removal should be considered as the method offering the best prognosis and least risk.

Purification of plasma membrane fractions from the bovine pars intermedia and neurohypophyseal lobe and properties of associated adenylate cyclase.

A procedure for the isolation of plasma-membrane-enriched fractions from bovine 'pars intermedia' and neurohypophysis is described. Various fractions are isolated by differential centrifugation and discontinuous sucrose density gradients. The plasma-membrane-enriched fractions have a density in sucrose of 1.14 and 1.16 and the yields are 1.8 mg and 1.5 mg per gram of tissue for the pars intermedia and neural lobe, respectively. The fractions are characterized by electron microscopy and enzymatic assays. The plasma membrane fractions are mainly vesicular in nature and are free of nuclei, mitochondria, and microsomes when examined by electron microscopy. 5'-Nucleotidase (EC 3.1.3.5) and Mg2+-(Na+ + K+)-ATPase (EC 3.6.1.3) activities are concentrated in the plasma-membrane-enriched fraction. Also, adenylate cyclase (EC 4.61.1) shows a 5 to 10-fold purification in the isolated membrane fraction. NaF (10mM) gives a two to three-fold stimulation of enzymatic activity in all fractions studied The yields of adenylate cyclase, 5'-nucleotidase, and Mg2+-(Na+ +K+)-ATPase are about 6% in the membrane fraction.
