Early experiences of endocardial catheter mapping of the left ventricle in patients with sustained ventricular tachycardia--efficacy, safety and complications.

In 23 patients with recurrent sustained ventricular tachycardia (VT) which originated from the left ventricle, endocardial catheter mapping has performed. In an additional 14 patients we also stimulated their left ventricle for non-sustained VT. Multiple sites could be mapped for the recording of local electrical activity, for pacing and for the induction of VT. These procedures could be done without complication. A careful, reasonable, and safe method of endocardial mapping will facilitate clinical electrophysiologic study.

Potency and receptors involved in coronary vasoconstriction caused by neuropeptide Y(NPY).

Using anesthetized dogs, the coronary vascular effects of neuropeptide Y (NPY) were studied and the action of alpha- or serotonergic receptor blockade on the action of NPY was evaluated. To demonstrate the biological significance of the action of NPY, the vasoconstrictor potencies of NPY and norepinephrine were compared. One to 5 nmol of intracoronary NPY reduced coronary flow in a dose-dependent manner. The action started rather gradually and lasted for 10 min or more. Since perfusion pressure and central venous pressure were unchanged, the decrease in coronary flow should be a result of coronary vasoconstriction. Intracoronary norepinephrine infusion caused vasodilatation but when dogs were pretreated with 0.5 to 1.0 mg/kg of systemic propranolol, a vasoconstrictor effect was observed at a 5 times higher dose than with NPY. Furthermore, the action of NE was only transient, lasting for 30 sec or less. The vasoconstrictor action of NPY was not antagonized by phentolamine or by ketanserin. Since NPY is an endogenous polypeptide found in the sympathetic nerve terminals around coronary arteries, it may participate in the regulation of coronary flow.

[The effect of cisapride on gastric emptying time and release of gastrointestinal hormones].

Cisapride is a novel gastrointestinal prokinetic drug. The aim of the present study was to investigate gastric emptying time and secretion of gastrointestinal hormones (Gastrin, Motilin, Glucagon) after single oral intake of 5 mg or 10 mg cisapride. The healthy men aged 23-55 years participated the study. The gastric emptying time and gastrointestinal hormones were measured by gamma-camera radioisotopic technique and radioimmunoassay, respectively. The half time of emptying phase was reduced from 31.78 min to 29.88 min, and emptying rate increased in 10 mg cisapride group. There was a tendency that cisapride accelerated a stimulation of Motilin secretion.

Correlation between gastric emptying time and the response of pancreatic polypeptide to test meals in peptic ulcer diseases.

In the present study, pancreatic polypeptide (PP) levels both in basal state and after stimulation by meal were determined in 21 controls, 28 patients with gastric ulcer and 21 patients with duodenal ulcer and their correlation with gastric emptying was examined. In patients with gastric and duodenal ulcers, basal levels as well as stimulated of plasma PP during 95 min of observation period were higher than those in controls except for the values at 5 min. As no correlation was seen between integrated PP response (IPPR) to the meal and T1/2 for gastric emptying in any of three groups, it was indicated that gastric emptying has only a minor role in postprandial PP response.

Mineralization and biodegradation of CSF shunting systems.

The mineralization and biodegradation of cerebrospinal fluid shunting systems were studied using material from 25 shunts that had been implanted for between 6 days and 10 years. New unused materials were also examined for comparison. Surface changes in six systems could be observed under an operating microscope. Substantial quantities of a white deposit had adhered to the tubing in four of the shunts. These changes were most advanced in the galeal penetrative portion of the shunts and are believed to have been caused by mechanical stress. Scanning electron microscopic analysis revealed surface wrinkles, microscopic holes, and tiny particles, suggesting deterioration of the material itself. An energy-dispersive analysis using x-rays demonstrated that the surface deposits were due to mineralization of calcium phosphate and that the tiny particle growth was aluminum. These changes may be a consequence of the degradation of silicone rubber. A discriminant analysis of the mineralization was carried out; thus, the age of the host and the duration of system implantation could be correlated with the incidence of mineralization (p less than 0.1). A measurement of the physical properties showed progressive change with a remarkable deterioration in systems implanted for more than 5 years.

[Analysis of the chemosensitivity test by subrenal capsule assay].

The antitumor activity of several chemotherapeutic agents against a total of 30 clinical samples obtained from 30 human tumors from the esophagus (12), stomach (10), colorectum (6) and lung (2), were tested by subrenal capsule assay (SRCA) using normal immuno-competent BDF, mice. The antitumor activity was evaluated by changes in both tumor size (delta TS method) and tumor growth inhibition rate (TGIR method). Among the 30 tumors, 15 were also tested by human tumor clonogenic assay (HTCA) and the results obtained in the two assays were compared. In the SRCA, adequate growth of the tumor in the control group for evaluation of the response of the treated group was obtained from 27 out of 30 tumors (90%). With activity criteria set at delta TS less than or equal to -1.0 dmm and TGIR greater than or equal to 50%, 41% of the drugs tested were active in delta TS while 27% were active in TGIR. When relationships between antitumor activities evaluated by delta TS and TGIR was compared, both activities were well correlated (r = -0.64). Correlations between tumor responses in the HTCA and in the SRCA were tested in 6 tumors treated with 20 drugs. The overall accuracy was 60% by the TGIR method and 70% by the delta TS method, respectively.

Endogenous production of tumor necrosis factor in normal mice and human cancer patients by interferons and other cytokines combined with biological response modifiers of bacterial origin.

The priming effect of endogenous biological response modifiers (BRMs), interferons (IFNs), and interleukin-2 (IL2), and the triggering effect of BRMs of bacterial origin, OK-432 and Corynebacterium parvum, on endogenous production of the tumor necrosis factor (TNF) were investigated in mice. TNF activity in serum was measured by in vitro cytotoxicity assay with L-929 cells as a target. The i.v. injection of OK-432 (3KE per mouse) triggered TNF maximally (mean value: 30 U/ml) after 2 h, with a similar time course to that of triggering by lipopolysaccharide. The priming activities of IFNs and IL2 were examined in the system of TNF-triggering by OK-432. The i.v. injection of recombinant IFN-gamma (rIFN-gamma, 10(4) U per mouse) increased TNF production to 790 U/ml; this priming effect was observed just after its injection, was maximal after 2 to 6 h, and disappeared after 24 h. Other types of interferon, rIFN-alpha A/D(Bgl) (2 X 10(5) U per mouse), rIFN-beta (10(6) U per mouse), and natural IFN-alpha/beta (10(6) U per mouse) showed maximal priming activity 6 h after their injection (200 to 800 U/ml) but no effect just after their injection. Recombinant IL2 (10(6) U per mouse) had priming activity that showed a similar time course to that of interferons other than IFN-gamma (a maximal TNF production: 100 U/ml). The i.v. injection of C. parvum, like OK-432, triggered TNF production at doses of 0.06 and 0.3 mg per mouse 2 h after its injection and the triggered TNF activity was enhanced by rIFN-gamma. These findings suggest that combinations of the above endogenous BRMs as priming agents and OK-432 or C. parvum as a triggering agent could induce endogenous production of TNF even in human cancer patients. In fact, combined administration of rIFN-gamma and OK-432 produced TNF in human cancer patients. The advantage of this method for treatment of human cancer patients is discussed.

[Islet cell carcinoma associated with concurrent Zollinger-Ellison syndrome and Cushing's syndrome].

A tissue specimen taken from the pancreatic tumor of a patient suffering from Zollinger-Ellison syndrome associated with a concurrent Cushing's syndrome was investigated both morphologically and immunohistochemically. Histologically, the tumor cells were found to be arranged in a cord or ribbon pattern. An immunohistochemical study revealed that the tumor nodule contained both gastrin and ACTH. Further, double stainings of the same section disclosed that the same tumor nodule possessed both hormones simultaneously, thus confirming the concurrent presence of Zollinger-Ellison syndrome and Cushing's syndrome. An electron micrograph of the tumor cell showed the presence of numerous endocrine-type granules in the cytoplasm. These findings substantiate the concurrent presence of the two characteristic clinical syndromes.

Immunohistochemical studies on the expression of pulmonary surfactant apoproteins in human lung carcinomas using monoclonal antibodies.

To reevaluate the expression of pulmonary surfactant apoproteins in lung carcinomas, cancerous tissues from 47 cases were immunohistochemically examined by the avidin-biotin peroxidase complex method using monoclonal antibodies. These antibodies recognized apoproteins in type II pneumocytes in normal and in hyperplastic alveoli and nonciliated columnal cells (Clara cells) in terminal bronchioles of noncancerous tissues. Cancer cells positive for surfactant apoproteins were observed in 11 out of 33 adenocarcinomas, including 1 case of bronchioloalveolar carcinoma. Interestingly, two cases of adenocarcinomas with ultrastructural features of Clara cells, which had metastatic lesions, were positively stained both in the primary and the metastatic sites. Two out of 4 adenosquamous carcinomas showed positive reaction, which was exclusively localized in the cells with papillary structure. The present study indicates that the expression of surfactant apoproteins in cancer cells occurs ubiquitously in adenocarcinomas, which are considered to be derived from the terminal air way.

1 alpha,25-Dihydroxyvitamin D3 induces anchorage-independent growth and c-Ki-ras expression of BALB/3T3 and NIH/3T3 cells.

1 alpha,25-Dihydroxyvitamin D3, a hormonally active form of vitamin D, induces anchorage-independent growth of BALB/3T3 A31-1-1 and NIH/3T3 cells with concomitant increase of their mRNA level of c-Ki-ras but not of c-Ha-ras or c-myc, through a receptor-mediated mechanism. Under the same conditions, 12-O-tetradecanoylphorbol-13-acetate did not induce anchorage-independent growth in these cell lines.

[Microangiopathic hemolytic anemia (MAHA) and renal failure induced by anti-neoplastic agents--a case report].

A 65-year-old man underwent left-upper lobectomy for large cell carcinoma of the lung on November 8, 1984 (pT1N0M0: Stage I a). He was treated with MMC, Futraful, CDDP and CPM as adjuvant chemotherapy. In April 1985, he was re-admitted to our hospital because of progressive dyspnea. He was diagnosed as having drug-induced interstitial pneumonia, and so steroid therapy was started. In July 1985, he suffered from anemia, thrombocytopenia, proteinuria and azotemia progressively, and died due to pulmonary hemorrhage and edema. At necropsy, no cancer recurrence was found. It thus seemed that the cause of death was microangiopathic hemolytic anemia and renal failure induced by anti-neoplastic agents.

Endogenous production of cytotoxic factor in mice induced by a combination of interferon-gamma and heterologous fibrinogen.

The ability of heterologous fibrinogen in combination with interferon (IFN)-gamma to induce endogenous production of cytotoxic factor was examined. Heterologous but not homologous fibrinogen induced high production of cytotoxic factor in IFN-gamma-primed mice. The cytotoxic activity was maximal 1 h after this triggering. The LD50 value of heterologous fibrinogen in mice was greater than 250 mg/kg i.v. But heterologous fibrinogen induced antibody, causing anaphylaxis. Therefore, the effect of successive injections of fibrinogens from a different species was tested. Cytotoxic factor could be produced repeatedly by successive treatments with a combination of IFN-gamma and heterologous fibrinogen from one species for 1 week, although the cytotoxic activity induced by successive injections gradually decreased. After the decrease of the triggering effect of heterologous fibrinogen of one species, heterologous fibrinogen from a different species could induce cytotoxic activity at the same level as that after the first triggering. Thus, a combination of IFN-gamma and heterologous fibrinogen is effective for cytotoxic factor production, provided different heterologous fibrinogens are used successively. This combination should be useful for endogenous cytotoxic factor production in clinical trials.
