Electrophysiologic study of patients with ventricular dysrhythmias during long-term follow-up after repair of tetralogy of Fallot.

We performed an electrophysiologic study (EPS) in 8 patients who had received corrective surgery for tetralogy of Fallot. The mean age was 30 years. An average of 15 years had elapsed after corrective surgery. Two patients had episodes of syncope. ECG showed normal sinus rhythm in 7 patients and atrial fibrillation in 1, and all had complete right bundle branch block. All patients had ventricular premature beats of grade 3 or higher of Lown's classification. Overdrive suppression test was performed in 6 patients. Corrected sinus node recovery time (CSNRT) ranged from 230 msec to 510 msec. Wenckebach block of atrioventricular nodal conduction occurred at rates of 130 to 170 bpm during atrial pacing. The H-V interval was prolonged to 60 msec in 1 patient, but was below 55 msec in the others. Programmed stimulation induced ventricular tachycardia (VT) in 3 patients, nonsustained VT in 2 and sustained VT in 1. In 2 of 3 patients, delayed potential or fragmentations were recorded in the outflow tract of the right ventricle. During the follow-up period of 20 months, 2 patients died suddenly. Their CSNRTs and H-V intervals were normal. Ventricular tachyarrhythmia seems to be important as a cause of late sudden death after repair of tetralogy of Fallot.

Plasma paracetamol concentrations measured by fluorescence polarization immunoassay and gastric emptying time.

Radionuclide labeled gastric emptying was compared with plasma paracetamol (acetaminophen) concentration measured by fluorescence polarization immunoassay. A significant correlation was found between both gastric emptying half-time of radionuclide (t1/2) and % isotope remained in the region of the stomach and paracetamol absorption kinetic parameters. The strongest correlation was found between t1/2 and the time required to reach the peak plasma level of paracetamol (r = 0.680, p less than 0.01). This study indicates that simple paracetamol marker method has enabled an indirect quantitative measurement of gastric emptying by sampling at varying time intervals.

[Interstitial pneumonia after CMC (CDDP, MMC, CPM) therapy].

Six patients receiving CDDP, MMC, and CPM chemotherapy for adjuvant chemotherapy after a resection due to lung cancer developed interstitial pneumonia. They were re-admitted for dyspnea, shortness of breath, and dry cough from 80 to 118 days from start of their treatment. On re-admission, their chest radiographs showed reticular infiltrates, and their laboratory data showed severe hypoxemia. The pathological findings of a transbronchial lung biopsy showed a thickening of the alveolar septa. Steroid therapy resulted in a complete resolution in one patient and a partial resolution the 5 others. One year later, two patients had died, one patient remains in complete resolution, but a shortness of breath still exists in the remaining three patients. Considering the disadvantages of that shortness of breath can cause to daily life, we should be more cautious about administering antineoplastic agents for adjuvant chemotherapy to patients with a cancer in an early stage.

[A study of 6-day subrenal capsule assay with cyclophosphamide pretreatment--histological evaluation of clinical samples].

In order to suppress the host immune reaction in subrenal capsule assay (SRCA), effect of cyclophosphamide (CPM) was evaluated histologically in 25 fresh clinical samples. In 21 out of 25 clinical samples, the extent of lymphocyte infiltration was minimal, and in all samples the host reaction was suppressed significantly by CPM pretreatment. However, tumor cells were well preserved in only 10 out of 20 samples obtained from esophageal cancer patients. With the samples obtained from gastric and colorectal cancer patients, there were no tumor cells in the subrenal space on day 6. Persistence of tumor cells was well recognized in the samples having rich tumor and less stromal cells. The xenografts with moderately to well-preserved cancer cells showed an increase of the tumor size as expressed by delta TS. Though many xenografts showed a positive delta TS, preservation of tumor cells was poor. In each of the drug treated groups, a decrease of the tumor size and the histological effects were well correlated. It is suggested that 6-day SRCA with CPM pretreatment is useful as a chemosensitivity test, especially for the esophageal cancer whose tumor cells were well preserved.

[Evaluation of therapeutic interventions of sustained monomorphic ventricular tachycardia guided by an electrophysiologic study: a case report].

Therapeutic evaluation of sustained monomorphic ventricular tachycardia (VT) using electrophysiologic study (EPS) is presented in a case of refractory VT. A 54-year-old man with a history of recurring syncope underwent coronary angiography which revealed total occlusion of the posterior descending branch of the right coronary artery. Left ventriculography showed a left ventricular aneurysm at the cardiac apex. Ejection fraction of the left ventricle was 36%. He had four VTs of different QRS morphologies in 12 lead electrocardiograms. According to our programmed ventricular stimulations, single or double, and rarely triple, extra stimuli were administered after eight basic stimuli at two basic cycle lengths. Rapid ventricular pacing, up to 210 bpm, was then added. The stimuli were delivered to two different sites in the right ventricle and to at least one site in the left ventricle. When the entire protocol could not induce VT, isoproterenol was given intravenously, and the same protocol was repeated. No drug could prevent VT attacks, even after the surgical resection of two VT foci, VT was still inducible. Postoperative drug therapy could not prevent VT induction in EPS. However, changes in the mode required for VT induction were observed. Among 47 patients with sustained monomorphic VT treated in our hospital, 24 had EPS to evaluate the efficacies of therapeutic interventions, such as drugs and surgery. In 14 patients, no VT was induced by the entire VT induction protocol. Among the remaining 10 patients, four showed changes in the VT induction mode, but VT recurred in their clinical courses even after their treatments.(ABSTRACT TRUNCATED AT 250 WORDS)

[Experimental study of the 6-day subrenal capsule assay by cyclophosphamide pretreatment].

For the purpose to suppress the host immune reaction in subrenal capsule assay (SRCA) using normal immunocompetent mice, the effects of cyclophosphamide (CPM) pre-treatment (CPM 180 mg/kg day-1 s.c.) were compared with cyclosporine A (CSA) treatment (CSA 60 mg/kg s.c. daily). CPM and CSA suppressed host reaction until day 6 and day 12, respectively. However, in the histological evaluation of day 6 tumor, there was no difference between two groups. Mouse serum levels of CPM were measured by a bioassay as indicated by the cytotoxicity against P388 cells, to evaluate its residual activity against implanted tumor. No cytotoxicity was observed in the serum of 36 hr. after CPM injection. The antitumor activities of several chemotherapeutic agents against serially transplanted human esophageal cancer xenograft (IMEs-1) in untreated control group and CPM pre-treated group of SRCA were compared with the subcutaneous transplantation assay in nude mice. In all agents, the drug activities in CPM pre-treated group were similar to that in nude mice assay system. The increase of toxicity in CPM pre-treated group was very light. The immunosuppressive activity of CPM were also tested in 14 clinical samples. CPM suppressed the host reaction satisfactorily in all samples. These results suggest the usefulness of CPM pretreatment for suppressing the host reaction in SRCA.

[A case of small squamous cell carcinoma with a grading of IIa + IIc, an early gastric cancer type].

A case of a 78-year old woman, complaining of epigastralgia, is reported. A series of gastrointestinal examinations revealed a small, elevated lesion with a central depression in the antrum. Since a fiberoptic biopsy had shown a Group V classification, a partial gastrectomy was performed. Tumor nodules, judged to be metastatic, were noticed in the liver and regional lymph nodes. Histologic scrutiny disclosed a keratinizing tumor that measured 1.0 X 0.9 cm in diameter and was largely a squamous cell carcinoma with a small focus indicating an adenocarcinoma. This tumor was confined to the muco-submucosal layers with prominent vascular permeation. No adenocarcinomatous components were found in the metastatic foci. The patient died eight months after operation. In light of our experience, together with what has been found in a review of the literature, we feel that a squamous carcinoma and an adenosquamous carcinoma of the stomach to be more aggressive than an ordinary adenocarcinoma and keeping this point in mind, they should be considered differentiated from an adenoacanthoma.

[99mTc-MAA perfusion studies in the treatment of preoperative intra-arterial infusion chemotherapy of malignant bone and soft tissue sarcoma].

Preoperative intra-arterial continuous chemotherapy combined with surgery in the treatment of malignant bone and soft tissue sarcomas has been used since 1968 in our clinic. This approach offers a theoretical advantage over systemic chemotherapy in that it delivers higher concentrations of chemotherapeutic agents to the tumor bed without increasing systemic toxicity. Tumor response depends on tumor sensitivity and the maximum amount of agent which is deliverable to the tumor tissues, so it is important to maintain a proper catheter placement to obtain good tumor response. 99mTc-Labeled macroaggregated albumin (99mTc-MAA) perfusion studies were carried out 19 patients who had histologically proven malignant bone and soft tissue sarcomas. The use of 99mTc-MAA perfusion studies in the treatment of intra-arterial chemotherapy offers an excellent way to evaluate catheter placement and tumor perfusion, in addition to providing a way to evaluate tumor A-Vshunts with lung scanning. The dynamic images of 99mTc-MAA obtained at each stage of the chemotherapy period also demonstrate the degree of tumor tissue response to intra-arterial chemotherapy. Therefore we conclude that 99mTc-MAA perfusion combined with preoperative intra-arterial infusion is a very useful method in cases involving limb salvage surgery.

Excess release of substance P from the spinal cord of mice during morphine withdrawal and involvement of the enhancement of presynaptic Ca2+ entry.

Possible presynaptic mechanisms involved in the naloxone-precipitated withdrawal jumping in morphine-dependent mice were studied. The withdrawal jumping precipitated by intrathecally (i.t.) administered naloxone (10 nmol) was abolished by the i.t. pretreatment with a substance P antagonist, [D-Pro4,D-Trp7,9,10]-substance P4-11 (10 nmol). The high KCl-evoked 45Ca2+ entry into synaptosomes and the release of immunoreactive substance P from slices were significantly enhanced by 10(-4) M naloxone in the morphine pelleted spinal cord, but not in the vehicle pelleted ones. These findings suggest that naloxone activates the depolarization-evoked Ca2+ entry into the nerve terminals, enhances the release of substance P from the spinal cord and the induction of withdrawal jumping in morphine-dependent mice then follows.

Presynaptic opioid kappa-receptor and regulation of the release of Met-enkephalin in the rat brainstem.

Opioid kappa-agonists had much more potent inhibitory effects on the high K+-evoked Met-enkephalin release from rat brain slices than did the mu- or delta-agonists. The opioid kappa- antagonist, MR2266 enhanced the evoked release of Met-enkephalin to a greater extent than did mu- or delta-antagonists in vitro and had a potent analgesia in mice in vivo. These findings suggest that the release of Met-enkephalin may be regulated in vitro and in vivo, mainly by presynaptic kappa-receptor-mediated mechanisms.

Di(2-ethylhexyl)phthalate enhances hepatic phospholipid synthesis in rats.

The effects of di(2-ethylhexyl)phthalate, a typical peroxisomal proliferator, on the activities of key enzymes in the glycerophospholipid synthetic pathway and the incorporation of lipid precursors into liver lipids in vitro were studied periodically in rats. When di(2-ethylhexyl)phthalate was fed at the 1% level to rats, glycerol-3-phosphate acyltransferase activity increased 2-3-fold in liver homogenates and microsomes in 2-4 days. The specific activity of microsomal CTP:phosphocholine cytidylyltransferase increased by 1.5-fold, whereas the cytosolic activity was depressed. The microsomal CDPcholine:diacylglycerol cholinephosphotransferase specific activity decreased, whereas the activity in the homogenates increased, suggesting the proliferation of the hepatic endoplasmic reticulum in di(2-ethylhexyl)phthalate-treated rats. The incorporation of [1(3)-3H]glycerol or [1-14C]acetate into liver phospholipids in vitro increased in 2 days and stayed at a high level up to 12 days. The present study confirmed that di(2-ethylhexyl)phthalate induced an enhancement of phospholipid synthesis in the liver. The increase in hepatic phospholipid synthesis by this drug is presumably linked to the proliferation of peroxisomes and other intracellular membranes.
