[Limb-saving surgery of malignant bone tumors using massive bone grafts].

The artificial materials in limb-saving surgery pose problems such as the age of patient, tolerance and stability. Thus, massive bone grafting should be reconsidered. In our clinic, out of 78 patients having malignant bone tumors, 29 received limb-saving surgery, and 10 of these cases received massive bone grafting using autografts and/or frozen allografts between 1968 and 1987. There were 7 cases with osteosarcoma, 1 case with mesenchymal chondrosarcoma, 1 case with leiomyosarcoma and 1 case with malignant fibrous histiocytoma. There were 5 males and 5 females ranging in age from 11 years to 44 years, and averaging 21 years. According to Enneking's staging system, these were all II-B cases. The median follow-up period was 71 mos, ranging from 13 to 239 mos. According to the modified Amago evaluation method, 80% of these cases had an excellent prognosis, one had a recurrence, but in all cases good bone union and stability were achieved without immunological reaction. Therefore we conclude that massive bone grafts in cases of malignant bone tumor surgery is a very beneficial method.

[A case of cardiac myxoma associated with congenital oculocutaneous albinism].

This case is a 43 year-old-woman of left atrial myxoma associated with oculocutaneous albinism, who was admitted to our hospital because of nocturnal cough everyday. Electrocardiogram showed normal sinus rhythm and complete right bundle branch block. Chest X-ray disclosed slight enlargement of left atrium. Echocardiography revealed the left atrial pedunculated tumor, arising from atrial septum, prolapsing into left ventricle during diastole, which was coincided with lucent tumor by levophase of pulmonary arteriography. Feeder artery of this tumor was visualized by right coronary arteriography. Cardiac catheterization represented normal pressure of right-sided heart. Thus the diagnosis of left atrial myxoma was established. At the operation, radical excision of the base of tumor and septum and patch closure was undergone. The gross morphology and microscopic findings were consistent with myxoma.

[The relationship between N-isopropyl-p-[123I]iodoamphetamine accumulation in the brain and its dynamics in other organs].

Single photon emission computed tomography of the brain and dynamic scintigraphy of the lung and liver with N-isopropyl-p-[123I]iodoamphetamine (IMP) were performed in 21 patients with and without cerebrovascular diseases to determine the relationship between IMP accumulation in the brain and its dynamics in other organs. The D/E ratios of the lung, liver and brain were calculated dividing the counts of the delayed image by that of the early image. The brain D/E ratio in the normal hemisphere was well correlated with the lung D/E ratio. So cases which showed more rapid lung washout tended to washout faster from the brain. We thought that this fact was affected by the systemic circulation. And the values of the brain D/E ratio of the cerebrovascular disease group were larger than that of the non-cerebrovascular disease group. This suggest the frequent existence of arteriosclerotic changes in patients of the cerebrovascular disease. Because the local brain washout was depended on the amount of the cerebral blood flow there. On the other hand, the lung D/E ratio was inversely correlated with the liver D/E ratio. In other words, more washout from the lung caused more accumulation of IMP in the liver. Also the brain D/E ratio was inversely correlated with the liver D/E ratio. In conclusion, there was a close relationship between IMP dynamics in the brain and that in other organs.

[Clinical evaluation of SPECT in cerebrovascular disease].

In 131 patients with cerebrovascular disease, regional cerebral blood flow was determined by 123I-IMP (N-isopropyl (123I)-iodoamphetamine) or 99mTc-HM-PAO (99mTc (d, l)-hexamethyl propyleneamine oxime) SPECT and findings were compared with those of X-CT or MRI. The perfusion deficit detected by SPECT was larger than the deficit by X-CT or MRI in every cases. The perfusion deficit area was more clearly demonstrated by SPECT than by X-CT or MRI in patients with acute cerebral infarction. The hypoperfusion area determined by 123I-IMP SPECT was wider than that by 99mTc-HM-PAO SPECT. The crossed cerebellar diaschisis was observed in 56 out of 131 cases (43%). The results of operation were quantitatively evaluated by 123I-IMP SPECT in 25 patients.

[Diagnosis of peripheral-type lung cancer using a new type of endoscope].

A new device, the BF-2.2T, has been designed to go through the 2.6 mm channel of the conventional fiberoptic bronchoscope (Olympus BF-1T20). It measures 2.2 mm in outer diameter; it has a visual angle of 75 degrees, a focal depth of 3 to 50 mm, a working length of 1150 mm, and a total length of 1400 mm. It bends 120 degrees in an upward direction and 120 degrees in a downward direction. The tip, 2.2 mm phi, of the BF-2.2 T angulates like a conventional bronchofiberscope, and this is the main characteristic of this instrument. The BF-2.2 T yields clinically satisfactory observation and photography, as a method for determining morphological changes in peripheral type lung cancer.

[A case of retroperitoneal schwannoma].

A 39-year-old man, who had been pointed out to have a lower abdominal mass, was admitted to our hospital. Physical examination revealed a child-head-sized mass with smooth surface in the middle part of the lower abdomen. Computerized tomographic scan showed the non-cystic mass located in the retroperitoneal space from the level of L5 to S5. An open biopsy was performed and pathological diagnosis was benign schwannoma (Antoni A and B type). Radical operation could not be performed because of his refusal. In addition to this case, 40 cases of retroperitoneal schwannoma reported in Japan are reviewed and discussed.

[Peculiar computed tomographic images after intracranial use of microfibrillar collagen hemostat: report of three cases].

After the intracranial application of microfibrillar collagen hemostat (Avitene), CT revealed contrast enhancement suggestive of brain abscess or residual or recurrent tumor in three patients. The first case was a 22-year-old male with left ventricular oligodendroglioma. At operation Avitene was applied to the ventricular wall and incised frontal lobe. Two months after the operation CT revealed ring enhancement at the left frontal lobe and ventricular enlargement. Craniotomy was performed again. Histological examination, however, revealed no recurrent tumor but residual Avitene and granulation. The second case was a 57-year-old female with left parietal glioblastoma multiforme. At operation Avitene was applied to the tumor cavity. Two months after the operation CT showed ring enhancement and craniotomy was performed again. Histological examination revealed residual Avitene and necrotic tissue. Five months after the first operation, craniotomy was performed for the third time, because recurrence was suspected after a CT scan. Histological examination revealed recurrent tumor and residual Avitene. The third case was a 19-year-old male with left frontal astrocytoma. Two months after the operation when Avitene was used for hemostasis, marked ring enhancement was observed. Five months after the operation, however, enhancement in CT was weak. The CT findings in these patients were characterized by the abnormally long duration of enhancement (five months) and by enhancement more marked two to three months after the operation than immediately after it. Based on these findings brain abscess or residual or recurrent tumor rather than normal healing was more suspect as the cause of this phenomenon. Brain abscess was ruled out, because the peripheral leukocyte count, erythrocyte sedimentation rate, CRP, etc. were normal.(ABSTRACT TRUNCATED AT 250 WORDS)

[Metallothionein as a resistance factor for antitumor drugs].

Metallothionein (MT), a metal binding protein induced by bismuth and other heavy metals, has been proved to have a potential to prevent toxic side effects of several antitumor drugs. It has also been demonstrated that the induction of MT in tumor tissues diminishes the antitumor activity of the drugs as well. These facts suggest a possibility that MT may play an important role in acquiring multi-drug-resistance in tumor cells. Thus, we were encouraged to examine the sensitivity of cultured cells overexpressing MT to various antitumor drugs which differ from each other in the mechanism of action. Cadmium-resistant cells (HeLa-R) were obtained from HeLa S3 cells (HeLa-S) by successive cultivation in a medium containing 100 microM CdCl2. MT level in HeLa-R was 180 times higher than that of HeLa-S. Comparison of the sensitivity of these two strains to the antitumor drugs, such as, cis-diamminedichloroplatinum (cis-DDP), adriamycin (ADR), peplomycin (PEP) and melphalan (MEL), revealed that HeLa-R was significantly more resistant to these drugs than HeLa-S. Further, there was no significant discrepancy between these strains, either in the level of major radical scavenging factors other than MT or in the cellular uptake of the drugs. The experimental results described above appear to support the hypothesis that MT may act as a multi-drug-resistance factor in tumor tissues.

[Clinical evaluation of indium-111-labeled leukocyte imaging in bone infection].

In-111-oxine-labeled leukocyte imaging was performed on twenty-one patients suspected of having bone infection. Nine of eleven cases (82%) were diagnosed as having active infection as demonstrated by abnormal accumulation of In-111-labeled leukocytes at the site of infection. There are two false negative (18%) cases. Two cases without active infection showed abnormal uptake. Four cases revealed cold defects on the scintigraphy. Marked uptake of radiotracer was noted not only in the case of acute osteomyelitis with acute septicaemia but also in the case with persistent chronic active osteomyelitis. It was observed that for precise evaluation of the test results it was equally important to compare the imaging findings with physical signs and laboratory investigations. It is concluded that In-111-oxine-labeled leukocyte imaging is a useful tool for the evaluation of the progression of bone infection.

[Therapeutic embolization of renal angiomyolipoma: a case report].

A case of bilateral renal angiomyolipoma without tuberous sclerosis is reported. A 49-year-old woman was admitted to the general practitioner with a sudden onset of severe left flank pain. An excretory urogram and ultrasonogram revealed an enlargement of the left kidney. She was subsequently referred to our clinic for further investigation and treatment. Computed tomographic scan and magnetic resonance imaging using Tl-weighted image showed several tumors with a fatty, dense area in the bilateral kidney. An arteriogram demonstrated a hypervascular renal mass with aneurysms in her left kidney. Diagnosis of bilateral renal angiomyolipoma was confirmed by percutaneous needle biopsy. Superselective embolization of the tumor was successfully performed, preserving normal renal tissue. Gelatin sponges containing Carboquone (CQ sponge) were used as embolic material. Angiomyolipoma has become relatively easy to diagnose by CT, ultrasound, MRI and so on. However, there are some cases of angiomyolipoma which are indistinguishable from renal cell carcinoma using these modes of testing. Therefore, in selecting a conservative management, we indicated that percutaneous biopsy or open biopsy should be done to confirm the results of the above procedures. Moreover, therapeutic embolization for angiomyolipoma was concluded to be very useful.

Involvement of cardiac metallothionein in prevention of adriamycin induced lipid peroxidation in the heart.

The effect of pretreatment of mice with heavy metals: Zn, Cu, Bi, Co, Cd or Hg, against lethal and cardiac toxicity of adriamycin (ADR) was investigated. The lethal toxicity of ADR was significantly reduced by pre-administration of these metals except for Cu. The levels of both malondialdehyde and conjugated dienes, determined as indicators of lipid peroxidation in the heart, markedly increased with ADR administration, but the increases were significantly prevented by the pre-administration of Zn, Bi or Cd. The survival rate of mice after ADR injection was significantly correlated with metallothionein (MT) levels in the hearts which was increased by the injection of each metal, but was not correlated with those in the liver and kidneys. A significant negative correlation between the cardiac MT concentrations and the levels of lipid peroxidation in the hearts was also observed. These results suggest that increased MT synthesis in the heart may be involved in the protective effect of the heavy metals tested against the lethal and cardiac toxicities of ADR.

Calcitonin gene-related peptide modulates calcium mobilization in synaptosomes of rat spinal dorsal horn.

Effects of synthetic human calcitonin gene-related peptide (hCGRP) on Ca2+ mobilization in crude synaptosomes prepared from rat spinal dorsal horn were studied using 45Ca2+ as a tracer. hCGRP promoted the influx of 45Ca2+ into synaptosomes of the spinal dorsal horn, in a dose dependent manner (at dose ranges from 10(-9) to 10(-6) M). The maximum effect of hCGRP on 45Ca2+ influx was achieved 5 min after addition of the peptide and was significantly inhibited by nifedipine. hCGRP did not alter the influx of 45Ca2+ into synaptosomes of the rat cerebral cortex, a brain area with reportedly few CGRP binding sites. In measurements of the 45Ca2+ efflux from synaptosomes of the spinal dorsal horn, 10(-7) M of hCGRP had no effect on the efflux of 45Ca2+ from the resting synaptosomes, but there was significant inhibitory effect on synaptosomes depolarized by 40 mM K+. The inhibitory effect of hCGRP on the efflux of 45Ca2+ was completely abolished by lowering the incubation temperature of the medium to 0 degrees C. The inhibitory effect of hCGRP on the efflux of 45Ca2+ was not observed in synaptosomes of the cerebral cortex. These findings suggest that hCGRP increases the levels of Ca2+ level in synaptosomes of the spinal dorsal horn, through interactions with its own receptors and the release of neurotransmitters from the nerve terminals is thus enhanced.

Modality-specific antinociception produced by intrathecal injection of anti-somatostatin antiserum in rats.

An intrathecal injection of antiserum against somatostatin significantly inhibited the nociceptive response to thermal but not mechanical stimulation in non-arthritic and adjuvant arthritic rats. The thermal antinociception induced by intrathecally injected anti-somatostatin antiserum was more remarkable in adjuvant arthritic rats than in non-arthritic ones. These results suggest that endogenous somatostatin participates in transmission of thermal nociception in the rat spinal dorsal horn in non-arthritic and arthritic conditions.

Different susceptibilities of long-term potentiations in CA3 and CA1 regions of guinea pig hippocampal slices to nootropic drugs.

Effects of the nootropic drugs, piracetam and bifemelane, on long-term potentiation (LTP) of population spikes in the CA3 and CA1 regions of guinea pig hippocampal slices were investigated. Piracetam (10(-6) to 10(-4) M) and bifemelane (10(-8) to 10(-6) M) significantly augmented the LTP in the CA3 region. The effects of these drugs were inhibited by scopolamine (10(-6) M). However, the LTP in the CA1 region was not affected by piracetam and bifemelane even at highly effective concentrations (10(-5) and 10(-6) M, respectively). Thus, the LTP in the CA3 is more susceptible to nootropic drugs than in the CA1.
