[Whole layer needle biopsy for evaluation of neoadjuvant therapy to invasive bladder cancer].

The most important information for treatment of bladder cancer is to know its exact staging. A whole layer needle biopsy technique has been developed for this purpose. Recently, neoadjuvant therapy has been used for invasive bladder cancer. Although down staging of bladder cancer after neoadjuvant therapy are evaluated by CT or ultrasound, these imaging are not reliable. We examined 11 invasive bladder cancer patients by whole layer needle biopsy pre and post neoadjuvant therapy. All cases were pT3-4 by pretreatment biopsy. After neoadjuvant therapy 4 were changed to pT0 by needle biopsy, other cases were no change or minimal change. In 4 changed to pT0, 3 were done total or partial cyctectomy and the results of pathological diagnosis of cystectomized specimens were also pT0. Another one case changed to pT0 is selected as candidate for bladder sparing and the patient is now in close surveillance. All 4 cases changed to pT0 were done combined treatment by chemo (internal iliac artery infusion) and radiotherapy. In remaining 7 cases, pathological staging of surgical or autopsy specimen agreed with that of whole layer needle biopsy. Whole layer needle biopsy showed no severe complication, except minor bleeding from the bladder wall. Tumor seeding into the needle tract was not observed up to present time. In conclusion, this biopsy system is useful for evaluation of the change of stage with chemotherapy and/or radiation therapy. By using this technique we can select the cases for candidate of bladder sparing.

[Evaluation of bone metastases from renal cell carcinoma].

Twenty-nine cases of bone metastases from renal cell carcinoma were examined. Eight had metastatic bone pain as the initial symptom and were diagnosed that the primary lesion was in a kidney. In eight cases bone metastases appeared after treatment of the primary site. Seven had only bone metastases and another 22 cases had multiple metastases in organs such as the lung and lymph node when the bone metastasis was found. Curable surgical treatment was performed in only 2 cases. The survival curve of these patients were: 1 year; 41 per cent, 2 year; 30 per cent and 3 year; 15 per cent. Bone scan used for detection of bone metastases of carcinoma frequently ends with false positive results. CT scan and angiography are available for differential diagnosis of bone metastasis. We examined 6 cases (9 lesions) of bone metastases from renal cell carcinoma (3 pelvic bones, 2 lumbar bones, 2 femurs and 2 humerus). All lesions were hypervascular by angiography and were easily diagnosed as bone metastases. For early detection of bone metastases from renal cell carcinoma, angiography is useful because hypervascularity and tumor stain are easily detected even in such small lesions as 2 cm. Angiography was also useful for chemoembolization.

Studies on beta-lactam antibiotics. III. Syntheses and antibacterial activities of new 3-(1,3-dithiolan-2-yl)cephalosporins, YM-22508, YM-16457 and their prodrug-type esters.

The syntheses and biological activities of new 7-beta-[(Z)-2-(2-amino-4-thiazolyl)-2-hydroxy-iminoacetamido]-3-(1 ,3- dithiolan-2-yl)-3-cephem-4-carboxylic acid (YM-22508, 1a), 7-beta-[(Z)-2-(2-amino-4-thiazolyl)-2- methoxyiminoacetamido]-3-(1,3-dithiolan-2-yl)-3-cephem-4-carboxyli c acid (YM-16457, 1d) and their prodrug-type esters are described. Among them, YM-22561 (1c), the 1-acetoxyethyl ester of 1a, showed good in vivo efficacy in mice against infections of Staphylococcus aureus Smith, Streptococcus pyogenes S 23 and Escherichia coli NY-17 and a long plasma T1/2 in mice.

Optimization of cell culture conditions for production of biologically active proteins.

We investigated the basic technology of cell culture conditions for production of useful substances such as cytokines, and related proteins produced by Namalwa cells. Namalwa cells (Klein, 1972), human B lymphoblastoid cells, were used for large scale production of alpha-interferon (Klein, 1979). Namalwa KJM-1, a subline of Namalwa cells, adapted to serum- and albumin-free medium, can grow at a high density above 1 x 10(7) cells/ml in suspension mode by the use of a perfusion culture system, Biofermenter, containing a cone-type cell-sedimentation column as cell separator (Sato, 1983). Several kinds of cytokine cDNA can be introduced and expressed in Namalwa KJM-1 cells (Miyaji, 1990a,b,c). Some of these were produced in large quantities by use of a gene amplification method with dhfr (Miyaji, 1990c), even though the Namalwa KJM-1 cells contained endogenous dhfr genes. For stable production of the target protein, Namalwa KJM-1 cells are very useful host cells, because they have no effective endogenous protease activity in the conditioned medium. Using Biofermenter with micro-silicone fibers and a dialysis system, the specific productivity of the target proteins was not depressed at a high cell density.

Effect of NGF-2 on the survival of chick sensory ganglion neurons in vitro.

The biological activity of NGF-2, the third member of the NGF family, was investigated. Conditioned medium of COS cells transfected with expression plasmid for human NGF-2 cDNA promoted the survival of sensory neurons from dorsal root ganglia (DRG), and nodose ganglia (NG). Supernatant of mock transfected COS cells was less effective for the survival of DRG neurons and ineffective for the survival of NG neurons. These results suggest that NGF-2 is a novel neurotrophic factor whose biological activity is distinct from NGF.

Properties of a cell line (MEG-01SSF) of human megakaryoblastic leukemia cells.

MEG-01SSF cells were prepared from MEG-01S by conditioning in a chemically defined, serum-free medium. The cells grew well in the medium and required transferrin for growth. Doubling time was about 20 hours. The cells retained GpIIb/IIIa as a marker of megakaryocytes and transferrin receptors on the cell surface. Some of the cells in the stationary phase of growth produced platelet-like particles, which bore characteristic microtubule rings.

Human papillomavirus type 31 DNA detected in part of the dysplasia but in no part of the squamous metaplasia in a specimen taken from one patient.

Using in situ hybridization, human papillomavirus (HPV 6, 16, 18, 31, 33) DNAs were detected in a cervical severe dysplasia accompanied by squamous metaplasia. It was found that, only HPV 31 DNA was harbored in the cervical severe dysplasia, but HPV DNAs were not identified in a lesion of squamous metaplasia. The in situ hybridization method will be of use, therefore, when dysplasia with squamous metaplasia or other lesions are examined for HPV DNA. In a cervical smear, HPV 31 DNA could be detected on the nuclei of dysplastic cells, so this method is applicable to cervical smears. If squamous metaplasia is to be considered as a precursor lesion to cervical dysplasia, the HPV DNA harbored in the dysplasia must also be detected in the accompanying squamous metaplasia. Our results suggested that not all squamous metaplasias were involved with HPV, as far as we were able to detect using five types of HPV DNA probe.

Multiple opioid receptors and GTP-binding proteins.

We believed that GTP-binding protein (G-protein)-coupling receptor always transduces stimulatory signals to G-proteins. From our recent experiments using reconstitution techniques, however, it was revealed that some receptors transduce an inhibitory or no signal to G-proteins in specific tissues, despite some interaction between them. Here we discuss the molecular basis of mechanisms of such diverse modes of functional coupling between different subtypes of opioid receptors and G-proteins.

[Kyotorphin like substance in human cerebrospinal fluid of patients with persistent pain].

Kyotorphin is an analgesic neuropeptide isolated from the bovine brain in 1979. Further studies showed that kyotorphin produces an analgesia through an increased release of met-enkephalin in the brain and the spinal cord. We showed that it is also found in the human cerebrospinal fluid and the concentrations of kyotorphin in normal human CSF is 1.19 +/- 0.51 pmol.ml-1. We also found that it is lower in patients with persistent pain (0.24 +/- 0.04 pmol.ml-1). Above results suggest that kyotorphin acts as a putative neuromediator and/or an endogenous pain modulator in the human brain.

Histogenesis and organogenesis of the gonad in human embryos.

The histogenesis and organogenesis of the human gonad in 12 embryos and 6 fetuses of ovulational ages 5 to 18 weeks was investigated by histological and ultrastructural examination, including observation of almost complete serial Epon-embedded sections of entire gonads of 10 embryos. This investigation revealed that the main constituent cells of the gonads are derived from the mesonephros, and that the coelomic epithelium is not involved in the formation of the main component at any stage. With the migration of the primordial germ cells into the gonadal ridge, the coelomic epithelium becomes stratified to form a moderate protrusion of the gonad into the coelomic cavity and the coelomic epithelial cells develop into short pillars which form cord-like structures, the so-called primary sex cords. Shortly afterwards, concomitantly with the development into the subsequent prominent protrusion of the gonad into the coelomic cavity, cells emerging from the mesonephros are incorporated into the gonad to form 'primordial sex cords'. At this stage, a stratified, pile-like arrangement of coelomic epithelium flattens into monolaminar or oligolaminar structures. In the testis, the 'primordial sex cords' differentiate into seminiferous sex cords by elaborating a surrounding basal lamina. In the ovary, these 'primordial sex cords' become displaced towards the peripheral regions of the gonad by the enlargement of these cords, as well as by the formation of the interstitium, or so-called medulla, at the base of the ovary; they differentiate into 'folliculogenous sex cords' which give rise to follicular cells.

[The activity of etoposide (VP16) in combination chemotherapy against human bladder cancer cells in vitro].

The activity of Etoposide (VP16) in combination chemotherapy against four human transitional cell carcinoma cell lines of bladder (TCCaB) was determined by in vitro colony formation assay. Four anti-tumor agents (methotrexate: MTX, vinblastine: VBL, adriamycin: ADM, cisplatin: DDP) were used for combination chemotherapy with VP16. The ADM + VP16 combination exhibited a strong synergistic antitumor effect against the human TCCaBs compared with other combinations in this study. The combination chemotherapy of ADM + VP16 may be useful as a new chemotherapeutic regimen for advanced bladder cancer.

TNF-driven inflammation during mouse liver regeneration after partial hepatectomy and its role in growth regulation of liver.

To determine the role of TNF-driven inflammation in self-regulation of cell growth and differentiation, mouse liver regeneration after partial hepatectomy was examined for TNF-driven inflammation. Hepatectomy provoked priming state for TNF production in both whole body and liver on day 3 when the peak mitotic response occurred. Histochemical studies of liver also showed an inflammatory symptom; hepatocellular necrotic foci appeared by 6 hours after hepatectomy. TNF itself was secreted spontaneously in liver transiently on day 1 to 2 after hepatectomy just before the proliferation of hepatocytes. Dexamethasone reduced both TNF secretion and hepatocyte proliferation after hepatectomy. Recombinant murine TNF stimulated the in vitro proliferation of hepatocytes. These findings indicate that hepatectomy induces short-term secretion of TNF in liver and TNF-driven inflammation has an important role in liver regeneration, at least in part by the direct stimulation of hepatocyte proliferation.

[A case of malignant rheumatoid arthritis associated with obstructive sleep apnea due to mandibular lesions].

A 55 year old man with malignant rheumatoid arthritis (MRA) was admitted because of severe snoring during sleep and hypersomnolence in the day time. The patient developed an episode of respiratory distress and cardiac arrest, which responded rapidly to tracheal intubation and mechanical ventilation. He could be easily weaned from ventilator after 3 days. Polysomnography showed that the patient had an obstructive sleep apnea syndrome. Lateral view of cranial radiography revealed acquired micrognathia and destruction of the temporomandibular joints caused by RA, which induced obstructive sleep apnea. It is important to consider that obstructive sleep apnea can occur in RA patients showing snoring, hypersomnolence and disordered sleep behavior.

Successful control of hemobilia secondary to metastatic liver cancer with transcatheter arterial embolization.

A 54-yr-old man with multiple metastatic liver cancer was admitted to our hospital because of melena and intermittent colicky pain on the right upper abdomen. He underwent partial gastrectomy for gastric cancer 3 yr before the admission. Endoscopic examination of the duodenum on the 3rd hospital day showed active bleeding from the papilla of Vater, and diagnosis of hemobilia was done. Transcatheter arterial embolization controlled the bleeding successfully, and no rebleeding occurred until his death due to the advanced cancer 8 months later. We considered that the high vascularity of his metastatic tumor caused hemobilia. To the best of our knowledge, this is the first report of hemobilia by metastatic liver cancer treated successfully with transcatheter arterial embolization.

[A case of Schönlein-Henoch purpura with myocardial complications].

A rare case of Schönlein-Henoch purpura with myocardial complications was reported. A 64-year-old man was admitted to our hospital for further examination of hematuria, associated with arthralgia, proteinuria, and previous episodes of anasarca and orthopnea. The examinations revealed moderate renal dysfunction due to mesangial proliferative glomerulonephritis secondary to Schönlein-Henoch purpura. In addition, the patient proved to have cardiac dilatation and dysfunction of the left ventricle. The histological examination of the right ventricular subendocardium sampled by myocardial biopsy showed myocardial damage, suggesting the invasion of microvasculitis in Schönlein-Henoch purpura to the peripheral coronary vessels. These findings indicate that myocardial damage induced in the patient may be one of the complications caused by Schönlein-Henoch purpura.

[Non-secretory primary plasma cell leukemia with hyperammonemia].

A 78-year-old man was admitted because of lumbago and chest pain. A diagnosis of non-secretory primary plasma cell leukemia was made based on the laboratory findings and his history. However, the plaque-forming cells assay of bone marrow cells revealed secretion of monoclonal immunoglobulin from the myeloma cells. Hyperammonemia was detected in the serum. Although the patient was treated with 4 courses of combination chemotherapy (vincristine, adriamycin, cyclophosphamide, methylprednisolone), he died of respiratory failure five months after diagnosis. Autopsy showed widespread multiple myeloma and prominent infiltration of myeloma cell in the sinusoid of the liver. Recently, there have been a few reports which increased the plasma ammonia concentration with multiple myeloma. This report strongly suggested that liver infiltration of myeloma cell caused hyperammonemia.
