Chemical and chemotaxonomical studies on Dicranopteris species.

Clerodane glycosides and flavonoids in Dicranopteris pedata and three varieties of D. linearis were investigated. All the ferns contained a new glycoside, (6S,13S)-6-[6-O-acetyl-beta-D-glucopyranosyl-(1-->4)-alpha-L-rhamnopy - ranosyloxy]-13-[alpha-L-rhamnopyranosyl-(1-->4)-beta-D-fucopyra nosyloxy]- cleroda-3,14-diene, as a chemical marker of this group. Flavonoids were limited to flavonol 3-O-glycosides. The ferns and isolated flavonoids are as follows; D. pedata: afzelin, quercitrin. D. linearis var. brevis: afzelin, quercitrin. D. linearis var. tenuis: quercitrin, isoquercitrin. D. linearis var. sebastiana: astragarin, isoquercitrin, rutin, kaempferol 3-O-(4-O-p-coumaroyl-3-O-alpha-L-rhamnopyranosyl)-alpha-L-rhamn opy ranosyl- (1-->6)-beta-D-glucopyranoside.

New 1,4-benzodiazepin-2-one derivatives as gastrin/cholecystokinin-B antagonists.

A novel series of 1-aroylmethyl-1,3-dihydro-2H-1,4-benzodiazepin-2-one derivatives was prepared and evaluated for activity as gastrin/cholecystokinin (CCK)-B receptor antagonists. In vitro binding studies showed that some derivatives exhibited potent affinity for gastrin CCK-B receptor and high selectivity over peripheral CCK(CCK-A) receptor. Furthermore these compounds potently inhibited pentagastrin-induced gastric acid secretion upon intravenous administration in an in vivo model in rats. Structure-activity relationship studies of this series suggested that 1-[(R)-2,3-dihydro-1-(2,3-dihydro-1-(2-methylphenacyl)-2-oxo-5-phe nyl-1H-1,4-benzodiazepin-3-yl]-3-(3-methylphenyl)urea (35b, YM022) was the optimal compound with IC50 values of 0.17, 0.11 and 150 nM for gastrin, CCK-B and CCK-A receptors, respectively, and an ED50 value of 9.5 nmol/kg (i.v.) in rats. The absolute configuration of the precursor of YM022, an (R)-3-amino-1,3-dihydro-2H-1,4-benzodiazepin-2-one derivative ((R)-25), was determined by X-ray crystallographic analysis of its (S) mandelate. It would be expected that YM022, a potent and selective gastrin CCK-B receptor antagonist, inhibits gastric acid secretion without inducing gastrin-mediated side effects such as hypergastrinemia and hyperplasia of oxyntic mucosa.

[Effects of feed and mating age on reproductive performance in Syrian hamsters].

In order to utilize Syrian hamsters in selection experiments, we investigated desirable conditions of feed and mating age for reproduction in females. Hamsters were subdivided into two groups, raised on pellet feed for herbivores (ZC-2) and breeding (MB-1), respectively. Half of the hamsters in each group were switched to the other feed, respectively, after mating. They were mated at 8 or 12 weeks of age. In the group that has been fed on ZC-2 before mating and MB-1 after mating, there was significantly more females giving birth and weaning, and exhibited the largest litter size and litter weight at 3 weeks of age (P < 0.01). The hamsters of the groups mated at 12 weeks of age exhibited a higher percentage of females giving birth and litter size and litter weight at birth than those at 8 weeks of age, but that was no significant difference in litter size and litter weight at 3 weeks of age. We conclude that Syrian hamsters raised by fibrous feed before mating gave optimal reproductive performance.

Malignant hemangioendothelioma of the aorta.

We present a rare case of aortic malignant hemangioendothelioma with a mural growth pattern in which the endothelial nature was confirmed by immunohistochemical demonstration of Factor VIII-related antigen. Aortic sarcomas and malignant hemangioendotheliomas in the literature are reviewed.

Repression of fat-dependent intestinal apo A-IV mRNA abundance by medium chain triacylglycerols and proteins, and elevation by carbohydrates of fat-dependent apo A-IV transport in suckling rat pups.

Nutrients regulating intestinal apo A-IV synthesis and secretion in developing rats have not been clearly defined. We examined the effect of ingestion of fats, carbohydrates and proteins on the abundance of intestinal apo A-IV mRNA and the serum concentration of apo A-IV in 14-day-old suckling rat pups fasted overnight. In pups ingesting long-chain fatty acid-fat (soybean oil, triolein: LCT), there was a prompt elevation of the mRNA at 1.5 h after ingestion, although a graded dose of soybean oil did not result in a comparable elevation of the apo A-IV mRNA. In pups on MCT, but not trilaurin, there was a repression of the LCT-dependent elevation of the message. In pups on Intralipid (composed of soybean oil, lecithin and glycerol), mRNA was elevated at 6 h after ingestion. Administration of Intralipid with lactose, glucose, fructose and sucrose induced a rapid elevation of mRNA together with elevation of serum apo A-IV, although administration of casein, whey proteins and soybean proteins resulted in repression of the Intralipid-dependent mRNA elevation. The message correlated weakly to the serum apo A-IV and triacylglycerols and with no correlation to intestinal fat accumulation. These results suggest that metabolic events following the ingestion of milk components modulate intestinal apo A-IV expression in developing rats, possibly through mucosal fatty acid utilization.

[Combined immunotherapy using interferon-alpha, interleukin-2 and lymphokine-activated killer cells--improvement of quality of life in patients with advanced renal cell carcinoma].

The goal of any treatment strategy for cancer is to improve not only patient survival but also quality of that survival. Between March 1990 and February 1993, we treated 10 patients with advanced RCC (9 men and 1 woman) by combined immunotherapy using natural interferon-alpha (IFN-alpha), recombinant interleukin-2 (rIL-2) and lymphokine-activated killer (LAK) cells, and resulting the quality of life (QOL) issues examined. The ages of the patients ranged from 36 to 78 years (mean: 60.2) and the performance status (PS) ranged from 30 to 100% (mean: 77%). There were 8 lung, 3 bone, 2 brain and 1 neck and para-aortic lymph node metastases. We could evaluate 8 patients, 2 patients dropped out because of bone fracture and acute pneumonia. The protocol was as follows; 1 x 10(6) IU of rIL-2 as an intravenous infusion and 6 x 10(6) IU of IFN-alpha intramuscularly on days 1-7 and 15-21. In additions LAK cells obtained from the patients were given on days 14, 21, 28, and 35 intravenously. This protocol was repeated for more than three cycles (mean: 4.13 cycles) in each patient. The maintenance therapy on outpatient basis were performed in 4 patients after confirmation of the safety of the combined immunotherapy. This outpatient regimen was composed of 1 x 10(6) IU of rIL-2 intravenously, 6 x 10(6) IU of IFN-alpha intramuscularly on days, 1, 8, 15, 22, and 29, plus LAK cells on days 15 and 29. [Posttraumatic pseudoaneurysm of the middle meningeal artery: a case report].

An epidural hematoma associated with posttraumatic pseudoaneurysm of the middle meningeal artery is a very rare clinical entity. Only 31 such cases have been reported in the literature. A 54-year-old man was admitted with lethargy and right hemiparesis. A skull X-ray film revealed a linear fracture in the left parietal and temporal bone, extending to the base of the skull. A brain CT scan disclosed an acute epidural hematoma over the left cerebral hemisphere, contusional hematoma in the right frontal lobe and thalamus, and subarachnoid hemorrhage in the left Sylvian fissure. During an emergency craniotomy a large epidural hematoma was evacuated and external decompression was performed. Angiography, which was performed about one month after the head injury, revealed an aneurysm of the middle meningeal artery. The aneurysm was removed to avoid delayed rupture at the time of cranioplasty. Histological diagnosis was pseudoaneurysm. We discussed the pathogenesis of a pseudoaneurysm of the meningeal artery and the necessity to pay attention to the presence of a pseudoaneurysm when an active bleeding point is not observed during surgery of the epidural hematoma.

Induction of metallothionein synthesis in transplanted murine tumors by X irradiation.

Although recent studies have shown that radiation can induce metallothionein (MT) synthesis in normal tissues, the induction of tumor MT synthesis by irradiation has not been reported. We examined the accumulation of MT in the Meth-A tumor (mouse fibrosarcoma cells) transplanted into mice exposed to whole-body X irradiation. In the present study, the MT content in the tumor cells was increased by X irradiation in a dose-dependent manner. The MT level induced in the tumor cells by X irradiation was elevated not only after a single exposure but also after repeated exposures. Several studies have shown that MT is one of the important cellular factors in resistance to various anticancer drugs and ionizing radiation. Thus our results suggest that the radiation-induced MT in the tumor cells may have to be taken into consideration when designing protocols for radio- and chemotherapy.

Manganese chloride treatment does not protect against acute radiation injury of skin or crypt cells.

Metallothionein (MT), the synthesis of which can be induced by metalloelement administration, is a known radical scavenger. This study investigated the possible protective effect of MT against acute radiation injury. Manganese chloride (10 mg of manganese/kg) was administered intraperitoneally to male C3H/He mice 24 h prior to irradiation. The paw of each mouse was irradiated locally, and the acute skin reaction was scored daily and averaged. Acute radiation injury of the small intestine was studied using an LD50/8 assay and a gut microcolony assay after abdominal irradiation. An LD50/8 value represents the radiation dose required to kill 50% of animals within 8 days. The number of microcolonies per tissue section was counted 3.5 days after irradiation. The level of MT in the liver, skin and intestine was determined by a modified 203Hg-binding assay. Acute skin reaction was not prevented by manganese pre-administration. The LD50/8 values of manganese-pretreated and control mice were 19.4 and 18.4 Gy, respectively. However, the difference was not significant. The number of microcolonies was not significantly different for these two groups in the dose range of 13-19 Gy. The level of MT in the skin and intestine was not increased by administration of manganese, although a sixfold increase was observed in the liver. In conclusion, manganese chloride treatment of mice 24 h prior to irradiation did not significantly protect skin and small intestine against acute radiation injury, because such a treatment did not result in increased levels of MT in the skin and small intestine.

[A case of malignant lymphoma arising from chronic pyothorax; usefulness of magnetic resonance imaging (MRI) for diagnosis].

A 79-year-old man had a history of pneumothorax induced to treat pulmonary tuberculosis when he was 40 years old, and of chronic pyothorax when he was 60 years old. He was admitted to our hospital because of chest pain and swelling of the right lateral chest wall. Soft tissues of the chest wall and rib had been destroyed, and a fistula had formed at the skin. The diagnosis was confirmed by examination of a biopsy specimen from the chest wall tumor. The tumor was successfully treated by radiation therapy. 67Ga scintigraphy, CT, and MRI were useful for diagnosis and follow up. Pyothorax and the tumor could be distinguished only with MRI.

Resting asynchronous left ventricular contraction abnormality analyzed by a phase method in spastic angina pectoris.

UNLABELLED: Quantitative phase analysis of equilibrium ventriculography was performed to study the character of left ventricular (LV) wall motion abnormalities in patients with spastic angina pectoris, who may have clinically and electrocardiographically silent ischemia combined with myocardial stunning, during rest and hyperventilation stress testing. METHODS: Phase analysis of the left ventricle at rest was performed by equilibrium radionuclide ventriculography in 13 control subjects and 36 patients with spastic angina pectoris. First-pass methodology along with hyperventilation stress testing was performed to assess spasm occurrences. Phase analysis of equilibrium multigated blood-pool scintigrams was performed to evaluate LV asynchrony at rest. RESULTS: The mean s.d. of LV phase distribution in the patients with variant and vasospastic angina was greater than that in the healthy control subjects (11.28 +/- 1.79 and 10.02 +/- 1.57 degrees versus 6.16 +/- 1.07 degrees). In addition, the mean s.d. of LV phase distribution in the variant angina group was greater than that in the vasospastic angina group. Furthermore, a linear correlation was found between the s.d. of LV phase distribution at rest and the percent decrease in ejection fraction during hyperventilation stress. CONCLUSION: Asynchronous LV contraction without significant hypokinesis was detected at rest in spastic angina pectoris. The severity of this asynchronous contraction corresponded well with decreases in ejection fraction during hyperventilation stress testing. Thus, analysis of the s.d. of LV phase distribution at rest is expected to provide useful information regarding LV asynchrony in spastic angina pectoris.

Role of free p70 (Ku) subunit in posttranslational stabilization of newly synthesized p80 during DNA-dependent protein kinase assembly.

The Ku antigen (p70/p80 heterodimer) is the DNA binding component of a DNA-dependent serine/threonine kinase (DNA-PK), the catalytic activity of which is carried by a 350 kDa polypeptide (p350). In the present studies, the assembly of p70, p80, and p350 was investigated in human K562 (erythroleukemia) cells, and rabbit (RK13) or murine (L-929) cells infected with recombinant vaccinia viruses directing the synthesis of human p70 and p80. Pulse-chase analysis and density gradient centrifugation revealed a pool of free p70 subunits in K562 cells that dimerized within minutes with newly synthesized p80, whereas Ku became associated with newly synthesized p350 1 to 4 h after the onset of p70/p80 heterodimer assembly. A stable pool of free p80 subunits was not detected, and newly synthesized p80 was degraded rapidly (t1/2 < 1.5 h) unless it became incorporated into a p70/p80 dimer. The explanation for the absence of unassembled p80 subunits in K562 cells was investigated further by expressing human p70 and p80 individually or together in Ku-deficient RK13 or L-929 cells infected with recombinant vaccinia viruses p70-vacc and/or p80-vacc. As in uninfected K562 cells, the t1/2 of the free recombinant human p80 subunit expressed in RK13 cells was < 1.5 h unless it was "rescued" by dimerization with p70. The t1/2 of human p70 as well as p70/p80 heterodimers was > 16 h in RK13 cells.(ABSTRACT TRUNCATED AT 250 WORDS)

[Alginate gel beads for chemoembolization: initial report].

Alginate gel beads (AGB) were studied as a new material for chemoembolization therapy. AGB were quickly produced by dripping a sodium alginate solution through a cannula into a calcium chloride solution, with bead size controlled by cannular size. AGB containing adriamycin or contrast medium were produced from the corresponding mixture. Sixty percent of adriamycin was eluted from AGB within one hour and 90% within 24 hours, while 95% of iopamidol was eluted in 30 minutes. AGB released these contents slower than gelatin sponge particles. Four dogs underwent transcatheter hepatic arterial embolization with AGB. Normal daily activity was recorded until sacrifice on the fourteenth day. There was no hepatic infarction in those embolized with AGB of 0.5 to 1.0 mm in diameter. Partial liver infarction was found in one with compelled embolization and in two with fragile beads. These observations suggested that AGB constitute an instantly preparable embolic material with slow-release activity and flexible solidity.

[A case of cutaneous malignant melanoma metastatic to the choroid].

A case of metastasis of cutaneous malignant melanoma to the choroid was reported. The patient, a 61-year-old man, had undergone an amputation of his right first finger 3 years ago due to cutaneous malignant melanoma. Ophthalmoscopic examination revealed a whitish yellow elevated choroidal lesion and serous retinal detachment in the left eye. Fluorescein angiography demonstrated multiple pinpoint leaks in the early phase and progressive pooling of dye into the subretinal space in the late phase. In magnetic resonance imaging, the tumor showed a hyperintense image in both T1 and T2-weighted images. Autopsy was performed and histopathological examination of the eye showed that the tumor cells were polygonal in shape, and had round or oval nuclei, but the cytoplasms had little melanin pigment. Immunohistochemistry for S-100 and HMB-45 antibody showed positive staining in choroidal tumor and other metastatic lesions, indicating that they were metastatic tumors from cutaneous malignant melanoma (amelanotic melanoma cells).

Effect of lactic acid on water content and osmotic fragility of erythrocytes in vitro.

Effects of lactic acid in red blood cells on osmotic fragility and water content of erythrocytes after hyperthermia were investigated. The osmotic fragility of erythrocytes increased following one-hour incubation with the addition of lactic acid at both 37 degrees C and 42 degrees C and that also increased after heating in vitro at 42 degrees C compared with those incubated at 37 degrees C, whether the lactic acid was added or not. The water content increased with the addition of lactic acid after heating in vitro at 42 degrees C. A high concentration of lactic acid and hyperthermia seem to cause the increase of intracellular water and the decrease of osmotic resistance of the red blood cells.

Radiological changes of frontometaphyseal dysplasia in the neonate.

The radiological manifestation of frontometaphyseal dysplasia has been well elucidated in later childhood and adulthood, but it has not been thoroughly clarified in the neonate. Here we report the radiological features of a neonate with frontometaphyseal dysplasia. Most features, including twisted ribs, increased interpediculate distances of the lumbar spine, flared ilia, mildly flared and bowed long bones, and arachnodactyly with undermodeled short tubular bones, were reminiscent of those found in older children and adults. A previously undescribed abnormality was advanced ossification of the femoral and tibial epiphyses.

[Sarcoidosis with pulmonary cavitation and calcification].

A 38-year-old man was seen at another hospital because of bilateral reticulo-nodular shadows on a chest X-ray film at the time of a regular health check-up. Pulmonary tuberculosis was diagnosed, and the patient was treated with antituberculous chemotherapy. The shadow did not improve, and the patient was transferred to our hospital. Bacteriological studies for pyogenic bacteria, mycobacteria, and fungi were all negative. Serum levels of angiotensin-converting enzyme and lysozyme were abnormally high. Examination of a specimen obtained by transbronchial lung biopsy revealed noncaseating epithelioid-cell granuloma associated with severe hyalinization. Sarcoidosis was diagnosed. Chest CT showed multiple nodular opacities, some of which were calcified. Oral steroids were given because the lesion was progressing rapidly. Cavitary lesions appeared during tapering of steroid therapy. Bacteriological and fungal studies were negative and transbronchial lung biopsy showed noncaseating epithelioid-cell granuloma. Therefore, the cavitation within the granuloma with hyalinization may have developed because of steroid therapy, and the calcification may have been caused by postorganizing dystrophy.
