[A case report of alternating abducens hemiplegia with special reference to the supranuclear pathway to the facial nucleus].

A case of alternating abducens hemiplegia was reported. A 16-year-old girl developed alternating hemiplegia characterized by the left abducens nerve palsy and right hemiparesis. In addition, she had right supranuclear facial nerve palsy. A brain MRI showed left mid to lower pontine lesion and vertebral angiography revealed medullary venous malformation in the left pons. SEP with right posterior tibial nerve stimulation showed a delayed central conduction time, suggesting that the lesion involved left medial lemniscus. We previously reported a 39-year-old man who developed pure alternating abducens hemiplegia. He did not show supranuclear facial nerve palsy or SEP abnormality. These findings support the idea that the supranuclear facial nerve fiber leaves the pyramidal tract at the upper to middle pons and descends in the area of the pontine tegmentum around the medial lemniscus.

[Staging of pancreatic ductal adenocarcinoma using dynamic MR imaging].

Single breath-hold gradient echo images were obtained before and immediately after bolus intravenous administration of Gd-DTPA (dynamic MR imaging) in the study of the pancreas. Of 37 patients with pathologically proved pancreatic ductal adenocarcinoma, seventeen patients who underwent both dynamic MR imaging studies and curative surgery were included in this study. Correlations between histologic findings in the resected specimens and MR images were analyzed as to tumor extension and staging according to the General Rules for the Study of Pancreatic Cancer (4th Edition) published by the Japan Pancreas Society. In comparison with conventional MR images, dynamic MR imaging improved the detectability of pancreatic carcinoma and delineation of the vasculature by clarifying the margin of the tumor and the vessels. Nonenhanced T1-weighted imaging is the best sequence to estimate peripancreatic tumor extension, because the contrast between the tumor and peripancreatic fat deteriorates with the use of contrast material. There is a tendency to overestimate vascular invasion on MR images, the reason for which is considered to be the contractive nature of fibrotic change induced by pancreatic carcinoma. The diagnostic efficacy of lymph node metastasis remains insufficient on MR images because some cases show no enlargement of lymph nodes in spite of the existence of pathological metastasis. Our results suggest that dynamic MR imaging has the advantage of improving the conspicuity of the tumor and the vasculature.

Antiandrogenic phenolic constituents from Dalbergia cochinchinensis.

Four new compounds, 9-hydroxy-6,7-dimethoxydalbergiquinol, 6-hydroxy-2,7-dimethoxyneoflavene, 6,4'-dihydroxy-7-methoxyflavan and 2,2',5-trihydroxy-4-methoxybenzophenone, in addition to eight known phenolic compounds including 7-hydroxy-6-methoxyflavone, have been isolated from the stems of Dalbergia cochinchinensis. Their structures were established by spectroscopic techniques including one- and two-dimensional NMR methods. The first two compounds showed potent inhibitory activity towards 5 alpha-dihydrotestosterone (DHT) which binds with an androgen receptor to form a DHT-receptor complex that causes androgen-dependent disease.

A case of retroperitoneal abscess: an unusual complication of cholecystitis and choledocholithiasis.

A man, 73 years old, who developed acute biliary tract infection complicated with retroperitoneal abscess formation in iliopsoas region was reported. The magnetic resonance imaging was especially effective to delineate the lesion, extending from the subhepatic down to the iliopsoas muscle. The echo-guided fine needle aspiration technique was successful to make the lesion subside.

[Stereochemical structure of d-borneol in "the Japanese Standards of Food Additives"].

d-Borneol is shown at "The Japanese Standards of Food Additives" the sixth edition. Though the absolute stereochemistry of this compound is described as 1S, 2R-form, the opposite optical rotation for the same structure is described in other literatures. The application of improved Mosher's method to d-borneol resulted in 1R, 2S-form for its absolute stereochemistry.

Morphological subclasses of lateral olivocochlear terminals? Ultrastructural analysis of inner spiral bundle in cat and guinea pig.

The lateral olivocochlear efferent pathway terminates in vesicle-filled swellings in the inner spiral bundles under inner hair cells (IHCs) and has been suggested to include at least two chemically distinct subclasses (see, e.g., Vetter et al. [1991] Synapse 7:21-43). In the present study, the ultrastructure and peripheral targets of vesicle-filled swellings in the IHC area of the cat and guinea pig cochleas were quantitatively analyzed to determine 1) whether morphological subclasses could be defined based on swelling size or on the density, size or shape of clear and dense-cored vesicles and 2) whether swellings with different postsynaptic targets differed morphologically. In both cat and guinea pig, all swellings contained large, round, clear vesicles and a variable number of dense-core vesicles. Although evidence of clear-cut subclasses was not compelling, the smallest swellings tended to be rich in dense-core and poor in clear vesicles and rarely formed synaptic contacts. Most of the larger swellings, which tended to contain few dense-core vesicles and a rich complement of clear round vesicles, formed synapses with radial afferent fibers. However, there were no morphological differences between swellings contacting afferents originating on the modiolar vs. pillar sides of the IHC (the source of afferents with low and high spontaneous discharge rates, respectively). We conclude that 1) if distinct gamma-amino butyric acid (GABA)ergic and cholinergic subclasses of lateral olivocochlear (LOC) fibers exist, then the vesicle morphology of their terminals does not differ as it does in the central nervous system and that 2) if peptide neurotransmitters, such as calcitonin gene-related peptide and enkephalins, are packaged in dense-core vesicles, then the LOC terminals synapsing with IHC afferent fibers are not particularly rich in these peptides.

Intracranial metastases of hepatocellular carcinoma: CT and MRI.

We report clinical characteristics and CT and MRI in 16 patients with brain metastases due to hepatocellular carcinoma (HCC). Eight of these 16 patients presented with apoplexy-like symptoms (50%). Pulmonary metastases were found in 13 cases (81.3%). The mean survival from the appearance of cerebral metastases to death was 6.2 weeks, which is one of the shortest survival terms in metastatic brain tumours. Haemorrhagic brain metastases were observed in 14 patients (87.5%) with a tendency for the frequency of bleeding to increase in proportion to the size of the tumour. On both contrast-enhanced CT and MRI, metastatic brain tumours enhanced strongly, suggesting that brain metastases, like HCC, are also hypervascular. MRI is useful in evaluating brain metastases from HCC, especially in order to differentiate tumour from haemorrhage. Our results demonstrated a poor prognosis and bleeding tendency of brain metastases due to HCC and showed the usefulness of CT and MRI in achieving a correct diagnosis.

Connexin43 is another gap junction protein in the peripheral nervous system.

That many cells express more than one connexin (Cx) led us to examine whether Cxs other than Cx32 are expressed in the PNS. In addition to Cx32 mRNA, Cx43 and Cx26 mRNAs were detected in rat sciatic nerve by northern blot analysis. Cx43 mRNA, but not Cx26 mRNA, was expressed in both the primary Schwann cell culture and immortalized Schwann cell line (T93). The steady-state levels of the Cx43 mRNA in the primary Schwann cell culture increased 2.0-fold with 100 microM forskolin, whereas that of Po increased 7.0-fold. Immunoreactivity to Cx43 was detected on western blots of cultured Schwann cells, T93 cells, and sciatic nerves but not on blots of PNS myelin. Immunohistochemical study using human peripheral nerves revealed that anti-Cx43 antibody stained cytoplasm around nucleus of Schwann cells but not myelin, confirming western blot results. Although Po expression was markedly decreased by crush injury of the sciatic nerves, Cx43 expression showed no apparent change. Developmental profiles showed that Cx43 expression in the sciatic nerve increased rapidly after birth, peaked at about postnatal day 6, and then decreased gradually to a low level. In adult rats, the Cx43 mRNA value was much lower than that of Cx32. These findings suggest that Cx43 is localized in Schwann cell bodies and that, compared with Po, its expression is less influenced by axonal contact and cyclic AMP levels. The high expression on postnatal day 6 indicates that Cx43 may be related to PNS myelination. Cx43 is another gap junction, but its function appears to differ from that of Cx32, as judged by the differences in their localization and developmental profiles.

Runt domain partner proteins enhance DNA binding and transcriptional repression in cultured Drosophila cells.

BACKGROUND: The Drosophila gene runt plays multiple roles during embryogenesis, including one as a pair-rule class segmentation gene. The runt protein (Runt) contains an evolutionarily conserved domain (the Runt domain) that is found in several mammalian proteins including the human protein AML1, which is involved in many chromosome translocations associated with leukaemia. Specific DNA binding activity of a mammalian Runt domain is enhanced by a partner protein called PEBP2beta/CBFbeta. DNA binding activity of Drosophila Runt is also stimulated by this protein, suggesting the existence of a similar Runt partner protein in Drosophila. RESULTS: We report here the cloning of two closely linked Drosophila genes, runt domain partner (rp) beta1 and beta2, that encode homologues of mouse PEBP2beta/CBFbeta. They are highly homologous to each other and to the mammalian counterpart. Either of the rpb proteins is capable of forming a complex with Runt and stimulating its DNA binding activity, but their temporal and spatial distributions are quite dissimilar, suggesting that functional specificity of Runt may be conferred by the interacting partner. Runt represses transcription dominantly when coexpressed with either partner in cultured cells, a function consistent with a direct role for Runt in regulating expression of the even-skipped gene in Drosophila embryos. CONCLUSIONS: Drosophila Runt can interact with either of two Runt domain partners, and the resulting complex functions as an active repressor of transcription.

Comparison of commercially available kits with standard methods for the detection of coliforms and Escherichia coli in foods.

Three commercially available kits that were supplemented with substrates for enzyme reactions were evaluated to determine their abilities to detect coliforms and fecal coliforms in foods. Japanese and U.S. Food and Drug Administration standard methods, as well as two agar plate methods, were compared with the three commercial kits. A total of 50 food samples from various retailers were examined. The levels of detection of coliforms were high with the commercial kits (78 to 98%) compared with the levels of detection with the standard methods (80 to 83%) and the agar plate methods (56 to 83%). Among the kits tested, the Colilert kit had highest level of recovery of coliforms (98%), and the level of recovery of Escherichia coli as determined by beta-glucuronidase activity with the Colilert kit (83%) was comparable to the level of recovery obtained by the U.S. Food and Drug Administration method (87%). Isolation of E. coli on the basis of the beta-glucuronidase enzyme reaction was found to be good. Levine's eosine methylene blue agar, which has been widely used in various laboratories to isolate E. coli was compared with 4-methylumbelliferyl-beta-D-glucuronide (MUG)-supplemented agar for isolation of E. coli. Only 47% of the E. coli was detected when eosine methylene blue agar was used; however, when violet red bile (VRB)-MUG agar was used, the E. coli detection rate was twice as high. Of the 200 E. coli strains isolated, only 2 were found to be MUG negative, and the gene responsible for beta-glucuronidase activity (uidA gene) was detected by the PCR method in these 2 strains. Of the 90 false-positive strains isolated that exhibited various E. coli characteristic features, only 2 non-E.coli strains hydrolyzed MUG and produced fluorescent substrate in VRB-MUG agar. However, the PCR did not amplify uidA gene products in these VRB-MUG fluorescence-positive strains.

Comparison of a fluorogenic assay with a conventional method for rapid detection of Vibrio parahaemolyticus in seafoods.

A conventional method and a fluorogenic assay for the detection of Vibrio parahaemolyticus were compared. Among 29 seafood samples examined for the presence of V. parahaemolyticus, 17 samples harbored V. parahaemolyticus, and trypsinlike activity was noticed in 19 seafoods. The added fluorogenic substrate was cleaved in single samples of shrimp, turbo, and cuttlefish from which V. parahaemolyticus could not be isolated by the conventional method. Vibrio alginolyticus, in addition to V. parahaemolyticus, was found to exhibit intracellular trypsinlike activity. Trypsinlike activity in seafoods was observed after the most probable number for the initial density of V. parahaemolyticus-like organisms was found to have reached > 10(2) per g. A V. parahaemolyticus inoculum at 10(4) CFU/ml in arabinose-glucuronate medium was required to attain growth to 10(6) CFU/ml, which is the level necessary for the release of detectable amounts of fluorescent compound from the added substrate.

Antiinflammatory effect of Trichosanthes kirilowii Maxim, and its effective parts.

This study was carried out to elucidate the antiinflammatory effect of 50% ethanol extract obtained from the fruit of Trichosanthes kirilowii Maxm, and its effective parts. The cut dried fruit was separated into the whole fruit, seeds and remaining parts, and each part was extracted with 50% ethanol. The antiinflammatory activity of these extracts was investigated on acetic acid-induced vascular permeability in mice, carrageenin-induced edema and cotton pellet-induced granuloma formation in rats, as well as writhing symptoms in mice. The whole fruit and seed extractions showed an antiinflammatory activity and analgesic activity, but the other part did not when given perorally. These results suggest that the antiinflammatory effect and analgesic effect induced by ethanol extract of the fruit of T. kirilowii may be due to the seeds contained in the fruit.

Accelerative effect of "nanshikon" and its constituents on the proliferation of granulation tissue in rats.

The present study was carried out to examine the accelerative effect of the ether extract obtained from "Nanshikon" on the proliferation of granulation tissue induced by cotton pellet in rats and to elucidate its active principles. Among naphthoquinone derivatives, the ether extract contained mostly teracrylshikonin. At corresponding doses based on the contents of the naphthoquinone derivatives in the ether extract, the accelerative potencies of teracrylshikonin, beta, beta-dimethylacrylshikonin, and a mixture of alpha-methyl-n-butylshikonin and isovalerylshikonin were about the same. Also, the accelerative potencies of these compounds were somewhat weaker than that of the ether extract. From these results, it is suggested that the accelerative effect of the ether extract on the proliferation of granulation tissue is mainly due to teracrylshikonin, beta, beta-dimethylacrylshikonin and a mixture of alpha-methyl-n-butylshikonin and isovalerylshikonin contained in the ether extract, and the accelerative effect induced by ether extract might be an additive effect of these naphthoquinone derivatives.

Pharmacologically active components of a Peruvian medicinal plant, huanarpo (Jatropha cilliata).

From Jatropha cilliata M. Arg., a Peruvian medicinal plant, isoorientin and orientin were isolated as anxiolytic components by using the isolation guide of anti-conflict effect in mice. Several related flavonoids were also tested for this effect. Another bioassay-guided isolation using the acetic acid-induced writhing method gave a coumarin compound, fraxetin, as an analgesic principle.

[Shichimotsu-koka-to prevents stroke and changes free-radical-related enzymes in stroke-prone spontaneously hypertensive rats (SHRSP)].

Shichimotsu-koka-to, a Chinese herb, is a medicine prescribed to treat patients with hypertension. We investigated the effects of Shichimotsu-koka-to on the lesions of stroke-prone spontaneously hypertensive rats (SHRSP). The SHRSP were given 1% or 2% Shichimotsu-koka-to solution (1 g/kg/day or 2 g/kg/day) instead of drinking water from 8 to 42 weeks of age. When the 2 g/kg/day Shichimotsu-koka-to was chronically administered to the SHRSP, their life span was significantly prolonged by prevention of stroke, but there was no effect on blood pressure. The Shichimotsu-koka-to treatment decreased superoxide dismutase (SOD) activities in the cytosol of the cerebral cortex and increased SOD activities in the cytosol of the brain stem. The treatment with 2 g/kg/day Shichimotsu-koka-to decreased xanthine oxidase (XOD) activities in the cytosol of the cerebral cortex, and Shichimotsu-koka-to quenched superoxide anion (O2-) as determined by electron spin resonance analysis in vitro. In addition, SOD activities in the cytosol of the cerebral cortex of SHRSP not administered Shichimotsu-koka-to were increased by the drug in vitro. XOD activities in the cytosol of the cerebral cortex of SHRSP not administered Shichimotsu-koka-to were inhibited by this herb in vitro. These results suggest that these preventive effects of Shichimotsu-koka-to on the stroke in SHRSP are due its ability to scavenge O2- and to inhibit O2- production by the hypoxanthine-XOD system in the cytosol of the cerebral cortex.

Isolation and structure of ciguatoxin-4A, a new ciguatoxin precursor, from cultures of dinoflagellate Gambierdiscus toxicus and parrotfish Scarus gibbus.

A new ciguatoxin congener, ciguatoxin-4A (CTX4A), was isolated from cultures of marine dinoflagellate Gambierdiscus toxicus, and its structure was elucidated to be 52-epiciguatoxin-4B on the basis of spectroscopic data. Chromatographic and spectral comparisons indicated that CTX4A was identical with a structurally unelucidated congener known as scaritoxin or SG1.

Stereoscopic visualization system for clinical angiography.

Four dimensional visualization (FDV) project for clinical angiography has started at National Cancer-Center since 1995 in cooperation with medical virtual reality projects in our center. All stereo pair of x-ray angiographic images, both digital images and film based conventional x-ray images, are available in this system. FDV system software was developed on a Silicon Graphics platform. Every stereo pair angiographic images can be viewed stereoscopically using a stereoscopic visualization interface. Series of image data sets provide three dimensional blood flow dynamics that is very useful to diagnose vascular encasement by the tumor and to determine vessel of purpose which locates in front or in back from superimposed angiographic images. FDV system is expected to improve the accuracy and efficiency of diagnostic radiology and interventional radiology (IVR) procedures. In future, angiography system will equipped with a stereoscopic visualization interface that will provide a more flexible and more effective guidance tool for diagnostic radiology, especially for IVR that requires image-guided procedure.
