The use of high resolution ultrasound in preoperative evaluation of cutaneous malignant melanoma thickness.

High resolution ultrasound (HRUS) was tested in 58 cutaneous malignant melanomas to check its validity in evaluating tumor thickness in vivo before surgery. The values obtained with this method were compared with histologic values (measured according to Breslow); a highly significant correlation was found (r = 0.895, p less than 0.001). The accuracy of HRUS in distinguishing between low- and high-risk cutaneous malignant melanoma was also found to be quite high. Our data therefore justify the use of such a technique in the preoperative staging procedure.

New prognostic criterion in adenoid cystic carcinoma of salivary gland origin.

Adenoid cystic carcinoma (ACC) of salivary gland origin shows histologic patterns (tubular, cribriform, solid) that have been correlated with the clinical course of the disease. Recognition of tubular pattern is generally easy. Discrimination between cribriform and solid types leaves some margin for the pathologist's subjective interpretation because it is essentially based on the amount of gland-like spaces, containing mucinous or hyalinized material, which are present in neoplastic nests. To overcome this problem and with the aim of identifying a more objective basis for prognostic evaluation of cribriform and solid ACC, the authors counted the number of gland-like spaces per square millimeter of tumor, excluding supporting stroma and small areas with the tubular pattern, in a group of tumors similar with regard to therapy, stage of disease, and microscopic evidence of tumor-free surgical margins. According to the authors' data, the number of gland-like spaces per square millimeter of tumor appears to be a reliable and objective measure for prognostic evaluation of cribriform and solid pattern ACC; in particular, the greater the number of gland-like spaces per square millimeter of tumor, the longer the survival of the patient.

Primary cutaneous follicular centre-cell lymphoma--a lymphoproliferative disease with favourable prognosis.

In this study the clinico-pathological and immuno-histological features, the methods of treatment and follow-up data of 11 patients with follicular centre-cell (B-cell) lymphoma primarily presenting in the skin are reported. All the patients had nodular, tumorous and/or papulonodular skin lesions on the trunk. In nine patients the disease was confined to a circumscribed area of the back. Small papulonodular or plaque-like lesions, as well as large nodules or tumours, were biopsied in six of 11 patients. No clear-cut correlation between the age and clinical morphology of the lesions and their histological growth pattern was found. Interestingly, however, a different immuno-architectural pattern was observed in large, late lesions compared to small, early lesions. Initial treatment consisted of orthovolt radiotherapy (in two patients associated with surgical excision), resulting in complete remission in all patients. Only one patient developed extracutaneous disease, which was limited to a single drainage lymph node appearing simultaneously with a cutaneous relapse. Five other patients had recurrent disease in the skin close to the initial site. The median disease-free period was 15.5 months. On relapse, radiotherapy alone or in combination with short courses of chemotherapy was performed. This resulted in a second complete remission. All the patients are still alive and in complete remission, with a median survival of 37 months. These results confirm the favourable prognosis of patients affected with primary cutaneous follicular centre-cell lymphoma limited to the trunk. Orthovolt radiotherapy proved to be the most suitable treatment for both initial lesions and relapses limited to the skin.

Common acquired melanocytic nevi, melanocytic dysplasia and malignant melanoma. A morphometric study.

Nuclei of melanocytes in 10 common acquired melanocytic nevi, 10 malignant melanomas and 10 dysplastic melanocytic nevi (melanocytic dysplasia) (MD) were morphometrically analyzed. MD group consisted of 6 lesions with (MD 1) and 4 without (MD 2) the clinical features of dysplastic nevi. The considered parameters were area, perimeter and form Ar. In the MD group, the results showed that median values of each considered parameter of MD 1 were close to those of MD 2. For each evaluated parameter, median values of MD turned out to be intermediate between those of common acquired melanocytic nevi and of malignant melanomas. Confirming the histologic evidence of a double - nonatypical and atypical - melanocytic population in melanocytic dysplasia, our data showed that nonatypical melanocytes were indistinguishable from melanocytes of common acquired nevi, while the atypical melanocytes were close to melanoma cells. The importance of atypical melanocytes is stressed in the diagnosis of melanocytic dysplasia.

Multicystic peritoneal mesothelioma. A fine structure study with special reference to the spectrum of phenotypic differentiation exhibited by mesothelial cells.

Multicystic peritoneal mesothelioma evidenced a complete spectrum of morphodifferentiation extending from classic epithelial-like surface mesothelial cells to myoid-fibroblastoid mesenchymal-like subsurface mesothelial cells. The contemporaneous presence of both epithelial- and mesenchymal-type submicroscopic features in individual cells strongly supported the concept of a holistic histogenetic/differentiative nature of surface and subsurface mesothelial cells on the one hand, and of a single origin for all serosal neoplasms, apart from their actual cytodifferentiation features, on the other. In addition, a profusion of smooth-membrane-bound intracellular collagen was found in myoid-fibroblastoid cells; this finding was interpreted to be, at least in part, due to tangential sectioning of convoluted cell surfaces enfolding extracellular collagen fibrils or to cell processes wrapping around them. A certain finalism might be seen in this event, especially considering the shorter-than-normal period length we found in collagen fibrils; the myoid-fibroblastoid cells might be hypothesized to play a contractile role in shortening collagen fibrils in a process of stroma contraction similar to wound repair or hypertrophic scarring.

Multiple marker studies on a malignant fibrous histiocytoma with primary cutaneous localization.

Continuing controversy exists concerning a possible relation between neoplastic cells of malignant fibrous histiocytoma (MFH) and the mononuclear phagocyte system. The aim of this study was to investigate the membrane and cytoenzymatic phenotype of a primary cutaneous MFH, storiform pleomorphic type, and to compare these data with ultrastructural observations. Cytoplasmic proteins (acid phosphatase, non specific esterase, alpha-1 antitrypsin, and lysozyme) suggestive of a mononuclear phagocyte origin were demonstrated in varying amounts in neoplastic cells infiltrating the dermis. Consistent with these data, two (LeuM3 and OKM5) out of four (OKM1 and LeuM1) monoclonal antibodies directed against mononuclear phagocyte antigens stained most of the neoplastic cells. Class II MCH antigens (DR and DQ) were variably expressed on distinct groups of neoplastic cells, suggesting different activation/differentiation states. The results favor the view that the present case of primary cutaneous MFH was of mononuclear phagocyte origin. However, the observed phenotypic profile was expressed on neoplastic cells irrespective of their ultrastructural morphology (histiocytic or fibroblastic). Together with previous data in the literature, the latter finding corroborates the view that distinction between these two cell types in MFH is likely to reflect divergent growth and differentiation patterns rather than histogenesis.

Giant cell lesion of the jaw. Case report.

A case is presented of a male patient affected by a giant cell lesion of the jaw, which had two recurrences in 8 years. Histologically, the lesion appeared to be composed of giant cells and mononuclear cells. Histoenzymatic study demonstrated acid phosphatase in both types of cells, and beta-glucuronidase in giant cells only. In some nuclei of giant cells, ultrastructural investigation showed filaments or microtubular structures of variable length, with irregular transverse periodicity, in addition to other expected findings. These characteristic features, found in giant cells of some giant cell tumors of the long bones, have never before been reported in a giant cell lesion of the jaw. The results are considered in order to assess the diagnosis, and the pathologic profiles of giant cell reparative granuloma, and of giant cell tumor are critically discussed.

Benign melanocytic lesions. A morphometric analysis.

Nuclei of melanocytes in 10 lentigines simplices, 10 junctional, 10 compound and 10 intradermal acquired melanocytic nevi were morphometrically evaluated, at three different skin levels: epidermis, papillary and reticular dermis. The considered parameters were: area, perimeter, form Ar, and form Ell. Mean values of nuclear area and form Ell demonstrated a progressive increase from lentigines to junctional to compound to dermal nevi. This trend was also observed for each considered level. In compound nevi mean values of nuclear area showed an increase from the epidermis to the papillary dermis. In compound and intradermal nevi they showed a decrease from the papillary to the reticular dermis. Mean values of nuclear form Ell presented a continuous increase from the epidermis to the papillary and reticular dermis. Our study shows that, in the evolution of the considered melanocytic lesions (from lentigo to dermal nevi), the nuclei of melanocytes become larger and rounder. In compound nevi, epidermal nuclei were smaller and more elongated than those of the papillary dermis. In compound and intradermal nevi, superficial dermal nuclei appeared larger but less round than the deep ones. These peculiar findings have been generally referred to as a melanocytic maturation process. They are also probably due to other factors, perhaps local factors, because a similar trend is observed in some malignant melanomas.

Neuroendocrinological evidence of an anti-dopaminergic effect of flunarizine.

The effect of a one month treatment with flunarizine (5 mg/day) on pituitary responsiveness to gonadotrophin-releasing hormone (GnRH), thyrotrophin-releasing hormone (TRH) and arginine infusion was assessed in 17 adolescents (11 M, 6 F) treated with the drug to prevent migraine attacks. Basal prolactin concentrations as well as the prolactin response to TRH were significantly (p less than 0.05) increased after flunarizine treatment. Flunarizine had no effect on the folliclestimulating and luteinising hormone response to GnRH stimulation, growth hormone response to arginine infusion or thyrotrophin response to TRH stimulation. Our data suggest that flunarizine may interfere with the hypothalamic-pituitary-prolactin axis decreasing dopaminergic inhibitory tonus.

Epilepsy with bilateral occipital calcifications: a benign onset with progressive severity.

We studied four patients with a focal epilepsy and bilateral occipital corticosubcortical calcifications without any sign of phakomatosis. The clinical course of the disease was similar in all the patients and evolved from a benign onset to a severe encephalopathy with progressive mental impairment. The question of whether these patients have an incomplete and atypical form of Sturge-Weber syndrome or a previously undescribed disorder is addressed.

Classic and immunodeficiency-associated Kaposi's sarcoma. Clinical, histologic, and immunologic correlations.

The evolutionary modifications of the clinical, histomorphologic, and immunopathologic features of both classic and immunodeficiency (ID)-associated Kaposi's sarcoma (KS) were investigated in relation to the immune status of the patients. The histologic picture was similar in the classic and ID-associated forms of the tumor. In classic KS, a variably dense reactive infiltrate was present, and its amount was inversely related to the age of the lesions; conversely, a scarce reactive infiltrate, with the absence of CD4+ cells, was always evidenced in ID-associated KS lesions, even when the immune status of the patient showed no abnormalities. This evidence supports the hypothesis that a specific impairment of skin-associated lymphoid tissue may be crucial to the development of ID-associated KS.

Multicentric reticulohistiocytosis. Report of three cases with the evaluation of tissue proteinase activity.

Three cases of multicentric reticulohistiocytosis showing typical clinical, histologic, and ultrastructural findings are reported. In one, gastric cancer occurred; in the other two cases, severe polyarthritis was the only detectable internal involvement. The serine proteinases, urokinase and tissue-type plasminogen activator, were evaluated both with the autohistographic technique and spectrophotometric assay in lesional skin and synovia. Urokinase levels appeared grossly increased in the lesional synovia and moderately increased in the lesional skin. We suggest that urokinase, presumably released by the activated proliferating histiocytes, may play a major role in the extracellular matrix degradation leading to erosion of cartilage and adjacent bone in multicentric reticulohistiocytosis.

A variety of Ehlers-Danlos syndrome type IV presenting with haematemesis and gastro-esophageal reflux.

A case of a variety of Ehlers-Danlos syndrome (EDS) type IV presenting with haematemesis and melaena and symptoms of gastro-esophageal reflux is described. At referral, the manometric esophageal findings similar to scleroderma and the abundant gastroesophageal reflux seemed noteworthy. 3 years after surgery, bleeding and reflux symptoms appeared, perfectly cured by a total biliary diversion operation. The reported case suggests that patients with EDS type IV especially when presenting with haematemesis and melaena should be investigated for reflux, since bleeding might be due to the action of reflux on the fragile-walled vessels of such patients.

Histologic-prognostic correlations in adenoid cystic carcinoma of major and minor salivary glands of the oral cavity.

Thirty-four cases of adenoid cystic carcinoma (ACC) of the major and minor salivary glands of the oral cavity, treated by wide surgical excision, were studied. The relationship between prognosis of the neoplasm and various morphologic factors were evaluated. Among the morphologic parameters previously studied, which according to various authors may be linked to the evolution of this tumor, we confirmed correlations for both histologic patterns and perineural invasion. In addition, the authors propose neoplastic growth type as a new parameter prognostically significant in ACC. In fact, none of the patients with the pushing type growth pattern died during the study period (p = 0.007). These same patients presented disease-free periods (mean 56 months, median 58 months) significantly longer than those with the infiltrating type growth pattern (mean 28 months, median 24 months).

L-5-hydroxytryptophan versus placebo in childhood migraine prophylaxis: a double-blind crossover study.

L-5HTP was tested versus placebo in a double-blind crossover study of 27 migraine children aged 6-12 years, who recorded their headaches in a headache diary for 1 month. Twenty-one patients subsequently started the trial. The mean daily dose of L-5HTP was 5 mg/kg body weight, and each treatment period with either L-5HTP or placebo lasted 12 weeks. In group A (L-5HTP-placebo; 10 patients) and group B (placebo-L-5HTP; 11 patients) both L-5HTP and placebo led to a significant reduction of the migraine index and frequency of migraine attacks during the 3rd month of each treatment period. However, we found a treatment X period interaction because the efficacy determinants decreased significantly during the first and the second treatment periods in both groups irrespective of the sequence of treatments. No differences were found between L-5HTP (first period of group A) and placebo (first period of group B).
