Efficacy, tolerability and influence on "quality of life" of nifedipine GITS versus amlodipine in elderly patients with mild-moderate hypertension.

OBJECTIVE: The main purpose of this study was to compare efficacy, tolerability and influence on quality of life (QOL) of nifedipine gastrointestinal therapeutic system (NI) 30-60 mg once a day vs amlodipine (AM) 5-10 mg once a day in elderly patients with mild-moderate hypertension. DESIGN: This was a randomized, double-blind, parallel-group, multicenter study. After a 2-week single-blind placebo run-in, patients were randomized to either NI 30 mg or AM 5 mg. Responders continued on the same dosage for 16 additional weeks, while non-responders were titrated to 60 mg NI or 10 mg AM. METHODS: Blood pressure was measured by mercury sphygmomanometer and efficacy equivalence of NI and AM tested by covariance analysis. Diastolic blood pressure (DBP) was the primary efficacy parameter, its baseline value being taken as covariate while centers effect and treatment interaction were included as fixed effects in the analysis model. The secondary efficacy variables systolic blood pressure (SBP) and scores for QOL were analyzed according to the same model. RESULTS: At the end of the study, overall mean DBPs, calculated as least-square means (LSMEANS), in the "by protocol" population were 87.5 mmHg for NI and 86.7 for AM (difference 0.8 mmHg with 90% CI -1.2 to 2.8 mmHg). In the "by intention to treat" (ITT) population LSMEANS were 87.6 mmHg for NI and 86.4 mmHg for AM (difference 1.2 mmHg with 90% CI -0.6 to 3.1 mmHg). SBP LSMEANS in the "by protocol" population were 147.7 mmHg for NI and 147.3 mmHg for AM (difference 0.3 mmHg, with 90% CI -3.7 to 4.3); corresponding values in the "by ITT" population were 148.0 mmHg for NI and 147.2 for AM (difference 0.8 mmHg, with 90% CI -2.8 to 4.6). Mean values for QOL parameters were not significantly different. A total of 173 episodes of adverse events were documented in 54 patients (26 NI and 28 AM), dropouts were 15 (20% of group) on NI and 21 (28%) on AM. CONCLUSIONS: NI 30-60 mg was shown to be as efficacious and safe as AM 5-10 mg in elderly patients with mild-moderate hypertension. QOL improved compared to baseline with no significant difference between the two drugs, thus confirming a positive class effect for calcium antagonists.

Prevalence of headache in an elderly population: attack frequency, disability, and use of medication.

OBJECTIVES: To assess the 1 year prevalence of tension-type headache (TTH), migraine headache (MH), and chronic daily headache (CDH), as well as of headache in general in a rural elderly population. METHODS: A door to door two phase survey was carried out on all elderly (>/=65 years) residents in three villages in central Italy. Participants completed a standardised headache questionnaire and underwent a clinical evaluation by a neurologist. Headache diagnosis was made according to the classification of the International Headache Society, with minor modifications for the classification of patients with MH with>/=15 attacks/month. RESULTS: Eight hundred and thirty three (72.6%) of the 1147 eligible persons completed the study protocol. One year prevalence rates were respectively 44.5% for TTH, 11.0% for MH, 2.2% for symptomatic headaches, and 0.7% for the remaining types of headache. The prevalence of headache in general was 51.0% because 62 residents had both TTH and MH attacks. Prevalence rates of patients with headache were higher in women than men (62.1% and 36.6% respectively) and decreased steadily with age for the 65-74, 75-84, and 85-96 age groups (56.7%, 45.2% and 26.1% respectively). Prevalence rates were 20.4% for patients with moderate to severe attacks, 18.0% for those with >/=1 attacks a month, and 4.4% for those with CDH. Of the 425 with headache 52 (12.2%) had not taken any drugs for their attacks in the previous year, 195 (45.9%) had taken them regularly, and 178 (41.9%) had taken them only when the headache pain interfered with activities that could not be postponed. Medication overuse was reported by 37.8% of patients with CDH with higher proportions for transformed migraine than for patients with chronic TTH (69.2% and 23.8% respectively, p=0.009) CONCLUSIONS: A consistent proportion of elderly people have primary headaches and consultation with a specialist is particularly recommended for patients with moderate or severe attacks, or with CDH.

Electrotherapeutic management of patients with heart failure.

Heart failure is associated with poor long term survival due to progressive refractory heart dysfunction and sudden cardiac death. Cardiac resynchronization through atrio-biventricular pacing has been introduced to treat patients affected by drug-refractory heart failure with desynchronized ventricular activation, as for complete left bundle branch block. The technique is aimed to overcome interventricular and intraventricular conduction delays leading to ventricular dysynchrony, paradoxical septal wall motion, presystolic mitral regurgitation and reduced diastolic filling times. Short term studies demonstrated that biventricular pacing (and perhaps left ventricular pacing alone) may improve both systolic and diastolic function. Initial studies in patients receiving long term pacing consistently showed significant QRS shortening associated with improvement in symptoms, left ventricular ejection fraction, exercise tolerance, quality of life and New York Heart Association functional class. As far as sudden cardiac death prevention in heart failure is concerned, implantable cardioverter defibrillator (ICD) implantation has been demonstrated to be the most effective therapy in patients with prior cardiac arrest due to ventricular fibrillation or poorly tolerated ventricular tachycardia. Low left ventricular ejection fraction, unsustained ventricular tachycardia and inducibility at electrophysiological study also may identify high risk patients requiring ICD implantation. Further studies are needed to evaluate the effect of cardiac resynchronization on hard end-points, such as survival and long term clinical outcome, and to upgrade risk stratification criteria to be used in selection of candidates for ICD implantation.

Prevalence and correlates of syncope-related traumatic injuries in tilt-induced vasovagal syncope.

BACKGROUND: Vasovagal syncope represents the most common form of syncope in the general population and is usually considered as a benign affection. However, syncope-related traumatic injuries may represent a major complication of such a condition in a relevant percentage of cases. The aim of this study was to assess the prevalence and clinical correlates of syncope-related trauma in a cohort of consecutive patients with recurrent vasovagal syncope. METHODS: Three hundred and forty-six consecutive patients were studied in whom a diagnosis of vasovagal syncope was established. All subjects were interviewed with a standard questionnaire in order to collect all possible information about their clinical history and the occurrence of trauma during syncopal spells. RESULTS: Ninety-four of the 346 patients (27.2%) reported at least one syncope-related traumatic injury. In 31/346 cases (8.9%) the severity of trauma had determined hospital admission and surgical treatment. When compared to the rest of the study population, patients with syncope-related trauma showed a higher prevalence of male gender (p < 0.01), a higher absolute number (p < 0.01) and frequency (p < 0.01) of syncopal episodes in their history. Patients with trauma also reported a shorter duration of warning symptoms preceding syncope (p < 0.01), while showing a higher prevalence of positive cardioinhibitory response to tilt table testing (p < 0.01). Moreover, the number of syncope-related injuries was found to correlate significantly with the number of syncopal spells (r = 0.64, p < 0.01). CONCLUSIONS: Most practicing physicians consider vasovagal syncope as simply a benign affection in young people. However, such a clinical view should be partially revised, as recurrent vasovagal syncope is associated with significant trauma-related morbidity.

[Blood pressure self-monitoring].

The blood pressure is measured, usually, in the last years by the patient at home. The method is useful for the screening of the true hypertensive patients, for the follow-up of "white coat" hypertension, for the control of antihypertensive therapy, and for the clinical trials. The home blood pressure is closer to real blood pressure of a subject and it is better correlated than clinical pressure to target organ damage. The normal values, 135/85 mmHg, are been established by meta-analysis studies. Automatic oscillometric devices should be used. The method, very useful in clinical practice, has to improve in accuracy and measures validation. Doctor has to spend more time to train the patient to use the devices correctly.

Facilitation of atrioventricular reentrant tachycardia by iatrogenic right bundle branch block.

The present case report describes the diagnosis of a concealed bypass tract in the right lateral wall revealed by electrophysiologic evaluation performed in a patient with rare palpitations. A iatrogenic right bundle branch block (RBBB) caused the occurrence of an incessant atrioventricular reentrant tachycardia. The disappearance of the RBBB determined a very difficult induction of the tachycardia that, when induced, showed a shorter cycle length and ventriculoatrial interval than those observed during RBBB tachycardia. The presence of a RBBB ipsilateral to the right free wall accessory pathway provided a critical delay within the circuit thus allowing the bypass tract to recover excitability. This relevant delay also allows the sinus beat to initiate and stabilize the tachycardia thus rendering it incessant.

Solitary muscle metastasis from lung carcinoma.

Recurrence after resection of non-small cell lung carcinoma is generally associated with a poor outcome. Limb muscle metastasis from lung cancer is extremely rare. We present a case of a 71-year-old man who presented with a solitary metastasis to his right lower limb two months after right upper lobectomy for lung cancer (stage: T2N0M0). Twenty-four months after surgical excision and chemotherapy he is alive without signs of neoplastic disease. We believe that a more aggressive approach might be considered for selected patients with solitary extracranial and extra-adrenal metastasis from lung cancer.

Paroxysmal atrial fibrillation in patients paced for atrioventricular block. Comparison between DDD and VDD single-lead pacing system.

BACKGROUND: Paroxysmal atrial tachyarrhythmias have been frequently detected in patients paced for atrioventricular block. However, it is not yet clear which is the actual incidence of such arrhythmias and if they were preexisting but not recognized, or if they could be related to the device. METHODS: One hundred and forty-five patients with a diagnosis of second or third degree atrioventricular block were enrolled into the study. One hundred and twenty-four received a pacemaker Medtronic Thera DDD and 21 a Medtronic Thera VDD. High rate atrial episodes were evaluated for 3 months after enrollment. Atrial electrograms documenting paroxysmal atrial fibrillation (AF) were stored and programming of atrial sensitivity and pacemaker diagnostics was very strict in order to exclude short and false positive AF episodes. [Role of the surgeon in the treatment of small cell lung carcinoma].

Surgery has never played a precise and well consolidated role in the planned treatment of lung microcytoma (SCLC). The acknowledged therapeutic strategy associates local treatment (radiotherapy) with general treatment (chemotherapy). Exeresis is particularly indicated in limited or peripheral forms, followed by intensive polychemotherapy. Scintigraphy with octreotide may be used for the initial screening of patients with widespread disease. Another minor role played by surgery is in the treatment of neoplastic foci remaining after chemotherapy. In some cases the use of a radioguided method which, after intravenous injection of radiolabeled octreotide, allows the accumulation of somatostatin analog in neoplastic foci to be assayed intraoperatively using a manual probe, might help the surgeon to check the radical nature of the operation. In addition, octreotide can be used as a radiotherapeutic pharmacological agent or to enhance the efficacy of chemotherapy in microcytoma and other lung tumours with neuroendocrine differentiation.

[Role of radio-guided surgery with 111In-octreotide in the treatment of thoracic neoplasms].

The study evaluates the role of radio-guided surgery (RGS) with 111In-octreotide in the treatment of lung neoplasms. RGS with octreotide appears to be useful in surgery, above all during the intraoperative staging of the tumour; it can define with greater precision the extent of the resections extended towards the lung wall and ensure a radical approach in minimal lung resections.

[Arrhythmia risk stratification in patients with heart failure. Foreword].

The challenge of preventing arrhythmic sudden death is one of the major issues of today's treatment of heart failure. To pursue this aim, an accurate selection of candidates for sudden death has to be routinely carried out, while a maximized and individualized drug treatment has to be extensively administered in all high-risk selected patients. However, in clinical practice there is no agreement on the selection criteria of sudden death risk, particularly in patients with advanced heart failure. Furthermore, the real impact of each category of drugs in reducing the risk of sudden death in heart failure patients is still under debate. As far as non-pharmacological options are concerned, implantable cardioverter-defibrillators (ICD) have been demonstrated to be the most effective therapy in patients with prior cardiac arrest due to ventricular fibrillation or poorly tolerated ventricular tachycardia. Low left ventricular ejection fraction, unsustained ventricular tachycardia and inducibility at electrophysiological study also may identify high-risk patients requiring ICD implantation. However, such a stratification seems to be effective in ischemic more than in non-ischemic patients, while generally the primary prevention of sudden death is still restricted to a minority of patients. Biventricular pacing has been proven to be effective in optimizing left ventricular function in more than 50% of left bundle branch block and advanced heart failure patients, while further studies are needed to evaluate the real impact of cardiac resynchronization therapy on hard endpoints, such as survival and long-term clinical outcome. Therefore, the selection criteria of "responders" to this novel non-pharmacological therapy still have to be defined. There is convincing evidence in the literature that tailored drug therapy can be highly effective in preventing heart failure progression as well as in reducing total and sudden mortality. Nevertheless, prevention of sudden death is still a debated point in heart failure treatment. For this reason, we aimed to provide heart failure specialists with updated reviews on this topic, such as those published in this issue of the Italian Heart Journal Supplement. Therefore, we are proud to present all the authors who contributed with the high quality of their articles to this editorial effort. Obviously, we have to thank the authors, but we also have to address the readers, thanking them in advance for their interest in such an initiative.

[Indications and potential benefits of implantable automatic defibrillator endowed with biventricular pacing].

Heart failure (HF) is associated with a poor long-term survival due to progressive refractory heart dysfunction and sudden cardiac death. Cardiac resynchronization through three-chambered atriobiventricular pacing has been introduced to treat patients with drug-refractory HF and unsynchronized ventricular activation due to left bundle branch block (LBBB). The technique is aimed to overcome inter- and intraventricular conduction delays leading to a ventricular dyssynchrony, characterized by paradoxical septal wall motion, presystolic mitral regurgitation, and reduction in diastolic filling times. Acute studies demonstrated that biventricular pacing (and maybe left ventricular pacing alone) may improve both systolic and diastolic function. First studies on chronically paced patients consistently showed that the QRS shortening was associated with a significant improvement in symptoms, NYHA functional class, left ventricular ejection fraction (LVEF), exercise tolerance, and quality of life. As far as sudden cardiac death prevention in HF is concerned, the implantable cardioverter-defibrillator (ICD) has been demonstrated to be the most effective therapy in patients with prior cardiac arrest due to ventricular fibrillation or poorly tolerated ventricular tachycardia. Low LVEF, unsustained ventricular tachycardia and inducibility at electrophysiological study also may identify high risk patients requiring ICD implantation. Further studies are needed in evaluating the impact of cardiac resynchronization on hard endpoints, such as survival and long-term clinical outcome, as well as in upgrading risk stratification criteria to be used in candidate selection to ICD implantation. However, HF patients with prior cardiac arrest and LBBB should be considered as the optimal candidates to the "ICD implantation combined with biventricular pacing". Conversely, HF patients with LBBB, but without cardiac arrest, could be considered for "biventricular pacing combined with an ICD". The selection criteria for this novel non-pharmacological therapy still have to be defined. The authors emphasize the main indication to ICD implantation combined with biventricular pacing, i.e. HF patients with prior cardiac arrest and LBBB; controversially, while they discuss the other indications to biventricular pacing combined with an ICD.

Repolarization and refractoriness in patients with persistent atrial fibrillation.

The use of monophasic action potential (MAP) recordings has been of important value during atrial fibrillation to understand the possibility of local pacing capture during the arrhythmia, while MAP and refractoriness determination after sinus rhythm restoration have highlighted the issue of electrophysiological remodeling owing to rate. Moreover the contemporary recording of MAP and refractoriness at the same atrial sites permitted to better understand the behavior of the ERP/MAP ratio in these patients. Local atrial pacing capture has been demonstrated in humans with chronic atrial fibrillation and suggests the presence of re-entrant circuits with large excitable gaps. The studies about atrial remodeling have shown a shortening of atrial ERP or monophasic action potential duration after cardioversion of persistent AF, while discordant results have been observed for what it concerns refractoriness adaptation to rate. Finally, the recording of a mean ERP/MAP90 ratio < 1 at all the pacing cycle lengths, indicates that no post-repolarization refractoriness was present after cardioversion of persistent atrial fibrillation.

Heart rate variability patterns before ventricular tachycardia onset in patients with an implantable cardioverter defibrillator. Participating Investigators of ICD-HRV Italian Study Group.

Time- and frequency-domain analysis of heart rate variability (HRV) has been proven effective in describing alteration of autonomic control mechanisms and in identifying patients with increased cardiac and arrhythmic mortality. Patients with implantable cardioverter defibrillators offer the opportunity to evaluate HRV patterns before ventricular tachycardia (VT) and under control conditions. We therefore analyzed time- and frequency-domain parameters of short-term HRV and power-law behavior of RR interval time series at rest, at 15 to 30 minutes, and immediately before VT. In comparison to control conditions, lower values of mean cycle length duration and total power were observed before VT. Spectral analysis indicated that the low- to high-frequency ratio was significantly higher (5.5 +/- 0.6 vs 2.8 +/- 0.3) immediately before VT than during rest. Both findings were consistent with the shift of sympathovagal balance toward sympathetic predominance and reduced vagal tone. Before VT, a more negative value of the scaling exponent beta of the power-frequency relation (-1.57 +/- 0.04 vs -1.33 +/- 0.04) also confirmed the presence of an altered HRV pattern in comparison to controls. Thus, both abnormal autonomic modulation and dynamic patterns of HRV seem to characterize the minutes before arrhythmia onset in these patients.

Postcardioversion atrial electrophysiologic changes induced by oral verapamil in patients with persistent atrial fibrillation.

OBJECTIVES: The aim of our study was to verify the effect of oral administration of verapamil on atrial electrophysiologic characteristics after cardioversion of persistent atrial fibrillation (AF) in humans. BACKGROUND: Discordant findings have been reported regarding the efficacy of verapamil in preventing the electrical remodeling induced by AF. METHODS: We determined the effective refractory periods (ERPs) at five pacing cycle lengths (300 to 700 ms) and in five right atrial sites after internal cardioversion of persistent AF (mean duration 238.1+/-305.9 days) in 19 patients. Nine patients received oral verapamil (240 mg/day) starting four weeks before the electrophysiologic study, whereas the other 10 patients were in pharmacologic washout. RESULTS: The mean ERPs were 202.0+/-22.7 ms in the washout group and 189.3+/-18.5 ms in the verapamil group (p < 0.0001). The degree of adaptation of refractoriness to rate was similar in the two groups (mean slope value in the washout group and verapamil group: 0.07+/-0.03 and 0.08+/-0.05, respectively), showing a normal or nearly normal adaptation to rate in the majority of the paced sites in both groups. The mean ERP was slightly longer in the septum than in the lateral wall and in the roof, both in the washout and verapamil groups. CONCLUSIONS: In patients with persistent AF, long-term administration of verapamil before internal cardioversion resulted in 1) shortening of atrial ERPs; 2) no change in refractoriness dispersion within the right atrium; and 3) no change in atrial ERP adaptation to rate.
