[Abnormal branching of left pulmonary artery to the lateral and posterior basal segments].

A 70-year-old male, underwent left lower lobectomy due to lung cancer, had an extremely rare form of left pulmonary artery branching. His pulmonary artery supplying to lateral and posterior basal segments (A9 + 10) was arisen anteriorly from the root of the left pulmonary artery in the mediastinum and ran below in the anterior portion of the upper lobe stem bronchus. Embriologically, this abnormality might be due to a derangement of anastomosis between the pulmonary arch system and splanic plexus in primitive lung, and was interpreted as one of the pulmonary sequestration. Clinically, particularly at thoracotomy, the presence of a large vessel in an unexpected location is very hazardous, so that preoperative pulmonary angiography is useful to detect the abnormal branching.

[A case of acromegaly associated with variegated spinal disorders].

A case of acromegaly associated with variegated spinal disorders was reported. The spinal disorders were multiple cervical disc herniations, spinal epidural cavernous angioma, multiple ossification of the spinal ligament and lumbar canal stenosis. A 51-year-old woman with acromegaly, complaining of disturbances of delicate hand movement and gate, consulted our department. Her past history included diabetes mellitus, hypertension and progressing enlargement of her extremities. Serum growth hormone level was 65.7 ng/ml and somatomedin-c level was 746 ng/ml. Brain MRI showed a pituitary tumor extending to the right cavernous sinus. Cervical MRI revealed disc herniations at C5/6 and C6/7. Thoracic MRI revealed osteoporosis, ossification of the posterior longitudinal ligament and multiple ossification of yellow ligament. Lumbar MRI disclosed ossification of yellow ligament and canal stenosis. Anterior fusion of C5-C7 and an intracapsular removal of the pituitary tumor were performed. Its pathology was that of eosinophilic adenoma. After 3 months, she suffered from paraparesis. On repeating MRI examination with Gd-DTPA, a spinal epidural mass was found at T4. Under laminectomy of Th3-5 and Th8-11, the epidural mass and ossified yellow ligament were removed. The epidural mass was cavernous angioma. She was able to walk without any assistance. An association of spinal canal stenosis with acromegaly is well known. But the association of disc herniation and with the ossification of spinal ligaments is rather rare in the literature. Spinal epidural cavernous angioma is very rare. We discussed the etiological aspects and the management of spinal disorders with acromegaly.

[Electro chemiluminescence immunoassay].

Boehringer Mannheim is starting into a new age of immunoassay systems. A new immunoassay technology should allow the development of random access immunoassay analyzers with short incubation times. To satisfy the such matter, we have found a new technology ECL (Electro ChemiLuminescence). This technology has been licensed to Boehringer Mannheim by IGEN inc. (Maryland, USA). It's based on a microparticle solid phase coated streptavidin and a very promising complete new electro chemiluminescence technology. The ruthenium complex label used in the system will enables extremely stable reagents compared to enzyme conjugates. The test principle is the following : 1) pipette a sample, biotinylated antibodies, antibodies labeled ruthenium complex, and streptavidin coated microparticles into a reaction cup, 2) after incubation, the reaction mixture is aspirated into an electrochemical measuring cell, and unbound conjugate are washed away by tripropylamine (TPA) and a magnet, 3) by applying a potential to electrode, the ruthenium complex become to exciting stage, and the signal generation initiates. The analyzers using this technology have developed 2 type of fully automated instrument. The use of this new technology and these analyzers will make immunoassay analyses as convenient as clinical chemistry.

A point mutation in glycoprotein IX coding sequence (Cys73 (TGT) to Tyr(TAT)) causes impaired surface expression of GPIb/IX/V complex in two families with Bernard-Soulier syndrome.

Bernard-Soulier syndrome (BSS) is a rare inherited bleeding disorder which is caused by abnormal expression or function of the glycoprotein (GP) Ib/IX/V complex, a platelet major receptor for von Willebrand factor. We studied four BSS patients in two unrelated families in which the same and novel mutation was found. Flow cytometric analysis showed that GPIX was completely absent but residual amounts of GPIb alpha and GPV were detectable in these patients. We analyzed all coding regions of GPIb alpha, GPIb beta, GPV and GPIX which were amplified from the patients' genomic DNA by the polymerase chain reaction (PCR). In all four cases, we identified a point mutation in the GPIX coding region that changes the codon for cysteine 73 (TGT) to a codon for tyrosine (TAT). Furthermore, we confirmed by a transient expression study that the mutation caused the loss of adequate surface expression of GPIX. Since cysteine might be important for the secondary structure, this mutation of GPIX gene would lead to a dramatic conformational change of GPIX protein, resulting in the reduced surface expression. We concluded that this novel point mutation of the GPIX gene was responsible for BSS in these families.

[Cavitary lung cancer with a fungus ball-like shadow].

A 75-year-old woman had an irregularly shaped cavitary lesion in the right upper lung field on a chest X-ray film and a CT scan. Primary lung cancer was suspected, but no evidence of malignancy or of infection was found on examination of specimens obtained by transbronchial biopsy and by lavage. Seven months after the first examination the cavity was found to have enlarged and an intra-cavitary fungus ball like shadow was seen. On the basis of these findings, pulmonary aspergilloma with or without primary lung cancer was suspected. Examination of a specimen obtained by transbronchial biopsy revealed squamous cell carcinoma. Right upper lobectomy was done, and the resected tissue included a polypoid nodule and a cavity wall composed of a milky-white solid tumor. Microscopic examination revealed a moderately differentiated squamous cell carcinoma in both the cavity wall and the polypoid nodule, and no evidence of fungal involvement.

Effectiveness of the anticholinergic agent oxitropium bromide in elderly patients with bronchial asthma.

The short-term and long-term effects of the anticholinergic agent oxitropium bromide (CAS 30286-75-0, OTB) were studied in nine elderly patients with bronchial asthma (8 men and 1 women, age: 69.5 +/- 5.2 years, 3 atopic and 6 (not atopic cases). 2 h after inhalation of 0.8 mg of OTB, forced vital capacity (FVC) had increased from 2.28 +/- 0.22 1 to 2.87 +/- 0.27 1 (p < 0.01), forced expiratory volume in 1 s (FEV1) had increased from 1.22 +/- 0.14 to 1.57 +/- 0.16 1 (p < 0.01), and respiratory resistance (Rrs) had decreased from 5.9 +/- 0.8 cmH2O/l/s to 4.9 +/- 0.7 cmH2O/l/s (p < 0.05). The patients inhaled OTB (0.2 mg t.i.d.) for 6 weeks. Their peak expiratory flow rates, measured each morning and each evening, increased significantly during the first 2 weeks of administration, and the increase was sustained for the entire weeks (p < 0.05). Other pulmonary-function tests were done before drug administration and again at the end of the 6 weeks. By the end of the 6th week of OTB administration, FVC had increased from 2.30 +/- 0.22 1 to 2.99 +/- 0.26 1 (p < 0.01), and FEV1 had increased from 1.25 +/- 0.17 to 1.70 +/- 0.22 1 (p < 0.05). These data indicate that OTB can be useful in the treatment of elderly patients with bronchial asthma.

Association of estrogen receptor dinucleotide repeat polymorphism with osteoporosis.

We investigated the association between dinucleotide (thymine-adenine) repeat polymorphism lying upstream of human estrogen receptor (ER) gene and bone mineral density (BMD) as well as biochemical markers for bone metabolism in 144 healthy postmenopausal Japanese women. The genotype was classified into 'A' through 'R' according to the number of the repeats, from 10 to 27. BMD was expressed in Z score (a deviation from the weight-adjusted average BMD of each age using the standard deviation as a unit). The people having genotype C (12 repeats of thymine-adenine) allele (n = 15) had significantly lower Z score of spine BMD (mean +/- SD; -1.11 +/- 1.3 vs. -0.06 +/- 1.2; p < 0.01) and of total body BMD (-0.58 +/- 1.0 vs. 0.31 +/- 0.9; p < 0.01) than those without this genotype (n = 129). They also had significantly higher levels of serum intact osteocalcin, urinary pyridinoline, and urinary deoxypyridinoline. These results suggest that genetic variation at the ER locus may be associated with some determinants for BMD and bone metabolism in postmenopausal women.

The activation and nuclear translocation of extracellular signal-regulated kinases (ERK-1 and -2) appear not to be required for elongation of neurites in PC12D cells.

The outgrowth of neurites was induced in PC12D cells, a subline of PC12 cells, that were treated not only with NGF but also with dbcAMP, staurosporine or bFGF. Simultaneous activation and rapid nuclear translocation of MAP kinases (ERK-1 and ERK-2) were observed in cells treated with NGF or bFGF. But staurosporine and dbcAMP induced no or only slight activation of the kinases. The nuclear translocation of the MAP kinases was not induced by the latter agents. These observations suggest a close relationship between the activation and the nuclear translocation of MAP kinases and, moreover, that stimulation and relocalization of MAP kinases might not be required for the outgrowth of neurites from PC12D cells. Staurosporine and dbcAMP may stimulate a down-stream step of the NGF pathway, or a parallel pathway(s) to the MAP kinase cascade in promoting neurite formation from PC12D cells. These agents mimic the effects of NGF in promoting neurite outgrowth in cultured sympathetic neurons, but not in conventional PC12 cells. Because of the similarity between PC12D cells and primed cells, it seems possible that activation and nuclear translocation of MAP kinases might be required for the transcription-dependent differentiation step but might not be necessary for the elongation of neurites at least in response to staurosporine or to dbcAMP.

Clinical correlates of Alzheimer's disease with and without silent radiographic abnormalities.

OBJECTIVE: To determine whether patients with Alzheimer's disease (AD) who do not have historical or clinical evidence of stroke but who do have computed tomographic or magnetic resonance imaging evidence of noncortical lesions smaller than 2 cm or periventricular "caps" differ from other patients with AD. METHODS: The computed tomographic or magnetic resonance imaging scans of 158 patients meeting criteria of the National Institute of Neurological Disorders and Stroke-Alzheimer's Disease and Related Disorders Association for probable AD were reviewed by one neuroradiologist. Two measures of disease severity--the Modified Mini-Mental State examination and the Blessed Dementia Rating Scale (Part I)--were subjected to two-way analysis of variance with scan type (computed tomography or magnetic resonance imaging) and lesion number as between-group factors and age and disease duration as covariates. RESULTS: No relationship was seen between lesion number or periventricular caps and disease severity. CONCLUSION: In this cross-sectional analysis using these clinical measures, patients with AD who have well-defined radiographic abnormalities cannot be differentiated from patients with AD who do not have them.

Neuropsychologic impairment in early HIV infection. A risk factor for work disability.

OBJECTIVE: To explore the functional significance of incident neuropsychologic impairment among initially asymptomatic subjects infected with human immunodeficiency virus. DESIGN: Prospective, observational cohort study of homosexual and bisexual men to examine the incidence of work disability related to the onset of neuropsychologic impairment. SETTING: A university clinical and behavioral research site in New York City. PARTICIPANTS: Sample of 207 homosexual and bisexual men; 123 were seropositive and 84 were seronegative. PRINCIPAL OUTCOME MEASURES: Incident work disability in the course of 4.5 years of follow-up, with disability defined as a persistent (> or = 24 months) change in work hours (from 20 or more to less than 20 h/wk). RESULTS: Compared with seronegative control subjects (n = 72), the relative risk of work disability among initially asymptomatic seropositive men (n = 44) was 2.76 (95% confidence interval, 1.2 to 6.5), nearly a threefold increase. Proportional hazards models show that this increased risk is attributable to the development of major neuropsychologic impairment in a subset (eight of 44) of the initially asymptomatic men, which is significantly associated with incident work disability (6/8 [75%]). Adjusting for symptom status and CD4+ cell count at the time of disability did not eliminate the increased risk associated with neuropsychologic impairment. CONCLUSIONS: In this cohort, the increased risk of work disability among initially asymptomatic human immunodeficiency virus-positive men was related to incident neuropsychologic impairment; such impairment predicted work disability independently of symptom status and CD4+ cell count over the follow-up period. Neuropsychologic impairment in the course of human immunodeficiency virus infection may indicate increased risk for poor outcomes over and above that associated with systemic disease.

A standardized technique for establishing onset and duration of symptoms of Alzheimer's disease.

OBJECTIVES: To develop an informant-based semistructured interview to determine the onset and duration of symptoms of Alzheimer's disease, and to use this instrument with informants to characterize a cohort of mildly impaired patients with Alzheimer's disease. DESIGN: In study 1, interrater and interinformant reliability was examined for the date of onset and the order of appearance for specific symptoms that were elicited by the semistructured onset interview. In study 2, the instrument was used to characterize disease onset in a cohort of patients with Alzheimer's disease who were participating in a large multicenter study. SUBJECTS: Informants of patients with Alzheimer's disease. RESULTS: In study 1, interrater reliability for duration of illness was excellent (intraclass correlation coefficient = .99, P < .001), and interinformant reliability was good (intraclass correlation coefficient = .86, P < .001). Agreement for the presence of a given symptom was highest for those that were most commonly reported (eg, memory and performance difficulty). In study 2, 89% of the cohort had memory problems, and 63.9% had performance difficulties as the first or second symptom. Depression and language problems were less commonly reported. Psychosis and behavioral disturbances were rarely reported as the first problem. CONCLUSION: This instrument provides a reliable procedure for standardizing the estimation of duration of illness based on retrospective report.

Lack of promoting activity of four pesticides on induction of preneoplastic liver cell foci in rats.

Four pesticides were examined for hepatopromoting activity using a medium-term bioassay based upon induction of glutathione S-transferase placental form (GST-P) positive foci in the rat liver. Male F344 rats were initially injected with diethylnitrosamine (DEN; 200 mg/kg body weight) intraperitoneally and 2 weeks later were treated with O-ethyl O-4-nitrophenyl phenylphosphonothioate (EPN; 75 and 150 ppm), diazinon (500 and 1,000 ppm), phenthoate (500 and 1,000 ppm), or iprobenfos (500 and 1,000 ppm) in the diet for 6 weeks and then killed, all rats being subjected to partial hepatectomy at week 3. All of the pesticides gave negative results, the numbers and areas of GST-P positive foci not exceeding the control values for animals given DEN alone. Indeed, a significant reduction of foci development was seen for EPN (75 ppm). These findings provide experimental evidence that the presently examined four pesticides do not have hepatocarcinogenic potential in rats.

Lack of carcinogenicity of monosodium L-glutamate in Fischer 344 rats.

Monosodium L-glutamate (MSG) was administered to groups of 50 male and 50 female Fischer 344 rats in the diet at levels of 0, 0.6, 1.25, 2.5 and 5.0% for a 2-yr period. Survival of dosed and control rats of both sexes was comparable, although animals receiving 5.0% MSG showed a trend towards (or significant) growth retardation. Urinary biochemistry values indicated increased pH and Na+ levels and decreased K+ in rats of both sexes given 2.5 or 5.0%. However, no significant increases in proliferative or neoplastic lesion development in the urinary tract were observed in any MSG-treated animals. In addition, occurrences of neoplastic lesions in other organs did not differ between treated and control groups. The present study thus did not demonstrate any long-term carcinogenic effect of MSG administered to rats at levels up to 5.0% in the diet.

The chronological change of vertebral bone loss following oophorectomy using dual energy X-ray absorptiometry: the correlation with specific markers of bone metabolism.

The changes of vertebral bone mineral density (BMD; g/cm2) following oophorectomy were studied, using dual energy X-ray absorptiometry (DEXA) in 38 premenopausal and 244 oophorectomized women. Two biochemical indices of bone remodeling, urinary deoxypyridinoline (DPyr) for bone resorption and serum intact human osteocalcin (hOC) for bone formation, were also measured at the same time. The rate of bone loss in the first year after oophorectomy was 10.7%, while that in the second year was 5.7% (rapid phase), followed by a slow phase at the rate of 1.1%. The bone mass finally reached an osteoporotic level (BMD < 0.767 g/cm2) at 12 years after oophorectomy. The DPyr increased to reach a peak level in the first year, whereas the hOC increased and reached its peak level in the second year after surgery. The maximal bone loss in the first year is considered to be caused by the remarkable increase of bone resorption and the biological delay of the maximal increase in bone formation.

Simple reaction time as a measure of global attention in Alzheimer's disease.

Alzheimer's disease (AD) is characterized by progressive decline in memory, language and other cognitive functions. Deficits in attentional processes have also been suggested. A simple reaction time (RT) task was used to assess global attention in AD. The length and consistency of a warning signal given prior to the response stimulus were manipulated to determine if patients with AD and age-matched controls benefit from predictability in RT tasks. Overall reaction time was slower in the AD group than in the and control group. Both groups demonstrated significant improvement in RT with long warning signals compared to short warning signals, but only the control group benefited from the consistency of the warning.
